The aim of this study was to compare the representatives international guidelines of pharmacotherapy for pediatric asthma. Methods: The recommendations related to pharmacotherapy for pediatric asthma were extracted from the latest representatives international guidelines, and comprehensive comparisons were conducted. Results: Major comparison outcomes between international guidelines were evaluated as follows: classification system on severity and pediatric age group, recommendation for inhaled corticosteroid dose, recommendation for pediatric age group of theophylline in mild asthma, and recommendation for pediatric age group of tiotropium in severe asthma. Clinical trials emphasized the adverse effects of theophylline, whereas tiotropium demonstrated beneficial actions for pediatric asthma. Therefore, theophylline was recommended for older patients with persistent asthma, and tiotropium was considered to be suitable for younger patients with severe asthma according to GINA guidelines. Conclusion: These findings address the requirement to harmonize international guidelines of pharmacotherapy in pediatric asthma. In addition, the findings suggest that KAAACI needs to update its pharmacotherapy guidelines of theophylline, tiotropium and other medicines recently approved. KEY WORDS: Asthma, pediatrics, pharmacotherapy, guideline, GINA 천식은천명과호흡곤란과같은지속적인증세와함께일시적인발작성악화를동반하는대표적인호흡기질환이다. 1) 특히소아에게서천식은다른연령대보다높은유병률을보일뿐만아니라급성악화와발작으로인한응급실내원및입원율이높은질병이다. 2) 따라서소아환자에게주요한관심을필요로하고있다. 3) 국내건강보험심사평가원보건의료빅데이터개방시스템의 국민관심질병통계 에따르면 2015년에 19 세이하에서질병코드 J45 천식 으로분류된환자수가인구 10만명당 6.4명 (642,150명) 으로나타났다. 4) 소아천식은성장하는과정에서자연스럽게호전되기도하지만, 적지않은경우에서성인까지연결되는만성질환으로진행되거나호흡기염증질환에영향을끼친다. 5) 따라서소아 천식의적절한질병관리, 환자교육그리고약물요법을중점으로한치료가반드시필요하다. 6) 특히소아환자는연령별발달수준에따라약물의대사차이가크며, 인종과같은유전적요인도동시에영향받기때문에보다신중한약물요법의설계와관리가필요하다. 7,8) 이에따라국제천식기구 (Global Initiative for Asthma, GINA) 는천식치료에대한국제가이드라인을발표하였고, 우리나라를포함한각국의주요기관들은별도의가이드라인을편찬하였다. 9) 그리고가이드라인에는흡입스테로이드제 (inhaled corticosteroids, ICS), 속효성베타효능제 (short-acting beta 2 - agonists, SABA), 지속성베타효능제 (long-acting beta 2 -agonists, LABA), 류코트리엔조절제 (leukotriene receptor antagonists, *Correspondence to: In-hwan Baek, College of Pharmacy, Kyungsung University, 309, Suyeong-ro, Nam-gu, Busan 48434, Republic of Korea Tel: , Fax: baek@ks.ac.kr 113

2 114 / Korean J Clin Pharm, Vol. 27, No. 2, 2017 LTRA), 테오필린 (theophylline), 항콜린성기관지확장제 (anticholinergic bronchodilators), 항면역글로불린E 항체 (antiimmunoglobulin E antibodies, anti-ige), 그리고항인터루킨 -5 항체 (anti-interleukin 5 antibodies, anti-il-5) 등천식에사용되는대표적인약물들에대한권고지침이수록되어있다. 하지만각가이드라인마다다소차이나는권고내용및권고수준을제시함에따라소아천식환자의약물요법설계에혼란을야기하고있다. 따라서본연구에서는국제천식기구 (GINA), 한국의대한천식알레르기학회 (Korean Academy of Asthma, Allergy and Clinical Immunology; KAAACI), 미국의 National Heart, Lung, and Blood institute(nhlbi), 영국의 British Thoracic Society (BTS), 일본의 Japanese Society of Allergology and Japanese Society of Pediatric Allergy and Clinical Immunology (JSA) 에서발행된최신가이드라인으로부터소아천식환자에대한약물요법에대한권고내용을비교분석하였다. 또한이러한가이드라인비교분석을통해소아천식약물요법의국제적인동향을파악하고, 향후국내천식약물요법가이드라인이보완하여야할사항에대해제시하였다. 연구방법 본연구에서는국내가이드라인과해외기관별 4개의가이드라인을채택하여소아천식약물요법에대한권고사항을비교분석하였다. 국내가이드라인으로대한천식알레르기학회 (KAAACI) 의한국천식진료지침 (Korean Guideline for Asthma, 2015년 ) 의내용을고찰하였고, 해외기관으로국제천식기구 (GINA) 의가이드라인 (Global Strategy for Asthma Management and Prevention, 2017년 ), 미국 NHLBI의가이드라인 (Guidelines for the Diagnosis and Management of Asthma, 2007년 ), 영국 BTS 가이드라인 (British guideline on the management of asthma, 2016년 ), 그리고일본 JSA 가이드라인 (Japanese Guideline for Childhood Asthma, 2014년 ) 의내용을심도있게분석하였다 (Table 1). 연구결과 국내 외주요가이드라인의개정현황대한천식알레르기학회 (KAAACI) 는 1994년에처음으로천식치료가이드라인을발표한이래로현재까지 1998, 2003, 2007, 2011, 2015년에걸쳐약 4년의주기로개정되고있다. 가장최근에개정된 2015년 KAAACI 가이드라인은 GINA의 2014년가이드라인과영국 BTS의 2014년가이드라인의내용을각각반영하였다고명시하고있다. 국제천식기구 (GINA) 의가이드라인은 2010년이래로격년또는매년개정판을내놓거나추가자료를발간해오고있다 (Table 2). 특히최근 3년 (2015년 2017년 ) 간매년개정판을발간하였다. 가이드라인개정에따른주요변경사항으로 tiotropium ( 항콜린성기관지확장제 ) 의권고수준은상향되고권고연령은점차낮아지고있다는점이다. 2015년 GINA 가이드라인에서는 tiotropium을 Evidence B수준으로 18세이상의악화된병력이있는천식환자에게중증도 4단계와 5단계에서투여할수있도록처음권고하였고, 이듬해 2016년 GINA는중증도 4단계서 12 세이상의환자에게 Evidence A 수준으로 tiotropium 을사용하도록가이드라인을개정하였다. 또한 mepolizumab, reslizumab 등새로이승인되는항인터루킨-5 항체에대한권고사항을신속히추가하였다. 가장최근에개정된 2017년 GINA 가이드라인에서는저용량 ICS의치료단계하향 (step-down), 경구스테로이드부작용에대한복약지도중요성, 천식치료제로써비타민D 연구의필요성등을강조하고있다. 영국 BTS는 2008년이래로매년또는격년의간격으로가이드라인을개정해오고있으며, 2016년개정된가이드라인에서 tiotropium과항인터루킨 -5 항체에대한권고내용이추가되었다. 비록 2014년가이드라인에서 tiotropium을언급한바는있으나 2016년가이드라인에서처음으로 tiotropium이 1++ 수준 (Evidence A와같은등급 ) 으로 12세이상의연령에서권고 Table 1. Revision of Global Institute of Asthma Guidelines 2003 Update: Pocket Guide for Asthma Management and Prevention 2004 Update: Pocket Guide for Asthma Management and Prevention 2005 Update: Pocket Guide for Asthma Management and Prevention 2010 Pocket Guide for Asthma Management and Prevention 2012 Pocket Guide for Asthma Management and Prevention 2014 Major Revision 2015 GINA Report, Global Strategy for Asthma Management and Prevention 2015 Pocket Guide for Asthma Management and Prevention 2015 Asthma, COPD and Asthma-COPD Overlap Syndrome (ACOS) 2016 Pocket Guide for Asthma Management and Prevention in Children 5 Years and Younger 2016 Pocket Guide for Asthma Management and Prevention Global Strategy for Asthma Management and Prevention (2016 update) Global Strategy for Asthma Management and Prevention (2017 update) 되었다. 다만흡입스테이로이드제용량의증량및지속성베타효능제와병용요법에비해우월성은입증하지는못하였으므로비공식 (off-label) 적인대체치료제 (alternative treatment) 로권고하고있다. 또한 2014년가이드라인에서 12세이상의천식환자에게 1++ 수준으로항인터루킨 -5 항체를권고하도록개정하였다. 국내 KAAACI와 GINA 가이드라인의비교국내 KAAACI와 GINA 가이드라인은질병분류체계, 치료제종류, 권고용량, 환자증상에따른치료단계상향 (step-up) 및하향 (step-down) 방법등에서전반적으로일치하였다. 하지만국내 KAAACI 가이드라인과 GINA 가이드라인은 theophylline과 tiotropium 에대한약물요법에서큰차이점을나타내고있다. 국내가이드라인은 theophylline의사용을중등도 5단계중 2단계부터 6세이상의소아환자에게권고하는반면, GINA 가이드라인은 12세이상의소아환자에게 Evidence B 수준으로사용하도록권고하고있다. 또한 GINA는천식에대하여 theophylline 이부족한효능과많은부작용으로고용량으로투여시소아환자의생명에지장을초래할수있다고강조하고있다. Tiotropium 의경우국내가이드라인에서는권고사항이없는반면 GINA 가이드라인에서는 12세이상, 중증도 4단계이상의중증환자에게 Evidence A 수준으로권고하고있다. 이외에 GINA 가이드라인은항인터루킨-5 항체등보다최신약물에대한권고내용을포함하고있다. 국내 KAAACI와미국 NHLBI 가이드라인의비교앞서국내와 GINA 가이드라인비교에서크게나타났던 theophylline에대한약물요법이미국 NHLBI 가이드라인과는일치하는권고내용을보이고있다. 미국 NHLBI 가이드라인은중증도 6단계중 2단계부터 5세이상의소아환자에게 theophylline의사용을 Evidence B 수준으로권고하고있다. 즉, 국내 KAAACI와미국 NHLBI 가이드라인에의하면모두지속적 (persistent) 천식의초기에서부터대체요법으로 theophylline 을사용할수있다. 또한미국 NHLBI 가이드라인은국내와마찬가지로 tiotropium에대한권고내용이없다. 국내 KAAACI와미국 NHLBI 가이드라인은각각 6세와 5 세미만의소아환자에게지속적천식질병을조절하기위하여흡입스테로이드제를 Evidence A 수준으로권고하고있으며, 중증도에따라병용요법대신흡입스테로이드제의용량을증량하는것을권고하고있다. 하지만국내가이드라인은 6세이상 12세미만환자에게중증도 2단계에서 3단계로상승시흡입스테로이드제와 LABA 병용요법을권고하는반면, 미국 NHLBI 가이드라인은흡입스테로이드제단독요법을유지하고투여량을증가하도록권고하고있다. 또한흡입스테로이드제중 beclomethasone과 budesonide의허용최대투여용량은국내가이드라인보다 1.6-2배높게권고하고있다 (Table 3). 또한미국 NHLBI 가이드라인은국내 KAAACI 가이드라인에비해흡입스테로이드제뿐만아니라류코트리엔수용체길항제 (LTRA) 와 cromolyn과같이이보다많은계열의약물들에대한투여권고용량을수록하고있다. 이중에는국내에서시판되지않는 zileuton 등을포함하고있다. 마지막으로국내 KAAACI 가이드라인에서중증단계에권고되는항면역글로불린E 항체에대한내용이미국 NHLBI 가이드라인에는명시되어있지않으며, 항인터루킨-5 항체에대한권고내용은두가이드라인모두없었다. 국내 KAAACI 가이드라인과영국 BTS 가이드라인의비교국내 KAAACI 가이드라인과영국 BTS 가이드라인간에다음과같은주요차이점이있었다. 국내 KAAACI 가이드라인은 GINA 및미국 NHLBI 가이드라인과마찬가지로성분명

Age (yrs) Medicine Budesonide (Nebules) Dose (µg) GINA KAAACI NHLBI Fluticasone (HFA) Beclometasone dipropionate (HFA) Budesonide (DPI) Budesonide (Nebules) Fluticasone (HFA) Low: Medium: High: >200 Medium: High: >400 Low: Medium: High: >1000 Medium: High: >500 Low: Medium: High: >200 Medium: High: >400 Low: Medium: High: >1000 Medium: High: >500 Low: Medium: High: >1000 Low:176 Medium: High: >352 Low: Medium: High: >320 Low: Medium: High: >800 Low: 500 Medium: 1000 High: 2000 Low: Medium: High: >352 DPI: dry powder inhaler; HFA: hydrofluoroalkane. 을기준으로흡입스테로이드제의투여량을권고하였으나, 영국 BTS 가이드라인은성분명대신각각의대표적인상품명에대한투여량을표기하고있다. 또한국내 KAAACI 가이드라인은지속적천식으로분류하는단계 ( 중증도 5단계중 2단계 ) 에서저용량의흡입스테로이드제를권고하고, 증상악화시 (3단계 ) 중간용량이흡입스테로이드제를권고하는반면에영국 BTS 가이드라인은 3단계에서저용량의흡입스테로이드제와 LTRA의병용요법을우선권고하고있다. 영국 BTS 가이드라인에서 tiotropium의사용을권고하는연령은앞서살펴본 GINA 가이드라인과흐름을같이하고있다. 즉, 국내 KAAACI 가이드라인은성인에대해서만흡입스테로이드제와병용요법으로 tiotropium 사용을권고 (Evidence A) 하고있으나영국 BTS 가이드라인은보다낮은연령인 12세이상에서 tiotropium의사용을 1++ 수준으로권고하고있다. 마지막으로영국 BTS 가이드라인은최근개발된항인터루킨 -5 항체에대한내용을포함하고있으나국내 KAAACI 가이드라인은아직권고내용이없다. 국내 KAAACI 가이드라인과일본 JSA 가이드라인의비교일본 JSA 가이드라인은국내 KAAACI 가이드라인및다른해외가이드라인과구별되는질병중증도분류체계를가지고있다. 국내 KAAACI 가이드라인은중증도단계를 5단계로설정하고있는반면일본 JSA 가이드라인은 4단계로구분하였다. 또한소아연령의구분에서국내 KAAACI 가이드라인은 소아환자를 6세미만, 6세이상 12세미만, 12세이상으로구분하는반면, 일본 JSA의가이드라인은 2세미만, 2세이상 6세미만, 6세이상 16세미만으로구분하고있다. 국내 KAAACI 가이드라인과일본 JSA 가이드라인은지속성천식의경증단계 (1단계) 에서권고하는약물요법에도차이를나타내고있다. 국내 KAAACI 가이드라인은중증도 1단계에서초기약물요법으로흡입스테로이드제를권고되는반면일본 JSA 가이드라인은동일한단계에서류코트리엔조절제또는 cromolyn을우선권고하고, 2단계부터흡입스테로이드제를류코트리엔조절제및 cromolyn과동등한수준또는선택치료제 (additional therapy) 로써권고되고있다. 지속적천식보다단계가낮은간헐적천식환자에게필요시 SABA를권고하는것은국내 KAAACI 가이드라인과일본 JSA 가이드라인이동일하다. 또한국내 KAAACI 가이드라인과일본 JSA 가이드라인은 LABA와 theophylline을권고하는소아연령에서차이를나타내고있다. 국내 KAAACI 가이드라인은 6세이상의소아환자에게 LABA와 theophylline을권고하는반면일본 JSA 가이드라인은중증도에따라 2세미만의소아환자에게도경구또는패취형태로투여할수있도록권고하고있다. 특히 theophylline은 2세미만, 2-5세, 6-15세의소아환자에게각각중증도 4단계중 4 단계, 3단계, 2단계부터 theophylline의사용을권고하고있다. 이는 GINA 가이드라인에서 theophylline의권고연령을점차상향화하는것과대조되는일본 JSA 가이드라인의특징이다.

5 대표적인국제소아천식약물요법가이드라인에대한비교연구 / 117 고찰 본연구에서는국제천식기구 (GINA) 에서발행하는가이드라인과한국, 미국, 영국, 일본등주요의약선진국의대표기관에서발행하는천식치료가이드라인을통해소아천식환자의약물요법을비교 고찰하였다. 천식은비교적관리가잘되고있는질환이며, 국제기구인 GINA에서국제적견해를취합하여가이드라인으로발표하고있는만큼기관마다비교적차이가별로없을것으로여겨왔다. 하지만본연구를통해 GINA 및각국의주요기관에서발행하는가이드라인사이에몇가지중요한차이가있음을확인하였다. 특히임상시험기술과정보기술이발달함에따라신뢰성있는정보들을얼마나빠르게반영하고있는가에따라가이드라인내용의차이가발생하는것으로여겨진다. 각가이드라인의약물요법에대한주요차이점은소아환자의중증도분류체계, 흡입스테로이드제의권고용량, theophylline 과 tiotropium의권고연령에서찾아볼수있었다. 국내 KAAACI 가이드라인과 GINA 가이드라인, 영국 BTS 가이드라인은간헐적천식으로포함한질병의중증도를 5단계로구분하는반면, 미국 NHLBI 가이드라인은 6단계로구분하였고, 일본은 4단계로구분하고있다. 이는각국에서선호하는단계적치료접근방식의알고리즘과이를구성하는약물요법의차이로부터발생한것으로사료된다. 따라서각기관별가이드라인간에차이를줄이고조화로운국제가이드라인마련을위하여우선질병중증도분류체계에대한일치가필요하다. 일본 JSA를제외한 GINA, 한국 KAAACI, 미국 NHLBI, 영국 BTS 가이드라인은모두지속성천식의초기 1차치료제로흡입스테로이드제를권고하고있다. 하지만소아환자에게권고하는흡입스테로이드제의용량에대하여불일치를보였다. 따라서어떤가이드라인을선택하느냐에따라흡입스테로이드제에대해권고하는소아용량이달라질수있다. 가장최근에개정된 GINA 가이드라인 (2017년 ) 은흡입스테로이드제에대한유해반응을더욱강조하고, 이를예방하기위해복약지도가중요하다고강조하였다. 따라서각기관별로소아환자에게서지속성천식을치료하기위한흡입스테로이드제에대한권고용량을일치시키고, 복약지도및모니터링의중요성을강조함으로써유해반응발생률을낮추고자하는노력이필요하다. Theophylline은천식치료에오랫동안사용하여온치료제이다. 국내에서도상대적으로낮은복약순응도와높은약가를가지는흡입제에비해경구로투여하는 theophylline이유지요법으로경증단계에서부터선호되어왔다. 10,11) 하지만최근 GINA는가이드라인개정을통하여 theophylline의권고연령을만 6세에서 12세이상으로상향화하는변화가있었다. 이는 theophylline의유해반응으로인해소아천식환자에게 theophylline의사용을다소줄여나가자는국제적동향을반영 하고있다. 하지만현재까지국내 KAAACI를포함한해외주요기관들은최근개정이이루어지지않아 theophylline을 12 세미만의소아에게사용가능한것으로명시하고있으며, 지속적인개정과후속연구를통해소아에대한 theophylline 약물요법의신뢰도를일치할필요가있다. GINA 가이드라인은 theophylline의권고연령은높인반면, tiotropium의권고연령은점차낮아지고있다. 과거 tiotropium은만성폐쇄성폐질환 (COPD) 의다소비중있는치료제인반면천식치료에는다소소극적인경향이있었다. 하지만최근천식과만성폐쇄성폐질환복합증후군 (Asthma-COPD overlap syndrome, ACOS) 에대한연구가진행됨에따라천식치료에 tiotropium을지지하는연구결과들이제시되고있다. 12,13) 이에 GINA는 2015년개정가이드라인에서 18세이상의성인에게 tiotropium을사용할수있도록처음권고하였고, 이듬해 2016 년개정가이드라인에서 12세이상의환자에게연무흡입기를사용하여사용하는것이가능하도록연령을낮추었다. 또한동일한시기에영국 BTS 가이드라인 (2016년) 도 tiotropium을 12세이상의소아천식환자에게사용이가능하도록개정되었다. 이는중증의소아천식환자에게고용량의흡입스테로이드및유해반응위험이큰 theophylline의병용요법대신흡입스테로이드제와 tiotropium의병용요법으로점차대체되고있음을시사하고있다. 따라서국내 KAAACI 가이드라인은이러한국제적동향을반영한가이드라인개정이필요하며, tiotropium 사용에대한국제적혼란을줄이기위한국제기관별노력이필요하다. 결론 본연구는국제천식기구 (GINA) 와한국등주요의약선진국의기관에서가장최근에발행한천식진료가이드라인을바탕으로소아환자에게권고하는약물요법을비교분석하였다. 각기관에따라중증도분류체계, 흡입스테로이드제의권고용량및초기치료제로써역할, theophylline과 tiotropium에대한권고연령등에서차이가있었다. 향후지속적인연구를통해최신의천식약물요법에대한견해를취합하고이를바탕으로국제적으로일관성있는가이드라인운영의필요성을제시하고있다. 이는임상과교육현장에서의혼선을줄일수있으며, 무엇보다도질병과유해반응에취약한소아환자들에게객관적이고신뢰할수있는약물요법을제공할수있을것으로사료된다. 감사의말씀 이논문은 2015년도미래창조과학부의재원으로한국연구재단의지원을받아수행된기초연구사업임 (NRF-2015R1C1A1A
