Torsades de pointes is characterized by beat-to-beat variations in the QRS complexes in any ECG leads with rates of per minute. Fluoroquinolones are widely used and well tolerated antibacterial agents. However, prolongation of the QT interval leads rarely to Torsades de pointes as a significant adverse effect. So, it should be used with caution in high-risk patients for developing Torsades de pointes. We report one case of 67-year old man with contact burns who experienced Torsades de pointes, which probably resulted from the use of levofloxacin, and no further episode occurred after its withdrawal. Torsades de pointes은 1966년프랑스내과의사 Dessertenne 가처음으로기술하였는데, 심전도에서 QRS 복합체가분당 회의속도로 beat-to-beat variations을보이는것이특징이다 2. 이는어지러움, 실신및심장마비등의증상을일으켜이환율및사망률증가로이어질수있다. 심전도감시를하지않으면부정맥발생유무를확인할수없기때문에, 일반인에서 torsades de pointes 진단은매우어렵다. 여성, QT간격연장증후군가족력, 기질적심질환, 심부전, 서맥및전해질불균형등이위험인자로알려져있으나, 약물에의해서도생길수있다 3,4. 하지만이들위험인자및약제들이항상 torsades de pointes 을발생시키는것은아니다. 따라서재발을방지하기위해서는적절한심전도감시를통해그연관성을입증하는것이중요하다. 저자들은접촉화상으로피부이식및재건술을위해입원한환자에서 levofloxacin 치료도중 torsades de pointes을경험하였기에보고한다. 474

양발은접촉화상으로인해궤양과함께피부층이소실되어있었다 (Figure 1). 검사실소견 : 입원당시시행한말초혈액검사에서백혈구 6,800/mm 3 ( 호중구 64.4%), 혈색소 8.2 g/dl, 혈소판 309,000/mm 3 이었다. 생화학검사에서 glucose 108 mg/dl, BUN 24.0 mg/dl, Cr 1.

No. 6, Dec 증례환자 : 김, 남 67세주소 : 양발의접촉화상현병력 : 만취상태에서찜질방에서자다가양발에접촉화상을입어응급실을통해입원하였다. 과거력 : 알코올성간염의기왕력이있으나, 고혈압, 당뇨등은없었다. 가족력 : 특이사항없음. 사회력 : 35갑년의흡연력과주 3회이상의음주력있음. 신체검사소견 : 신장 157 cm, 체중은 52 kg이었다. 입원당시활력징후는혈압 120/80 mmhg, 맥박 76회 / 분, 호흡수 20회 / 분, 체온은 37.5 o C였다. 의식은명료하였고만성병색을보였다. 흉부청진에서양측아래폐야에서악설음이들렸다. 양발은접촉화상으로인해궤양과함께피부층이소실되어있었다 (Figure 1). 검사실소견 : 입원당시시행한말초혈액검사에서백혈구 6,800/mm 3 ( 호중구 64.4%), 혈색소 8.2 g/dl, 혈소판 309,000/mm 3 이었다. 생화학검사에서 glucose 108 mg/dl, BUN 24.0 mg/dl, Cr 1.1 mg/dl, total protein 6.0 g/dl, albumin 2.2 g/dl, AST 41 IU/L, ALT 23 IU/L이었다. 혈청전해질은 Na/K/Cl 139/3.5/100 meq/l 였으며, TCO 2 는 26 mmol/l, HbA1c 는 6.8% 였다. 요검사에서비중 1.027, ph 6.0, protein +++, glucose +++, blood +++였으며, 나머지특이사항은없었다. 대기중동맥혈가스검사에서 ph 7.464, 이산화탄소분압 43.1 mmhg, 산소분압 78.4 mmhg, 중탄산염 30.4 mmol/l, 산소포화도 96.2% 였다. 24시간소변단백 3,046 mg/day이었다. 혈청 β2-microglobulin 4.4 mg/l (ref, mg/l), FANA: positive (nucleolar type), RF 24.9 IU/mL (ref, 0 20 IU/mL), C3 126 mg/dl (ref, mg/dl), C mg/dl (ref, mg/dl), cryoglobulin negative 였다. 혈액전기영동검사에서감마분획이 37.3% (ref, %), 24시간소변전기이동검사에서는 IgG-lambda 유형단세포군단백뇨로보고되었다. 방사선학적소견 : 입원당시가슴 X-선사진에서는특이소견은없었다. 치료및임상경과 : 양발의접촉화상치료를위해성형외과로입원하였다. 창상감염증치료를위해서는 cefobactam 을투여하였다. 창상배양에서는 methicillin-sensitive Staphylococcus aureus가동정되었다. 치료도중 6병일에갑자기호흡곤란이발생하여동맥혈가스검사를시행했더니 ph 7.436, PCO mmhg, PO mmhg, BE -1.9 mmol/l, HCO mmol/l, SaO % 였으며, B type natriuretic peptide (BNP) 는 1,620 pg/ml, CRP 35.2 mg/l였다. 가슴 X-선사진에서는폐부종소견이었다 (Figure 2). 기계호흡및혈역학적감시가필요하다고생각되어중환자실로이송되었다. 활력징후는혈압 170/102 mmhg, 맥박 154회 / 분, 체온 36.8 o C로비교적안정된상태였다. 8병일에집중치료를위해호흡기내과로전원되었다. 활력징후는비교적안정된상태로유지되었다. 수액과다 Figure 1. Normal sinus rhythm was returned. 에의한폐부종으로생각되어이뇨제 (furosemide) 를간헐적으로투여하였다. 항균제 cefobactam 은창상감염때문에유지하였다. 12병일부터설사증세를보여 13병일에는항균제 levofloxacin 750 mg을하루한번정주하였다. 14병일에 levofloxacin 정주 2시간후갑자기 torsades de pointes 이발생하였다 (Figure 3). 마그네슘정맥주사에반응이없어직류심장율동전환 (DC carioversion) 후정상리듬으로회복하였다. 이당시혈액검사에서 Na/K/Cl 476

그러나, 마그네슘정주에도불구하고심실빈맥, T파전위및 QT간격연장이간헐적으로나타났으며, 일시적인 torsades de pointes 리듬도반복되었다 (Figure 4). 약제에의해서유발되었을가능성이있어 17병일째부터는 levofloxacin 을끊고 panipenem 으로변경하였다. 그후부터는계속해서심전도가안정되었다 (Figure 5).

혈청및소변검사에서감마글로불린병증이의심되어골수조직검사결과미결정유의성단세포군감마글로불린병증 (monoclonal gammopathy of undetermined significance) 으로보고되었다 (Figure 7). 33병일에는화상치료를위해다시성형외과로전과하였으며, 인공피부이식및재건술후퇴원하였다.

No. 6, Dec /4.9/106 meq/l, Ca mmol/l (ref, mmol/l), Mg mmol/l (ref, mmol/l), CK 146 U/L (ref, U/L), CK-MB <0.5 mg/ml (ref, mg/ml), Troponin-I <0.04 ng/ml (ref, ng/ml) 이었다. 심장초음파검사에서는특이소견은관찰되지않았다. 15병일및 16병일에도 levofloxacin 은계속정주되었다. 그러나, 마그네슘정주에도불구하고심실빈맥, T파전위및 QT간격연장이간헐적으로나타났으며, 일시적인 torsades de pointes 리듬도반복되었다 (Figure 4). 약제에의해서유발되었을가능성이있어 17병일째부터는 levofloxacin 을끊고 panipenem 으로변경하였다. 그후부터는계속해서심전도가안정되었다 (Figure 5). 가슴 X-선 Figure 6. Chest X-ray showed marked improved pulmonary edema and pleural effusion at the 21st hospital day. 사진에서이전에보였던폐부종및늑막삼출은많이호전되었다 (Figure 6). 전신상태가호전되어성공적으로기계환기를이탈할수있었으며, 21병일에는일반병실로이송하였다. 혈청및소변검사에서감마글로불린병증이의심되어골수조직검사결과미결정유의성단세포군감마글로불린병증 (monoclonal gammopathy of undetermined significance) 으로보고되었다 (Figure 7). 33병일에는화상치료를위해다시성형외과로전과하였으며, 인공피부이식및재건술후퇴원하였다. 고찰선천성 QT연장증후군과함께저칼륨혈증, 저칼슘혈증, 저마그네슘혈증등의전해질이상, 서맥성부정맥, 뇌출혈등으로인해후천적으로 QT간격이연장된경우정상동율동으로즉시전환시키지않으면바로사망에이르는 torsades de pointes 이발생할수있다 5. 중요한원인약물로는각종항부정맥제, 항균제, 항히스타민제및진정제등이있다 3. 국내에서는 terfenadine, hydroxyzine, ketoconzole, cisapride, ofloxacin 등이원인약물로보고된바있다 6-8. 본증례의경우 torsades de pointes 발작당시전해질이상, 심근허혈및기저심장질환은관찰되지않았다. 병력에서도어지러움, 심계항진및실신등의기저심장질환을의심할만한사항이없었으며, 선천성 QT연장증후군및급사등의가족력도없었다. 처음에는기저감마글로불린병증에의해파라단백질의심장침윤에의한 torsades de pointes 가능성을염두에두었다. 하지만, 심장초음파검사에서특이소견이관찰되지않았고, 문헌고찰에서는치료약제와관련해서보고된적은있으나, 감마글로불린 Figure 7. 5 SA Kwon et al: Levofloxacin and torsades de pointes 병증질환자체가 torsades de pointes 을일으켰다는보고는없어감별진단에서배제하였다. 또한, 다발성골수종감별을위해시행한골수조직검사도미결정유의성단세포군감마글로불린병증 (monoclonal gammopathy of undetermined significance) 으로보고되어부정맥의원인으로작용했을가능성은낮다고생각되었다. 한편, 전해질이상을수반하지않고 torsades de pointes 을일으킬수있는이뇨제는 thiazide 계통의 indapamide 가있다 9. 증례에서는 indapamide를사용한적이없고, furosemide 를간헐적으로사용하긴했지만전해질이상은수반하지않았으며, 문헌고찰에서도 torsades de pointes 과의연관성에대한보고가없어원인약물에서제외하였다. 본증례에서나타난 QT간격의연장및 torsades de pointes 이순전히 levofloxacin 에의해유발되었다고단정하기는어렵다. 하지만 QT간격연장을시사할만한과거병력이없고, 입원해서새로이나타났으며, QT간격연장의다른뚜렷한요인이없었던점, 투여된약물중 QT간격을연장시킬수있는것은 levofloxacin 이유일했으며, 이의중단후 QT간격이기저치로회복함과함께 torsades de pointes 리듬도완전히사라졌음을고려할때, 다형심실빈맥의원인은아마도 levofloxacin 이었을것으로추정된다. 국내에선보고된바가없으나외국의경우 levofloxacin 에의한 torsades de pointes 예는드물게보고되고있다 10. 발생빈도는미국의경우천만처방건수당 5.4예이다 11. QT간격은 500 mg 사용했을경우는정상이나, 1,000 mg 에서 1,500 mg으로증량하면용량에비례하여연장된다고한다 12. 증례의경우, levofloxacin 은 750 mg씩총 4회정주되었다. 첫용량에서는별문제가없었으나두번째용량부터심전도에이상이생겼다. Levofloxacin 의반감기가 6 8시간으로비교적길고, 환자의체중이 52 kg 임을고려하면일회용량 750 mg은고용량에해당될수있다. 또한긴반감기로인해약물누적효과도나타났을것으로생각된다. Fluoroquinolone 에의한 QT간격의연장기전은심장의전압작동칼륨통로 (voltage-gated potassium channel) 를차단함에의해서다 13. 특히, 칼륨전류의지연성정류기 (delayed rectifier) 의 rapid component (IKr) 가관여하는것으로알려져있다. IKr을억제하면심근세포로부터의칼륨방출이차단되고칼슘유입이늘어나심장재분극이지연되어결과적으로 QT간격이늘어나게된다. Ikr은 human either-a-go-go-related gene (herg) 에의해암호화된다. 모든 fluoroquinolone 은 IKr의대항제로작용할수 있으나, IKr 의차단정도및 QT간격의연장은투여한 fluoroquinolone 의종류및용량에따라다양하게나타난다. 특히 moxifloxacin, gatifloxacin 등에비해 levofloxacin, ciprofloxacin 및 ofloxacin 등은 herg 통로에대한잠재적억제능력이유의하게낮기때문에다형심실빈맥의빈도도적게발생하는것으로알려져있다 14. Fluoroquinolone의분자구조는항균작용을나타내는부위와 herg통로를차단하여잠재적으로부정맥유발성에기여하는특이적부위를가진구조로이루어져있다. 다시말해서, 각약물의 5번위치에있는라디칼 (radical) 이무엇이냐에따라 QT연장정도가결정된다. 예를들어, sparfloxacin 은아미노기 (amino group), grepafloxacin 은메틸기 (methyl group), moxifloxacin, levofloxacin, gemifloxacin, gatifloxacin, ciprofloxacin은양성자기 (proton group) 가 5번위치에존재한다. Sparfloxacin 및 grepafloxacin 은 QT간격연장으로인한잠재적부정맥유발가능성이높고실제로 torsades de pointes 임상발현이빈번하여시장에서퇴출된예이기도하다. QT간격을연장시킨다하더라도해당되는모든약물이 torsades de pointes 을유발하지는않는다. 부정맥을유발하기위해서는 QT간격연장뿐만아니라, 부정맥유발성 (arrhythmogenic potential) 소인에대한재분극의이질성또한필요하기때문이다. 그런데, QT간격이연장될수록이에따라서 torsades de pointes 의위험도증가하는것으로알려져있다. 선천성 QT연장증후군및약물-유발 torsades de pointes 연구에서교정 QT간격이 500 ms 이상이면그위험성은현저하게증가한다 3. 본증례를경험하면서몇가지제안하고자한다. 첫째는사용하고있는약물이 QT간격및 torsades de pointes 을일으킬수있는약물인지확인할필요가있다. 성인호흡기감염증에사용하는많은항균제 ( 예, fluconazole, levofloxacin, azithromycin, ciprofloxacin, trimethoprim-sulfamethoxazole, clindamycin) 가원인약물에포함된다. 다른선행질환을갖고있는경우는타과병용약물도꼼꼼히살펴보아야한다. 웹사이트 ( 혹은 를방문하면쉽게확인할수있다. 둘째는원인약물투여전후에교정 QT간격을측정해서비교해보는것이다 (Figure 8) 15. 기저심전도와비교해서 QT간격이유의하게늘어나있거나교정 QT간격이 500 ms 이상이면다른안전한약제로변경할것을추천한다. 그래야치명적 torsades de pointes 을사전에예방할수있기때문이다. 478
