Understanding of the mechanism of those injuries is important for the radiologist to give more information to the orthopedist to identify ligament injuries and then to fix the fractures. 1 n 1948, Lauge-Hansen devised a classiti 딩 tion of ankle fractures based on the position of the foot and direction of injuring forα at the time of injury. On the basis of location and appearance of the fibular fracture, 4 typt> '"'. ìnjuries are identified; pronation-abduction (PA), pronation-external rotation (PER), supination-adduction (SA), and supination-external rotation (SER). 60 cases of ankle fractures treated at Kyu ng Pook National U niversity Ho spital from J an to Dec were analized and classified. The results are as follows: 1. Male was more frequently affected than female and the sex ratio was 4 : The most commonly affected age group was 20 s. 3. R ight side wasmore frequently affected than left. 4. A mong 4 types, the mostωmmon was SER type (33.3%) and next were PER (30.3%), SA(16.7%}, and PA (13.3%) types. 1. 서론 각종사회활동의증가에따라인체각부위의골절상은임상에서흔히접하는외상성질환의하냐가되었다. 특히족관절은체중의약 5 배까지하중을겨디고보행에중요한역할을하며, 그기능면이나해부학적위치로인하여외력에대해쉽게손 %L을받을수있다특히골절시관절연의침범및주위의인대손상이유발되기쉽이논문은 83 년 10 월 24 일에채택되었음 다. 따라서족관절팔절은정확한진단과적절한치료가 이루어지지않으연그손상정도에비해 심한기능의 장 애가나타날수있다. 족관절골절의정확한진단을위해서는외상기전을아 는것이중요하나불행하게도대부분의환자가손상시의 급박한상황때문에이를장설명할수없다. 그러므로 방사선학적소견으로외상기전을추정하는것이 골절의 치료에매우중요한일이다. 그러냐지금까지는족관절골절의 x- 션상판독시단 순히해부학적골절의부위와골절형의명시에만그쳐임상가의치료에별도움을주지못한것같다.

2 저자을은비교적전형적인족관절골절 선학적소견으로그외상기전을추정하여 en 의 60 예를방사 Lauge - Hans' 분류 1,2,3,4) 플중심으로분류해봄으로써몇가지 지견을얻었으므로운헌고찰과함께보고하는바이다. 저자들은 1973 년 1 월부터 1982 년 12 월까지 10 년 동안경북대학교의과대학부속병원에서업원및 통원 가료한족관절부위골절환자 183 예를겸토하여이중 x - 선상과임상기록부등의자료미바로검토불가능 한환자와족관절부위의다발성내지는복합골절로외 상기전의추정이불가능한교통사고환자를제외하고층 대상환지는 60 예이었다. x- 선장은족관절의전후상, 측면상및 Mortise View 를시행하였 며원위경비관절이개및 불안정성이 의심되는환자에서는 Stress View 및양측사변촬영을 같이이용하였다. 골절상의분류는 L auge- H 뻐 sen 의분 류 1, 2, 3,4) 에따랐다. 이들의 분륜를요약하면다음과 같다 II. 연구대상 Supination - Adduction (SA) Stage 1 : Transverse fracture ()f the lateral malleolus or tear of the lateral collateralligament. Stage 11 : Plus vertical or oblique fracture of the media1 m a11eolus. Supination - External Rotation (SER) Stage 1 : Rupture of the inferior anterior tibiofibular ligament. Stage II : Plus obliqe spira1 fracture of the lateral ma11eolus. Stage III: Plus fracture of the posterior lip of the tibia. Stage IV: Plus transverse or oblique fracture of the medial malleolus or tear of the deltoid ligament. Pronation - Abduction (PA) Stage 1 : Transverse fracture of the medial ma11eolus or.tear of the deltoid ligament. Stage II : Plus rupture of the anterior and posterior tibiofibular ligaments and inferior transverse tibiofibular ligament with fracture of the posterior lip of the tibia (The interosseous ligamentremains intac.) Stage III: Plus an oblique supram a11eolar fracture of the fibula remains intact.) Pronation - External Rotation (PER) 및방법 (The interosseous membrane Stage 1 : Transverse fracture of the medial mallelous or tear of the de1toid ligament. Stage 11 : Plus tear of the inferior anterior tibiofibular and interosseous ligaments. Stage 111: Plus tear of the interosseous membrane to the level of a spiral fracture of the fibula 7 to 8 cm or more proximal to the tip of the la teral m a11eolus. Stage lv: Plus avulsion fracture of the posterior lip of the tibia at the attachemnt of the posterior inferior and inferior transverse tibiofibular ligament. 대상환자의연령은 8 세에서 성적 62 세까지였으며 20 세 에서 29 예까지가 20 예 (33_3 %) 로가장많았으며, 활 동이왕성한연령층인 20 세에서 39 세까지가 3 4 예 ( 56 _6 %) 로전체의절반이성을차지하였다. 남녁의비율은남자가 48 예 (80 %). 여자가 (20 %) 로남녀의비는 4 : 1 로남자가많음을보여 었다. (Table 1). 골절부위는총 60 예중우측이 12 예 ;ζ T 37 예 (61. 7%) 이고 좌측이 2.3 예 (38.3 % ) 로우측이많았으며양측성은없 었다. 1J 연령및성별분포 Age m. 연구 Table 1. Distribution of the Ankle Fractures II III IV Total (%) SA (1 6.7) SER (33.3) PA (13.3) PER (30.0) U nc1assified 4 ( 6.7) 60 (1 00.0) mortise View 및양측사면촬영도추가시행하였다. 측면상은중족골 (metarsal bone) 의기저부 (base) 를반드시포항하여촬영하여야한다 6) 그이유는족관절손상시 5 번째중족골과거골 (talusj 의골절이일어나서족관절골절과혼돈을일으키기쉬우므로이를막기위함이다. 족관절골절의분류와치료에중요한역할을하는방사선학적소견은첫째, 비골골절의부위, 형및방헝이고 7 ) 풀째, 내측과골절 (med ial malleolar fracture) 과삼각인대 (deltoid lig려nent) 의파열유무이며셋째, 후경플골절 (posterior tibi혀 fracture) 의유무및겸비인대결합부 (tibiofibular syndesmosis) 의파열파열유무 * SA : Supination-Adduction SER : Supinatio-External rotation PA : Pronation-Adbuction PER : Pronation-External rotation IV. 고 잘 2 족관절은체중을부하하는관절로서역학적으로중요 한역할을하는경첩관절 ( 비nge joint J 이다. 족관절골절의진단에있어서골절시발의위치와가해진힘의방향을아는것이중요하며 1,2,5) 족관절골철의치료 Supination-adduction type. Stage 1, 11. 에있어서골절기전이매우중요한위치를차지한다. 불행하게도거의대부분의환자가골절시힘의방향과발의위치를찰모론다. 족관절골절의분류는 1948 년부터 Lauge -벼nsen 에의해서힘의방향과발의위치에의해분류가되었다. 3 그러나방사선학적소견만으로정확한 Lauge - Hansen 분류를할수없는경우도있으나 1980 년에 Arimoto Howard K 와 Forrester D. M. 에의해서 Lauge - Hansen 의분류를중심으로하여방사선학적소견만으로족관절골절을분류하여보고하였다 5) 이들분류의 Supination-externa1 rotation type. 목적은골절의방사선학적소견만으로골절기전을조속 S.tage 1, 11, 111, 1 V 히알아서임상적으로골절치료에도움을주기위함이며, 그정확도는 90 % 내지 95 % 로보고하였다. 족관 Fig The 4 types of the ankle fracture. 절골절에서골전기전을추측함으로써골절과수반되어일어나는골절부위의인대손상을추측할수있으며명비골골절시손상기전에따라골철형이다르므로골확하지않은골절을더관찰하여혹은추가로다론검절분류가다르다예를들면회외 -외회전형시는저비사방법으로정확한골절을찾아볼수있다. 골골절 ( Low fibular f ract ure : 외측과의끝에서 7 cm 족관절골절시사용되는사진은전후상과측면상사이하에일어나는콜절 ) 에서나선골철이고회내 -외회전 쩌 진을필수적으로사용하였고핑요에의해 Stress View 형은횡골절이다 5,8)

Posterior tibial fracture is seen. 부는족관절사진 %L에서는보이지않는수가 있으므로 회내-외회전형의 Stage III 혹은 Stage N가 의심될 시는비골의근위부가나오도록촬영하여야한다. 회외 -내전형은외측과의견인골절 (Traction fracture) 이 일어나며, Stage II 에서는특징적인내측과골절이 거 의수직에가까운수직골절이 온다. 회외 - 외회전형은 비골의골절선 (fracture line) 주행이전하멍에서 후 상방으호달리는형이고 IS)(Fig. 3), 회내 - 외회전형은 전상 $L에서 후하방으로달리는모양이며 이것은측면 상사진에서잘보인다. 저자들이족관절골절을분류한결과는회외 - 외회전 형의빈도가가장많았으며이는 Lauge -Hansen 과 Burwell H.N. 등 16) 의보고의빈도와비슷하며권고웅 등 17) 정태영등 18) 및한창동등 19) 의보고에는회 내 - 외회전형의빈도가가장많다고보고하였다. 이는 교통사고환자의수에의해서영향을받는것으호사료 된다. V. 결 론 1973 년 1 월부터 년 12 월까지의 10 년동안경 북대학교의과대학부속병원방사선학교설에서족관절 골절 60 예에대한 Lauge - Hansen 의분류에 기초를두 고방사선학적소견만요로분류한결과는회의 -외희전 형이 20 예 (33.3 %) 로서가장많았으며그다음이 회 내 - 외회전형 18 예 (30.0 % ), 회외-내회전형 10 예 (1 6.7 % ), 회내 - 외전형 8 예 (13.3 %), 및 미분류형 4 예 (6.7 %) 의빈도순으로나타났다. 연령층은 20 세에서 29 세사이가 20 예 (33.3 %) 로가장많았으며 20 세에 서 39 세사이가 34 예 (56.6 %) 로전체의절반이성을 차지하였다. 족관절의해부학적중요성과족관절골절의분류가치 료에중요한역할을함으로족관절골절의판독시 학적골절부위와골절형의영시에중점을두고 는것보다 Lauge - Hansen 의분류를사용하는것이 해부 판독하 절시의손상기전을이해하고족관절골절과방사선학 적소견이잘안보이는인접부위의 골 인대손상을추측 할수있으므로신속하고적절한치료를하는데 가에게더도움을줄수있다. 5.
