We compared the preoperative stress radiographs with the intraoperative radiographs under anesthesia to determine the accuracy and efficacy of stress radiographs in an outpatient clinical environment. Materials and Methods: Data was collected from patients who underwent a modified Broström operation for painful chronic unilateral lateral ankle instability between January 2014 and June Subjects were divided into three groups complete tear, partial tear, and instability without rupture according to the status of preoperative MRI findings of the anterior talofibular ligament. The anterior drawer and varus stress radiographs were taken preoperatively and intraoperatively under anesthesia. Results: Ninety-six patients, with a mean age of years, were enrolled. There were 39, 46, and 11 patients in the complete tear, partial tear, and instability without rupture groups, respectively. On the anterior drawer and varus stress radiographs of the affected limb, talar anterior translation and varus tilting were significantly increased by 2.56 mm and 2.0. The gaps between the unaffected limbs were also increased by 2.47 mm and 1.32 after anesthesia. Although the stress radiographs were taken under anesthesia, the results were often smaller than the diagnostic value. Conclusion: Stress radiographs for painful chronic lateral ankle instability taken at the outpatient clinic might be inaccurate for diagnosis. Stress Radiographs under Anesthesia 13 치에대해서여러이견이있어왔으나 3-5) 현재는전위의절대치가 10 mm 이상이거나건측과비교하여 5 mm 이상일때의미가있는것으로판단하고있다. 6) 내반스트레스검사 (varus stress test) 의경우거골경사 (talar tilting) 의절대치가 9도이상이거나건측과비교하여 3도이상일때양성이라는보고가있으며, 7) 또다른문헌에서는절대치가 15도이상일때로규정하고있기도하다. 6) 스트레스검사를시행할때결과에영향을미칠수있는요소중하나로고려되어야하는근성방어 (muscle guarding) 란손상받은부위주변의근육이긴장됨으로써움직임을최소화하여통증을줄이고부목의효과를나타냄을말한다. 만성내측주관절불안정성에있어서환자가깨어있는상태로스트레스검사를시행할시에근성방어로인하여불안정성이제대로검사되지않는다는보고도있으므로 8) 근성방어의영향을최소화하며스트레스방사선검사를시행하는것이가장이상적인방법이라할수있다. 이에저자들은동통을동반한족관절의만성외측불안정성을호소한환자에대하여외래에서흔히시행되는스트레스방사선검사의정확도및효용성을평가해보고자하였다. 근성방어로인하여정확한스트레스검사를시행함이어렵기때문에마취전과마취후의검사결과에유의한차이가있을것이라가정하였다. 대상및방법 1. 연구대상 2014년 1월부터 2016년 6월까지 W병원에서, 첫손상후 3개월이상지속되는전방거비인대부위 (anterior talofibular ligament) 의통증, 반복적인염좌및주관적인족관절의불안정성으로내 원한환자가운데 MRI 촬영이후에변형 Broström 술식 (modified Broström operation) 을시행받은 121명의환자를대상으로연구를시행하였다. 이들중양측손상이거나전신관절유연성이있는경우에는연구에서제외되었으며총 96예가포함되었다. 환자의평균나이는 29.63세 (15 76세) 로우측이 59예, 좌측이 37예였으며, 남성이 65예, 여성이 31예였다. 동반된수술의경우발목관절경이 42예로가장많았으며, 비골하부골제거술이 20예로두번째로많았다. 발목관절경을함께시행받은 42예중전방충돌증후군을가진환자가 41예, 거골의골연골병변을가진환자가 21예였다 ( 내측 15예, 외측 6예 ). 그외과상부절골술을함께시행한것이 1예, 발목터널유리술을시행한경우가 1예있었다. 이들 96예의환자들을 Lee 등 9) 이제시한 MRI상전방거비인대파열을분류하는방법에의거하여 complete tear, partial tear, instability without rupture의세가지아형으로분류하여각아형에대해서도비교, 분석을시행하였으며, 각각 39예, 46예, 11예가이에포함되었다. 2. 검사방법모든수술은척추마취하에시행되었으며, 수술실에서척추마취하에서는 Telos TM (Telos GmbH, Holstein, Switzerland) 를이용한족관절스트레스검사를시행하기가불가능하였기때문에본연구에대해고지된바없는상태로스트레스검사를시행하는방법만교육시킨 2명의방사선사가저자의감독하에검사를진행하였다. 본격적인연구를시행하기전, 초기 5명의환자를대상으로수술실에서마취를시킨이후에변형 Broström 수술을시행하기직전및수술직후환자가수술실을빠져나가기전건측에대한스트레 Table 1. Stress Radiographic Values of the Affected Side Subtracted by the Unaffected Anterior drawer test (mm) Varus stress test ( ) Before anesthesia After anesthesia p-value Before anesthesia After anesthesia p-value Overall (n=96) Complete tear (n=39) Partial tear (n=46) Instability without rupture (n=11) 검사를시행하였음에도불구하고많은저자들이전방불안정성의기준으로제시한절대수치 10 mm 이상의거골전방전위에는모든아형에서미치지못함을확인하였다. 환측에서시행한내반스트레스검사의경우에도마취이후에평균 2.0도가증가하여통계적으로유의한변화를보였으며 (p=), 아형별로나누어비교하였을때에는 instability without rupture군에서 1.51도가증가하여 complete tear군의 2.04도및 partial tear군의 2.09도보다유의하게증가량이적었다 (p<0.001). 전방당김검사와마찬가지로절대치 15도이상의거골경사의기준 6) 에는마취를시행하였음에도불구하고어떤아형에서도부합하지못하였다. 또다른진단기준으로사용되는환측과건측의차이를살펴보면 (Table 3), 전방당김검사의경우마취전보다마취후에건측과의차이가통계적으로유의하게증가하는양상을보였다 (p=0.005). 아형간증가된정도를비교하였을때에는 complete tear군에서 2.67 mm로가장크게측정되기는하였으나 partial tear 군이 2.29 mm, instability without rupture군이 2.59 mm로측정되어다른군과통계적으로유의한차이는없었다 (p=0.52). 내반스트레스검사의경우에도평균 1.32도가증가하는양상을보였으며, complete tear군이 1.13도, partial tear군이 1.42도, instability without rupture군이 1.23도로아형별차이는없었다 (p=0.31). 고찰 족관절의안정성을담당하는구조물중전방거비인대의경우, 외과원위부의전방에서기시하여거골체부에부착하게되며길이는 2 cm, 폭은 6 8 mm로보고되고있다. 발의종축으로위치하고있어서원위경골관절면에대한거골의전방전위를막아주는동시에중립위의족관절에서내반력에대하여저항하는역할을하게된다. 10) 또다른구조물인종비인대의경우, 외과원위부에서기시하여족관절과거골하관절을지나서종골의외측에부착하며 4 8 mm의폭을가지는것으로알려져있다. 족관절이족저굴곡되었을시에내반력에저항하는역할을갖게되며서있는자세에서는이완된상태로유지되게된다. 10) 그러므로전방거비인대파열이있을때에는전방당김검사에서거골의전방전위가나타나거나중립위에서내반스트레스검사를시행할시에거골경사가증가될수있으며, 종비인대가파열되었을때에는족관절을족저 굴곡시킨상태로내반스트레스검사를시행하였을때거골경사가증가되는소견이관찰될수있다. Lee 등 9) 은 MRI상관찰되는전방거비인대및후방거비인대손상이스트레스방사선상경골-거골간경사각및전방당김검사에유의한영향을미친다고보고한바있으며, Cox와 Hewes 11) 는외상없이정상적인상태에서거골이 5도이상경사를나타내지는않는다고보고하였다. 그러나정상에서도 21도의거골경사를보이거나 10도이상양측의차이가나타날수있다는보고가있으므로 12) 스트레스방사선검사결과의해석에있어서검사결과가의미있는수치를나타내었다고해도무조건발목의불안정성이있다고판단할수만은없을것이다. 족관절의만성외측불안정성을호소하는환자에게있어서스트레스방사선검사는손쉽게시행할수있는방법이지만, 검사가가지고있는한계점이많이있기때문에이러한결과가나타날것이라저자들은생각한다. 즉, 족관절자체가골성구조로안정성이유지되는구조물이기때문에당기는각도에따라전위나경사의정도가이에영향을받는다는점과사람마다관절의이완정도가달라수치화하기어렵다는점, 통증이심하면근성방어에의해서도영향을받는다는점등이한계점으로지적될수있다. 8,10) Becker 등 13) 은급성발목염좌에대한스트레스검사를시행함에있어서좀더믿을수있는결과를얻기위해서는마취가필요하다는보고를한적이있다. 이들은 110명의환자를대상으로비골신경차단술을시행한이후에전방당김검사를시행하였으며 65명의환자에서마취를시행한후에야급성인대파열여부를확인할만한결과를얻을수있었다. 하지만일반적으로급성발목염좌에있어서스트레스방사선검사를시행하게되면남아있는인대마저파열시킬위험성이있기때문에시행함에있어서주의를해야하는것으로알려져있다. 14) 아직까지만성외측불안정성에대한스트레스방사선검사를시행함에있어서마취여부에대한결과차이에대해서는발표된것이없으며, 근성방어가만성환자에서도영향을미칠것인지에대해서도밝혀진바가없다. 저자들은본연구를통하여족관절의외측불안정성의진단을위해서시행되는전방당김검사및내반스트레스검사를, 동통을동반한만성일측성환자에게마취를시행한이후에진행하여급성발목염좌에서의보고 13) 와유사하게절대적수치및건측과의비교치가모두유의하게증가됨을확인하였다. 또한 MRI상확인되는전방

5 16 Vol. 21 No. 1, March 2017 거비인대의파열정도와수치의증가정도에는유의한관계가없으며, 마취하에스트레스검사를시행할때 MRI상완전파열소견을보이더라도불안정성의진단기준에미치지못하는작은값이측정될수있음역시확인하였다. 그러므로동통이동반된족관절의만성외측불안정성을보이는환자를진단함에있어서반복적인염좌가있었는지등을포함하여주의깊게병력을청취하는것이중요하고, 15) 초음파 16) 나초음파하에서시행하는전방스트레스검사, 혹은 MRI 17-19) 등의추가적인영상의학적검사방법을시행하는것이필요할것으로생각한다. 본연구의한계점으로는스트레스검사가 Telos TM 등의기구를이용하지못하고수동적으로이루어졌으므로일정한힘을가할수없었을가능성이있다는점이있으며, 동반되어있는종비인대손상여부에대해서는따로연관성을분석하지않았다는점이있다. 또한상대적으로적은 96예가연구에포함되었다는점역시한계점으로지적될수있을것이다. 결 동통을동반한족관절의만성외측불안정성을진단하기위하여외래에서흔히시행되는스트레스방사선검사를, 수술직전마취하에시행하여보니외래에서시행한수치와비교하여통계적으로유의한수치의증가를보이기는하였다. 하지만불안정성의진단에있어서변별력을크게높여주지는못하였다. 론 REFERENCES 111 Freedman LS, Jenkins AI, Jenkins DH.
