1 The Korean Journal of Gastrointestinal Endoscopy Session 3 의료현장에서 QI 활동의필요성 조유경 가톨릭대학교의과대학내과학교실 Improvement in Digestive Endoscopy Yu Kyung Cho, M.D. Department of Internal Medicine, The Catholic University of Korea College of Medicine, Seoul, Korea 질관리 ( assurance) 와질향상관리 (QI; improvement) 란무엇인가? 의료에서질은위험 (harm) 은피하고유해성은최소화하면서적절한치료효과를유지 (benefit) 하는것을말한다. 의료에서질에대한보장은 확인된효과또는효율의문제들을측정을통하여의료의정량화를이루고, 그결과로이런문제들을해결하고측정을반복하여결과적으로의료의질이향상되어야한다. 질향상관리 (QI; improvement) 는전통적인질관리 (QA; assuarance) 와차이점이있다. 전통적인질관리 (QA) 가질적측정에있어표준에서이탈하거나, 적정수준을초과또는미달하는소수의개인의관리에중점을두고있다면, 질향상관리 (QI) 란업무를수행을모든사람들의수행능력을향상시키는데중점을준다. 즉 QA와비교되는 QI의중요한요소는 1) 질적지표에의해고정된기준치를정해놓지않고지속적인향상을꾀하며 2) 의료소비자와의료공급자모두에게초점을맞추고 3) 적정수준이하의소수를알아내기보다모두를위한과정과결과를개선하는것이다. 1 의료현장에서 QI 는왜필요한가? 1) 모든의료인과의료기관의목표와윤리는결국양질의의료서비스를제공하는것이다. 2) 의료소비자인환자의식이변화하였다. 환자는내시경의사의수준, 환경과시설, 편리함, 접근성등여러요소에대한정보를획득하고종합하여최종적으로의료기관을선택한다. 3) 병원경영의효율화, 즉효율을향상시키고비용을절감하는것이다. 4) 의료기관간의경쟁체제가도입되었다. 5) 정부의보건의료정책의한분야이다. 그렇다면내시경분야에서의 QI는왜필요한가? 다음의여 러이유들을들수있을것이다. 1) 진료의전반적인질을향상시키기위해 2) 타당하지않은시술을제한하기위해 3) 사망률이나질병유발율을제한하기위해 4) 내시경교육을향상시키기위해 5) 환자의불평과소송을제한하기위해 6) 의료수가를얻거나높이기위해위와같은여러가지이유로소화기내시경분야의 QA 또는 QI는미국, 영국등일부선진국에서는이미의무사항이다. 우리나라도보건산업진흥원주관의의료기관평가, 보건복지부와국립암센터주관의국가암조기검진사업내시경질평가사업이시행되고있어가까운장래에의무적으로될것이다. 그러므로의료비용을제공하는환자와정부의질보장압력에있어서먼저의사들이주도적으로질향상계획을개발, 실행하여야할것이다. 질평가의접근현재의의료의질에대한적절한평가 (quality assessment) 는질관리및질향상관리의전제조건이된다. 보건진료분야의질관리에서많이이용되는 Donabedian 2 의논문에의하면의료의질평가를위한접근법 (approach to quality assessment) 을구조, 과정, 결과의세범주로나누어제시하였다. 내시경의예를보면구조는내시경시설및장비, 의료인력이며과정은실제의내시경시술, 결과는내시경시술에의해변화된건강상태라고할수있다. 질관리는이런진료의세분야에서검토되어야한다. 훌륭한구조는훌륭한과정의가능성을증가시키고훌륭한과정은훌륭한결과의가능성을증가시킬것이다. 소화기내시경의질관리는내시경을시행하는의사의질관리와내시경실 (endoscopy unit) 의질관리로나누어생각해볼수있을것이다. 본문에서는특히내시경의사의질관리에초점을맞추어 2009년 OMED (Organisation Mondiale d'endos- Vol. 40 (Suppl 1), 2010 (25-29) 25

2 copie Digestive, World Organisation of Digestive Endoscopy) 에서제정, 발표된 소화기내시경에서자격부여와질보장에대한 London OMED 성명 (The London OMED position statement for credentialing and quality assurance in digestive endoscopy) 3 을중심으로리뷰하고자한다. 11) 내시경결과를문서화하고결과에대해환자와토의할수있어야한다. 12) 핵심적인성과지표를유지하여야한다. 내시경시술의수련과질보장 내시경의사의자격과권한의부여 의료기관은시술할능력이있는내시경의사에게만시술할권한을부여할책임이있다. 역량또는시술능력 (competency) 이란최소수준이상의기술과지식을가지고수련과경험을통해안전하고능숙하게지도감독없이시술을시행할수있는전문성을의미한다. 4 의료기관은역량을가진의사에게특정내시경시술을할수있는권한을부여한다. 임상권한은각내시경시술마다다르다. 예를들어내시경초음파의시술할권한과소장캡슐내시경을할수있는권한을의미하지않는다. 의료기관은임상권한을부여하는원칙이있어야하며이는적절한가이드라인을따라야한다. 역량있는내시경의사를만드는수련에는다음사항이요구된다. 1) 내과또는외과수련을마치고내시경을임상진료에적절히적용할수있어야한다. 2) 내시경의적응증, 합병증, 시술에따른위험도를이해해야한다. 3) 환자에게위험을비롯한시술전반을쉽게설명하고동의서를획득할수있어야한다. 4) 내시경해부학을이해하여야한다. 5) 내시경과부속기구의특징과안전에대해인지하고소독에대한지식을습득하여야한다. 6) 내시경검사결과를이해하고해석할수있어야한다. 7) 진정마취제의약리적특징을이해하고위험성을인식하여야한다. 8) 조직검사와보편적인치료시술을할수있어야한다. 9) 합병증을빨리진단하고관리할수있어야한다. 10) 내시경기술이나시술능력의한계를인지하여야한다. 특정시술을할수있는능력을측정하기위한객관적인지표가있어야한다. 예를들어내시경시뮬레이터는유용한수련도구이지만적절한수련을받았다는평가도구는될수없다. 미국소화기학회수련위원회, 유럽소화기학회에서는모두수련받은의사가지도감독없이시술을시행할수있는시술의최저수를정하는권장사항을만들었다. 각학회에서권장하는시술의최저수 (threshold numbers) 를 Table 1에표시하였다. 3 이숫자는의사가특정시술을할능력 (competency) 을갖추었음을평가할수있는수준에이르렀다는것을말하며임상권한을부여하는것에사용되어서는안된다. 5 다른면으로는시술의최저수 (threshold number) 에이를때까지는지도감독하 (supervised) 에시술이이루어져야함을의미하기도한다. 질관리지표 ( indicator) 의결정 현재의질수준을평가하기위해서는우선질관리지표를결정해야한다. 그렇다면무엇을질관리지표를삼을것인가? 적절한질관리지표는다음조건이있어야한다. 1) 환자가얻는성과와연결되어야하며 2) 자료를수집하기비교적용이해야하고 3) 환자기록의일부로서이미수집된정보를기술할수있어야하며 4) 전자데이터베이스를이용하여수집을할수있어야한다. 내시경분야의적절한질지표는시술의수준을고려하여야한다. 예를들어직장내시경은위험도가낮으므로주된합병증이드물다. 이런경우부작용의모니터링이나합병증의발생률만으로는안전도와시술능력을확인하기어렵다. 높은질의내시경서비스는특정한결과시점이있으므로이를질관리지표로사용될수있다. 내시경의질지표는일반적질지표와 Table 1. 3 특정시술의질지표로나눌수있다. 특정시술의질지표는특정시술의성공률, 합병증, 환자의만족도등을말한다 년 OMED에서적절한질관리지표로추전한일반적질지표와 특정시술의질지표를 Table 2 로정리하였다. 3 영국은 Joint advisory group (JAG) 이결성되어내시경질관리를담당하고있다 ( 아래설명 ). 2004년부터영국의료기관의 Table 2. Global rating scale에제시된질지표와안전성지표의표준을내시경시술별로 Table 3 5에정리하였다. 내시경보고서 내시경보고서의완성도자체로질을측정할수있다. 미국소화기내시경학회에서는내시경보고서는다음의내용을포함할것을권장하고있다. 6 1) 시술일, 2) 환자의신원, 3) 시술의, 4) 내시경보조자, 5) 관련된병력과신체검사, 6) 동의서획득유무, 7) 내시경시술명, 8) 시술적응증, 9) 내시경기기, 10) 시술중사용한약제, 11) 검사가시행된범위 12) 검사의한계와장정결의정도, 13) 조직검사유무, 14) 내시경소견, 15) 내시경진단, 16) 시행한내시경치료, 17) 치료의결과, 18) 합병증, 19) 합병증의결과, 20) 합병증을처리한결과 ( 예, 입원 ), 21) 향후계획 내시경실의질향상과내시경실인증제 내시경의사뿐아니라간호사, 관리자, 및기타의료지원인력이모두관여하는내시경실의질향상이중요한과제이다. 내시경실은지속적으로교육및직원의역량을평가하여야한다. 내시경장비및시설유지및보수, 환자모니터링및평가, 적절한의료인력및자원의배정, 대기시간의축소, 내시경방및직원회전율, 환자만족도, 시술의사후관리, 투약관리, 환자의회복, 장비소독과감염의예방등이질관리에포함된다. 7 일부선진국에서는다양한내시경실인증제도가있어내시경실의질관리사업이시행된다. 몇가지예를살펴보면미국에서대다수내시경실을다양한인증제도를통해인증을받았고일부지역에선의료기관개설을필수사항으로규정하는곳도생기게되었다. 미국에서는내시경검사를대상으로한인 증제는세종류가있다. 이들은각각 the Accreditation Association for Ambulatory Healthcase (AAAHC), the Joint Cmmision (OC, JCAHO), the American Association for Accreditation of Ambulatory Surgery Facilities (AAAASF) 로불린다. 이들인정기관은인증을받으려는기관이서면으로인증심사를신청하면주기적으로이미알려진기준에대한현장실사를하고인증하는형식을취한다. 인증기준은각기관에따라다르지만일반적으로구조와과정에초점이맞추어져있다. 인증받는기관의적절한감독여부, 충분한시설과장비, 적절한수련기회의제공, 확립된정책과안전하고높은수준의내시경행위가가능하도록하는확립된원칙이있는지여부를판단한다. 영국에서는내시경에관한 Joint advisory group (JAG) 이결성되었고내시경시술자수련에대한기준을제정되어있다. 8 JAG는내시경수련과교육에필요한기준을만들고시술자의기준, 내시경수련기준, 수련에필요한내시경질관리, 수련과정의질관리, 우리나라보험공단과유사한 National health Services (NHS) 에서시행하는암검진의질관리를위해다양한기준을작성하였다. 영국의모든내시경실은 2004년부터 Global rating scale이라는규정을지키게되었고이평가기준은시술뿐아니라환자와관련한평가기준을포함하고있는데이기준은진료의질과안전성을판단하게되어있다. 모든내시경실이인터넷을통해작성한후매년이결과를공표하고있다. 2004년부터 2008년까지평가한바에따르면다양한평가지표가시간이갈수록좋아지고있어좋은평가모델로여겨지고있다. 질향상프로그램의시작 질향상을위한노력은구체적으로다음의단계를거쳐진행된다. 1) 주된지표들의확인 2) 우선순위의결정 28 The Korean Journal of Gastrointestinal Endoscopy
