두정엽 및 후두엽 간질에 대한 수술전략 MRI 촬영상 병변이 발견되지 않았던 3명의 환자에서는 3차 에서는, 일반 MRI에서 궁륭부 및 기저부에 병변이 위치하 원 입체화뇌에서는 모두 상부 두정소엽의 회 및 구가 위축된 는 경우에는 병변의 위치가 정상부위와 비교되어 뚜렷하게 소견이 보여 이 부위를 간질병소로 의심할 수 있게 하였다 잘 나타났으나, 내측에

The operative field showing resection of the epileptogenic zone corresponding to the region revealed abnormal gyral and sulcal pattern in the 3D-surface rendered brain, in patient with parietal lobe epilepsy. 기 위해 대뇌피질 전기자극검사를 하였다. 9명의 두정엽간 질 환자 모두에서 운동중추를 확인할 수 있었고, Amytal test 결과 우성대뇌가 좌측에 있으며 간질병소가 두정엽 및 측두엽 후방까지 추측되었던 2명의 두정엽 간질환자에서는 3. 뇌 파(Table 1, 2) 1) 두피뇌파 모든 환자에서, 발작간 두피뇌파검사는 광범위한 부위에 서 간질성뇌파가 기록됨으로써 간질병소를 예측할 수 있을 만큼 충분한 정보를 제공하지는 못했다. 두정엽 간질 환자 들에서는 양측 대뇌의 전두부, 두정부 및 측두부에서 광범 위하게 분포되는 극파(spike)를 관찰할 수 있었고, 우측 후 두엽에 간질병소를 가졌던 환자에서만 일측의 후뇌부(unilateral posterior head region)에 국한된 극파를 관찰할 수 있었다. 각 환자에서 5 7회씩의 발작을 관찰하며 얻은 각성마취를 이용하여 감각성 언어중추를 확인할 수 있었다. 5. 병리조직검사 소견(Table 1, 2) 병리조직검사상, 두정엽간질 환자중 6명은 피질이형성증 (cortical dysplasia)을 보였고, 2명은 뇌염후 나타나는 신 경교증(neuronal gliosis) 소견을 보였다. 후두엽 간질환자 중 4명은 피질이형성증을 보였으며, 이들중 2명은 결절성 경화증(tuberous sclerosis)을 동반하였다. 다른 4명에서는 신경교증을 보였으며, 과오종(hamartoma)과 신경절교종 (ganglioglioma) 소견도 각각 한명의 환자에서 관찰되었다. 발작시 뇌파에서, 9명의 두정엽 간질환자 모두는 발작시 뇌 6. 수술결과(Table 1, 2) 수술후 6개월에서 37개월(평균 19.7개월) 동안의 추적 파기록상, 양측 대뇌의 후두부(T7, T8, P7, P8, O1, O2)에 관찰기간 동안, 두정엽환자중 5명은 전혀 발작을 보이지 않 서 동시에 시작하는 고속 율동성 간질파(fast rhythmic 았다.
