In patient with dextrocardia as a part of situs inversus, coronary heart disease may occur with similar frequency and manifestation as in the general population. A case is reported of angina and thyrotoxicosis in patient with situs inversus totalis in which the angina was manifested by ST-segment elevation during graded exercise, relieved by antithyroid treatment. 신체검사상 특이한 소견은 없었으며 양 아침에 가슴이 답답한것같은 우측의 흉부 불쾌감이 있 측 폐음은 깨끗하였고, 수포음은 들리지 않았다. 심음 었고 그때마다 오른쪽 팔로 방사하는 방사통이 있었으 은 오른쪽 흉부에서 가장 강하게 청진되었는데 규칙적 며 내원 1주일전부터는 매일밤과 아침으로 같은 증상 이었고, 심잡음은 없었다.

Plain abodomen showing right sided gas shadow in stomach and left sided liver. 치료한 적이 있으며, 내원 1개월전부터 주로 저녁이나 36.7 C였다. 신체검사상 특이한 소견은 없었으며 양 아침에 가슴이 답답한것같은 우측의 흉부 불쾌감이 있 측 폐음은 깨끗하였고, 수포음은 들리지 않았다. 심음 었고 그때마다 오른쪽 팔로 방사하는 방사통이 있었으 은 오른쪽 흉부에서 가장 강하게 청진되었는데 규칙적 며 내원 1주일전부터는 매일밤과 아침으로 같은 증상 이었고, 심잡음은 없었다. 검사실 소견에서 말초혈액 이 있었다. 과거 병력에서 8년전 갑상선기능항진증으로 검사상 혈색소치는 12.7 g/dl, 적혈구 용적은 43.2%, 백 진단받고 불규칙적으로 항갑상선제제를 개인 의원에서 혈구는 6,200/mm3, 혈소판은 336,000/mm3 였다. 일 치료하고 있었다. 사회력상 흡연이나, 음주력은 없었고, 반 생화학 검사상 간기능검사나 신장기능검사 및 콜레 가족력상 특이소견은 없었다. 내원당시 혈압은 110/ 스테롤 수치는 정상범위였다. 갑상선기능검사상 T3는 85 mmhg, 맥박은 76회/분, 호흡수는 24/분, 체온은 ng/dl(정상범위 까지), FT4는 Korean Circulation J 1999;29(11):

다. 단순 흉부 방사선 소견상(Fig. 2) 심장이 오른쪽으 로 위치한 우심증을 보였으며, 단순 복부 촬영(Fig. 3) 상 간과 위의 가스 음영이 반대로 위치하여 내장역위임 을 알 수 있었다. 경흉부 심초음파상 심장은 특이한 이 상소견은 없었다.

유발을 시도하였으나 심전도에서 변화는 없었으며, 흉 통도 호소하지 않았다. Modified Bruce protocol로 stage Ⅱ로 진행 중에 환자가 갑자기 심한 흉통을 호 소하고 심전도상(Fig. 5) 여러 유도에 걸쳐 심한 ST절 의 상승을 보였다.

환자는 갑상선기능항진증과 내장역위를 가지고 있으면서 허혈성 심질환이 있는 경우로 생각하 고 관동맥 조영술을 권하였으나, 거절하여 현재 nitrate제 제와 칼슘길항제 및 항갑상선 제제만 복용하고 있으며 현재 5개월간 경과 관찰중에 흉통은 전혀 없는 상태로 Fig. 5. Electrocardiogram during exercise showing the diffuse ST elevation.

3 다. 단순 흉부 방사선 소견상(Fig. 2) 심장이 오른쪽으 로 위치한 우심증을 보였으며, 단순 복부 촬영(Fig. 3) 상 간과 위의 가스 음영이 반대로 위치하여 내장역위임 을 알 수 있었다. 경흉부 심초음파상 심장은 특이한 이 상소견은 없었다. 흉통의 감별진단을 위해 운동부하 심 전도 검사를 시행하였는데, 환자의 전흉부 유도를 오른 쪽으로 반대로 부착하고 양팔의 유도를 바꿔달고 시행 하였다. 운동 시작전의 기본 심전도(Fig. 4)는 비특이 적인 ST-T절의 변화이외에는 다른 이상은 없었다. 운 동 시작전에 과호흡을 약 30초간 시켜 관동맥 연축의 Fig. 4. Modified Bruce protocol로 stage Ⅱ로 진행 중에 환자가 갑자기 심한 흉통을 호 소하고 심전도상(Fig. 5) 여러 유도에 걸쳐 심한 ST절 의 상승을 보였다. 즉시 운동을 중단하고 Isoket spray 를 입안에 뿌리고, 경과를 관찰한 결과 약 3분후에 흉 통이 소실되고, 심전도상(Fig. 6)에 ST절의 상승의 소 실을 보였다. 환자는 갑상선기능항진증과 내장역위를 가지고 있으면서 허혈성 심질환이 있는 경우로 생각하 고 관동맥 조영술을 권하였으나, 거절하여 현재 nitrate제 제와 칼슘길항제 및 항갑상선 제제만 복용하고 있으며 현재 5개월간 경과 관찰중에 흉통은 전혀 없는 상태로 Fig. 5. Electrocardiogram during exercise showing the diffuse ST elevation. 외래에서 추적 관찰중이다. 고 안 협심증은 갑상선기능항진증 환자에서 약 20%에서 발생하는데, 대부분 관동맥 죽상종과 교감신경계의 항 진으로 일어난다.7) 갑상선 호르몬이 심장에 작용하는 기전으로 첫째, 세포에 직접 영향을 미치거나, 둘째, 교 감신경과의 상호작용에 의해서, 셋째, 말초 순환과 에 너지 대사에 영향을 미쳐 심장에 작용을 한다고 한다.8) 갑상선 기능항진증은 기존의 심장질환을 악화시킬뿐만 Fig. 6. Electorocardiogrma during recovery phase showing the loss of ST elevation. 아니라, 그 자체가 심장질환을 초래한다.2)9)11) 갑상선 기능항진증에서 협심증의 기전으로 제시되고 있는 것 은 기존의 동맥경화성 질환이 있는 경우 산소요구량이 ng/dl(정상범위 까지)로 증가되어 있었고, 증가하거나,7) 일차적으로 동소발생부위(in situ)의 관 TSH는 0.01 uiu/ml(정상범위 0.4 4까지)로 심하게 동맥의 혈전이나12) 갑상선 호르몬으로 인한 직접적인 감소되어 있었다. 심전도(Fig. 1)상 유도 I에서 P, QRS, T 심근의 장애,13-15) 소동맥 질환16) 이나, 관동맥 연축17-19) 파가 모두 역위된 소견과 전흉부 유도에서 QRS파의 등이다. 협심증의 대부분은 기존의 관동맥질환이 있던 진행이 떨어지는것으로 전형적인 우심증의 소견을 보였 경우이지만, 드물게는 관동맥조영술상 정상인 경우도 있 1247
