E, F Postoperative AP and lateral x-ray showing well replaced the bone fragments with miniplates and screws. 시킨 후 철사나 miniplate와 나사못 등으로 잘 고정하였다. 내원 당시의 신경학적 상태를 Glasgow 혼수계수에 따라 수술후 항생제의 투여는 내원 당시의 상처 부위의 오염 분류해 보았을 때, 5점이 7례(77.2%)로 가장 많았으며, 정도에 따라서 투여기간을 결정하였으며, 비교적 깨끗한 경 2점에서 4점 사이가 4례(8.%), 점이 례(4.5%)로 우에는 수술후 2주, 오염 정도가 심한 상처의 경우에는 수 대부분이 비교적 양호한 상태였다. Glasgow 혼수계수가 술후 3주간 투여하는 것을 원칙으로 하였다. 이 때 항생제 점이었던 례도 뇌압항진의 소견이나 뇌손상의 정도가 비 는 광범위의 세파 계열 항생제 종과 aminoglycoside 계 교적 심하지 않았던 경우이었다. 열 항생제 종을 정맥 주사 및 근육주사로 복합 투여하였다. 손상 부위로는 전두골이 포함된 손상이 6례(72.7%)로 대부분을 차지하였으며, 9례(4.9%)는 전두동의 골절이 동 결 과 반되었다(Table 2). 수술 전이나 수술 중 뇌경막의 열상이나 결손이 발견되었 대상 환자의 성별과 연령 분포는 남자가 2례로 압도적으로 많았으며, 연령 분포는 4세에서 63세까지로 평균 26.9±7.4 세 이었고, 대부분이 2대 이하이었다. 그 중에서 5 세 소 아 환자는 7례(3.8%) 이었다(Table ). 수상 원인으로는 오토바이 사고를 포함한 교통사고가 2 례(54.5%)로 가장 많았으며, 둔기나 흉기에 의한 수상이 7 례(3.8%), 추락 사고가 3례(3.6%) 순이었다. 67 던 8례 중 5례는 직접 봉합술을 시행하였고, 3례는 두개골 막반 이식으로 결손 부위를 봉합하였다(Table 3). 내원 당시의 상처 부위의 오염정도를 대략적으로 분류하여 보았을 때, 이물질의 함입이 적어 비교적 깨끗한 상처로 분류 되었던 경우가 대부분이었으나, 7례(3.8%)는 흙이나 모래, 머리카락 등에 의하여 심하게 오염되어 있었다(Table 4). 22례 모두 수상후 24시간 이내에 본원에서 수술을 받았 J Korean Neurosurg Soc/ Volume 29 / May, 2
