Debates over classification and diagnostic criteria, and controversy over identifiable treatment targets will continue until distinct pathophysiological mechanism of vascular dementia is found. Clinical diagnostic criteria are sufficiently strong to be useful for clinical trials, but need further refinement. Cognitive changes in vascular dementia are more variable than other disorders, and are dependent on the vascular pathology. Accurate diagnosis of vascular dementia is known to need the presence of reliable cerebrovascular disease on brain imaging. Although it seems obvious that cerebrovascular disease causes pathological damage and impaired cognition, it is very difficult to find the accurate contribution of cerebrovascular pathology to cognitive decline. Most studies have shown a small but significant benefit of cholinesterase inhibitors on cognition, the significance of this effect has been slight and benefits on global functioning, activities of daily living, and behaviour have not been consistently reported. Management of vascular dementia should focus on identifying and managing vascular risk factors. Subtypes of vascular dementia Multi-infart dementia (cortical vascular dementia) Small vessel dementia (subcortical vascular dementia) Strategic infarct dementia Hypoperfusion dementia Haemorrhagic dementia Hereditary vascular dementia (CADASIL) Alzheimer s disease with cardiovascular disease Multiple cortical infarcts Imaging and pathological changes Lacunes, extensive white matter lesions ; pathologically, infarcts, demyelination, and gliosis Infarct in strategic location (e.g., thalamus) Watershed infarcts, white matter lesions ; pathologically, incomplete infarcts in white matter Haemorrhagic changes, may be associated with amyloid angiopathy Multiple lacunes and white matter lesions, temporal lobe white matter affected Combination of vascular changes and atrophy, especially medial temporal lobe ; pathologically, mixture of vascular and degenerative (plaque and tangle) pathology CADASIL : cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy 기억장애가 알츠하이머병에서는 핵심 증상으로 적절할지 모르나 다양한 범위의 영향을 받는 혈관성 치매에는 적절하지 않다 이러한 제한점 때문에 뇌혈관질환이 주로 다른 병리적 변화와 더불어 인지기능 장애를 일으킨다는 의견이 지지를 받아왔으며 보다 넓은 의미를 가지는 혈관성 인지장애라는 용어가 사용되었다 혈관성 인지장애에서는 다양한 성질의 혈관성 병리가 치매 발병에 기여를 하며 다양한 아형이 존재한다 정신질환의 진단 및 통계편람 제 판 - 이하 진단체계에서는 치매 진단기준에서 기억장애를 필수 진단기준에서 삭제하고 주요신경인지장애라 새로이 정의하였다 또한 혈관성 인지장애의 범주에 혈관성 주요신경인지장애 와 혈관성 경도신경인지장애 를 포함시켰다 기존에 널리 사용되던 혈관성 치매를 대신해서 혈관성 주요신경인지장애라는 용어로 변경하였다 혈관성 치매의 진단 및 질병분류학적 부분에 대한 논란은 혈관성 치매에 대한 명확하고 접근하기 용의한 병태생리학적 기전이 밝혀지기 전까지는 지속될 것으로 전망된다 이 글에서는 혈관성 치매의 역학 위험요인 진단 그리고 치료에 대해서 다루고자 한다 역학대부분의 연구는 전형적인 협의의 정의에 따른 혈관성 치매를 대상으로 이루어졌다 혈관성 치매 발병 위험도는 나이에 따라 증가하며 매 년마다 약 배가량 증가한다 이러한 지수적인 증가는 년마다 배가 증가하는 알츠하이머 치매에는 약간 못 미치는 정도이다 뇌졸중 생존자의 약 정도에서 뇌졸중 후 치매가 발생한다고 알려져 있다 뇌졸중 후 새로운 치매 증상의 발생은 뇌졸중 후 년 경과 시 정도이지만 년 경과 시에는 까지 증가하는 것으로 보고되었으며 혈관성 치매의 사망률이 알츠하이머병보다 더 높게 나타나는데 이는 동반된 관상동맥 질환의 영향에 기인한 것으로 여겨진다 뇌졸중 후 치매 는 병태생리가 밝혀져 있지 않기 때문에 종종 혈관성 치매의 하위부류로 분류된다 그러나 뇌졸중 후 치매는 여러 가지 특성이 있는데 이미 존재하는 인지장애나 치매를 드러나게 하는 것 반복된 뇌경색 후 발생하는 혈관성 치매 장기적으로 치매의 높은 위험요인을 가지는 사람들에게 뇌졸중이 발생하는 것 등이 포함된다 혈관성 병리와 알츠하이머병리 사이의 밀접한 상관관계로 인해 지연성 치매의 병태생리가 혈관성 질환에 기인하는지 퇴행성 병리에 기인하는지 아니면 그 둘 모두에 기인하는지에 대한 연구의 필요성이 대두되었다 몇몇 연구들은 뇌졸중을 경험한 사람들 중에서 알츠하이머병이 더 흔하다고 보고하였지만 알츠하이머병리의 높은 위험요소를 가진 세 이상의 뇌졸중 생존자들을 장기간 추적하여 부검한 연구에서는 이상이 퇴행성이 아닌 혈관성 치매가 원인이라고 보고하였다 위험요인뇌졸중 후 치매에 대한 위험요인으로는 연령증가 낮은 교육수준 여성 혈관성 위험요인들 병변의 위치 현재 뇌졸중의 유무 그리고 측두엽 전체나 내측두엽의 위축이 있다 뇌졸중 없이 발병하는 혈관성 치매에서는 고령과 혈관성 위험요인들이 가장 중요하다 고혈압 당뇨 고지혈증 심방세동 흡연 음주 비만 등의 혈관성 위험요인들이 혈관성 대뇌병변을 일으키는 위험요인임은 분명하다 그러나 이들 위험요인이 혈관성 치매 발병에 어떤식으로 기여하는지에 대해서는 명확히 journal.biolpsychiatry.or.kr 81

3 Korean J Biol Psychiatry 2016;23(3):80-88 밝혀져 있지 않다 등 의 메타 분석에 의하면 인생의 후반기에 발생한 우울증이 알츠하이머병에서와 마찬가지로 혈관성 치매의 위험요인으로 나타났다 뇌영상 과 병리 에 대한 확인 결과 인생 후반기에 발생한 우울증이 많은 혈관 이상과 관련이 있으며 혈관에 대한 기전은 우울증과 혈관성 치매 사이의 적절한 기전적 연관성을 제공한다 혈관성 질환의 위험요인은 알츠하이머병의 주요한 위험요인이기도 하다 연령과 더불어 알츠하이머병의 위험요인으로는 고혈압 흡연 ε 보유 허혈성 심장 질환 심방세동 증가된 콜레스테롤 과 호모시스테인 - 당뇨 비만 등이 있다 이러한 위험요인들 중 상당수는 중년의 나이에 존재할 때 알츠하이머병과 매우 강한 연관성을 가지며 이러한 연관성은 나이에 따라 변한다 예를 들어 치매 발병 전 혈압 콜레스테롤 체중이 감소되는 경향은 알츠하이머병에 밀접한 위험요인이라고 하기에는 불명확하고 단면연구에서 연관성이 없다고 나타나기도 한다 알츠하이머병과 혈관성 치매 간 공유하는 혈관성 위험요인의 존재는 이미 알려져 있는 두 질환 간 병리학적 상호작용에 중요한 의미를 지닌다 비슷한 알츠하이머병리를 가지고 있더라도 혈관성 질환이 동반된 경우에 치매의 임상적 증상이 더 심하다 지역사회 연구들에 따르면 알츠하이머병과 혈관성 치매는 신경정신적인 특징들을 공유한다 행동증상으로는 우울증 무감동 정신운동성 지연 신체증상에 대한 걱정 감정조절의 어려움 성격 변화 불안 등이 흔히 나타나고 망상과 환각 같은 정신증상 또한 나타날 수 있다 그러나 혈관성 치매에서는 우울증과 무감동 같은 증상들이 더 빈번하게 나타나며 망상과 환각 같은 증상은 드물게 나타난다 신경학적 검사 시 국소 신경학적 징후가 동반될 수 있고 보행 장애 구음 장애 삼킴 장애 등의 이상 소견이 나타날 수 있다 혈관성 치매의 다양한 특징으로 인해 임상적 결과는 매우 다양하게 나타나지만 인지능력 저하에 대해서는 알츠하이머병과 비슷한 결과를 보인다 또한 기억장애 집행기능 장애 정신운동성 지연 성격 변화와 기분 변화 등이 빈번하게 동반되므로 알츠하이머병과 감별이 어려운 경우가 있다 그러나 피질하 혈관성 치매는 전두엽의 기능장애가 두드러지고 기억력 손상은 비교적 가볍게 나타나며 신경학적 징후가 비교적 초반부터 나타난다는 차이점이 있다 또한 심혈관계 또는 뇌혈관계 요인들로 말미암아 혈관성 치매에서 사망률이 더 높게 나타나며 평균 생존율은 년으로 나타난다 임상양상전통적으로 혈관성 치매의 특징적 임상양상은 인지기능 장애의 갑작스러운 발병 계단식 악화 증상 진행의 변동성 국소 신경학적 이상 소견 및 징후 등이 있다 혈관성 치매에서의 인지기능 변화는 알츠하이머병과 같은 다른 질환들보다 훨씬 다양한데 혈관이 분포하는 대뇌 어느 부위에서건 혈관성 병변이 발생할 수 있고 영향을 받은 신경기질에 따라 달라지기 때문이다 또한 피질하 혈관성 병변이 흔하게 존재하기 때문에 전두 선조체 회로를 교란하고 주의력 정보 처리와 집행기능의 현저한 결핍을 보이지만 대뇌 병변의 분포에 따라 영역별로 뚜렷한 기능장애를 보이기도 하고 기능이 잘 보존되기도 하는 조각난 형태가 나타나는 경우가 많다 알츠하이머병을 위해 고안된 간이정신상태검사와 같은 표준화된 치매 선별검사는 앞서 열거한 특징적인 장애에 대한 민감도가 낮다 몬트리올 인지평가척도 나 혈관성 치매 평가척도 - 와 같이 주의력과 집행기능에 대한 평가를 강조한 검사들이 이러한 인구 집단에서는 더 유용하게 사용될 수 있다 기억 언어 실행과 같은 기능들은 혈관성 치매에 의해 매우 다양한 영향을 받을 수 있다 다른 치매들처럼 비인지적인 특징들도 빈번하게 나타나는데 특히 이는 환자와 가족 모두를 힘들게 할 수 있다 진단임상기준 대에 들어서 혈관성 치매에 대한 다양한 진단기준들이 등장하였다 년 등 이 제안한 - 년 세계건강기구 에서 제안한 진단기준 년 에서 제안한 진단기준 년 - 에서 제안한 진단기준 등이 있으며 현재 - 에서 제시한 진단기준이 널리 사용되고 있다 에서 제시한 진단기준은 치매와 심혈관질환 간의 인과관계에 대한 기준이 가장 엄격하고 혈관성 인지장애의 이질성과 다양성을 포용하였으며 높은 특이도를 가지고 있어 대부분의 혈관성 인지장애 연구에서 주로 사용되고 있다 에서 제시한 진단기준에서는 허혈성 질환만을 혈관성 치매의 원인으로 규정하고 반드시 영상의학적 증거와 소뇌 이외의 한군데 이상의 뇌경색소견이 존재해야 한다 의 진단기준은 비대칭적 분포의 인지기능장애와 명확한 뇌혈관질환이 존재해야 하며 가지의 아형으로 규정하고 있다 과 진단기준 모두 뇌영상소견을 요하지 않는다 서론에서 언 82

The criteria for the clinical diagnosis of probably vascular dementia include all of the following : 1.0 Dementia defined by cognitive decline from a previously higher level of functioning and manifested by impairment of memory and of two or more cognitive domains (orientation, attention, language, visuospatial functions, executive functions, motor control, and praxis), preferably established by clinical examination and documented by neuropsychological testing ; deficits should be severe enough to interfere with activities of daily living not due to physical effects of stroke alone. Exclusion criteria : cases with disturbance of consciousness, delirium, psychosis, severe aphasia, or major sensorimotor impairment precluding neuropsychological testing. Also excluded are systemic disorders or other brain diseases (such as AD) that in and of themselves could account for deficits in memory and cognition. 2.0 Cerebrovascular disease, defined by the presence of focal signs on neurologic examination, such as hemiparesis, lower facial weakness, Babinski sign, sensory deficit, hemianopia, and dysarthria consistent with stroke (with or without history of stroke), and evidence of relevant CVD by brain imaging (CT or MRI) including multiple large-vessel infarcts or a single strategically placed infarct (angular gyrus, thalamus, basal forebrain, or PCA or ACA territories), as well as multiple basal ganglia and white matter lacunes or extensive periventricular white matter lesions, or combinations thereof. 3.0 A relationship between the above two disorders, manifested or inferred by the presence of one or more of the following : (a) Onset of dementia within 3 months following a recognized stroke ; (b) abrupt deterioration in cognitive functions ; or fluctuating, stepwise progression of cognitive deficits. B.0 Clinical features consistent with the diagnosis of probable vascular dementia include the following : B.0 (a)0early presence of a gait disturbance (small-step gait or marche a petits pas, or magnetic, apraxic-ataxic or parkinsonian gait) ; (b) history of unsteadiness and frequent, unprovoked falls ; (c) early urinary frequency, urgency, and other urinary symptoms not explained by urologic disease ; (d) pseudobulbar palsy ; and (e) personality and mood changes, abulia, depression, emotional incontinence, or other subcortical deficits including psychomotor retardation and abnormal executive function. C.0 Features that make the diagnosis of vascular dementia uncertain or unlikely include : C.0 (a)0early onset of memory deficit and progressive worsening of memory and other cognitive functions such as language (transcortical sensory aphasia), motor skills (apraxia), and perception (agnosia), in the absence of corresponding focal lesions on brain imaging ; (b) absence of focal neurologic signs, other than cognitive disturbance ; and (c) absence of cerebrovascular lesions on brain CT or MRI. D.0 Clinical diagnosis of possible vascular dementia may be made in the presence of dementia (section I-1) with focal neurologic signs in patients in whom brain imaging studies to confirm definite CVD are missing ; or in the absence of clear temporal relationship between dementia and stroke ; or in patients with subtle onset and variable course (plateau or improvement) of cognitive deficits and evidence of relevant CVD. E.0 Criteria for diagnosis of definite vascular dementia are : C. (a)0clinical criteria for probable vascular dementia ; (b) histopathologic evidence of CVD obtained from biopsy or autopsy ; (c) absence of neurofibrillary tangles and neuritic plaques exceeding those expected for age ; and (d) absence of other clinical or pathologic disorder capable of producing dementia. NINDS-AIREN : National Institute of Neurological Disorders and Stroke-Association Internationale pour la Recherche et l Enseignement en Neurosciences, AD : Alzheimer s disease, CVD : cardiovascular disease, ACA : anterior cerebral artery, PCA : posterior cerebral artery 급한 바와 같이 에서 로 넘어오면서 혈관성 치매 진단에 대한 많은 변화가 일어났다 좁은 의미의 혈관성 치매 대신 넓은 의미의 혈관성 인지장애의 개념을 적용하였다 이로써 기억장애가 치매의 필수 진단기준에서 삭제되었고 치매라는 용어 대신 주요신경인지장애와 경도신경인지장애로 바뀌었다 또한 뇌혈관의 원인으로 생긴 경우에는 각각 혈관성 주요신경인지장애와 혈관성 경도신경인지장애로 진단하도록 하였다 영상정확한 혈관성 치매의 진단을 위해서는 임상적으로 보고된 인지 장애의 정도를 설명할 수 있는 뇌혈관질환의 영상학적 근거가 충분해야 한다 자기공명영상이 더 세부적으로 뇌혈 관질환의 정도 위치 영역 등을 알 수 있지만 전산화단층촬영 또한 뇌경색과 백색질 병변을 보기에 충분하다 뇌혈관질환과 치매 간의 명확한 연관성이 없다는 것은 혈관성 치매의 영상학적 진단기준을 비교하는 연구를 통해 알 수 있는데 이 연구 는 뇌졸중 생존자들 중 치매가 있는 환자와 치매가 없는 환자를 비교하였으며 이들 사이에는 유의한 차이가 없다고 보고하였다 그러나 여러 연구에서 다수의 열공 선조체 경색 이상의 백색질 변성 혹은 이들의 여러 조합이 혈관성 치매와 공존하는 것으로 나타났다 주로 피질하 혈관질환을 나타내는 백색질의 병변은 다른 비허혈성 원인에 의해서도 나타날 수 있기에 특히 주의가 필요하다 그러나 통상적으로 세 이상의 인구에서는 대부분 혈관성이 원인이며 전향적인 연구들에 따르면 백색질의 병변이 초기에는 인 journal.biolpsychiatry.or.kr 83

Supporting features awaiting further research 1.0 Relatively early gait disturbance and urinary incontinence 2.0 Extensive PV and deep WM lesions 3.0 Focal changes in electrophysiologic or physiologic neuroimaging studies Possible ischemic vascular dementia : either single stroke without temporal correlation with dementia or Binswanger syndrome 의 위축이 관찰되는데 이는 적어도 혈관성 질환과 마찬가지로 치매와도 강하게 연관되어 있음을 나타낸다 그러나 이러한 연관성은 위축이 혈관성 질환에 따른 부가적인 것인지 아니면 혈관성 변화와 퇴행성 변화의 총체적인 것인지 명확히 설명하지 못한다 알츠하이머병리에 기여하는 인자를 평가하는 데 있어 생체 영상과 아밀로이드 및 타우 에 대한 뇌척수액 표지자는 상당히 유용하다 영상학적 변화 외의 혈관성 치매에 대한 생물표지자는 알츠하이머병에서 보다 덜 발전되어 있긴 하지만 알부민 메탈로프로테이나제 염증표지자 등이 거론되고 있다 유전치매에 대한 대부분의 유전학적 연구들은 알츠하이머병에 대한 것이었고 혈관성 치매에 대한 연구는 주로 희귀 가족성 증후군에 대한 것으로 특히 번 염색체의 패임유전자 의 해독틀 이동 돌연변이와 관련 있는 피질하 경색과 백질뇌증을 동반하는 상염색체 우성 뇌동맥질환 - 에 대한 것이었으며 이는 Table 4. The ICD-10 criteria for vascular dementia G1. The general criteria for dementia (G1 to G4) must be met. G2.0 Unequal distribution of deficits in higher cognitive functions, with some affected and others relatively spared. Thus memory may be quite markedly affected while thinking, reasoning and information processing may show only mild decline. G3.0 There is clinical evidence of focal brain damage, manifest as at least one of the following : (1) Unilateral spastic weakness of the limbs ; 84 (2) Unilaterally increased tendon reflexes ; (3) An extensor plantar response ; (4) Pseudobulbar palsy. G4.0 There is evidence from the history, examination, or tests, of a significant cerebrovascular disease, which may reasonably be judged to be etiologically related to the dementia (e.g., a history of stroke ; evidence of cerebral infarction). The criteria met for major or mild neurocognitive disorder. B. The clinical features consistent with a vascular etiology as suggested by either or the following : 1. Onset of the cognitive deficits temporarily related to one or more cerebrovascular events. 2. Evidence for decline is prominent in complex attention (including processing speed) and frontal-executive function. C.0 There is evidence of the presence of cerebrovascular disease from history, physical examination, and/or neuroimaging considered sufficient to account for the neurocognitive deficits D.0 The symptoms are not better explained by another brain disease or systematic disorder. Probable vascular neurocognitive disorder is diagnosed if one of the following present : otherwise possible vascular neurocognitive disorder should be diagnosed : 1.0 Clinical criteria are supported by neuroimaging evidence of significant parenchymal injury attributed to cerebrovascular disease (neuroimaging-supported). 2. The neurocognitive syndrome is temporarily related to one or more documented cerebrovascular events. 3. Both clinical and genetic (e.g., CADASIL) evidence of cerebrovascular disease is present. Possible vascular neurocognitive disorder is diagnosed if the clinical criteria met but neuroimaging is not available and temporal relationship of neurocognitive syndrome with cerebrovascular events not established. 6 Vascular Dementia Kim TW, et al 성인의 혈관성 치매와 뇌졸중의 가장 흔한 유전적 원인으로 알려져 있다 이러한 희귀 질환들이 혈관성 뇌 허혈의 발생 기전을 이해하는 데 있어 중요한 역할을 하고 있지만 이들 질환들과 후기 발병 혈관성 치매의 유전적 요인과 관련성은 명백하지 않은 실정이다 혈관성 인지 장애와 관련된 연구들에 대한 체계적문헌고찰 에 따르면 혈관성 인지장애와 강한 연관성을 가진다고 알려진 이하 - - 이하 유전자들 중에 ε 와 만이 유의한 연관성을 가진다고 나타났다 메타분석 에서 ε 는 알츠하이머병과 심혈관계 질환 간의 강한 연관성을 보인다고 보고하였다 이러한 연관성은 혈관성 치매에 특이적인 발병기전과 치료법을 밝히는 데에는 도움이 되지 않는다 또한 선행된 메타분석들 에서 유전자 다형성 이 확인되었다 은 호모시스테인 대사와 관련이 있는 혈관성 유전자 이며 혈관성 치매가 높은 호모시스테인 농도와 관련이 있다고 여겨졌기 때문에 혈관성 치매와 직접적인 연관성이 있다고 여겨졌다 그러나 그 효과가 경미하였고 혈관성 치매에 특이적이지 않았다 혈관성 치매에 대한 유전학적 연구는 매우 부족한 실정이며 또한 혈관성 치매에 특이적인 연관성을 가지는 유전자의 발견은 이루어지지 않고 있기에 추후 많은 연구가 필요하다 신경병리뇌혈관계 질환이 병리학적 손상과 인지기능 장애를 초래하는 것이 명백하게 보일지라도 뇌혈관성 병변이 인지기능 저하와 치매에 정확히 어떠한 기여를 하는지 밝히는 것은 매우 어렵다 크고 작은 혈관의 죽상경화증 다른 혈관질환들은 피질경색 피질하 경색 아경색 허혈성병변 크고 작은 뇌내출혈을 일으킬 수 있다 이러한 병리적 변화들은 뇌 전반에 발생할 수 있고 혈관성 치매를 일으킬 수 있다 부검 연구를 통해 생전의 인지장애 정도와 사체의 병리적 소견 간의 연관성을 파악하는 것은 전향적 연구 자료를 사용한다고 하더라도 힘든 일이다 알츠하이머병의 원인들은 비교적 잘 정의되어 있으며 병의 경과가 예측가능하지만 혈관성 치매의 경우는 그렇지 않다 혈관성 뇌병변은 세 이상의 인구에서 일반적이며 경미한 인지손상뿐만 아니라 심한 치매 발병에도 기여한다 알츠하이머병이 없이 혈관성 치매가 발병하기 위해서는 광범위하고 심한 뇌혈관병변이 존재해야 하므로 순수 혈관성 치매의 유병률은 매우 낮아 보이기도 한다 뇌혈관질환 발생에 대한 부담은 나이 및 인지장애의 중증도와 함께 증가한다 그리하여 나이 및 치매의 중증도가 증가하면 다양한 뇌병변이 더 많이 존재하게 되며 순수 혈관성 치매의 비율은 감소하게 된다 이는 세 이상의 고령 인구에서 혼재성 치매가 일반적인 이유를 설명할 수 있으며 만약 인지장애와 관련된 영상학적 소견의 근거가 부족할 경우 혈관질환에 대해 고려해볼 필요가 있음을 시사한다 치료혈관성 치매의 일반적인 관리 원칙은 정확한 조기진단 동반질환의 평가 및 치료 환자와 보호자에 대한 정보 제공과 지지 환자 자립성의 극대화이다 그러나 혈관성 치매에 효과적인 치료법을 찾아가는 과정은 알츠하이머병의 경우보다 훨씬 어렵다고 알려져 있다 는 혈관성 치매의 임상 경과는 위험단계 전 치매단계 치매단계 로 나누고 위험단계와 전 치매단계에서는 예방을 치매단계에 들어서면 치료에 주력하도록 하였다 혈관성 치매의 일차예방은 위험인자를 조절하여 뇌졸중을 예방하는 것이다 이에 일차예방과 관련된 여러 연구들이 진행되었지만 일차예방이 혈관성 치매의 예방에 효과가 있다는 근거가 부족한 실정이다 과 위약을 임의로 배정하고 두 그룹의 년 후 인지장애를 비교한 연구에서 두 그룹 간의 유의한 차이를 보이지 않았으며 과 위약을 심혈관질환이나 당뇨병 환자에게 임의로 배정하고 년 후의 인지장애를 비교한 연구에서도 두 그룹 간의 유의한 차이를 보이지 않았고 각 그룹에서 가 치매로 발전하였다 낮은 콜레스테롤이 혈관성 치매를 막는 일차예방의 근거가 없음을 나타내는 결과를 보였으나 결과 측정의 민감도가 낮고 인지기능 감소의 비율이 적은 비교적 건강한 사람들이 포함된 한계점이 있었다 연구 는 항고혈압제 치료가 유의한 정도는 아니지만 치매 유병률을 감소시키는 경향을 보였고 연구 는 이 위약에 치매를 줄이는 유의한 효과를 보인다고 보고하였으나 대부분의 예방된 치매들은 혈관성 치매보다는 알츠하이머병이었다 이차예방과 관련된 연구 에서도 항고혈압제의 치매 예방에 대한 이득이 없다고 보고하였으며 항혈소판제에 대해서도 유의한 결과를 보이지 못하였다 이에 반하여 - 연구 는 고위험군의 혈관성 위험요소 제어 영양 관리 인지 훈련 운동을 병합하여 진행한 연구로 년 후 인지저하의 정도를 감소시킬 수 있었다고 보고하였다 journal.biolpsychiatry.or.kr 85

7 Korean J Biol Psychiatry 2016;23(3):80-88 현재 혈관성 치매에 대한 최선의 치료법은 콜린분해효소 억제제와 메만틴이다 이 두 약물은 알츠하이머병에 승인된 약물들이지만 혈관성 치매와 알츠하이머병이 신경병리학적 그리고 신경화학적인 요인을 공유한다는 설득력 있는 근거들이 제시되고 있어 이들 약물 사용의 근거가 되어 왔고 특히 콜린 결핍 혈관성 치매에서도 동일하다 연이은 신경화학적 분석을 통하여 순수 혈관성 치매에서는 콜린성 체계가 온전할 수 있음이 제기되었지만 혼재성 치매의 경우에는 알츠하이머병과 같은 정도의 영향을 미친다고 되어 있다 다만 콜린분해효소 억제제가 순수 혈관성 치매에는 효과를 보이지 않지만 알츠하이머병과 혈관성 치매가 혼재된 경우에는 효과적일 수 있다 초창기 갈란타민 연구에서는 순수 혈관성 치매와 혼재성 혈관성 치매 모두에서 효과를 보일 수 있다고 나타났다 그러나 잇따라 수행된 대규모의 잘 고안된 무작위대조군 연구 등에서는 일치된 결과를 보이지 않았다 대부분의 연구 결과들이 인지기능에 있어 콜린분해효소 억제제가 작지만 유의미한 이점을 보인다고 하지만 이 영향의 정도가 매우 경미하며 전반적 기능 일상생활 행동에 대해서는 일관된 결과를 보이지 못하였다 이러한 결과들 진단적 정확성과 발생 가능한 부작용들에 대한 우려를 모두 종합하여 여러 규제기관들과 가이드라인 그룹들 모두는 혈관성 치매 환자에게 콜린분해효소 억제제와 메만틴을 사용하지 않도록 결론 내렸다 이미 잘 알려진 신경화학적 기전들을 바탕으로하여 알츠하이머병의 치료를 혈관성 치매에 적용하는 것은 부적절하다는 것은 혈관성 치매에서 콜린분해효소 억제제와 메만틴의 연구를 통해 알 수 있다 현재로서는 혈관성 치매의 예방적 치료에 대한 근거가 부족한 상황이나 위험요인에 대한 다차원적인 접근을 통해 예방적 효과를 기대해 볼 수 있다 또한 공존질환에 대한 진단과 치료 혈관성 위험요인에 대한 최적의 관리 비인지적 증상에 대한 적절한 평가와 관리 환자와 보호자의 삶의 질을 극대화할 수 있도록 하는 적절한 정신사회적 그리고 여러 방면에서 적절한 지지 체계를 확고히 하는 것이 혈관성 치매에 대한 최선의 치료라 할 수 있겠다 본고에서는 혈관성 치매에 관련된 최신의 문헌들을 검토하였다 우선 혈관성 치매의 역학과 위험요인에 대해 알아보았으며 임상양상 및 로 변화된 진단기준 및 기타 진단기준 등을 정리하였다 아울러 혈관성 치매에 관련된 유전 신경병리에 대한 최신 지견을 고찰하였다 그리고 현재까지 연 구된 인지기능개선제의 치료효과에 대한 한계를 설명하였고 일차예방에 대한 대규모 연구결과를 논의하였다 향후 본고 가 현재까지의 혈관성 치매에 관한 임상적 소견을 정리하였기 에 혈관성 치매에 진단 및 치료에 더 많은 연구가 필요한 분 야를 알 수 있고 현재까지 연구결과를 바탕으로 임상현장에 서 혈관성 치매 환자에 대한 전반적 이해와 진료에 참고가 될 것으로 사료된다 중심단어 : 혈관성 치매 혈관성인지장애 인지손상 치매 치료 뇌혈관질환 Conflicts of interest The authors have no financial conflicts of interest.
