Therefore, systemic risk by general anesthesia can be avoided, and more accurate surgeries can be performed through active joint motion during surgery without discomfort due to tourniquet used during local anesthesia. Also, the time and cost associated with anesthesia can be reduced and the hospital day can be reduced by performing the day surgery. The authors will introduce the concept of the wide-awake hand surgery and discuss the indications, drug usage, and injection methods. Key Words: Anesthesia, Local, Lidocaine, Epinephrine, Tourniquet 서론 수부외과수술의마취방법은국소 (local), 부위 (regional), 전신 (general) 마취가있으며이중국소마취는환자와소통하며수술부위의움직임을실시간으로확인, 평가하여이를수술에즉각반영할수있는장점이있다. 국소마취를통한수부외과수술은간단한손가락수술을제외하면대부분무혈성의좋은수술시야 (bloodless field) 를위해공기압지혈대 (pneumatic tourniquet) 를사용한다. 대개상완부지혈대에 250 mmhg의압력을적용하게되는데여러연구에따르면, 대다수의환자들은 20 분이상의지혈대적용시간을견디지못하는것으로밝혀졌다 1-3. 따라서국소마취를통한수술의경우, 시간이길 어지게되면지혈대로인한통증으로수술에어려움을겪을수있다. 일부저자들은이를극복하기위해전완부지혈대를사용하거나지혈대의압력을 200 mmhg로줄이는방법을제시하기도하였으나이는통증을견딜수있는약간의시간을늘려줄뿐근본적으로지혈대로인한통증자체를없애주지는못한다. 그밖에수술직전이나도중, 진정제 (sedatives) 나진통제를사용하기도하는데이는국소마취의큰장점인수술중환자와의소통이어려운단점이있다. 이처럼수술부위의통증은국소마취제로차단할수있으나지혈대로인한통증은해결하기가어려우며, 특히시간이지날수록통증을점점심하게느끼는환자에게협조를기대하기는굉장히어렵다. 이는결국환자와의사모두수술에집중하기어려운상황을야기할수있으며궁 Received February 19, 2018, Revised February 22, 2018, Accepted February 23, 2018 Corresponding author: Ji-Kang Park Department of Orthopaedic Surgery, Chungbuk National University Hospital, 776 1sunhwan-ro, Seowon-gu, Cheongju 28644, Korea TEL: , FAX: , carm0916@hanmail.net Copyright c 2018 by Korean Society for Surgery of the Hand, Korean Society for Microsurgery, and Korean Society for Surgery of the Peripheral Nerve. 2 Archives of Hand and Microsurgery Vol. 23, No. 1, March 2018 극적으로는수술의최종결과에악영향을미칠수있다. 따라서국소마취의최대장점을살리기위해서는지혈대로인한통증을없애는것이매우중요하다. 이에최근지혈대를하지않고국소마취를통한수부수술이시행되고있는데이를각성국소마취및무지혈대 (wide awake local anesthesia no tourniquet, WALANT) 라한다. 결론을내렸다. 하지만최근 Zhang 등 9 에의해방아쇠손가락수술에서 1% 리도카인과 1:100,000 에피네프린의복합투여로인한손가락의괴사가보고된바있어에피네프린주사로인한수지허혈상태가수술후에도지속될경우상황에따라알파차단제인펜톨아민을통해혈관수축을되돌릴수있도록준비하는것이좋겠다 (Fig. 1). WIDE AWAKE LOCAL ANESTHESIA NO TOURNIQUET WALANT 마취방법은리도카인 (lidocaine) 을이용한 국소마취로수술중발생하는통증을막고, 에피네프린 (epinephrine) 을혼합하여지혈대를사용하지않으면서 수술부위의출혈을막아장시간의각성및지혈상태를얻 는마취및수술방법으로, 운동성을유지한무통상태에서 다양한수부수술을할수있는장점이있다. 2. 국소마취제로리도카인을사용하는이유? 부피바케인 (bupivacaine) 과비교하여 부피바케인은심장독성 (cardiotoxicity) 이있어위험할 수있다. 반면에리도카인과에피네프린조합은수십년간 치과영역에서별도의모니터링없이안전하게사용됐음이 입증되었다. 다만, 수술시간이 2.5 시간이상길어질것이 예상될경우 1:200,000 에피네프린에 0.5% 부피바케인 10 ml 를추가하여마취시간을지속시킬수있다. 1. 수부수술에서에피네프린의안정성 과거괴사에대한우려때문에손가락에대한에피네프린의사용은금기시되었으나최근의여러대규모연구에서수부외과수술에있어에피네프린의사용에대한안정성이검증되었다. 6개도시에서 9명의각기다른술자가에피네프린을이용한수부수술 3,110예에대한전향적연구에서단한건의수지괴사도없었으며 1,111예를대상으로한유사연구도비슷한결과를보고하였다 4,5. 또한 4,000예이상을대상으로한, 두그룹의대규모연구에서에피네프린주사로인한수지의허혈상태를되돌릴수있는알파차단제 (alpha blocker) 인펜톨민 (Phentolamine Mesylate; Reyon Pharm., Seoul, Korea) 의사용이거의필요하지않았음을증명하였고 6,7, 심지어 1:1,000의고농도에피네프린의사용도수지괴사를일으키지않았다고보고하였다 8. 이러한결과를바탕으로 Thomson 등 6 은수부수술에있어에피네프린의사용은더이상위험하지않다는 Fig. 1. WALANT 수술의장점 국소마취중사용하는지혈대로인한통증을막기위한 진정제의사용이필요없으며진정제로인한오심, 구역, 전신에대한부작용도없다. 전신마취로인한위험을피할 수있어고위험마취군의수술이가능하며전신마취나부 위마취에필요한수술전검사를할필요가없어경제적이 다.

WALANT 수술의장점 국소마취중사용하는지혈대로인한통증을막기위한 진정제의사용이필요없으며진정제로인한오심, 구역, 전신에대한부작용도없다. 전신마취로인한위험을피할 수있어고위험마취군의수술이가능하며전신마취나부 위마취에필요한수술전검사를할필요가없어경제적이 다. 또한각성상태에서수술을하므로수부수술의특성상 수술부위를착각할수있는여지를원천적으로막을수있 고, 지혈대에의한불편감없이수술중환자의능동적인 관절운동을통해보다정확한수부수술을할수있다. 4. WALANT 수술의용법 1) 리도카인의안전용량 일부연구에따르면, 35 mg/kg 도안전한것으로알려져 있으나 10,11 대체로받아들여지는안전용량은 7 mg/kg 이 다. 70 kg 의성인을기준으로이를적용하면, 안전용량은 1% 리도카인 50 ml 정도이다. 대부분의수부수술은 50 ml 이하로가능한데 3 개이상의수지건수술이나손목 및전완부수술과같이넓은수술부위의경우더많은양 이필요할수있다 (Table 1) 12. 2) 주사방법 (Fig. 2, 3) 리도카인과에피네프린의최대효과발현시간은최소 26 분이상소요되는것으로밝혀졌다 13. 따라서피부절개 최소 26 분이전에주사가되어야한다. 피하조직보다대 Fig. 3. A simple technique to decrease movement during injection is to stabilize the syringe holding hand with fingers, and too much local anesthetic is better than not enough. 략 1,000 배의산성도를띠는리도카인 ( 평균 ph 4.7) 이체 내에들어오게되면통증을느끼게되는데, 이를줄여주기 위해서리도카인 10 ml 당 8.4% 바이카보네이트 (bicarbonate) 1 ml 를섞어 10:1 의비율로중화시킨다 14. 또한 마취제를체온과비슷하게유지하고, 27 게이지이하의얇 은바늘을사용한다 15. 계획된절개부위의가장근위부에서 부터주사를시작하고, 처음에약물 1 ml 를피하에주사한 후약 초동안시간차를두어주사부위에더이상의 통증이없어졌을때나머지마취용액을 1 분이상에걸쳐 천천히주입하여급속한피부팽창에의한통증을줄일수 있다. 이때주사바늘을피부에직각으로삽입하여통증을 최소화하여야하며주사바늘이흔들리지않도록주의하여 3

Hagert와 Lalonde19는 최근 삼각 섬유 연 수술 중 능동적인 관절 운동을 함으로써 유착 부위 등 문 골 복합체 수술에도 활용하였으며, 손목 터널(Fig. 14) 및 제를 일으키는 곳을 보다 쉽게 알 수 있고 때로는 환자가 팔꿈치 터널 증후군, Dupuytren 구축증(Fig. 15), 골종양 유착을 스스로 없애는 경우도 있다. 또한 수술을 마무리 하 수술 등 수술 범위가 점차 늘어가고 있다. 기 전에 수술의 적정성 여부를 최종 평가하여 예후를 알 수 있다(Fig. 9, 10). 4) 기타 결론 수부 각성 수술은 안전하고, 경제적이며 효율적이다. 무 각성 수술을 통한 관절 유합술은 수술 중 적절한 유합 각 엇보다 수술 중 환자의 문제점을 정확히 평가할 수 있고 이 도를 설정할 수 있으며, 수부 골절 수술의 경우 주먹을 쥐 를 수술에 즉각적으로 반영할 수 있으므로 좋은 결과를 가 어 보게 하여 회전 변형 여부를 정확하게 평가할 수 있다 져올 수 있다. 다수의 연구에서 안전성이 확보되었지만, 수 6
