Materials and Methods: A total of 22 patients (22 cases) with a severe hindfoot deformity and arthritis underwent tibiotalocalcaneal arthrodesis with retrograde intramedullary nails. The average age was 57.4 years (22 82 years) and the mean follow-up was 29.6 months (12 74 months). The radiological outcomes included an assessment of the preoperative and postoperative coronal ankle alignment, hindfoot alignment, sagittal alignment, and postoperative union time. The clinical outcomes were evaluated using the visual analogue scale (VAS), American Orthopaedic Foot and Ankle Society (AOFAS) score, and postoperative complications. The results were analyzed statistically by dividing the patients into two groups based on a 10 angle of deformity. Results: Regarding the preoperative coronal ankle alignment, 14 patients had a mean varus deformity of 17.8 ±14.5 and six had a mean valgus deformity of 8.1 ±6.6. Postoperatively, a satisfactory postoperative coronal ankle alignment of less than 5 was obtained in all patients. Regarding the preoperative hindfoot alignment, 12 patients showed a mean varus deformity of 15.2 ±10.5 and six had a mean valgus deformity of 8.1 ±4.2. In total, 94.4% (17 patients) had satisfactory postoperative hindfoot alignment of less than 5. Radiological union was achieved in 90.9% at an average of 19.2 weeks (12 32 weeks) and there were 2 cases of nonunion. The clinical outcomes showed improvement in the mean VAS and AOFAS scores (p<0.001, p<0.001, respectively). Even a preoperative severe deformity more than 10 showed a significant deformity correction of coronal ankle alignment and hindfoot alignment, postoperatively (p<0.001, p<0.001, respectively). No significant differences were found between the patients with a preoperative coronal ankle deformity more than 10 and those less than 10 regarding the mean postoperative coronal ankle alignment (p=0.162). Conclusion: Tibiotalocalcaneal arthrodesis using retrograde intramedullary nailing is an acceptable technique for achieving satisfactory deformity correction, high union rate with minimal complications, and improvement of the clinical outcomes. In addition, tibiotalocalcaneal arthrodesis using retrograde intramedullary nailing is considered an effective treatment option, particularly in severe ankle and hindfoot deformities. 2 134 Jae-Gu Park, et al. 에서사용할수있는수술법이다. 수술의목적은보행시통증없는안정된척행족 (plantigrade foot) 을얻기위한것이다. 1) 경거종골관절유합술에서나사못, 스테이플, 핀, 외고정, 골수강내금속정등과같은많은고정기법들이소개되고있다. 외고정, 압박나사못과금속판은보통술후장기간의비체중부하또는부분체중부하를요하며, 낮은골유합률과관련이있다고알려져있다. 2,3) 반면골수강내고정은다른고정법과비교하여안정성과압박력이우수하여골유합률을향상시키며, 특히골질이좋지않은환자에서도좋은결과를보이고있다. 4) 유합술후의적절한정렬상태는좋은결과와높은환자만족도를위해중요하다. 따라서본저자들은심각한후족부변형및족관절과거골하관절염환자에서역행성골수강내금속정을이용한경거종골관절유합술을시행하여방사선적결과와임상적결과를분석해보고자하였다. 대상및방법 1. 연구대상 2011년도 11월부터 2016년 5월까지인제대학교상계백병원에서역행성골수강내금속정을이용하여경거종골관절유합술을시행받은환자 22명 (22예) 을대상으로후향적으로연구하였다. 심각한족관절및후족부변형, 진행된족관절및거골하관절염, 골지주판 (bone stock) 이불충분하거나불안정한고정이예상되는거골의무혈성괴사나샤르코관절병증, 심한급성골절등이있는경우수술의적응증으로하였다. 환자의평균연령은 57.4세 (22-82세) 였으며, 남자는 12명, 여자는 10명이었다. 평균추시기간은 29.6개월 (12-74개월) 이었다 (Table 1). 본연구는인제대학교상계백병원임상연구윤리위원회 (institutional review board) 의승인을받았다 ( ). 2. 수술방법척추마취또는전신마취를시행한후환자를앙와위로눕힌상태에서수술을시행하였다. 모든예에서병변대퇴부에지혈대를사용하였다. 주로경비골접근법 (transfibular approach) 을사용하였고, 전방접근법을사용하였던수술병력이있었던 3예에서는이전의전방접근법을이용하였으며, 이때에는거골하관절에대해서경비골접근법을추가하였다. 경비골접근법의경우족관절및거골하관절의후방관절면을노출한후연골을제거하고, 해면골을노출시켰으며, K-강선을이용하여연골하골에다발성천공을시행하여유합될부위를전처치하였다. 정렬과해면골의노출이관절유합술에서가장중요한부분이기때문에금속정삽입전에투시검사장치를이용하여정렬상태와관절면의상합성을확인하였다. 금속정이삽입될뒤꿈치패드의바로앞에종적피부절개를하였고, 박리를조심스럽게하여종골에접근 하였다. 족관절이시상면에서중립위, 5 외회전, 5 외반상태를 유지한후족저면을통하여역행적으로골수강내금속정 (Dyna Locking Ankle Nail TM ; U&I Corporation, Seoul, Korea) 을삽입하 였다. 이후경골잠금나사와거골잠금나사를차례대로삽입한 뒤, 종골후면에서전방으로종골잠금나사를삽입하였다. 골결 손부위에제거된비골을전부또는내측일부를이용하여자가 골이식을시행하였으며, 골결손정도에따라자가장골이식또 는동결동종골이식등의추가적인골이식을시행하였다. 술후 8 주간단하지석고붕대고정을포함하여체중부하를금하였으 며, 관절유합의진행상태에따라체중부하의시기가조정될수 있으나문제점이없으면술후 8 주경부터부분체중부하를시작 하였다. 3. 방사선적평가 방사선적평가로수술전후체중부하족관절전후면및측면영 상, 후족부정렬영상 (hindfoot alignment view) 을촬영하여관상 면상족관절정렬각 (coronal ankle alignment) 과후족부정렬각 (hindfoot alignment), 시상면상정렬각 (sagittal alignment), 골유합 Table 1. 결과 역행성골수강내금속정을이용한경거종골관절유합술을시행받은 22명 (22예) 환자의진단명과추가적으로시행한수술은다음과같다 (Table 1). 방사선적결과에서전체환자의평균관상면상족관절정렬각은술전변형각 14.9 ±13.3 에서술후외반 0.2 ±1.9 로의미있게교정되었으며 (p<0.001), 평균후족부정렬각은술전변형각 12.9 ±9.4 에서술후외반 1.7 ±3.

3 135 Tibiotalocalcaneal Arthrodesis Using Retrograde Intramedullary Nailing in Severe Hindfoot Deformity 시기를평가하였다. 후족부정렬영상은기존의 Saltzman의방법으로촬영하였는데, 체중부하상태에서방사선이경골후방에서전방을향하고지면에서 20 경사가되도록투사하였다. 관상면상족관절정렬각은체중부하족관절전후면영상에서경골의해부학적축과거골체의관절면에수직인선이이루는각도로정의하였으며, 5,6) 후족부정렬각은후족부정렬영상에서경골의해부학적축과종골의종축이이루는각도로정의하였다. 7) 시상면상정렬각은체중부하족관절측면영상에서경골의해부학적축과거골의후방결절 (posterior tubercle) 의하부와거골경부의하연을이은선이이루는각도로측정하였다 (Fig. 1). 8) 내반, 외반변형에따른변형정도의해석오류를피해기위해술전변형각을절대값으로측정하여분석하였다. 즉변형각을측정하였을때내반은양의값, 외반은음의값으로측정하였는데, 술전변형의정도의크기를평가하기위해절대값을적용하였다. 술후체중부하족관절전후면및측면영상에서족관절과거골하관절면의 50% 이상에서골소주의연결이보일때를유합이라고정의하였고, 9) 술후 12개월이후에도상기소견이관찰되지않을경우불유합으로정의하였다 (Fig. 2). 10) 유의하다고판단하였다. 결과 역행성골수강내금속정을이용한경거종골관절유합술을시행받은 22명 (22예) 환자의진단명과추가적으로시행한수술은다음과같다 (Table 1). 방사선적결과에서전체환자의평균관상면상족관절정렬각은술전변형각 14.9 ±13.3 에서술후외반 0.2 ±1.9 로의미있게교정되었으며 (p<0.001), 평균후족부정렬각은술전변형각 12.9 ±9.4 에서술후외반 1.7 ±3.5 로의미있게교정되었다 (p<0.001) (Table 2). 전체환자의평균시상면상정렬각은술전 ±17.0 에서술후 ±5.5 로측정되었다. 전체환자에 4. 임상적평가임상적평가로수술전및최종추시시 visual analogue scale (VAS) 과 American Orthopaedic Foot and Ankle Society (AOFAS) 점수를조사하였다. 또한수술후발생한합병증을분석하였다. 5. 통계통계적방법을이용하여결과값을분석하였다. 전체환자및각군의연속변수의정규분포여부를 Kolmogorov-Smirnov test 를이용하여검정하였으며, 모두정규분포를보이고있었다. 수술전과후의방사선적또는임상적결과를분석하기위해 paired T-test를시행하였으며, 두군간의비교는 independent t-test를시행하여통계처리하였다. p-value가 0.05 미만인경우통계적으로 Figure 2. 12명의환자에서술전 10 이상의심한관상면상족관절정렬각을보이고있었으며, 술후평균내반 0.2 ±2.3 로만족스럽게교정되었다 (p<0.001). 또한술전족관절정렬각이 10 이상인군과 10 미만인군사이에술후평균족관절정렬각을비교해보았을때통계적으로유의한차이를보이지않았다 (p=0.162) (Table 3).

5 137 Tibiotalocalcaneal Arthrodesis Using Retrograde Intramedullary Nailing in Severe Hindfoot Deformity 정되었고, 평균교정각은 10.2 ±9.9 였다. 술전 10 이상의심한 후족부변형을보이고있는환자 10 명에서술후평균외반 1.1 ± 4.1 로만족스럽게교정되었으나 (p<0.001), 1 명의요내반족변형 (equinocavovarus deformity) 환자에서술후후족부내반 7.5 로불 충분한교정을보였다. 술전후족부변형이 10 이상인군과 10 미만인군사이에술후평균후족부정렬각은통계적으로유의 한차이를보이지않았다 (p=0.410) (Table 3, Fig. 3). 술전관상면상족관절정렬각이 10 이상인군과 10 미만인군 사이에술전나이, 성별, 추시기간, 진단명의유의한차이는없 었다. 방사선적골유합은 90.9% 에서평균 19.2 주 (12-32 주 ) 에이루 어졌다. 수술중제거된비골을부분적으로또는전부사용하는 것이외에추가골이식이필요한경우 5 예에서자가장골이식, 4 예에서동결동종골이식을시행하였다. 족관절정렬각이 10 이 상인군과 10 미만인군에서골유합시기는각각 18.6±3.7 주, 20.6±6.7 주로두군간에통계적으로유의한차이를보이지않았 다 (p=0.418). 3 예의거골의무혈성괴사에서골유합시기는평균 24.0±4.0 주였으며, 2 예의샤르코관절병증에서골유합시기는평 균 28.0±5.7 주였다. 전체환자에서 2 예의불유합이발생하였으 며, 1 예에서방사선검사에서거골하관절불유합을보였으나, 임 상적증상이경미하여경과관찰하다술후 2 년째내고정물제거 술및골이식술을계획하였고, 수술시내고정물제거후거골하 관절의불안정성이관찰되지않아추가적인골이식술은시행하 지않았다. 1 예에서경거골간의골소주연결이부족하였으나증 상이경미하여경과관찰하였다. 임상적결과에서 VAS 는술전 7.6 에서술후 2.9 로감소되었고 (p<0.001), 평균 AOFAS 점수는술전 35.4 에서술후 70.6 으로증 가되었다 (p<0.001) (Table 2). 관상면상족관절정렬각이 10 이상 인군과 10 미만인군사이에술후 VAS, AOFAS 점수는유의한 차이를보이지않았다 (p=0.192, p=0.101) (Table 4). 술후합병증으로 2 예의불유합, 1 예의종골잠금나사못이완, Table 4. AOFAS, American Orthopaedic Foot and Ankle Society. 1예의골수정후방부위의부분파손이있었다. 모든예에서수술부위감염이나창상문제는발생하지않았다. 고찰 경거종골관절유합술은족관절과거골하관절에영향을미치는심각한후족부질환에시행되는구제술이다. 고정방법으로외고정, 나사못고정, 금속판고정, 역행성골수강내금속정고정등이사용되고있다. 외고정은오랜고정기간으로인해환자에게불편감을주며, 높은불유합과감염률이보고되었고, 11) 나사못고정은금속물이완과낮은유합률과관련이있다. 12) 금속판고정은스트레스골절발생률이높으며, 광범위한연부조직박리를요하는단점이있다. 13) 역행성골수강내금속정은부하전달 (load-sharing) 효과를얻을수있고, 상대적으로작은절개를통해연부조직을보존하는장점이있어많은인기를얻고있다. 유합술후의적절한정렬상태는좋은결과와높은환자만족도를위해중요하다. 따라서본연구의목적은심각한후족부변형, 진행된족관절및거골하관절염에서역행성골수강내금속정을이용한경거종골관절유합술을시행하여방사선적결과와임상적결과를분석해보고자하였고, 특히심각한족관절및후족부변형을가지고있는환자에서변형교정에있어서의가치를분석해보고자하였다. 이전에시행된대부분의연구에서변형교정에대한결과가없거나연구가시행된경우에도술전변형의정도가정량화되어있지않았고, 오직일부연구에서변형교정에대한정량화된결과가보고되었다. Brodsky 등 14) 은심한각변형을동반한환자 29명 (30예) 에서역행성골수강내금속정을이용한경거종골관절유합술을시행하였으며, 수술후 76% 에서관상면상변형각이 5 미만임을보고하였다. 또한 Fang 등 4) 은 22 명의환자에서외반곡선형 (valgus-curved) 금속정을이용한경거종골관절유합술을시행하였고, 수술후모든환자에서후족부정렬각 5 미만의만족스러운교정을얻었음을보고하였다. 본연구에서전체환자에서평균관상면상족관절정렬각및후족부정렬각은술후만족스럽게교정되었으며, 또한술전 10 이상의심한변형을보이고있는경우에도수술후의미있는변형교정을보였다. 한편본연구에서시상면상정렬각은술전 ±17.0 에서술후 ±5.5 로측정되었으며, 표준편차값을보면술후시상면상정렬각이술전에비해평균값에근접하여일관되게교정되었음을알수있었다. 또한저자들의연구에서측정된술후시상면상정렬각은평균 로 Buck 등 8) 이시행한연구에서시상면상중립으로고려하였던각도인 106 와비슷하게측정되었다. 역행성골수강내금속정을이용한수술법의유합률은여러저자들에의하여 76% 에서 100% 의높은유합률을보고하고있으며, 유합기간은대략 16주이다 ) 저자들의경우에도 90.9% 의골유

6 138 Jae-Gu Park, et al. 합률을보였으며, 19.2주 (12-32주) 의평균유합기간을확인할수있었다. 본연구에서 2예의불유합이있었으며, 1예에서수술전족관절외반변형 10 이상의심한변형을동반한원위경골부불유합환자, 1예에서변형이경미하였지만골질이좋지않은류마티스관절염환자에서발생하였다. 골유합률은환자의기저질환, 수술전골질, 수술중해면골이노출될때까지관절연골의완전한제거와적절한정렬, 내고정시사용하는기구의생역학적특성, 수술후환자의순응도등에의해영향을받는것으로알려져있다. 본연구에서수술전변형의정도에따라골유합률의차이를나타내지않았지만골유합이여러요인에의해영향을받을수있기때문에추후이러한요인들을충분히고려한추가적인연구가필요할것으로생각된다. 또한본연구에서 3예의거골의무혈성괴사에서골유합시기는평균 24주였으며, 2예의샤르코관절병증에서골유합시기는평균 28.0주로상대적으로골유합시기가지연되는것을확인하였지만그증례의수가적어통계적분석에제한점이있었다. 하지만골질이불충분하거나불안정한고정이예상되는경우골유합을도모하기위해추가적인고정기구의사용또는골이식등을고려할수있겠다. 역행성골수강내금속정을이용한경거종골관절유합술후기존의연구에서평균 AOFAS 점수는 54점에서 80점으로보고되고있으며, 1) 본연구에서는술전평균 AOFAS 점수 35.4에서술후 70.6으로증가를보였다. 한편평균 VAS 점수는술전평균 7.6에서술후 2.9로의감소를보였다. 본연구에서수술시접근법은주로경비골접근법을사용하였고, 전방접근법을사용하였던수술병력이있던 2예에서이를이용하였다. 전방접근법을사용하였던경우중 1예에서비골전절제술을시행하였으며, 이를이용하여골결손부위에이식하였다. 전체환자에서비골전절제술을시행한경우는 10예였으며, 비골부분절제술을시행한경우는 8예였다. 본연구에서는수술후비골절제술과관련된합병증은보이지않았다. 역행성골수강내금속정의형태로곡선형또는직선형금속정이있고, 술자의선호도따라적절한금속정을선택하게된다. 종골이경골의해부학적축에대해외측으로치우쳐있는관계로직선형금속정을사용할때종골의내측화 (medialization) 없이중앙부위로금속정을삽입할경우너무외측으로경골을통과하게되며, 경골의협부 (isthmus) 를지날때후족부내반이발생할수있으며, 경골의스트레스골절발생위험이있다. 20,21) 이러한합병증을피하기위해외반굴곡된금속정이개발되었으며, 외측삽입구로인해신경혈관구조물에대한손상가능성을줄일수도있다. 하지만심하거나불안정한변형교정이필요한경우외반굴곡된금속정의삽입은금속정삽입을위한관절확공으로인해교정의소실을일으킬수있다. 22) 저자들은직선형의금속정을사용하여경골의해부학적축에가능한일치하도록종골삽입부위를내측으로위치시켰으며, 수 술중투시검사장치를사용하여족관절정렬및후족부정렬상태의적절성을확인하였다. 수술중체중부하영상을촬영할수없는제한점이있기때문에본연구에서는경골의축을중립으로위치시킨상태에서족부를족배굴곡하여방사선이후족부중앙을향하게 40 정도머리방향 (cranially) 으로경사가되도록투사하는종골축상영상을이용하여경골에대한후족부정렬상태의적절성을확인하였다. 기존의금속정삽입지점에서신경혈관손상은알려진합병증이지만 23) 저자들의경우에조심스러운박리와적절한견인을통해신경혈관손상발생에주의하였고, 술후족부의발바닥감각이상이나통증을경험하지는않았다. 역행성골수강내금속정을이용한경거종골관절유합술의수술후합병증은 20% 에서 50% 보고되고있으며, 19,24,25) 창상회복지연, 감염, 종골골절, 금속자극증상, 고정실패, 스트레스골절, 절단등의합병증이있다. 저자들은 2예의불유합, 1예의종골잠금나사못이완, 1예의골수정후방부위의부분파손을경험하였다. 경골스트레스반응과골절은금속정근위부에서증가된굽힘력으로인해발생할수있지만, 15) 본연구에서경골스트레스골절은발생하지않았다. 수술부위감염은 10% 이내로보고되고있으며, 26-28) 저자들의경우수술부위감염이나창상문제는발생하지않았다. 본연구의제한점으로는후향적연구이며, 상대적으로연구대상의수가부족한점을들수있다. 연구에포함된환자들이동일한질환이아닌다양한질환군의환자를대상으로유합술을시행하였으므로연구의결과를일반화하기에는제한이있다. 또한수술전후관상면상족관절정렬각, 후족부정렬각, 시상면상정렬각을측정하였지만관절유합술후발생할수있는인접관절관절염발생등장기추시결과를조사하지못했기때문에향후이와관련한추가적인연구가필요할것이다. 결론 역행성골수강내금속정을이용한경거종골관절유합술은만족스러운변형교정력, 높은골유합률과낮은합병증을보였으며, 임상적결과의향상을보였기에유용한수술방법중의하나라고생각된다. 특히심각한족관절및후족부변형을동반한환자에서도효과적인치료방법으로고려될수있다. 8 140 pissn : , eissn : Original Article J Korean Orthop Assoc 2019; 54: 심한후족부변형및경거종골간관절염에서골수강내금속정을이용한경거종골관절유합술의가치 박재구 * 정형진 배서영 이정환 김휘영 이준석 인제대학교상계백병원정형외과, * 대림성모병원정형외과 목적 : 심한후족부변형및족관절및거골하관절염에서역행성골수강내금속정을이용한경거종골관절유합술을시행하여방사선적결과와임상적결과를분석해보고자하였다. 대상및방법 : 심한후족부변형이나진행된관절염으로역행성골수강내금속정을이용한경거종골관절유합술을시행받은 22명 (22예) 을대상으로하였다. 연구대상의평균연령은 57.4세 (22-82세), 평균추시기간은 29.6개월 (12-74개월) 이었다. 방사선적평가로수술전후관상면상족관절정렬각, 후족부정렬각, 시상면상정렬각을측정하였으며, 술후골유합시기를평가하였다. 임상적평가로 visual analogue scale (VAS), American Orthopaedic Foot and Ankle Society (AOFAS) 점수및합병증을분석하였다. 또한변형각 10 를기준으로두군으로나누어결과를통계적으로분석하였다. 결과 : 전체환자에서평균관상면상족관절정렬각은술전내반변형을보인환자 14명에서 17.8 ±14.5 였으며, 외반변형을보인환자 6명에서 8.1 ±6.6 였다. 술후모든예에서관상면상족관절정렬각은 5 미만으로만족스럽게교정되었다. 평균후족부정렬각은술전내반변형을보인환자 12명에서 15.2 ±10.5 였으며, 외반변형을보인환자 6명에서 8.1 ±4.2 였다. 술후 94.4% (17명) 에서후족부정렬각은 5 미만으로만족스럽게교정되었다. 방사선적골유합은 90.9% 에서평균 19.2주 (12-32 주 ) 에이루어졌으며, 2예의불유합이있었다. 임상적결과에서술후평균 VAS 및 AOFAS 점수는유의하게향상되었다 (p<0.001, p <0.001). 술전변형각 10 이상의심한변형을보인경우에도술후관상면상족관절정렬각과후족부정렬각은의미있게교정되었다 (p<0.001, p<0.001). 또한관상면상족관절정렬각이 10 이상인군과 10 미만인군사이에술후평균관상면상족관절정렬각은통계적으로유의한차이를보이지않았다 (p=0.162). 결론 : 역행성골수강내금속정을이용한경거종골관절유합술은만족스러운변형교정력, 높은골유합률과낮은합병증을보였으며, 임상적결과의향상을보였기에유용한수술방법중의하나라고생각된다. 특히심각한족관절및후족부변형을동반한환자에서도효과적인치료방법으로고려될수있다. 색인단어 : 변형, 관절염, 경거종골관절유합술, 골수강내금속정 접수일 2018 년 2 월 1 일수정일 2018 년 4 월 3 일게재확정일 2018 년 4 월 29 일책임저자정형진 01757, 서울시노원구동일로 1342, 인제대학교상계백병원정형외과 TEL , FAX , orthoman@paik.ac.kr, ORCID 대한정형외과학회지 : 제 54 권제 2 호 2019 Copyright 2019 by The Korean Orthopaedic Association This is an Open Access article distributed under the terms of the Creative Commons Attribution Non-Commercial License ( which permits unrestricted non-commercial use, distribution, and reproduction in any medium, provided the original work is properly cited.
