1 VI FEV 1 Decline in Asthma 이진국 가톨릭대학교서울성모병원호흡기내과 Although asthma is generally considered as mild and reversible disease, decline of FEV 1 is observed in some patients according to previous studies. Decline of FEV 1 is significantly higher in asthma than normal control. Smoking is the most important factor to contribute decline of FEV 1 in asthma patients. Severe asthma in children, difficult to treat asthma, and life-long asthma are also important risk factors for decline of FEV 1. These factors should be corrected early in asthma patients to prevent COPD in later life. Key Words: Asthma, FEV 1, Decline Corresponding author: Chin Kook Rhee, M.D., Ph.D. Division of Respiratory, Allergy and Critical Care Medicine, Department of Internal Medicine, The Catholic University of Korea, Seoul St Mary s Hospital, 222, Banpo-daero, Seocho-gu, Seoul , Korea Tel: , Fax: , chinkook@catholic.ac.kr 1. 서론 천식은가역적인기도폐쇄를동반하는질환으로, 대부분치료에잘반응하여호전되는질환이다. 따라서많은의사들은천식이가벼운병이고, 일시적인병이며, 후유증이남지않는병으로오인하는경우가많다. 하지만시간을길게두고생각하여보면, 천식이항상경한질환만은아니다. 일시적인치료로호전되는듯보이지만재발되는경우가흔하고, 가역적인기도폐쇄가시간이지남에따라비가역적으로변하는경우도있다. 또한오랜기간동안천식을앓게되면궁극적으로는폐기능장애라는후유증이동반되기도한다. 따라서천식환자들의폐기능이어떻게감소되는지및그와관련된인자가무엇인지를알아보는게매우중요하다고하겠다. 따라서본 review 에서는천식환자의 FEV 1 decline과관련된문헌들을고찰해보았다. 2. Asthma vs. Non-asthma Rural community 의 1,303명거주민을대상으로 7년간추적관찰한연구에따르면, 정상인에비해천식이있는환자가폐기능이더많이감소되었다 1. 남자에서정상인은 6년간 37.8 ml의 FEV 1 저하가있던반면, 천식환자에서 ml의감소가있었다. 여성에서도역시정상인 36.0 ml에비해천식환자 ml로큰차이를보였다 (Figure 1). 또한이연구결과에따르면, 천식이없던환자가 6년뒤에도천식이없을경우폐기능저하가거의없었고, 천식이있었더라도 6년뒤천식이사라진환자에서도폐기능저하가심하지않았다. 반면천식이없다가 6년뒤천식이있는경우많은폐기능감소가관찰되었고, 천식이처음부터 6년뒤까지있던경우가장심한폐기능감소가관찰되었다. 따라서이런사실로보아천식자체가폐기능저하에큰영향을미치는것을알수있으며, 더나아가현재증상이있는 active 천식이폐기능저하에중요한요소임을유추해볼수있다. 107

2 3. Smoking Asthma vs. Non-smoking asthma 흡연은그자체만으로폐기능을감소시킴이잘알려져있다. 만약천식환자가흡연을하게되면이효과는배가되어더많은폐기능감소를야기하게된다. Busselton Health Study 2 연구결과에의하면정상여성, 비흡연천식여성, 흡연천식여성의경우 FEV 1 decline 이각각 24.5 ml/yr, 28.4 ml/yr, 35.7 ml/yr 였다. 남성에서도역시같은결과가관찰되었다. 연구자들은시간에따른 FEV 1 decline 을천식및흡연유무에따라각각계산하여제시하였다 (Figure 2). 한가지흥미로운사실은천식환자의경우소아때이미폐기능장애가발생되어 20대의 maximal lung function 도달점이낮다는것이다. 따라서이런환자들이흡연을하게되면궁극적으로매우낮은 FEV 1 값을가지게된다. Copenhagen City Heart Study 3 에서도역시같은결과가관찰되었다. 40대남성을 15년간추적관찰한결과정상인 24.2 ml/yr에비해비흡연천식환자 32.8 ml/yr, 흡연천식환자 57.6 ml/yr로 FEV 1 이크게감소되는것을알수있었다. 4. 3 한다. 젊은나이의천식이 COPD 의 risk factor 가될수있음은이미잘알려져있는사실이다. 그렇다면어떤요소를가지고있는천식환자가 COPD 로진행을하게되는것일까? Vonk 등 4 은 21 33세의천식병력이있는 228명의환자를 enroll 한후 23년뒤폐기능검사를다시시행하였다. 그결과 16% 의환자가 23년뒤비가역적인기도폐쇄소견을보였고, 23% 가 DLCO/VA 의감소소견을보였다. 비가역적기도폐쇄를일으키는 risk factor 로써는낮은폐기능및스테로이드치료가필요했던경우등이었다. 9세때부터 26세때까지총 7번의폐기능검사를시행하며추적관찰한 population study 5 에서, 천식환자들은크게세군으로구분되었다. 처음부터마지막까지 FEV 1 /FVC 값이정상으로유지된군과, 처음에는정상이었다가후에감소되는경우와, 처음부터끝까지감소된경우로환자들이구분되었다. FEV 1 /FVC값이처음부터끝까지정상으로유지된군에비해나머지두군은다음과같은 risk factor 들을가지고있었다 - 9세때천식이있는경우, 남성, 기도과민성이있는경우, 9세때 FEV 1 값이낮았던경우. 한가지흥미로운사실은 FEV 1 /FVC 값이처음부터마지막까지낮은환자군은 26세때이미 FEV 1 /FVC가 70% 미만이라는점이다 (Figure 3). 따라서이런사실을바탕으로보아소아때의천식상태가환자의평생폐기능에미치는영향이매우크다고하겠다. 최근 Tai 등 6 은소아때천식상태와 COPD 와의연관성에대한중요한연구결과를발표하였다. 연구자들은 6 7세의천식환자를 enroll 하여상태를구분한후 7년마다정기적으로 FU 한후환자가 50세가되었을때 COPD 여부를조사하였다. 소아때 mild wheezy bronchitis 였던경우 50세때 COPD 인환자의비율은 4% 에불과하였으나, 천식이었던경우 16% 에서 COPD 가발생하였다. 소아때중증천식인경우에는무려 44% 가 50세때 COPD 가되어있었다 (Figure 4). 다변량분석에서소아때천식은성인 COPD 에대한 odd ratio가 9.6이었고, 소아때중증천식은성인 Figure 3. COPD 에대한 odd ratio가 31.9였다. 이는소아때천식유무및중증도가성인때 COPD 발생에얼마나큰영향을주는지를매우잘보여주는귀한자료라고하겠다. 흡연은 COPD 를일으키는중요한인자로잘알려져있고, 천식역시위에기술된연구에서처럼 COPD 를일으키는중요한인자이다. 그럼천식의 COPD 에대한 contribution 정도를 smoking 양으로비교계산해볼수있을까? 이에대한답은 2013년발표된연구 7 에서명쾌히얻을수있다. 1968년 enroll 된 8,583명의 school children을 2008년까지추적관찰한연구에따르면, 전체대상자의약 6% 에서 COPD 가발생되었다. 연구자들은천식이 COPD 발생에기여하는정도와흡연이기여하는정도를통계적인방법으로비교분석하였다. 그결과, early-onset current clinical asthma 의경우 33갑년흡연과동일한 effect 를보였고, late-onset current clinical asthma 의경우 24갑년흡연과동일한 effect 를보였다. 따라서이결과를통하여천식이있고계속증상이있어온비흡연자의경우에천식이없는수십갑년흡연자와동일한정도의 COPD 발생위험이있다는것을쉽게가늠해볼수있겠다. 이상의연구결과들을종합해보면, 천식환자는일반인에비해폐기능저하속도가빠르고, 흡연을하는경우기능저하가더빨리진행된다. 소아천식환자들은정상인에비해소아때이미폐기능저하가발생되어 maximum lung function 에도달하지못하게된다. 이런사실들을종합하여모식적으로표현해보면 Figure 5와같은결론을얻을수있게된다. 위에기술된여러연구이외에도천식환자의폐기능저하에기여하는요소들은많은연구를통해밝혀져왔다. 지금까지기술된요소들말고도 blood eosinophilia, β2 reversibility, chronic mucus hypersecretion 8, high FENO 9, aspirin sensitivity 10, GLCC I1 variant 11, severe exacerbation 12 등이천식환자의폐기능저하에기여하는요소로보고되었다. 5. 결론 천식환자는일반인에비해폐기능저하속도가빠르고, 흡연, 소아천식, 중증천식등이동반되게되면더많은폐기능저하가관찰된다. 천식환자에게서이런위험인자들을조기에발견하여교정해주는것이추후 COPD 발생예방을위해매우중요하다고하겠다.
