Summary and Literature Review: Calcium phosphate is a biocompatible alternative to PMMA for vertebral augmentation in painful osteoporotic vertebral fracture as it is osteoconductive, non-exothermic, and injectable. Materials and Methods: We analyzed 45 cases treated from April 2005 to August 2006 with a minimum of 1 year follow-up. Preoperative and post operative pain scores (visual analogue scale), ambulatory status, and patient satisfaction were measured. Anterior vertebral height, as well as the status and size of cement were assessed radiologically preoperatively, postoperatively, and at 3 months and 1 year. Results: Pain scores (visual analogue scale) and ambulatory status improved significantly after kyphoplasty and remained unchanged during follow-up. Overall patient satisfaction was 93%. Radiological findings showed that mean vertebral height was significantly higher than preoperative (p 0.05). According to follow-up radiological finding, we divided cases into 4 groups: Group 1, 2; maintained vertebral height with minimal or some cement resorption; Group 3, 4; cement crack resorption and vertebral collapse. Group 1, 2 and Group 3, 4 had 38 patients (84%) and 7 patients(16%) respectively. Revision surgery was needed in 3 cases (antero-posterior surgery in 2 cases of group 4, and decompression in 1 case of extravasation into the neural canal). Conclusions: Kyphoplasty with calcium phosphate may be a good alternative for treatment of osteoporotic vertebral fracture, but non-union of the vertebral body with a large cleft showed a high risk of premature resorption and collapse of the vertebral body. The presumed advantage over PMMA needs longer follow-up. 2 대한척추외과학회지 Vol. 14, No. 4, 2007 서 론 골다공증성척추골절의치료로 PMMA 를이용한경피적풍선후만성형술이보편적으로시행되고있다. 풍선후만성형술은골다공증이나종양의전이로인한병적척추골절의치료로서최소침습적이면서낮아진추체의높이의회복과함께후만을복원하고임상증상을호전시키는데있어서효과적이다 1,2,3). 이시술은풍선을이용하여척추체의높이를회복시킨후공간을확보하고거기에골시멘트를주입하는데현재까지추체내부에충전하는물질은 PMMA (polymethylmethacrylate) 시멘트로오랜기간인공관절치환술에사용되면서그효과가입증된물질이다. PMMA 시멘트는가격이싸고사용하기에편리하며추체내로주입후즉각적인강도와안정성을획득할수있다. 하지만굳는과정에서상당한열이발생하며방출된 monomer 가세포독성을갖고있고, 주의의뼈와골유합이되지않기때문에거의항상뼈와시멘트사이에섬유층이형성되며강도가일반뼈보다강해인접한추체의골절을유발할수있다. 이상적 PMMA 시멘트대체물로는생체에적합하고 (biocompatible) 골전도성이있으며시간이지나면서점진적으로뼈로대치되는성질을갖는물질이여야하며 PMMA 시멘트를대체하는생물학적물질로 Calcium- Phosphate 시멘트가풍선성형술의충전제로장점이있을것으로생각된다. Calcium-Phosphate 시멘트는외상분야에서많이사용되어왔으나척추에시술할시문제는즉각적체중지탱을위한최초의강도가약하고뼈로의대체가매우느리다는점이나여러연구에서 Calcium-Phosphate 시멘트의생화학적강도나임상적효용성에대해긍정적보고가되고있다 4,5,6,). 이에저자들은골다공증성척추골절에서 Calcium-Phosphate 시멘트를이용하여풍선후만성형술을시행후그결과를분석하였다. 측정상 T-score 가 -3.0 이하로골다공증진단을받은상태였으며 2 주이상의보존적치료에도반응하지않는골다공증성추체압박골절을가진환자였다. 전신동위원소검사 (whole body bone scan) 가술전시행되어현재통증이있는골절부위를확인하는데사용되었으며단순방사선사진상다발성추체골절이있는환자에서는동위원소검사상음영증가를보이는추체를수술하였다. 수술받은추체는부위별로흉추부가 3 례, 요추부가 13 례, 흉요추이행부 ( 흉추 10 번 - 요추 2 번 ) 가 35 례였다 2. 수술재료및방법 Calcium-Phosphate 시멘트 : Calcibon 은가루와액체의두가지물질로구성되어있으며가루는 α-tcp (α-tricalcium-phoasphate) 61%, CaHPO 4 (calcium-hydrogenphosphate) 26%, CaCO 3 (calcium-carbonate) 10%, pha (precipitated hydroxyapatite) 3%, 의 4 가지물질로구성되어있고, 액체는 aqueous Na 2 HPO 4 (di-sodium-hydrogenphosphate) 로구성되어있다. 가루와액체를혼합하면사용하기에적합하게죽모양의형태로변화하게되며공기중의이산화탄소와반응하여결정화하게된다. 결정화과정은체온과비슷한온도에서가장빠르게진행되며 1 분간혼합하여혼합이끝나는시점부터 4 분내에주입하게되며주입이끝난시간부터 5 분간경화되어혼합시작부터굳는데까지총 10 분이걸리며발열반응은없다. Calcibon 의압박강도는시험관에서의실험상혼합후 6 시간째에 25 MPa 에도달하여해면골의강도 (10~30 MPa) 정도를가지며 3 일이지나면 60 MPa 에도달하여피질골의강도 (25~100 MPa) 에근접하게된다 연구대상및방법 1. 연구대상 2005 년 4 월부터 2006 년 8 월까지본원에서동통성골다공증성추체압박골절로풍선성형술을시행한환자들을대상으로하였다. 수술은모두제 1 저자에의해시행되었으며총 45 명의환자에서 51 개의추체를대상으로전례국소마취하에서시행되었다. 환자들의평균연령은 69.1(54~83) 세였으며성별은남자 8 명, 여자 37 명이었다. 모든환자들은이중에너지방사선골밀도 Fig. 1. 3 골다공증성추체골절에서 Calcium Phosphate Cement (Calcibon ) 를이용한경피적풍선후만성형술 김응하외 (Fig. 1). 수술방법 : 복와위에서국소마취하에영상증폭장치를사용하여골절된추체의양측척추경을통해확장가능한풍선 (KyphX, Kyphon Inc.) 을삽입하고조영제를풍선내부로주입하여풍선을확장시켜골절된추체의상부종판을들어올려골절을정복한후공간을만들었다. 정복은추체의경계밖으로나가지않게주의하였고환자의통증이심할경우더이상팽창시키지않았다. 이후풍선을제거한후추체내부의공간에 Calcium- Phosphate 시멘트 (Calcibon ) 를주입하였다. 반죽후 PMMA 시멘트와달리빨리굳고점성이높아실제주입실린더에담은후주입가능시간이아주짧아 2~3 분이내이다. 따라서양측척추경을통한풍선팽창후에냉장고보관된재료와용기를꺼내와반죽을시행하고팽창된풍선의체적보다약간많이들어가게양측척추경을통해시멘트를동시에주입하는것이좋다. 주입후시멘트가흘러나오지않게입구를막고 1~2 분기다리며 PMMA 와달리점도가높고빨리굳어압력을가해주입하더라도추체바깥이나척추강내로의누출은별로일어나지않는다. 술후 12 시간경과후보행하고퇴원하였으며모든환자에서술후추시기간동안골다공증에대한내복약을투여하였다. 3. 연구방법 모든환자들은술전전 - 후면및측면방사선사진을촬영하였고이중에너지방사선골밀도측정과전신동위원소검사를시행하였으며술후 24 시간에단순방사선사진및 CT 촬영을하여 Calcibon 이추체내에적절히위치하였나를확인하였다. 술후 1 개월에외래에서추시관찰및단순방사선촬영을하였으며이후 3 개월마다외래추시관찰하였다. 임상결과는최종추시상시술받은골절부위에대하여환자의만족도와함께시상통증척도 (visual analogue scale, VAS) 를술전, 술후및최종추시상측정하였는데환자가통증이전혀없을시를 0, 심한통증이있을시를 10 으로측정하였다. 보행가능정도는환자가도움없이혼자걸을수있는경우, 보조기나목발등이필요한경우, 휠체어를타고있는경우, 침상에누워있는경우총 4 단계로분류하여시술전후의환자의보행상태비교분석하였다. 방사선결과의측정은술전, 수술직후, 술후 3 개월및최종추시상의추체의전주높이를측정하였는데골절된추체와인접상 - 하추체의전주높이를측정하여인접상 - 하추체높이의평균에대한골절된추체높이의비율을측정하여압박율 ( 골절된추체의전주높이 / 인접추체전주높이의평균 100) 을술전, 술후및추시기간에측 정하였다. 추시방사선사진상척추체의높이의변화와시멘트의균열, 파쇄. 흡수, 이동여부등을분석하였다. 결 과 1. 임상적결과 술전평균통증치수는 8.9 였으며수술 1 개월째외래추시상평균통증치수는 3.9 로호전되었으며술후 3 개월째평균 3.2 술후 6 개월째평균 2.9 최종외래추시상평균통증지수는 2.9 를보여통계학적으로유의 (P<0.05) 하게통증정도가감소하였으며그중에서도수술 1 개월째통증치수가크게감소함을보였다 (Fig. 2). 보행상태는수술전환자가도움없이혼자걸을수있는경우가 29 례 (69%) 보조기나목발등이필요한경우가 6 례 (12%) 휠체어를타고있는경우가 7 례 (12%) 침상에누워있는경우가 3 례 (7%) 에서최종외래추시상환자가도움없이혼자걸을수있는경우가 36 례 (83%) 보조기나목발등이필요한경우가 7 례 (12%) 휠체어를타고있는경우가 1 례 (2%) 침상에누워있는경우가 1 례 (2%) 로통계학적으로유의 (P<0.05) 하게보행상태가호전되었음을알수있었으며 (Fig. 3) 최종추시상조사된환자평균만족도는 93% 였다. 2. 방사선학적결과 인접상 - 하추체높이의평균에대한골절된추체전방높이의평균은술전 66.8%, 풍선후만성형술시술 1 일후측정한추체전방높이는평균 82.2%, 술후 3 개월째높이는평균 74.5%, 최종추시상의전방추체높이는평균 73.7% 였다. 술전과술후추체높이의차이는통계 Fig. 2. 4 대한척추외과학회지 Vol. 14, No. 4, 2007 학적으로의미있게 (P<0.05) 증가하였으며술후와최종추시사이의추체높이의변화는통계학적으로의미있는결과를보이지않았다 (P>0.05)(Fig. 4). 추시방사선사진상척추체의붕괴와시멘트균열, 파쇄, 흡수는서로연관되어나타나다음의 4 가지형태로분류되었다 (Fig. 5). 그룹 1 은시멘트의흡수없이척추체의높이가유지된경우, 그룹 2 는경도의시멘트균열흡수가있으나척추체의높이가유지된경우, 그룹 3 은시멘트균열이뚜렷하고중등도의시멘트흡수와함께중등도의척추체높이의붕괴를보인경우, 그룹 4 는시멘트흡수파쇄와추체외또는척수관내로이동과함께중등도이상척추체높이의붕괴를보인경우였으며그룹 1 이 24 명 (53%), 그룹 2 가 14 명 (31%), 그룹 3 이 3 명 (7%), 그룹 4 가 4 명 (9%) 이었다 (Fig. 6). 이차적추체의붕괴를보인전례에서시멘트괴의균열과흡수를보였으며균열이없는경우나경미한경우추체의높이가잘유지되어있어시멘트괴의균열과시멘트의흡수정도와이차적추체붕괴가연관성을감지할수있었다. 시멘트의균열, 파쇄와척추체의붕괴를보인그룹 3, 4 가 16% 를차지하며이 7 례의원인을분석해보니풍선후만성형술시행후외상이 1 례, 술전불유합이 5 례, 술전방출 Fig. 3. (Group 1+Group 2) 38 patient(84%) showed good maintenance of vertebral height with minimal resorption of calcibone cement 성골절이 1 례였다. 풍선성형술후추시기간동안인접분절의이차적골절을보인경우는 2 례로서이중 1 례는타병원에서추체성형술을시행하였고나머지 1 례는보존적치료를시행하였다. 방사선학적결과와임상결과의연관성을살펴보면조기붕괴로재수술이필요했던경우는 2 례였으며나머지 5 례의조기붕괴의경우는시술후초기의통증완화효과는만족스러웠고붕괴가일어난추시기간중급성의통증악화는보이지않았다. 3. 합병증 경과관찰중재수술이필요한경우가 3 례가있었는데 1 례는풍선후만성형술시행후별다른이상없이지내시다가술후 1 개월째넘어지면서방출성골절과함께시멘트의신경관으로유출로인해신경증상이발생한경우였고다른 1 례는술후시멘트의흡수와점진적인후만변형이진행된례였다. 이두례에서전방감압술및전후방유합술을시행하였고또다른 1 례는신경관내측벽의붕괴로인한시멘트의누출로신경근손상이발생하여감압술을시행하였다. 주입과정에서 Calcibon 이추체전외측으로의누출이 2 례, 상방디스크으로의누출이 2 례로총 4 례모두술전추체불유합을보였던환자였으며별다른증상을호소하지않아경과관찰중이다. 고 찰 풍성성형술에사용하는충전제로 PMMA 는현재표준적시료로사용되고있는데값싸고사용하기쉬우며즉각적인안정성을제공하며유해성에관해인체에사용도검증된상태이다. 그러나추체의충전제로서의 PMMA 의가장큰단점은골전도가되지않고추체내에 Fig. 7. Some osteoblastic reaming was noted suggesting osteointegration process. 이물질로계속남는다는점이다. 따라서골전도가되면서생물학적으로인체내에서분해됨과동시에뼈로대치되는물질을사용하는것이가장이상적일것이라고생각되며또한이러한물질은주입후즉시충분한강도를획득하여술후환자를즉시보행시켜도문제가없을정도로강해야만한다. PMMA 는시험관내실험상약 50~90MPa 정도의압박강도를보이며이는시멘트혼합시의점도나방사선대조물질의첨가정도에따라약간씩차이가있을수있다. 사람의건강한해면골의압박강도는 30MPa 이고 Calcibon 의압박강도는 20~ 60 MPa 정도이므로 5), 30 MPa 이상의압박강도를가진 Calcium-Phosphate 시멘트는이론적으로는골절된추체내에충전제로사용하기에적합할것으로생각된다. 만약추체내에삽입되는충전제의압박강도와경도가너무강할시는인접추체에부하가가중되어인접추체의골절을유발할수있는데 7,8,9), 이것이 PMMA 시멘트를사용할시대두될수있는문제점이며 PMMA 보다는약하지만일반적인해면골보다는강한 Calcibon 을사용시이러한문제점을줄일수있을것으로예상된다. Calcium phosphate cement 가추체내에서충분한강도를유지하는가에대한생역학적실험과이후몇개의임상적보고가있다. Bai 등 4) 은생역학적실험상골절이없는골다공증성척추체의압박강도가평균 527 N 인데반해 Calcium phosphate 로보강한척추체와 PMMA 로보강한척추체의압박강도의평균이각각 1063 N 과 1036 N 으로증가하였다고보고하였고 Tomita 등 10) 도 Calcium phosphate 와 PMMA 를사용한그룹모두척추체의압박강도를증가시키며이두그룹간압박강도는의미있게차이가나지않는다고하였다. 실험실에서정상적추체에서의작용은만족스럽다고보고하고있으나실제골절상황에서의결과는차이가있을수있다. 특히두가지면에서차이를지적할수있는데첫째실험실에서의인위적골절상황과실제임상에서의다양한환경에는
6 대한척추외과학회지 Vol. 14, No. 4, 2007 차이가있으며두번째로는임상에서실제시멘트주입이기술적으로완벽하지않을수도있기때문이다. 즉불유합이나방출성골절같이압력하에서주입이어려운경우시멘트강도의차이가있을수있으며골절부위의움직임이심한경우시멘트에가해지는전단력으로인해시멘트에균열이발생할수있다. 실제증례에서도불유합등움직임이많고추체내공간이폐쇄되어있지않은경우아무래도시멘트괴의견고성에문제가생길가능성이높아균열과흡수로이어지는결과로나타난것으로생각된다. PMMA 대용으로사용할물질에있어서필요충분조건은강도뿐만이아니며 PMMA 시멘트처럼다루기가쉽고초기혼합직후액체와비슷한상태를유지할수있어야만관을통하여추체내로주입할수있다. 임상적으로추체내로주입하기위해서는초기혼합후주입하기전까지다루는과정에서는어느정도충분한시간동안액상을유지하고주입중에는최대한응집력을가져야하며인체내로주입후에는최대한빨리굳어서단단해져안정성을바로확보할수있는것이중요하다. 또한 PMMA 와는달리 Calcium-phosphate cement 는중합반응 (polymerization) 이아니라결정반응 (crystallization) 을거치게되는데 11), 이결정화반응은사람체온에서상당히빨리일어나기때문에주입시에충분히주입하기도전에굳어버려더이상주입이불가능하게되는경우도있다. 따라서 Calcium-phosphate cement 는 PMMA 와는시술법이다른면이있다. 보통 PMMA 는혼합후액상상태를상당히오래유지하기때문에추체내로주입시천천히주입하게되지만 Calcium-phosphate cement 는혼합이끝난후주입까지최대 3~4 분의시간밖에없고인체내부에서빨리굳어버리므로풍선추체성형술시에는양쪽척추경을통해동시에최대한빨리주입하여야만한다. 이러한방법을사용할시는추체외부로 cement 가샐가능성과정맥으로의누출로인한색전증의가능성이더높아지므로 12), 주입시최초추체내부에서부풀렸던풍선의부피보다비슷하게나약간많은정도의양만큼 cement 를주입하는것이좋다. Calcibon 을사용시저자들은최초혼합후주입과정에서조금이라도더많은시간을확보하기위하여혼합직전까지 Calcibon 은냉장보관하였으며주입관등의기구들도냉장고에같이보관하였다. 주입후굳은후완전한경도를확보하는데있어서는 Calciumphosphate cement 가 PMMA 보다더장시간을요하게되는데주입후 6 시간이경과하여야만해면골정도의강도인 25 MPa 를확보할수있기때문에시술후최소 12~24 시간동안환자를침상안정시킨후거동시키도록하여야한다. PMMA 시멘트의또다른단점으로인체내에서굳는과정에서세포독성이있는 monomer 를 방출하고상당한열을방출하지만 13), Calcium-phosphate 는굳는과정에서열발산이없고독성물질의방출이없다는장점이있다. 일부에서는 PMMA 가방출하는열과 monomer 가골조직내의통증신경을제거하여통증을줄인다는가정을보고하였으나현재 Calcibon 을사용한임상결과가이전발표된 PMMA 를사용한임상결과와거의동일한점으로미루어보아이러한가정에대한의문이발생하며수술후통증이사라지는것은통증신경에대한독성효과보다는즉각적인추체의안정성확보에의한것으로보인다. Calcium-phosphate 가골로대치되는과정은파골세포에의해흡수되면서조골세포에의한신생골형성을유도하는것으로보고되었으며 4,14,15,16,17), 동물실험결과에서 Calcium-phosphate 와골의경계면에서신생골이형성되고또한 Calcium-phosphate 내부에서도새로이생성된골조직이관찰되는것으로나타났다. Libichter 등 6) 은개의 histomorphometry 연구에서 Calcium-phosphate cement 의거의전표면에서뼈로덮혀지며 PMMA 는약 30% 만뼈로덮힌다고하였다. Calcibon 의장점인골전도와골로대치되는지의여부에대해서도경계면의 CT 검사상 PMMA 와비교하여점진적골흡수소견을보이고경계면의 Calcium-phosphate cement 내에서골세포의점진적증가를보인다고하였다. Shibata 등 18) 은동물실험결과에서 Calcium-phosphate 와골의경계면에서신생골이형성되고또한 Calcium-phosphate 내부에서도골조직이새로이생성되어해면골의강도를증가시킨다고보고하였다. Ikenagna 등 19) 은토끼대퇴골원위부골단판에생긴공동에 Calciumphosphate cement 주입후 16 주내에 Calcium-Phosphate 시멘트가원래의골과유사한강도의신생골로대치된다고보고하였고 Knaack 등 11,15) 은쥐대퇴골골결손부위에 Calcium-Phosphate 시멘트를주입후 4 주째결손부위가해면골로대치되는것을확인하였다. Calcibon 의붕괴로재수술을요했던환자 2 명에서제거한추체의조직학적소견으로는골조직과 Calcium-phosphate 사이에섬유조직의형성과함께일부 Calcibon 주변에조골세포가붙어신생골을형성하고있었다 (Fig. 7). Calcibon 주입후조직학적표본을얻기까지가상당히단기간 (2 개월및 4 개월 ) 이었기때문에단정짓기는힘들지만 Calcium-phosphate cement 주위로골형성이일어남을알수있었으며시멘트괴의안전성이장기간유지된다면 Calcium-phosphate cement 의골전도와신생골형성이더이루어질것으로예상된다. 현재저자들이외래에서추시관찰한바로는시술후 3 개월째까지는단순방사선검사상주입된 Calcibon 의주변부에음영에변화가없었지만 6 개월째의단순방사선사진에서는일부환자에서 Calcibon 의음영이조금씩옅어지는것을확인할수있

Calcium phosphate cement 주입후인접추체붕괴에대해서는 Pflugmacher 등 20) 은 2 년추시상 67 개의추체에서 11 개 (18.3%) 의인접추체의붕괴를보고하였으며그중 8 개의추체에서풍선추체성형술을시행하였고우리의경우 1 년추시상 51 개의추체에서 2 개 (3.9%) 에서인접추체의붕괴를보여 Calcium-Phosphate cement 의장점인적은인접추체붕괴율은장기적인추시가필요할것으로사료된다. 결 론 골다공증성추체골절환자의치료에서경피적풍선후만성형술을시행함에있어서 Calcium-Phosphate cement 는임상양상의호전정도나단순방사선사진상좋은결과를보여술후안전하고만족스러운임상결과를기대할수있는치료법으로사료된다. 척추체불유합으로시행한경우에시멘트의조기흡수및추체의붕괴의빈도가높아사용에주의가필요하며 Calcium-Phosphate cement 만의장점인뼈로대치되는가에관하여서는좀더장기간의추시가필요한것으로사료된다. 참고문헌 01) Fourney DR, Schomer DF, Nader R, et al: Percutaneous vertebroplasty and kyphoplasty for painful vertebral body fractures in cancer patients. Acta Radiol. 2006; 47(8): 국문초록 연구계획 : 후향적연구연구목적 : Calcium-Phosphate cement를사용한경피적풍선후만성형술을시행후방사선학적분석및임상적결과를통해서 Calcium-Phosphate cement의효용성에대해알아보고자하였다. 대상및방법 : 2005년 4월부터 2006년 8월까지본원에서골다공증성동통성추체골절로진단받고경피적풍선성형술을시행한후 1년이상추시가가능하였던 45명의환자들을대상으로하였다. 임상적평가는환자의만족도와함께시술전후의시상통증척도 (visual analogue scale, VAS) 의변화, 시술전후의환자의보행상태비교분석하였다. 방사선학적평가는술전, 수술직후, 술후3개월및최종추시상의추체의전주높이를측정하였고또한추시 1년째방사선사진상흡수된시멘트의양, 시멘트누출여부등을분석하였다. 결과 : 풍선성형술을시행한환자에서의 VAS score를이용하여비교한통증수치는시술전 8.9점에서시술후 2.9점으로향상을보였고보행상태의호전과함께환자의만족도는 93% 였다. 추체전방높이의평균은술전 66.8%, 술후 1일평균82.2%, 으로술전과술후추체높이의차이는통계학적으로의미있게 (P<0.05) 증가하였다. 또한추시 1년째방사선사진상시멘트의상태와추체붕괴여부에따라 4개의그룹으로나눌수있었는데결과를비교분석시그룹3(7%) 과그룹4(9%) 의 7명의환자 (16%) 에서시멘트의뚜렷한흡수와척추체의붕괴를보였고그중에서그룹4 중 2 례와 trocar삽입경로를통한시멘트누출로인한신경근손상 1례에서관혈적수술이필요하였다. 결론 : 골다공증성추체골절환자의치료에서 Calcium-Phosphate cement는술후안전하고만족스러운임상결과를기대할수있는치료법으로사료된다. 시술상 PMMA보다빨리굳으므로술기의숙지가필요하고문제점으로는척추체불유합이있었던예에서시멘트및척추체의조기붕괴의빈도가높으므로 Cacium-Posphate cement의사용에있어서신중하여야한다. 색인단어 : 골다공증성추체골절, Calcium-Phosphate cement, 경피적풍선후만성형술. 통신저자 : 김응하경기도부천시원미구중동 1174 순천향대학교부천병원정형외과 Tel: Fax: eungha@unitel.co.kr
