1 대한내과학회지 : 제 87 권제 2 호 의학강좌 - 개원의를위한모범처방 (Current Clinical Practice) 천식치료의최신지견 가톨릭대학교의과대학호흡기알레르기내과 권순석 New Guidelines for Asthma Management Soon Seog Kwon Division of Allergy and Respiratory Medicine, Department of Internal Medicine, Catholic University of Korea College of Medicine, Seoul, Korea Asthma is a chronic airway inflammatory disease that is characterized by symptoms such as shortness of breath, wheezing, chest tightness, and nighttime or early morning coughing, together with variable expiratory airflow limitation. Asthma is a heterogeneous disease involving various underlying processes that have been described based on causal or exacerbating factors, the pattern of airflow obstruction, severity of disease, pattern of cellular inflammation, and structural changes among patients with asthma. However, there is considerable overlap among these clinical variants, which are not correlated strongly with specific pathological processes or treatment responses. The Global Strategy for Asthma Management and Prevention 2014 (GINNA 2014) emphasizes the importance of confirming the diagnosis to minimize both under- and over-treatment with respect to the heterogeneity of asthma. To assess asthma control, it is necessary to assess not only the symptom control but also the future risks of an adverse outcome. Lung function is the most used indicator of future risk. This article reviews the treatment of adult asthma, including patients already receiving treatment according to GINA 2014 and the Health Insurance Review and Assessment Service. (Korean J Med 2014;87: ) Keywords: Asthma; Heterogeneity; Pulmonary function test 서론기관지천식은만성기도염증질환이면서호흡곤란, 천명음, 가슴답답함과야간이나새벽에발생되는기침등의호흡기증상이하루중에도변동이다양한양상으로나타난다 [1]. 최근에는원인, 기도장애정도, 염증세포의구성및구조적 으로다른여러가지아형으로천식이분류되고있다 [2]. 그러므로임상적으로다른양상을보이는천식들을시기에맞게적절히치료함으로써천식의악화도예방하고약의부작용을막을수있다. 2014년 5월에개정된 GINA 지침 [3] 에근거하여증례중심으로살펴보고보험심사기준도소개하고자한다. New guideline for asthma management - 증례 1: 46세여자환자가알레르기성비염으로 15년전부터간헐적으로치료를받아왔다. 한달전부터천명음과호흡곤란이생겨서일상생활이불편하여내원하였다. 밤에는호흡곤란이없었다고한다. 1주일전부터흡입용스테로이드 (inhaled glucocorticosteroid, ICS) 를하루에두번씩사용을하고있으며, 호흡곤란이심하면흡입속효성베타2 항진제 (short acting beta2 agonist, SABA) 를하고있다고한다. 폐기능검사상, FEV 1 = 56% predicted value, bronchodilator response = 7% 였다. 치료의원칙은? 배경 : 일차의료에서기관지천식을진단된환자의 25-35% 정도에서다양한기도제한의정도를객관적으로평가하지않고임상증상만으로치료를하는경우들이많다 [3]. 그러므로이런환자들에대해서는기관지천식에대한정확한진단이요구된다. 질병조절제 (controller) 를사용하고있는환자에서기관지천식의확진 1. 변동이심한호흡기증상과기도장애 (variable respiratory symptoms and variable airflow limitation) 가있는경우천식으로진단할수있으며증상조절정도를평가하여질병조절제를선택하여사용하게한다. 2. 변동이심한호흡기증상은있으나가변적인기도장애가없는경우 SABA 는 4시간, 지속성베타2 항진제 (long-acting β 2 agonist, LABA) 는 12시간정도중단한다음기관지확장제가역성 (reversibility) 을다시측정하여가역성은없으면서호흡기증상이지속되는경우는다른진단을고려해본다. 1) FEV 1 이예측치의 70% 이상이면기관지유발검사를시행한다. 만일음성이면기존에사용하고있는질병조절제의단계를한단계낮추어사용하고 2-4주후에다시재평가한다. 2) FEV 1 이예측치의 70% 미만이면질병조절제의단계를한단계올려서 3개월정도사용한다음호흡기증상과폐기능을다시평가한다. 만일증상호전이없는경우에는이전단계로낮추어치료를하면서다른질환을찾아보아야한다. 3. 호흡기증상이미약하면서폐기능이정상이고, 기도가역성도없는경우 SABA 는 4시간, LABA 는 12시간정도중단한다음기관지확장제가역성을다시측정하여가역성이정상임에도불구하고호흡기증상은있는경우에는질병조절제단계를한단계를낮추면서다른진단을찾아본다. 1) 한단계를낮춘다음호흡기증상이발생되면서폐기능이감소하면질병조절제단계를이전에효과가있는가장낮은단계의질병조절제로단계를올려치료를한다. 2) 가장낮은단계의질병조절제를사용한후에도호흡기증상이없으면서폐기능도정상이면질병조절제를중단을할수도있지만반드시 12개월이상추적검사가필요하다. 4. 지속적인호흡곤란이있으면서비가역적인기도장애가있는경우질병조절제를한단계올려서 3개월동안사용한다음다시호흡기증상및폐기능을평가한다. 만일반응이없을경우에는이전치료단계로유지하면서다른질환즉, asthma- COPD overlap syndrome의가능성을고려하여야한다. 답변 1: 기존의치료에도불구하고 FEV 1 이예측치의 70% 미만이므로질병조절제를한단계올려서저용량의 ICS와 LABA 를함께 1 puff bid로사용하면서 SABA 를증세완하제로사용하거나 ICS/formoterol 경우에는완화제로사용할수있다. 치료하는 3개월동안기침, 천명음, 호흡곤란의증세를관찰하고폐기능검사를추적검사를시행하여호전여부를확인한다. 증례 2: 35세남자환자가 3개월동안저용량 ICS와 LABA 로 1 puff bid로치료하는동안호흡곤란이나일상생활에불편함이없었으며 SABA 는한달에한번이하로사용하였다. 과거작년겨울에천식악화로두차례응급실로내원하여치료받은적이있다고한다. 폐기능검사상 FEV 1 이예측치의 88% 이면서기관지확장제반응이 2% 였다. 환자는그만사용하고싶어한다면? 배경 : 조절상태유지치료천식이잘조절되면천식조절상태를유지하면서비용을최소로하고안전성을최대로하는가장낮은치료단계및용량을정하기위해서지속적인모니터링이필수적이다. 일반적으로환자는첫방문후 1개월후추적하고천식이조절되면 3개월마다방문을권장한다. 급성악화가있을경우에

3 - 대한내과학회지 : 제 87 권제 2 호통권제 648 호 는 1주이내반드시다시병원을방문하여야한다. 5. 천식조절천식조절은질환의예방또는완치를의미하는것이아니라질환발현의조절을의미한다 [4,5]. 천식의조절상태를평가를하는데는두가지구성요소가있으며호흡기증상 ( 주간증상, 야간증상으로인한수면장애, 증상완화제사용횟수와일상활동제한 ) 의정도를나타내는증상조절과지난한해동안한번이상의급성악화, 흡입기순응도미숙및사용미숙, 폐기능저하, 호산구성객담, 흡연과같은미래위험요소 (future risk) 를같이고려하여치료하는것이중요하다고강조하고있다 [3]. 천식증상은천명음, 호흡곤란, 답답함과기침으로발생시간과정도가지속적이기보다는변동이많다. 이러한증상이잘조절되지않으면천식의급성악화로이어질가능성이매우높다 [6]. 1) 증상조절증상조절을평가하기위해서는지난 4주동안 1주일에며칠정도증상이있었는지, 천식증상으로수면장애나일상활동에제한이있었는지, 증상완화제를얼마나사용하였는지를알아야한다. 이러한증상들이두가지이내로평가되면부분조절이며세가지이상이동반되면조절안됨으로평가한다 (Table 1). 2) 나쁜예후를예상할수있는미래위험요소천식조절을평가하는두번째구성요소에는나쁜예후를예상할수있는위험요소인급성악화, 비가역적인기도제한이나약물부작용등을미리파악하는것이다. 이러한미래위험요소를가지고있는환자들이천식증상이본인들이인식하기에는문제가없는것으로평가해버리는것이문제가된다. 예를들면기도염증을전혀치료하지않고부적절하게 LABA 만을사용하여증상이조절된다고생각하는경 우, 또는폐기능이감소되어있으나증상이별로없는경우들이이에해당된다 (Table 2). (1) 급성악화증상조절평가에서조절안됨은급성악화로진행될가능성이매우높다. 또한미래위험요소인지난 1년이내에한번이상급성악화가있거나치료순응도가떨어지는경우, 흡입기사용이미숙하거나흡연을하는환자는급성악화의위험이매우높다. (2) 비가역적인기도장애천식환자에서점차적으로폐기능이감소되면기도장애가완전히회복되지를않는다. 흡연을하거나만성적으로가래가배출되거나또는 ICS를사용하지않아서급성악화가반복되면비가역적인기도장애가발생되어지속적호흡곤란을호소하게된다. (3) 약물부작용고농도 ICS를오래사용하면멍이잘들고골다골증, 백내장, 녹내장과부신기능저하가동반된다. 흡입기사용미숙으로국소적으로는구강칸디다증, 목소리이상이발생될수가있다. 6. 폐기능검사낮은 FEV 1 은천식증상과상관없이급성악화의중요한위험인자이며폐기능과천식증상이꼭일치하지는않기때문에천식조절을평가하는데에필요하다. 질병조절제를사용하면폐기능이점차호전되어약 2개월후정상에가깝게도달하게된다. 폐기능검사는진단당시뿐만아니라치료를시작하면서 3-6개월간격으로반드시시행하여야한다. 1) 추적폐기능검사에서 FEV 1 이정상이하로나오는경우 (1) 특히 FEV 1 < 60% predicted 이면증상조절정도와상관없이급성악화의위험이매우높다 [7]. Measure FEV 1 at start of treatment, after 3-6 months of controller treatment to record the patient's personal best lung function, then periodically for ongoing risk assessment. Potentially modifiable independent risk factors for flare-ups (exacerbations) Uncontrolled asthma symptoms [7] Excessive SABA use (> dose canister/month) [8] Inadequate ICS: not prescribed ICS; poor adherence [9], incorrect inhaler technique [10] Low FEV 1, especially if < 60% predicted [11] Major psychological or socioeconomic problens [12] Exposures: smoking [11]; allergen exposure if sensitized [12] Comorbidities: obesity [13]; rhinosinusitis [14]; confirmed food allergy [15] Sputum or blood eosinophilia [16] Pregnancy [17] Other major independent risk factors for flare-ups (exacerbations) Ever intubated or in intensive care unit for asthma [18] 1 severe exacerbation in last 12 months [19] Risk factors for developing fixed airflow limitation lack of ICS treatment [20] Exposures: tobacco smoke [21]; noxious chemicals; occupational exposures Low initial FEV 1 [22]; chronic mucus hyperseveretion [22]; sputum or blood eosinophilia [22] Risk factors for medication side-effects Systemic: frequent OCS; long-term, high dose and/or potent ICS; also taking P450 inhibitors [23] Local: high-dose or potent ICS [23]; poor inhaler technique [24] ICS, inhaled glucocorticosteroid; OCS, oral corticosteroid. Having one or more of these risk factors increases the risk of exacerbations even if symptoms are well controlled (2) 증상조절정도와상관없이점차적으로폐기능이감소될가능성이있다 [22]. (3) 일상활동장애같은증상이조금이라도있으면기도염증이치료가되지않고있음을시사한다 [25]. 2) 천식증상은있으나 FEV 1 이정상이상인경우다른원인질환즉, 심장질환, 후비루증후군또는역류성식도염등을감별하여야한다. 3) 지속적인기관지확장제가역성이있은경우질병조절제를복용하거나 SABA 흡입후 4시간이내또는 LABA 흡입후 12시간이내에도불구하고기관지확장제흡입후 FEV 1 > 12% 이면서 > 200 ml로나타나면천식조절이안되는상태를말한다. 7. 천식치료보험심사기준부분조절단계이상의기관지천식에서는 ICS/LABA, ICS, 항류코트리엔제제, SABA 의사용을인정하고있으며반드시 3-6개월간격으로폐기능검사를비롯한천식조절정도를평가하고기록하여야한다. 기관지천식에투여되는흡입제중분무용용액 (nebulizer solution) 은노인, 소아, 안면마비환자, 의식불명환자등일반적인흡입제사용이곤란한경우나응급치료시에만요양급여를인정하며, 동인정기준이외에는약값전액을환자가부담토록한다. 8. 천식치료단계낮추기의실제증상조절이잘되며, 폐기능이정상인상태로 3개월이상유지되는경우단계를낮출수가있다. 하지만급성악화나비가역적인기도제한의위험요소가있는경우에는세밀한관찰후에낮추는것을고려하여야한다. 원칙적으로는 3 개월마다 ICS를 25-50% 감량하여조절을하거나다른조절제를우선적으로중단하고최소용량의 ICS로유지한다. 단

5 - The Korean Journal of Medicine: Vol. 87, No. 2, 계를낮춘후 2-4주이내방문하여폐기능검사및증상조절의정도를평가하여야한다. 1) 5단계치료 (1) 고용량 ICS/LABA 와경구용스테로이드 (OCS) 를사용하는경우 고용량 ICS/LABA 를사용하면서객담지침에따라서 OCS를감량한다. OCS를격일투여한다. 고용량 ICS와 OCS로전환치료한다 (2) 고용량 ICS/LABA 와다른약제를추가사용한경우 천식전문가에의뢰한다. 2) 4단계치료 (1) 중등용량또는고용량 ICS/LABA 를조절제로사용하는경우 LABA 를중단하면악화되는경우가많으므로 ICS를우선적으로 50% 감량하여사용한다. (2) 중등용량 ICS/formoterol을조절제와증상완화제로사용하는경우 저용량 ICS/formoterol로조절제와증상완화제로사용한다. (3) 고용량 ICS와 2차조절제를사용하는경우 ICS를 50% 감량하고 2차조절제는계속유지하여치료한다. 3) 3단계치료 (1) 저용량 ICS/LABA 를조절제로사용하는경우 LABA 중단후악화소견이많으므로 ICS/LABA 를하루에한번으로사용하도록한다. (2) 저용량 ICS/formoterol을조절제와완화제로같이사용하는경우 저용량 ICS/formoterol를하루한번사용하면서증상완화제로도같이사용한다. (3) 중등용량혹은고용량 ICS를사용하는경우 ICS의용량을 50% 감량하여사용한다. 4) 2단계치료 (1) 저용량 ICS을사용하는경우 하루에한번사용하게한다. (2) 저용량 ICS 또는류코트리엔조절제를사용하는경우 증상 6-12개월동안없으면서위험요소가없는경우에 질병조절제의중단을고려하여본다. 답변 2: 3개월이상증세가없으면저용량 ICS/LABA 를하루에한번으로감량하여사용할수있으나과거력상 1년이내두번이상급성악화의경험이있으므로기존대로 1 puff bid로계속사용하게하는것이좋다. 증례 3: 65세여자환자저용량 ICS/formoterol를 1 puff bid 로사용중에 3일전감기가시작되면서금일오전부터가슴이답답하고기침이난다고내원하였다. 내원시폐기능검사에서 FEV 1 = 75% predicted 였다면치료는? 배경 : 천식이조절되지않는다면치료단계를높이기에앞서흡입기를제대로사용하는지를확인하고매일규칙적으로사용하는지, 집이나직장에서간접흡연, 알레르겐노출, 공해나미세먼지에노출이있는지또는베타차단제나 NSAID를복용하는지를먼저확인할필요가있다. 또한현재의증상이천식이조절이되지않아서나타나는것인지를반드시확인하여야한다. 9. 천식치료 1) 1단계치료 : 필요할때증상완화제사용 (1) 주간증상이별로없고즉, 기침, 천명및호흡곤란이한달에 2회이하이면서야간증상은없고폐기능검사가정상인경우 (2) 치료에는증상완화제인 SABA 를사용한다. SABA 대신에흡입성항콜린제제또는경구 SABA, 단기데오필린을사용할수는있으나작용시간이늦다. (3) 급성악화위험요소가있는경우즉, FEV 1 < 80% 이거나지난 12개월동안급성악화를 1회이상경험한경우에는 ICS를사용한다 [26]. 2) 2단계치료 : 저용량질병조절제와증상완화제사용 (1) 저용량 ICS를사용하는것을권장한다. 흡입스테로이드대신사용할수있는조절제로는류코트리엔조절제가있다. 류코트리엔조절제는흡입스테로이드를사용할수없거나사용하려하지않는환자, 흡입스테로이드로인하여목이쉬는등부작용이심한경우또는알레르기비염이동반된천식환자에게사용할수있다. (2) 저용량 ICS/LABA 를사용하면저용량 ICS보다증상완화나폐기능호전에효과가있으나악화위험요소를더감소시키지는않는다고한다

New guideline for asthma management - (3) 계절적인알레르기성천식환자에서는증상이있을때부터꽃가루비산시기가끝난후 4주까지 ICS 사용할수도있다. 3) 3단계치료 : 하나혹은두가지조절제와증상완화제사용 (1) 저용량 ICS와 LABA 흡입제와증상완화제 SABA 를사용하거나저용량 ICS/formoterol를조절제및증상완화제로함께사용하는것을추천한다. 저용량 ICS와 LABA 의병용요법은부가적인효과가나타나서증상호전이나폐기능향상을훨씬더증가시키면서급성악화의위험도를감소시킨다. 3단계치료의조절제의다른방법은중등용량흡입스테로이드를단독사용할수는있으나 LABA 병용보다효과가없다. (2) 그외저용량 ICS와류코트리엔조절제또는방형테오필린을함께사용하는방법이있으나효율적이지는않다. 4) 4단계치료 : 두가지이상의조절제와증상완화제사용 (1) 저용량 ICS/formoterol을조절제및증상완화제로함께사용하거나중등용량 ICS/LABA 와증상완화제로 SABA 를사용하는것이다. 지난한해동안급성악화가한차례이상인경우에서는저용량 ICS/formoterol를조절제와증상완화제로사용하는것이급성악화를더줄일수있다 [27]. 흡입스테로이드는중등용량에서고용량으로증가시켜도추가로얻는효과가상대적으로적으므로중등용량의 ICS/LABA 를사용하여도천식조절이되지않을때만고용량 ICS/LABA 를 3-6개월만일시적으로사용하여효과여부를판단하기를권한다. 5) 5단계치료 : 고단계치료와조절제의추가투여사용 (1) 되도록천식전문가에게의뢰하여야한다. (2) 항IgE를다른조절제에추가하여사용하면중등도또는중증알레르기천식을호전시킬수있다. (3) 객담지침치료 : 유도객담에서호산구 > 3% 를기준으로치료하면효과가있다 (4) 경구스테로이드 ( 7.5 mg/day): 3개월이상사용시에는골다공증에대한위험성이높다. 10. 천식치료단계높이기의실제 가 2일이상 20% 이상감소가되면기존의 ICS를두배이상증량하여 2-4주간사용을하고증상조절과악화요인을평가하여단계를조절하여치료한다. 2) 흡입스테로이드제를성인에서천식의급성악화때용량을 4배이상증가시켜서 7-14일동안계속사용후다시기존의방법으로사용하는방법도있다 ( 단기간사용한 OCS 의효과와대등 ). 3) 급성악화때에는 SABA 의사용은조절제의효과가나타나기까지증상악화를견디게하는효과이므로하루이상사용시에는조절제의증량이필요하다. ICS/formoterol은증상완화제로사용할수가있으며하루최대 formoterol 72 mcg까지사용할수가있다. 4) FEV 1 이예측치의 60% 미만이거나증상완화제와조절제를증량치료후 48시간에도호전이없는경우에는 OCS를 1 mg/kg/day ( 최대 50 mg) 으로 5-7일만사용을한다. 답변 3: 감기로인한악화이므로기존 ICS/formoterol를 1 puff qid 또는 2 puff bid로 2-4주간사용한후다시방문하여증상조절정도와폐기능을평가하여호전된경우에는 1 puff bid를사용하게한다. 결론기관지천식의치료는환자의천식조절상태가호전되어유지하는것이중요하다. 이를위해서흡입치료약을제대로복용하는여부에대한교육및확인이필요하며보다세분화된단계별천식치료를통하여약의부작용이최소화상태에서호흡기증상이조절이잘유지될수있다. 환자가호소하는호흡기증상뿐만아니라향후천식악화가일어날수악화요인을분석하여치료단계에첨가함으로써보다천식조절상태를잘유지할수있게되었다. 그러므로천식치료조절제를감량, 유지또는증량에있어서환자가느끼는주관적인호흡기증상을잘기록하여평가하는외에도추가적으로객관적인평가인폐기능검사를주기적으로시행하는것이필요하다. 중심단어 : 기관지천식 ; 다양성 ; 폐기능검사 1) SABA 를 1-2 일이상반복적으로사용하게되거나 PEF
