Some bacterial organisms were identified as etiologic agents, including, Micrococcus species and Dermatophilus congolensis. However, in Korea, studies to prove the causative organisms have not been performed. Objective: We performed this study to identify causative organisms of pitted keratolysis in Korea. Method: Twelve normal healthy men and 27 pitted keratolysis patients were enrolled. We cultured the scraped specimens of the stratum corneum and identified the cultured organisms. We compared the cultured organisms of pitted keratolysis group with those of control group. We also compared the distribution of cultured organisms in pitted keratolysis with and without tinea pedis. Result: Micrococcus species and were identified in pitted keratolysis group much more frequently than in normal control group. In most cases of pitted keratolysis combined with tinea pedis, the identified organisms were Micrococcus species. Conclusion: Micrococcus species and are thought to be the major causative organisms of pitted keratolysis in Korea. Micrococcus species might play a certain antagonistic role, especially in patients of pitted keratolysis with tinea pedis. 2 김범준등 : 소와각질융해증의원인균연구 서론 소와각질융해증 (pitted keratolysis, 이하 PK) 은발바닥의압박부위에주로발생하는표재성세균감염질환으로, 분화구모양의작은구멍을특징으로한다. 대부분증상이없지만종종가려움증이나화끈거림또는통증이동반되기도한다 1, 년대에 Keratoma plantare sulcatum 으로처음명명되었으며 1930 년대에는 Keratolysis plantare sulcatum 로불리다가, 이후원인균과병리기전이밝혀지면서현재의명칭으로재명명되었다 1. 해외의보고에따르면대표적인원인균으로 Micrococcus species, Corynebacterium species, Dermatophilus congolensis 의세균주가알려져있으며, 이들이땀에의해부드러워진각질층을침범하여각질층을녹이는효소를분비하여구멍을만드는것으로알려져있다 1~3. 국내에서소와각질융해증의원인균에대한연구는아직까지보고된바없으며, 이에저자들은한국인의소와각질융해증의원인균의종류와이들의분포를확인하고자본연구를시행하였다. 재료및방법 1. 연구대상 2005년 8월 1일부터 2005년 11월 30일까지서울대학교병원, 중앙대학교용산병원, 분당서울대학교병원, 서울시립보라매병원, 국군수도병원, 해군포항병원의피부과를내원한환자중소와각질융해증으로진단된환자 27명을대상으로균주배양검사를시행하였다. 대조군은피부질환이나내과적질환이없는건강한자원자 12명의발에서균주배양검사를시행하였다. 울대학교병원진단검사의학과배양실로이동시켰다. 배양실에선검체를직접 blood agar 및 brucellar agar에접종하여이틀간호기성및혐기성배양을시행하였다. 균의동정은환자 1인당 3~4종의균이분리되었고, 균동정에는 Vitek Identification System (Biomerieux, France), BBL Crystal Identification System (Becton Dickinson, MD, USA) 및생화학적수기검사가이용되었다. 3. 통계분석 PK 환자와정상인에서동정된균의종류를비교하여 PK의원인균으로추정되는균주를확인하였다. 그리고 PK 단독이환자와 PK와족부백선이동시이환된환자에서동정된균을비교하여, 족부백선이병발하는경우 PK의원인균분포가어떤차이를보이는지확인하였다. 이를위해 SPSS version 7 프로그램을이용하여 Fisher s exact test를시행하였다. 결과 조사대상자 39명은모두남자였고, 연령은 20세에서 27세로평균 23세였다. 유병기간은최소 1주부터최대 3개월로평균 1.3개월이었다. 대조군에서는 S. epidermidis가가장많은수에서동정되었으며전체 12명중 8명에서동정되었다. 다음으로 S. aureus, S. warneri 가각각 3명에서동정되었으며, 그외 S. simulans, S. lugdunensis, S. saprophyticus, S. cohnii sp cohnii 등이소수에서동정되었다. 대조군중 PK의원 2. 연구방법 환자의성별, 나이, 발생부위, 동반질환을조사하여기록하였다. 환자들중본연구에참여할의사가있는환자들만을대상으로균주배양검사를시행하였다. 우선발의병변부를생리식염수와거즈를이용하여세척한후 15번블레이드 (blade) 로각질을긁어서소독된 Cornical tube (Falcon, USA) 에담아분당서 Fig. 1. Fig. 2. Cultured organisms of pitted keratolysis in Korea. 많은수에서동정되었으며전체 27명중 18명에서동정되었다. 이들 18명중 13명에서는 Micrococcus species가단독으로동정되었으며, 5명에서는 Micrococcus species와 가함께동정되었다.

Micrococcus species가동정된비율은 PK 환자군에서 66.7% 로대조군의 8.3% 와비교하여통계적으로유의하게높았으며 (p = 0.001), 가동정된비율도 PK 환자군에서 33.3% 로정상인의 8.3% 와비교하여높았지만이는통계적으로유의한수준은아니었다 (p = 0.131). PK 환자군중족부백선이함께있는경우와없는경우의균주분포를비교한결과, 족부백선이있는환자군에서는 Micrococcus species 단독감염이전체 7명중 6명으로가장많고 단독감염은 1명이며두종의혼합감염및기타균의감염은없었던데반해, 족부백선이없는환자군에서는 Micrococcus species 단독감염이전체 20명중 7명, 단독감염이 3명, 두종의혼합감염이 5명, 기타균의감염이 5명으로다양하게나타났다 (Fig. 3). 고 찰 Fig. 3. PK 환자군에서는 Micrococcus species가가장 PK의원인균은 1967년 Zaias 1 등이 Corynebacterium 을증명한이래로 Micrococcus species, Dermatophilus congolensis 등이밝혀져왔다. 이들원인균은공통적으로각질분해효소나단백분해효소를생산한다는특징을가지고있으며, 이효소들의작용으로 PK의특징적병변인소와가형성된다. 국내에서는현재까지 PK의원인균에대한연구가없었으며외국의자료로미루어추정만해왔을뿐이다. 본연구를시행하기에앞서 PK의균주를배양하기위해여러가지다양한시도를하였다. 우선세가지접종방법을시도하였으며, 검체를배지에직접접종하는방법, 멸균식염수에불린후접종하는방법, 멸균식염수에불린액을여과하여접종하는방법을시행하였고, 이중검체를

(B) Cultured Micrococcus luteus and (C) light microscopic examination. 직접접종후배양하는방법이가장정확도와재현성이높아서본연구의표준검사로채택하였다. 본연구결과, Micrococcus species와 Corynebacterium species가대조군과비교하여 PK 환자군에서통계적으로유의하게더많은비율로동정되었으므로, 이두균주가외국에서와마찬가지로국내에서도 PK의원인균이되는것으로생각할수있다. 한가지특이한점은, 외국에서 Micrococcus species, Corynebacterium species와함께 PK의대표적원인균주의하나로알려져있는 Dermatophilus congolensis는동정된예가없었다는것이다. PK 환자군중족부백선이함께있는환자군에서는 Micrococcus species 단독감염이전체의 85.7% 로대부분을차지하였으며, 혼합감염은없었고, 단독감염만확인되었다. 그러나이에반해, 족부백선없이 PK에만단독으로이환된환자군에서는 Micrococcus species와 의단독또는혼합감염이다양하게나타났다. 이로미루어볼때족부백선의원인균주인진균과 Corynebacterium 사이에길항작용이있는것이아닌지의심해볼수있었다. 그러나 본연구의모집단의크기가충분하지않았고, 균주간의상호작용에관한실험이병행되지않았으므로, 좀더추가적인연구가필요할것으로생각된다. 앞으로여러병원의협력을통해소와각질융해증에대한본격적인연구가진행되길기원한다. 결론 소와각질융해증으로진단된환자 27명과건강한자원자 12명을대상으로균주배양검사를시행한결과다음과같은결론을얻을수있었다. 국내에서발생하는소와각질융해증의원인균은 Micrococcus species와 가주요균주로추정되며이들균주들이단독혹은혼합감염의형태로다양하게존재한다. 소와각질융해증과족부백선에함께이환되는경우, 원인균은 Micrococcus 단독인경우가대부분이었다. 감사의글본연구의수행을위해아낌없는지원과협조를해
