It begins early in life in many subjects, and it is well recognized that over % of asthmatic children go into long-term clinical remission, defined as the complete absence of asthmatic symptoms and no asthma medication for at least 24 months, during adolescence. Several studies have shown spirometric abnormalities and BHR during clinical remission. It is unknown whether these functional abnormalities, which are supposed to be indicative of asthma severity with respect to symptomatic asthma, reflect persistent airway inflammation or merely indicate residual airway damage or are related to another mechanism such as a familial predisposition. It is likely that the nature of BHR in asthma remission is not same as that in symptomatic asthma. We have shown that the former condition is associated with lower levels of blood eosinophils and eosinophilic cationic protein, a lower degree of bronchial responsiveness to exercise, and a more common formation of plateau on the dose-response curve to high-dose inhaled methacholine (i.e., limited maximal airway narrowing), compared to the latter condition. It is still controversial whether BHR in adolescents with asthma remission is reduced by inhaled corticosteroids. Better understanding of the mechanisms that lead to asthma remission, especially that seen during adolescence, is likely to lead to significant advances in our understanding of asthma pathogenesis, and should provide insights into how remission might be induced with therapy. We still have minimal understanding of the mechanism underlying BHR in adolescents with asthma remission. Elucidation of this mechanism would be an important step towards new perspectives that see remission as the next therapeutic frontier in asthma. ( 1(1):11-19, 213) Keywords: Adolescence, Asthma, Bronchial hyperresponsiveness, Clinical remission 서론천식은처음발병하여증상이발현된후, 기간이지나감에따라증상이같은정도로지속되기도하고, 때로는악화되거나경감되기도하는다양한경과를취한다. 또한일부에서는장기간에걸쳐천식증상이더이상나타나지않는상태를시현하기도한다. 천식에대한치료없이, 즉약제를사용하지않으면서, 천식증상이장기간나타나지않는상태를장기간천식관해 (long-term asthma remission) 라한다. 장기간 의정의는연구마다다르나, 계절적천식이있음을고려하면최소한 1년이상의조건을필요로하며, 대부분은 2년이상인경우로정의한다. 이러한장기간천식관해상태는청소년시기에서뚜렷이관찰된다. 따라서소아천식환자에서청소년기에천식증상이지속되는지또는관해상태가유도되는지여부는자연경과상중요한이정표라할수있다. 한편청소년기천식관해상태의일부분에서는폐기능저하, 기관지과민성이존속하며, 또한기도염증과기도개형의증거도제시되고있다. 이종설에서는청소년기천식관해상태의양상과, 이상태에서나타나는폐기능저하, 기관지과민성, 기도염증, 기도개형등이현증천식에서와차이가있는지, 이들사이에서어떠한연관성이있는지를고찰함으로써, 청소년기장기간천식관해상태의이해를돕고자한다. 2 Koh YY Long-term asthma remission during adolescence 장기간천식관해의양상소아천식환자를성인연령까지장기간추적관찰한연구들은이들중 16 74% 에서장기간천식관해상태를보고하고있다. 1) Jenkins 등 2) 은 7세에천식으로진단받은 741 명중 29 36세에서 19명 (2.6%) 만이천식증상을호소한다고보고하면서, 소아천식환자들의일부에서만이성인연령까지증상이지속한다고기술하였다. Kjellman과 Gustafsson 3) 은 4 14 세의천식환아 명을 2 3세시점까지추적관찰하여 9명 (16%) 에서최근 1년동안천식관해상태를보고하였다. De Marco 등 4) 은 44세의 18,873 명자료를분석하여천식으로진단받은환자들중최근 2년동안의천식관해상태가 4.8% 이었으며, 남자에서 49.%, 여자에서 41.6% 로남자에서높아있음을보고하였다. Vonk 등의연구 ) 에서는 14 세천식환아 91명을평균 3년동안추적관찰하여 32 42세시점에서 47명 (1.6%) 이최근 3년동안의천식관해상태를보였다. 이러한장기간천식관해상태는이미청소년또는청년시기에서부터뚜렷이관찰된다. 이시기의환자들을대상으로한연구를보면, Martin 등의연구 6) 에서는 7세이전에천명이시작된 331 명중 14세까지 번이상천명이 223명에서있었는데, 이들중 79명 (3.4%) 이최근 1년이상의천식관해상태를보였으며, Tucson Children s Respiratory Study (TCRS) 에서는사춘기이전에천식으로진단받은 166명의천식환자를 16세 ( 사춘기이후평균 4.2±1년후 ) 까지추적관찰한결과 69명 (42%) 에서사춘기이후증상이소실되었다. 7) Taylor 등 8) 도 176 명의소아천식환자중 18세에 68명 (38.6%) 에서최근 1년동안의천식관해상태가있었음을보고하였고, Sekerel 등의연구 9) 에서는 1명의소아천식환자를평균 11년동안추적관찰하여 61명 (3%) 에서 1년이상의천식관해상태를보였다. 캐나다 Ontario 주에서 1994년에출생하여 6세이전에천식으로진단받은 34,216 명에서, 12세까지천식으로인한병원방문이필요없었던경우가 48.6% 이었다. ) 한편 Childhood Asthma Management Program (CAMP) 연구에서는이보다훨씬낮은천식관해율을보고하였다. 11) 이연구에서는 4 6년간의 budesonide/cromolyn/ 위약치료후 4년간의관찰기간중마지막 1년동안 (13 22 세 ) 천식상태를 remitting (no asthma activity in the last year), persistent (asthma activity in the last year), periodic (neither remitting nor persistent) 으로구분하였는데, 각각 6%, %, 39% 의분포를보였다. 이와같이낮은관해율은이전의다른연구들이환자 / 보호자의기억에의존했던것에반해자주 (1년에 4회 ) 면담을통해상태를파악하였기때문이며, 또한상당히넓은범위로천식의활동성 (activity) 을인정하였기때문이다. 다만이연구에서 persistent군이나 periodic군에서도천식의심한정도는과거투약기간에비하여훨씬경감되어있었다. 이와같이천식관해상태가사춘기-청소년시기에많이이루어진다는증거는연령에따른천식유 Percent Female Male 병률의분포에서도드러난다. 최근의미국인구통계자료에서천식 유병률이 2 세경에, 특히남자에서, 현저히저하되는현상이 보고되었는데 (Fig. 1), 12) 이는성인연령에서새로발생하는천식이 있다는점을감안하면, 사춘기 청소년시기에확연히천식의관해 가유도됨을가리킨다. Prevalence of current asthma among males and females aged 1 8 years, by age: United States, annual average Adapted from Centers for Disease Control and Prevention. 12) 이와같이소아천식환자의 % 내외는청소년또는성인연령 에서장기간관해상태를보이는데, 그렇다면천식증상의지속 (persistent) 또는관해상태를유도하는요인이무엇인가? 많은연 구들은소아기천식에서폐기능의감소가심한경우, 기관지과민 성의정도가심한경우, 천명의빈도가자주있는경우, 혈중호산구 수가많은경우등이천식증상지속상태의위험인자로제시하고 있다. 1) 이는소아기천식의중증도가높을수록, 천식증상이지속 될가능성이많음을가리킨다. 또한아토피, 특히집먼지진드기에 대한감작상태가관해를유도하는데저해요인으로작용하며, 부 모가천식이있는경우에도천식증상의지속상태로유도하는경 향이있다. 2,13) 남자에서여자보다천식관해상태비율이높은데, 4,9) 이는천식환자가소아연령에서는남자가여자에서보다많으나, 청 소년기를지나면서성비가역전하는현상 (Fig. 1) 에서도짐작할수 있다. TCRS 에서는청소년기에천식증상을지속시키는요인으로 다른요인들 ( 천명의빈번함, 아토피, 부비동염 ) 과함께비만을제시 하였다. 7) 발병연령도중요한것으로보이는데 De Marco 등 4) 은천 식관해율을, 세이하에서발병한환자의 62.8%, 2 세이상에서 발병한환자의.% 로보고하였다. 이는소아천식에서성인천식 에서보다예후가좋다는사실에부합하는소견이다. CAMP 연구 에서 budesonide 군, cromolyn 군, 위약군사이에 remiting/persistent/periodic 의세가지상태비율이다르지않게나타난것 11) 은 inhaled corticosteroid (ICS) 등의천식치료가천식관해상태로의 유도에별로영향을미치지않는다는점을시사하며, 이는 ICS 가 항염증제로서천식의치료에중요하며, 특히조기치료의이점이강 조되지만, ICS 를중단한후장기적으로폐기능등자연경과를호 12

3 고영률 청소년기장기간천식관해 전시킨다는증거는부족하다는소견 14) 과부합한다., P<.1 P<.1 폐기능저하와기관지과민성, 장기간의임상적관해상태에서천식증상이오랫동안나타나지않았다면, 폐기능이정상범위로회복되고, 기관지과민성도소실될것이라예측할수있지만, 많은연구들에의해실제는그렇지않다는것이밝혀져있다. Boulet 등 ) 은 2년이상천식치료제없이무증상시기를경험한 18세부터 61세사이의 3명중 21명에서폐기능이저하되거나기관지과민성이지속함을보고하였다. Van den Toorn 등의연구 16) 에서는 1년이상임상적관해상태의 18 2 세환자 21명에서 forced expiratory volume in 1 second (FEV 1) 이정상대조군보다저하되어있고, 이들중 11명에서기관지과민성이있었다. Obase 등의연구 17) 에서는 년이상천식관해상태의 2세전후환자 23명중 2명에서기관지과민성이있었으며, Komatsu 등 18) 은 년이상임상적관해상태의 24명환자에서 maximal midexpiratory flow (MMEF), forced expiratory flow at 7% of forced vital capacity (FEF 7) 가정상대조군에비해저하되고있고이들중 12명 (%) 에서기관지과민성이있었는데, 기관지과민성이있는군에서없는군보다소아기천식유병기간이길고, 관해기간은짧았음을보고하였다. Yoshikawa 와 Kanazawa 19) 도 3년이상천식관해상태 19세전후의환자 24명에서 FEV 1, MMEF가정상대조군보다낮고, 이들중 18명에서기관지과민성이있음을보고하였다. Gruber 등 2) 은치료종료후 1년이상무증상시기를경험한 세환자 118 명을아토피 (n= 81) 와비아토피 (n= 37) 로구분하였는데아토피군에서비아토피군보다기관지과민성이잔존하는경우가유의하게높았으며 (41/81 vs. 7/37, P =.1), 아토피군에서비아토피군보다 FEF 2 가유의하게낮았다. 이는아토피유무가천식관해상태를유도하는데중요한요인임과동시에, 관해상태에서기관지과민성의유무를결정하는데도중요한변수임을시사하였다. 이와같이천식관해상태에서폐기능저하 / 기관지과민성의존재가있는것이알려지면서일부연구에서는관해상태중단순히증상이없는상태를임상적관해 (clinical remission), 폐기능저하 / 기관지과민성도없는상태를완전관해 (complete remission) 로구분하고있다. Panhuysen 등 21) 은 13 44세의환자 181 명을평균 2년추적관찰하여 41% 가증상이없는상태, 즉임상적관해가있었지만 11% 만이기관지과민성과폐기능저하가없는, 즉완전관해상태였다고보고하였다. Vonk 등의연구 ) 에서 2% 를차지한임상적관해환자중 43%, 즉전체환자중 22% 가완전관해상태이었으며, Sekerel 등의연구 9) 에서는전체 3% 의임상적관해상태의환자중 27% 가완전관해상태였고, 나머지 26% 에서는폐기능저하또는기관지과민성이있었다. Broekema 등 22) 은 세전후의천식환자 16명을분류하여 62명이최근 3년이상의임상적관해상태였으 PD2 AMP (μg) 1, Asthma (n=21) (n=21) 며, 이들중 18 명이기관지과민성이나폐기능저하가없는완전관 해상태였다고보고하였다. 이와같이모든연구에서, 전체임상적 관해상태의 % 이하에서완전관해상태가있다는보고를하고 있는데, 이는대체로천식관해상태의 % 이상에서는폐기능저 하또는기관지과민성이잔존함을가리킨다. 기도염증및기도개형 Controls (n=18) Fig. 2. Van den Toorn 등 16) 은 21 명의임상적관해환자에서호기산화질소 (fraction of exhaled nitric oxide, FeNO) 농도가정상대조군보다높았고현 증천식환자군과비슷하였으며, adenosine -monophosphate (AMP) PD 2 은정상대조군보다낮았지만, 현증천식환자군보다 높아있음을보고하였다 (Fig. 2). 이들은후속연구 23) 에서기관지생 검을시행하였는데기관지내호산구, T 세포, 비만세포, IL-, 혈액 내호산구수가임상적관해군에서정상대조군에비해높아있었 으며, 망상기저막 (reticular basement membrane) 도비후되어있어 (Fig. 3) 기도개형의증거가있음을보고하였다. 그러나이들지표 에서현증천식환자군보다는낮은경향성이있어서, 기도염증과 기도개형이현증천식환자의수준에는미치지못함을보여주었 다. Obase 등 17) 은 23 명의천식증상이 년이상없었던대학생환 자 23 명에서유도객담소견상호산구수, eosinophil cationic protein (ECP), tumor necrosis factor-α (TNF-α), granulocyte macrophage-colony stimulating factor 가정상대조군보다높았으며, 경 증현증천식환자군보다는통계적으로유의하지는않았지만낮아 있음을보고하였다 (Table 1). Hara 등의연구 24) 에서도 년이상무 13

Komatsu 등의연구 18) 에서 도임상적관해상태의 24 명에서객담호산구수가정상대조군보 다높았으며, 객담호산구 3% 이상을기준으로하는호산구증가는 8 명 (33%) 에서발견되었다. 또한 FeNO 농도가정상대조군보다높 았다. 이연구에서는현증천식군이포함되어있지않아비교할수 는없었다. Volbeda 등 2) 은 세전후의천식완전관해군과현증 천식환자군의비교에서기저치객담내호산구수는차이가없었 지만, AMP 유발검사에의한호산구의증가는차이를보여서, 현증 천식환자군에서는유의한증가가있었던반면천식완전관해군 에서는증가를보이지않았다. 이는천식관해상태와현증천식사 이에미세하지만기도염증의차이를보여주는자료라할수있다. Broekema 등의연구 22) 에서객담내호산구수는관해환자와현증 천식환자사이에차이가없었으나, 객담 ECP 는완전관해군에서 현증천식환자군에비해유의하게낮았으며, 기도개형소견은관 해군과현증천식군사이에차이가없었다. 이연구는정상대조군 이포함되어있지않아관해군에서염증및기도개형이증가해있 는지여부를평가할수는없었다. 이러한연구들을종합하여보면, 기도염증과기도개형은천식관해상태에서대체로정상대조군 보다증가되어있으나현증천식보다는그정도가덜한것으로보 인다. 이에따라천식관해는기도염증또는폐기능저하, 기관지과 민성등의정도가천식증상을나타내는역치 (threshold) 에미치지 못하는상태에서나타나는현상이라는가설이제기될수있다 (Fig. 4). 1) Symptom threshold Fig. 4. Pharmacol Ther 211;13: 38-4, with permission of Elsevier. 1) 천식관해상태의기관지과민성이기도염증또는기도개형과관련이있는가? 천식관해상태에서기도염증또는기도개형의소견이있다면, 천식관해상태의기관지과민성에서이들과의연관성이있을것이 라추정할수있으나, 이에대한연구결과는서로일치하지않고있 다. Obase 등의연구 17) 에서천식관해상태의 23 명중 2 명에서기 관지과민성을나타내어, 기관지과민성유무에따라두군을통계 적으로비교할수는없었지만, 23 명에서 methacholine PC 2 과객 담내호산구수또는 TNF-α 농도가서로유의한상관성을보여, 기관지과민성이기도염증과연관되어있음을시사하였다. Komatsu 등의연구 18) 에서는기관지과민성이있는관해군에서기관지 과민성이없는군에비해객담내호산구수가유의하게높음을관 찰하였다. Ann Allergy Asthma Immunol 23;91:297-32, with permission of Elsevier. 28) 의변화를분석하였다. 그결과, 위약군뿐만아니라 budesonide 군에서도투약전에비해 3, 6, 9개월에각각 PC 2 의유의한변화가없었다. 이는동일한용량의 budesonide 를투여한후현증천식환자 (n=19) 에서의 methacholine PC 2 의유의한증가와대비되었다 (Table 2). 이와같은결과는관해상태의기관지과민성이 ICS에의해변화하지않은것이 ICS의용량, 기간이충분하지못한것에기인하지않았음을가리킨다. 이와같은결과로써필자등은관해상태의기관지과민성이기도염증과관련이없고, 장기간의기도염증에따른기도개형과같은구조적인변형또는가족적성향과관련이있을것이라는가설을제시하였다. 필자등 27) 은이에따라청소년기관해환자를기관지과민성유무에따라구분하여, 이들의부모에서기관지과민성을측정하여, 관해상태의기관지과민성이가족적성향에따라표현될것이라는가설을세우고이를검정하였다. 결과, 기관지과민성이있는환자들의부모들에서기관지과민성이없는환자들의부모들보다기관지과민성의빈도및기관지반응도가유의하게높아있음을관찰하였다. 이는청소년기관해상태의기관지과민성은가족적성향이중요한요인임을시사하였다. 필자등 28) 은또한기관지과민성이있는관해환자, 기관지과민성이없는관해환자, 현증천식환자, 정상대조군사이에서혈액호산구수와혈청 ECP를비교하였다. 결과, 호산구수및 ECP는관해환자중기관지과민성이있는군과기관지과민성이없는군사이에차이가없었으며, 이두군에서는정상대조군과는차이가없고, 현증천식군보다는유의하게낮아있음을관찰하였다 (Fig. ). 이는, 비록혈액내염증지표로써기도염증을평가하는데는한계가있지만, 천식관해상태의기관지과민성이기도염증에기인하지않음을시사하였다. 위와같은연구결과들은청소년기장기간천식관해상태의기관지과민성이기도염증과무관하다는점을부각시키는동시에, 천식관해상태에서기도염증이존재한다는다른연구결과들에강력히의문을제기하는소견이다. 이와같이연구결과가상충되는이유는명확하지않지만, 첫째대상연령이다른연구들 17,18) 에서는청년내지장년등의성인인데반하여필자등의연구 26,28) 에서는청소년에국한하였으며, 둘째필자등의연구 26,28) 에서는천식관해상태판정을정기적인면담 (3개월내지 6개월간격 ) 으로확인하였으나다른연구들 17,18) 에서는후향적으로기억에의존하여대상환자를모집함으로써천식관해상태정의의엄격함에차이가있었기때문이라추정된다. 또한연구대상에서천식과흔히동반되는알레르기비염의합병여부도중요했을것으로생각된다. 왜냐하면알레르기비염환자들에서도기도염증의소견이어느정도는나타나기때문이다. 29) 필자등의연구 26,28) 에서는알레르기비염이합병된경우연구대상에서가능한한배제하였다. Broekema 등의연구 22) 에서도기관지과민성유무에따라천식관해환자에서혈액과객담의호산구수가차이가없었으며, 다만기관지생검상 eosinophil peroxidase 염색호산구수에서근소한차이를보여호산구의활성화에는차이가있을가능성을기술하였

6 Koh YY Long-term asthma remission during adolescence 다. 이연구에서는기도개형의지표가기관지과민성유무에따라 천식관해환자에서차이가없었으며, 이들환자에서는현증천식 환자보다유의하게작게측정되어천식관해상태의기관지과민성 이기도개형에기인하지않음을시사하였다. 천식관해상태의기관지과민성이현증천식에서와같은가? 장기간천식관해상태환자의일부에서기관지과민성이지속된 다면, 이러한환자의기관지과민성은현증천식환자에서와같은 가? 즉천식기도의다른특징, 기관지확장제반응이나폐기능의일 중변동률또는최대기도반응의증가의소견이현증천식과비교하 면어떤가? Boulet 등 ) 의연구에서천식관해상태에서기관지확 장제반응과 peak expiratoy flow rate (PEFR) 의일중변동률은정 상대조군보다증가되어있었다. 그러나현증천식환자군이포함되 어있지않아서이들과의비교는이루어지지않았다. 필자등 3) 은 청소년기장기간천식관해상태의기관지과민성이현증천식환 자에서와같은정도의최대기도반응과동반하는지여부를알아보 고자, 기관지과민성이있는천식관해상태의청소년 46 명과현증 천식청소년 44 명에서최대기도반응을측정하였다. 결과천식관해 상태에서는 34 명 (73.9%) 에서, 현증천식에서는 19 명 (43.2%) 에서 (P <.1) 각각최대반응평행을보였으며, 이들을포함하여전체 Maximal airway response (%) P=.1 대상에서최대기도반응은천식관해상태에서 44.%±7.2% 로서, 현증천식환자에서의 48.6%±.8% 보다유의하게낮았다 (Fig. 6). 이와같은최대기도반응은 ICS에의해변화되지않았다. 즉 6개월간의 budesonide 에의해천식관해상태의최대기도반응은, 위약투여와마찬가지로투약전에비해유의한변화가없었는데, 이는현증천식환자의최대기도반응이유의하게감소한것과대비되었다 (Fig. 7). 이와같은결과는필자등의이전연구 26) 에서 PC 2 으로표현되는기관지과민성이 ICS에반응하지않는다는소견과더불어, 천식관해상태의최대기도반응또한기도염증과연관성이없음을시사하였다. 필자등 31) 은이어서최대기도반응과비례한다고알려진 ΔFVC (percentage fall in forced vital capacity [FVC] at the methacholine PC 2) 의차이가있는지도검정하였다. 4명의현증천식청소년과 8명의천식관해상태청소년에서 methacholine 유발검사를시행하여천식관해상태청소년을기관지과민성이있는 44명과기관지과민성이없는 36명으로구분하였다. 이들의 ΔFVC 를비교한결과, 기관지과민성이있는관해환자와기관지과민성이없는관해환자사이에는 ΔFVC가유의한차이가없는 (12.8%; 9% confidence interval [CI], vs. 11.%; 9% CI, ; P = not significant) 반면, 이들두군보다현증천식환자에서는유의하게높았다 (.%; 9% CI, ) (P<.) (Fig. 8). 이와같은결과들은, 천식관해상태의기관지과민성의성상이현증천식에서의기관지과민성과동일하지않음을시사하였다. 필자등 32) 은또한천식관해상태의기관지과민성에서 PEFR 일중변동률과운동유발성기관지수축의정도가현증천식환자및정상대조군과비교하여어떠한지알아보았다. Chest 26;129:272-7, with permission of American College of Chest Physicians. 31) 지과민성이있는경우 PEFR 일중변동률이증가할수있다는다 른연구결과들에부합하였다. 즉, 알레르기비염환자중기관지과 민성이있는환자군에서기관지과민성이없는환자군에비해 PEFR 일중변동률이높으며, 33) 무증상기관지과민성이있는개체 들에서도 PEFR 일중변동률이높았다 34) 는보고가있다. 반면운동 유발성기관지수축정도는천식관해상태 (7.4%±3.9%) 에서정상 대조군 (6.%±2.7%) 과유사하였으며, 이들군에서현증천식군 (22.3%±.%) 보다는유의하게낮았다 (Fig. ). 이와같은결과 는천식관해상태의기관지과민성이현증천식에서의기관지과 민성과달리운동유발기도반응과동반되지않음을가리킨다. Mochizuki 등 3) 은천식관해청소년과현증천식청소년에서기관 지과민도 (bronchial sensitivity) 의지표로 threshold of methacholine (Dmin) 을, 기관지반응도 (bronchial reactivity) 의지표로 slope of methacholine (Sm) 을각각측정하여이들을비교한결과 Dmin 은두군이비슷하였지만, Sm 은관해군에서현증천식군보다유의 하게낮았으며, 이에따라기관지반응도의차이가있음을보고하 였다. Broekema 등 22) 의보고에서는기관지확장제에대한반응이 완전관해군과기관지과민성이있는관해군사이에는차이가없 고, 현증천식군보다는유의하게낮았다. 이결과는기관지확장제 반응이천식관해상태에서정상인보다증가되었는지는알수없지 만천식관해상태의기관지과민성이현증천식에서의기관지과 민성과동일하지않다는결과로해석할수있다. 치료의필요여부 몇몇의연구들은천식이관해상태에있다가일부에서는상당한 Fig. 9. Martin 등 6) 은 14 세에서관해상태를나타냈던환자의약 4% 에서 21 세 까지천식증상의재발이있었음을보고하였고, Sears 등 13) 의연구 에서는 9 11 세사이에천명이있었던 27 명중 26 세때까지추적하 여 89 명은증상이지속하고, 76 명은관해상태를지나다시재발하 였고, 92 명은관해상태를유지하였다. Taylor 등 8) 은 18 세까지관해 상태에있었던환자의약 1/3 이 26 세까지재발하였음을보고하였다. 이와같이청소년기장기간관해상태가, 비록논란이있지만, 기 도염증또는기도개형과관련이있으며, 향후증상재발의가능성 이있다면, 이에대한대책이필요하지않겠는가? 이에대한해답은 현재로서는명확하지않다. 앞서기술한필자등의연구 26,28) 결과는 청소년기장기간관해상태에서는, 기관지과민성이있더라도치료 가필요하지않다는견해를뒷받침한다. 그러나이에상충되는견 해를제시한연구결과도있다. Van den Toorn 등 36) 은그들의연구 결과 16,23) 를배경으로하고, 또한무증상의천식환자에서소량의항 원노출에의하여비록증상이나타나지않으면서도기도염증을 유발하고, 이는 ICS 에의해예방될수있다는소견 37) 을근거로, 천 식관해상태청년들에게 fluticasone/salmeterol 을 3 개월간투여하 17

Respir Med 2;99:779-87, with permission of Elsevier. 36) 고, 전후에 methacholine, AMP 유발검사와 FeNO 를측정하고, 기 관지생검을시행하였다. 결과 methacholine 과 AMP 에대한기관 지과민성이호전되었고 (Fig. 11), FeNO 가감소하며, 기관지생검상 tryprase 와망상기저막두께가유의하게감소되었음을보고하였 다. 이와같은결과는천식관해상태에서도 ICS 와같은치료가필 요하다는점을시사하였으며, Koh 등 26) 의연구결과와상반되었다. 이는사용한약제 (fluticasone/salmeterol vs. buesonide) 와연구대 상환자의연령 (2 세전후의청년 vs. 세전후의청소년 ) 이다르 기때문이라해석할수도있지만, 충분히설득력이있는설명은되 지못한것같다. 만약치료가필요하다는견해에부응한다면, 실제치료는얼마 나오랜기간동안지속할것인가? 가시적인효과가없는상황에서 장기적인치료에환자들이과연높은순응도를보일것인지? 이러 한치료가추후의재발은과연방지할수있을것인지? 이에대한 해답은미상으로남아있으며, 추후많은연구가필요할것이다.
