Bronchial challenge with methacholine or adenosine-5 -monophosphate (AMP) has been used to diagnose asthma. Recently, measurement of exhaled nitric oxide (eno) can also be used for the diagnosis of asthma. The aim of this study was to compare the diagnostic value for asthma between challenge with methacholine or AMP and eno in children with chronic nonspecific respiratory symptoms. Methods: One hundred thirty-three children who have chronic nonspecific respiratory symptoms were enrolled. Bronchial challenge with methacholin and AMP were performed, and eno was measured in all subjects. Subjects were defined as asthma based on the clinical symptoms and bronchodilator response during follow-up of at least 3 months after test. Results: Thirty-three subjects (34%) were finally diagnosed as asthma among 97 patients after 3-month follow-up. The area under the receiver operating characteristic curves for the diagnosis of asthma were (95% confidence interval [CI], ; P< 0.001) for methacholline challenge, (95% CI, ; P< 0.001) for AMP challenge, and (95% CI, , P= 0.156) for eno measurement. The cutoff values of these tests were methacholine PC20 (provocative concentration of methacholine causing a 20% fall in forced expiratory volume in one second) 12.0 mg/ml (sensitivity, 87.9%; specificity, 82.8%), AMP PC mg/ml (sensitivity, 84.8%; specificity, 85.9%), and eno 18.5 ppb (sensitivity, 45.5%; specificity, 71.9%). Conclusion: Measurement of eno may be inferior to challenge with methacholine and AMP for the diagnosis of asthma in children. 2 윤지선외 소아천식진단에서호기산화질소와기관지유발시험의비교 Initial recruitment Chronic respiratory symptoms (cough, wheezing, dyspnea, chest discomfort) over at least 1 month (Exclusion criteria: diagnosed or treated as an asthma previously, and other diseases with chronic respiratory symptoms*) (n=133) Initial evaluations PFT, Mch, AMP, eno SPT, serum total lge, and eosinophil Subjects for the analysis Clinical asthma (n=33) vs. nonasthma (n=64) 여러단계의농도를흡입하는과정이반복되므로 30 40분의시간이소요되고, 검사도중기관지수축에의한기도폐쇄의위험성이존재한다. 6,7 호기산화질소측정은그수치가호산구성기관지염증을반영하는것으로잘알려져있고, 8-10 기관지유발시험보다방법이쉽고, 간편하며반복적으로검사가가능한장점이있어천식의중증도및항염증치료의정도를평가하는지표로쓰인다 그뿐만아니라, 폐활량계에의한기도폐쇄정도, 기관지확장제에의한기도가역성, 기관지유발검사에의한기관지과민성을평가하는다른진단방법과비교하여호기산화질소측정으로천식을진단하기위한 cutoff 수치를제시하려는노력이있었고, cutoff 수치는연구대상과방법에따라다양하게제시되고있으나, 아직까지그값이확정되지못하고있다 기침, 천명, 호흡곤란등의천식의가능성을시사하는비특이적호흡기증상이 3개월이상지속된 150명의학동기소아를대상으로한연구에서천식진단의기준으로호기산화질소의 cutoff 수치를 19 ppb로제시하였고, 이때민감도, 특이도는각각 80%, 92% 로보고하였다. 11 그러나호기산화질소는검사중호기속도에따라그결과가유동적일수있고, 바이러스감염, 천식외다른아토피질환, 비부비동염, 기관지확장증및만성폐질환에서도비특이적으로높게측정될수있다. 7,10 본연구의목적은만성비특이적호흡기증상을주소로내원한대상자들에서소아천식을진단하는데있어호기산화질소검사의진단적가치를메타콜린과 AMP를이용한기관지유발시험과비교하는것이다. 대상및방법 1. 연구시기및대상 2010년 10월부터 2014년 11월까지 1개월이상의기침, 천명, 호흡곤란, 가슴답답함등의만성호흡기증상을주소로서울아산병원소아천식아토피센터를방문한 133명의소아를대상으로폐기능검사, 메타콜린및 AMP 기관지유발시험을시행하였고, 호기산화질소를측정하였다. 기존에천식을진단받거나치료중인대상자는연구에포함시키지않았고, 대상환자들은과거력상기관지폐이형성증, 기관지확장증, 폐쇄성세기관지염 (bronchiolitis obliterans), 선천성심질환, 면역결핍질환, 악성종양, 발달장애가없었다. 이후맹검 (blind) 연구는아니었으나, 전향적으로적어도 3개월이상추적관찰하여, 임상적으로반복적인천명, 호흡곤란, 또는기침을호소하고, 폐기능검사시기관지확장제에의해 FEV 1 이 12% 이상증가하거나임상증상이호전되는경우를천식으로진단하였다. 36명의대상자는 3개월까지추적되지않고중간에탈락하였다. 최종적으로천식으로진단된대상자는 33명, 비천식군은 64명이었다 (Fig. 1). 이중천식으로진단된대상자는추적관찰하면서 National Asthma Education and Prevention Program 의 Expert Panel Report 3 guideline 에따라치료를하였다. 14 본연구는사전에부모나보호자에게조사내용에대해설명한 후동의서를받았으며서울아산병원임상연구심의위원회의승인 아래진행되었다 (IRB No.: ). 2. 연구방법 1) 폐기능검사, 메타콜린및 AMP 기관지유발시험 연구에참여한 133 명모두기저폐기능검사를시행하였고, FEV 1, 노력성폐활량 (forced vital capacity, FVC), 노력성중간호기 유량 (forced expiratory flow at 25% to 75% of FVC) 을측정하였다. 각폐기능지표는정상예측치의 % (%pred) 로표현하였다. 15 기저폐기능검사에서 FEV 1 이정상예측치의 70% 이상일때메 타콜린기관지유발시험을시행하였고, 이후 AMP 유발시험을같 은날시행하였다. AMP 유발시험은메타콜린유발시험때의기저 FEV 1 의 95% 이상으로회복될때수행하였다. 대상자모두검사시 행전최소 48 시간이내에항히스타민제제및다른약물복용이나 기관지확장제를사용하지않았고, 14 일이내에흡입용스테로이드 제를사용하지않았다. 연구에포함된대상자들은검사시행 3 주 이내상기도감염병력은없었다. 36 Subjects drop-off due to follow-up under 3 months Fig. 1. 3 Yoon J, et al. eno and BHR in the diagnosis of childhood asthma 수에녹여 0.625, 1.25, 2.5, 5, 10, 25 mg/ml을준비하였고, 각각의농도를 5분간격으로순차적으로흡입후 60 90초후 FEV 1 값을측정하였다. AMP 기관지유발시험은 AMP를 3.125, 6.25, 12.5, 50, 100, 200, 400 mg/ml 농도로준비하여각각의농도를 5분간격으로순차적으로흡입후 1분 30초에 FEV 1 값을측정하였다. FEV 1 이생리식염수흡입후수치에서 20% 이상감소할때까지또는감소하지않은경우는마지막농도에도달할때까지농도를증가시켰고, 용량반응곡선에서로그-선형보간법을이용하여 PC 20 을구하였다. 메타콜린기관지과민성의기준은 PC 20 이 16 mg/ml 미만인경우로하였고, AMP 기관지유발시험에서양성기준은 200 mg/ml 미만으로하였다. 메타콜린과 AMP 기관지유발검사에서각각의최종농도인 25 mg/ml, 400 mg/ml에도 FEV 1 이기저치의 20% 이상감소하지않은경우는각각두배값인 50 mg/ml, 800 mg/ml 를중도절단자료 (censored data) 로처리하였다. 추적관찰시기관지확장제반응측정은 1개월, 3개월때정기적으로, 그리고천식증상으로비정기적인외래방문시측정하였다. 기관지확장제반응을평가하기위해기저 FEV 1 을측정한후 400 µg (4 puff) 의살부타몰 (Ventolin Evohaler, Glaxo-Smith-Kline, London, UK) 을스페이서 (AeroChamber Plus, Trudell Medical International, St. London, ON, Canda) 를통해흡입하고 15분후 FEV 1 을재측정하였다. 기관지확장제흡입전후의 FEV 1 변화를기관지확장제흡입전 FEV 1 값에대한비율로나타낸것을기관지확장제반응으로정의하였다. 2) 호기산화질소측정호기산화질소수치는 Niox Mino (Aerocrine, Solna, Sweden) 를이용하여측정하였다. 연구에포함된대상자들은각각산화질소가없는공기를총폐용적 (total lung capacity) 까지흡입한후 50 ml/sec의속도로지속적인호기를시행하도록하였다. 호기시간은적어도 6초이상이되었을때산화질소농도를측정하였다. 총세번측정하여그평균값을호기산화질소수치로이용하였다. 3) 혈액내호산구분율, 혈청총 IgE 및피부단자시험혈액내호산구분율은 automated blood analyzer를사용해측정하였고, 혈청총 IgE 수치는 UniCAP system (Phadia AB, Uppsala, Sweden) 을이용하여측정하였다. 피부단자시험은집먼지진드기 (Dermatophagoides pteronyssinus and Dermatophagoides farinae), 진균 (Alternaria, Aspergillus, Cladosporium, and Penicillium), 화분 (grass, trees, weeds, ragweed, mugwort, oak, beech, nettle, willow, elm, pine, hop, elder, hazel, oats, lambs, quarter, ash, alder, birch, timothy, and rye grass), 바퀴벌레, 개및고양이털을포함하는주요흡입항원을사용하여표준화된방법에따라대상자등에점적하여시행하였고히스타민과생리식염수를각각양성 대조, 음성대조로사용하였다. 19 팽진장경과단경을측정하여평균팽진의크기를결과로사용하였고, 각항원에대해팽진이 3 mm 이상이면서양성대조군보다큰경우를양성으로판정하였다. 20 한개이상의항원에양성반응을보이는경우를아토피라정의하였다. 3. 통계분석자료는 SPSS ver (SPSS Inc., Chicago, IL, USA) 통계프로그램을이용하여분석하였다. 각변수는일반적으로평균 ± 표준편차로제시하였고, 혈액내호산구분율, 혈청총 IgE, 호기산화질소농도, 메타콜린및 AMP PC 20 은로그화하여분석하였고, 결괏값은기하평균 (1 표준편차범위 ) 으로표현하였다. 천식및비천식군간의폐기능, 메타콜린및 AMP PC 20, 호기산화질소, 혈액내호산구분율, 혈청 total IgE 수치의비교는 t-test를이용하였고, 메타콜린및 AMP 유발시험과호기산화질소측정의천식진단에대한진단적가치비교를위해서 receiver operation characteristic curve (ROC) 곡선을이용하여 cutoff 수치와민감도및특이도를구하였다. 유의수준은 P 값이 0.05 미만일때통계적으로유의한것으로정하였다. 결과 1. 연구대상의임상적특성총 133명의대상자중 3개월미만의추적기간으로탈락한대상자를제외하고최종적으로연구에포함된 97명과제외된 36명의특징을비교한결과, 제외된군의나이는 10.0±1.4 세, 연구에포함된군의나이는 7.8±1.4 세로유의하게높음으로써키, 몸무게도두군간유의한차이를보였다. 그외혈청총 IgE, 혈액내호산구분율, 폐기능검사및호기산화질소는두군간차이가없었으나, 메타콜린및 AMP PC 20 수치는제외군에서유의하게높음을보였다 (data not shown). 본연구에서분석에포함된대상자 97명의특성은 Table 1과같다. 대상자들이호소하는주요만성비특이적호흡기증상으로는기침단독 (n= 48, 49.5%), 가슴답답함단독 (n= 7, 0.07%), 천명단독 (n= 6, 6.2%), 호흡곤란단독 (n= 4, 0.04%), 심호흡단독 (n= 2, 0.02%), 흉통단독 (n= 2, 0.02%), 기침및천명 (n=18, 18.6%), 기침및호흡곤란 (n= 2, 0.02%), 기침및천명및호흡곤란 (n= 2, 0.02%) 등이었다. 천식군과비천식군모두성별의차이는없었고, 천식군의나이가비천식군보다유의하게어렸고, 이에따라키, 몸무게모두유의하게낮은수치를보였다. 체질량지수또한천식군에서유의하게낮은수치를보였다. 혈액내호산구분율은천식군에서 3.6% ( 범위, 1.5% 8.5%), 비천식군에서 2.6% ( 범위, 1.1% 6.2%) 에비해높았으나통계적으로유의하지않은반면, 혈청총 IgE는천식군에서 102

FEV1, forced expiratory volume in one second; %pred, percentage predicted; FVC, forced vital capacity; Mch, methacholine; PC20, provocative concentration causing a 20% fall in the FEV1; AMP, adenosine-5 -monophosphate; eno, exhaled nitric oxide. **P<0.001, compared to nonasthma group ku/l ( 범위, ku/l), 비천식군에서 71.3 ku/l ( 범위, ku/l) 으로, 천식군에서유의하게높았다 (P < 0.001). 천 식군에서는아토피가 84.8% 로비천식군의 44.8% 보다유의하게높 았으며, 아토피피부염은천식군에서 72.7%, 비천식군에서 12.5% 의 유병률을보여두군간유의한차이를보였다 (P<0.001) (Table 1). 2. 천식군과비천식군에서폐기능, 메타콜린및 AMP PC 20, 호기산화질소의비교 폐기능검사결과 FEV 1 %pred, FVC %pred 및 FEV 1/FVC 수치는 천식과비천식군에서유의한차이를보이지않았다 (Table 2). 천식 군의메타콜린 PC 20 은기하평균 3.8 mg/ml (1 표준편차범위, mg/ml) 로비천식군의 30.9 mg/ml ( 범위, mg/ml) 보다유의하게낮았으며 (P<0.001), 마찬가지로메타콜린 PC Table 3. PC20, provocative concentration causing a 20% fall in the forced expiratory volume in one second decrease; AUC, area under curve; CI, confidence interval. mg/ml 이하를양성으로할때천식군의양성분율은 90.9% (n= 30) 로비천식군 20.3% (n=13) 보다유의하게높았다 (P<0.001) (Table 2). 천식군의 AMP PC 20 또한 68.1 mg/ml ( 범위, mg/ml) 로대조군의 mg/ml ( 범위, ,422.1 mg/ml) 와 유의한차이를보였고 (P<0.001), AMP PC mg/dl 이하를양 성으로할때천식군의양성분율은 81.8% (n = 27), 비천식군은 12.5% (n= 8) 로유의한차이를보였다 (P<0.001) (Table 2). 그러나호기산화질소측정값은천식군에서 17.0 ppb ( 범위, ppb), 비천식군에서 13.8 ppb ( 범위, ) 로통계적으로유 의한차이를보이지않았다 (Table 2). 3. 메타콜린및 AMP 기관지유발시험과호기산화질소측정의 천식진단적가치비교 연구대상에서메타콜린및 AMP 기관지유발시험과호기산화질 소측정의천식진단에대한 ROC 곡선분석을시행하였을때, 메타 콜린 PC 20 이 12.0 mg/ml 이하일때민감도 87.9%, 특이도 82.8%, ROC 곡선하면적 (95% confidence interval [CI], ; P<0.001) 이었다 (Table 3, Fig. 2). AMP PC 20 은 mg/ml 이하 일때민감도 84.8%, 특이도 85.9% 였으며, ROC 곡선하면적은 (95% CI, ; P < 0.001) 이었다 (Table 3, Fig. 3)

AMP, adenosine-5 -monophosphate; PC20, provocative concentration causing a 20% fall in the forced expiratory volume in one second decrease; AUC, area under curve; CI, confidence interval. 반면에호기산화질소측정값은 18.5 ppb 보다높을때민감도 45.5%, 특이도 71.9%, ROC 곡선하면적은 (95% CI, ) 로, 메타콜린및 AMP PC 20 과비교하여천식을예측하는데 진단적정확도가낮았다 (Table 3, Fig. 4). 고찰 만성비특이적호흡기증상을호소하는소아를대상으로전향 적으로추적관찰하여천식을진단하였을때, 메타콜린및 AMP 기관지유발시험과호기산화질소측정의민감도, 특이도를비교해 보았을때, 메타콜린, AMP PC 20 에비해호기산화질소가천식진단 을예측하는데있어상대적으로진단적정확도가낮음을보였다. 소아에서천식진단에대해서상기세가지검사를비교한연구 는없는가운데, Berkman 등 9 이비특이적호흡기증상을호소하는 성인을대상으로 2 년간추적관찰하여천식진단의 cutoff 수치로 제시한호기산화질소수치는 7 ppb 이상일때, 민감도 82.5%, 특이 도 88.9% 였다. 함께시행한기관지유발시험결과, 메타콜린과 AMP PC 20 의 cutoff 수치및민감도, 특이도는각각메타콜린 PC 20 3 mg/ml (sensitivity, 87.5%; specificity, 86.7%); AMP PC mg/ml (sensitivity, 89.5%; specificity, 95.6%) 로, 호기산화질소가 천식진단을예측하는데다른두검사에상응하는유용한비침습 적도구임을제시하였다. 이결과는본연구와는상반되는결과로, 이는연구에참여한대상의차이때문일수있고, 호기산화질소가 아토피가있는경우호흡기증상이없더라도높게나타날수있다 는점을고려했을때, 13 본연구의비천식군에서아토피양성및아 토피피부염과알레르기비염대상들이포함되어있는것이결과에 영향을미쳤을가능성이있다. 또한 Shim 등 6 은아토피천식군에 Fig. 4. AUC, area under curve; CI, confidence interval. 서대조군과비교하여호기산화질소가증가한반면, 비아토피천 식군에서는메타콜린에대한기관지과민성은있으나, 호기산화질 소는정상대조군과유의한차이가없음을보여주어소아에서비 아토피천식의기전은기도염증과관련이없거나, 또는호기산화 질소에반영되지않는염증에의할수있음을제시하였다. 이와같 은맥락으로, 119 명의 세경증및중등증천식대상자를아토 피정도에따라대상군을나누어시행한호기산화질소수치는흡 입항원감작정도에따라다름을보고한연구가있었다. 흡입항원 비감작군의호기산화질소수치는 18.6 ppb, 한가지흡입항원에감 작된군은 28.8 ppb, 두가지이상에서감작된군은 44.7 ppb 로아토 피정도가강할수록호기산화질소가증가하는양상을보였다. 21 이 번연구에서도비아토피천식 (15.2%) 이천식군에포함되어있으므 로호기산화질소의천식에대한진단적가치에영향을미쳤을가 능성이있다. 또한본연구에서는비천식군에서나이, 키, 몸무게가 천식군보다유의하게높았으므로, 호기산화질소가나이가많을수 록, 키가클수록수치가유의하게높아짐을고려했을때, 10,22 호기산 화질소의진단적가치에영향을미쳤을가능성이있다. 소아를대상으로한호기산화질소의천식진단에관한연구로 Sivan 등 11 이기침, 천명, 숨참등의비특이적호흡기증상을적어도 3 개월이상호소한학동기대상자를 1 년간추적관찰한결과, 천식 에대한진단 cutoff 수치는 19 ppb 로, 민감도 80%, 특이도 92% 를 보여, 호기산화질소가천식의진단에가치가있음을제시하였다. 그러나호기산화질소가천식진단에한계가있음을제시한연구 또한존재하는데, Henriksen 등 23 은 8,571 명의노르웨이소아에서 151 명의천식군과 213 명의대조군을선정하여호기산화질소를측 정하였고, 메타콜린기관지유발시험을시행하였다. 메타콜린기관 지유발시험결과, 천식군에서메타콜린 PD 20 cutoff 수치를 2 mg 으 로했을때, 천식군에서 134 명 (75%), 대조군에서는 46 명 (21.6%) 이 104

6 윤지선외 소아천식진단에서호기산화질소와기관지유발시험의비교 기관지과민성양성을보였다. 이에비해호기산화질소 8 ppm을양성으로간주했을때, 천식군에서는 52% 가, 대조군에서는 20% 가양성을보여메타콜린기관지유발시험에비해천식진단의민감도가낮음을보였고, 메타콜린기관지유발시험과호기산화질소측정을함께시행해위의기준을만족시킬때천식을진단하는데유용한도구가될수있음을제시하였다. 그리고천식군을아토피와비아토피로나누어호기산화질소수치를비교하였을때, 각각의호기산화질소기하평균값이 11.7 ppb, 5.6 ppb로유의한차이가있어호기산화질소가아토피에영향을받음을보여주었다. 본연구의천식군도앞에서언급한것처럼아토피뿐만아니라비아토피천식군이포함되어있으며, 더나아가서소아천식은균일한집단이라기보다는다양한천식의표현형이혼재되어있어, 호기산화질소가메타콜린및 AMP 기관지유발시험보다진단적가치가낮았을가능성이있다. 본연구결과에서진단적가치가낮게나왔지만호기산화질소측정은천식에서기관지염증상태및항염증약물치료반응을모니터하는데있어서여전히유용하다. 10,24 소아에서천식을진단하기위해일반적으로사용하는메타콜린기관지유발시험의천식진단에대한 cutoff 수치를제시한연구들을살펴보면, 각연구에서제시하는 cutoff 수치는다양하다. Carlsten 등 25 이 348명의 7세소아를대상으로한연구에서는메타콜린 PC 20 cutoff 수치는 3 mg/ml, 민감도 80%, 특이도 49.1% 로보고하였다. Liem 등 26 의연구에서는 8세소아를대상으로메타콜린기관지유발시험을시행한결과, cutoff 수치 4 mg/ml, 민감도 66.1%, 특이도 64.0% 로보고하였다. 본연구에서제시한메타콜린 cutoff 수치인 14.9 mg/ml는위두연구에서보다높지만, 민감도 87.9%, 특이도 82.8% 로두수치모두높게나타났다. 이러한결과의차이는대상군과진단기준의차이에기인할수있다. 상기두연구모두코호트를기반으로하여일반인구집단을반영하는반면에, 본연구는외래에서흔하게접할수있는만성호흡기증상이있는대상자로제한하였다. 또한상기두연구는천식진단기준이의사진단또는 International Study of Asthma and Allergies in Childhood 설문지에기반을두고있는반면에, 본연구는적어도 3개월간추적하여기관지확장제반응을기반으로두고있다. 기관지유발시험중간접자극제로기관지과민성을보여주는 AMP는직접유발시험인메타콜린과는다른기전으로기관지수축을유발하는데, AMP에의한기관지과민성은메타콜린기관지과민성과비교하여기도염증과더관련이있고, 비아토피천식보다는아토피천식에서기관지과민성이더높게나타나는것으로알려져있다. 6,27 Avital 등 1 은 133명의 6 25세대상자에게 AMP 기관지유발시험을시행하여 AMP PC 20 <200 mg/ml일때민감도 95.5% 임을제시하였고, 동시에시행한메타콜린유발시험결과 PC 20 8 mg/ml일때, 민감도 98% 에상응하는천식진단에유용한도구임을제시하였다. 본연구에서는 AMP 기관지유발시험은 AMP PC mg/ml 이하일때민감도 84.8%, 특이도 85.9% 로 메타콜린 PC 20 의민감도와특이도에상응하는결과를보였다. 그 러나 cutoff 수치가 mg/ml 로마지막농도를넘어실제임상 에적용하기어려운점이발견되었다. 이는 AMP 에반응하지않는 비아토피천식이천식군에포함되어있어 cutoff 수치가높아졌을 가능성이있다. 본결과에제시하지는않았지만, 비아토피군에서 AMP PC 20 <200 mg/ml 의분율은 10% 로미만으로낮기때문이 다. Suh 등 28 이 122 명의천식소아를대상으로메타콜린및 AMP 기 관지유발시험을시행한결과, 아토피천식에서비아토피천식에비 해 AMP PC 20 가유의하게감소하였고, 메타콜린 PC 20 은유의한차 이를보이지않았다. AMP 기관지유발시험은동일한날메타콜린기관지유발시험후 시행하였다. 동일한날두유발시험이시행되었으므로메타콜린기 관지유발시험이 AMP 기관지유발시험의결과에영향을미쳤을 가능성을완전히배제할수는없다. 그러나 AMP 기관지유발시험 은메타콜린기관지유발시험후기도폐쇄의증상이없고 FEV 1 의 수치가메타콜린유발시험때의기저 FEV 1 의 5% 이내로회복될때 시행하였다. 또한실제두유발시험의기저 FEV 1 수치는유의한차 이가없었다 (data not shown). 본연구의한계점으로는만성비특이적호흡기증상을가지고 3 차병원을내원한소아를대상으로하였고, 최종분석에사용된대 상수가 97 명으로비교적많지않은숫자이므로연구의결과를일 반인구집단에적용하기는어렵다. 그러나만성비특이적호흡기 증상을보이는대상군이실제천식을진단하기위한검사를필요 로하는집단이라는점에서연구의임상적의미가있다. 다른한계 점으로 3 개월간의추적은임상적으로천식을진단하기충분하지 않은기간일수있다. 또한진단하는과정이맹검법으로이루어지 지않았으므로연구자가이미시행한검사결과에영향을받는오 류를배제할수없다. 결론적으로본연구의결과는만성비특이적호흡기증상을가 진소아에서호기산화질소측정은메타콜린및 AMP 기관지유발 시험과비교하였을때천식에대한진단적정확도가낮을수있음 을제시한다.
