1 = 증례보고 = 소아갑상선안병증의임상적특징 대한안과학회지 2013 년제 54 권제 8 호 J Korean Ophthalmol Soc 2013;54(8): pissn: eissn: 배정범 1 정혜미 1 유재호 2 권윤형 1 안희배 1 동아대학교의과대학안과학교실 1, 동아대학교의과대학소아청소년과학교실 2 목적 : 소아갑상선환자를대상으로소아갑상선안병증의임상특징과치료에대하여알아보고자하였다. 대상과방법 : 2010 년 9 월부터 2012 년 9 월까지본원소아과에서자가면역성갑상선질환을진단받고, 갑상선안병증진료를위해내원한환자 34 명을대상으로소아갑상선안병증의발생률과임상적특성에대하여의무기록을후향적으로분석하였다. 결과 : 추적관찰기간 24.1 개월동안그레이브씨병환자가 14 명 (41.2%), 하시모토갑상선염환자가 20 명 (58.8%) 이었고, 34 명중 15 명 (44.1%) 에서안증상이발생하였다. 눈꺼풀부종 14 명 (41.2%), 눈꺼풀후퇴 8 명 (23.5%), 눈꺼풀내림지체 6 명 (17.6%) 등눈꺼풀증상이가장많았다. 결막충혈 5 명 (14.7%), 안구돌출 3 명 (8.8%), CT 상외안근비대 6 명 (17.6%) 이었으며, 그외시력저하, 시야결손, 외안근운동장애등의심한안증상은없었다. 3 명 (8.8%) 에서눈꺼풀후퇴교정술이필요하였으나, 대부분은보존적치료만으로유지되었다. 결론 : 소아갑상선안병증은그레이브씨병과하시모토갑상선염에서모두빈번하게관찰되었고, 성인에비해임상양상의발현과경과가경미하여보존적치료로유지되는특징을가진다. < 대한안과학회지 2013;54(8): > 갑상선안병증 (Thyroid associated ophthalmopathy, TAO) 은안와의염증성질환으로그레이브씨병 (Graves disease) 에서흔히발생하지만, 하시모토갑상선염 (Hashimoto s thyroiditis) 에서도발생한다고보고되고있다. 1 성인에서의 Graves' disease는연간 100,000명당 15-20명에서발생한다고알려졌다. 2 하지만소아에서의 Graves' disease는흔하지않아서 Perrild et al 3 은 1,000,000명당 7.9명, Kraiem and Newfield 4 는 4세이하에서 1,000,000명당 1명, 4세이상의소아에서 1,000,000명당 38명이발생한다고보고하였다. 성인에서갑상선안병증의주된증상으로눈주위부종, 눈꺼풀후퇴, 안구돌출, 결막부종, 외안근제한과같은연조직침범소견이발생하며, 심한경우각막궤양및압박시신경병증으로인한시력상실과같은증상이나타난다. 대부분의환자는가벼운연조직부종과눈꺼풀후퇴만을겪게되지만, 10-15% 의환자는심한임상양상이발현된다. 반면소아에서갑상선안병증의임상양상은경한안구돌출과눈 Received: Revised: Accepted: Address reprint requests to Hee Bae Ahn, MD, PhD Department of Ophthalmology, Dong-A University Medical Center, #26 Daesingongwon-ro, Seo-gu, Busan , Korea Tel: , Fax: hbahn@dau.ac.kr * This study was presented as a narration at the 108th Annual Meeting of the Korean Ophthalmology Society * This article has been supported by the Intramural Research Fund of the Dong-A University of medicine. 꺼풀부종, 눈꺼풀후퇴등대개증상이경미하여수년내에자연호전되는경우가많아경과관찰을주된치료로한다. 5,6 지금까지소아갑상선안병증의임상적특징에관한연구는성인을대상으로한연구에비해적고, 또한중국인을대상으로한몇몇연구들 7,8 이외엔북미와유럽등백인을대상으로한연구가대부분이었다. 따라서아직국내에서는소아갑상선안병증에대한연구가아직보고된바없기에그특징에대한연구가필요하다고생각한다. 이에본연구에서는본원소아과에서갑상선질환을진단받고갑상선안병증진료를위해내원한환자들을대상으로우리나라소아갑상선안병증환자의발생률과임상양상, 특성, 치료에대해알아보고자하였다. 대상과방법 2010년 9월부터 2012년 9월까지동아대학교병원소아과에서자가면역성갑상선질환을진단받고안구돌출및눈꺼풀부종이관찰된환자 34명을대상으로임상양상및동반질환등의특성에대하여환자의의무기록으로후향적검토및분석을하였다. 대상환자 34명중 1명은눈꺼풀부종증상으로안과로내원하여갑상선안병증을진단받고갑상선기능검사를시행하였으며, 갑상선기능항진증으로소아과협의진료를시행하였다. 한국인에서소아갑상선안병증환자의발생률과임상양상, 특성에대해알아보기위해성별, 갑상선질환진단

2 - 대한안과학회지 2013 년제 54 권제 8 호 - 시나이, 갑상선기능검사, 갑상선병증의유형, 갑상선치료병력, 가족력, 간접흡연력, 안증상및검사, 치료여부및그결과를조사하였다. 소아과초진시시행한갑상선기능검사항목으로 TSH, free T4, 갑상선자극호르몬수용체 (thyroid stimulating hormone receptor antibody, TRAB), 항갑상선과산화효소항체 (thyroid peroxidase antibody, TPO-Ab) 및항티로글로불린항체 (anti-thyroglobulin antibody, Tg-Ab) 검사를시행하였으며, 갑상선혈청자가항체와갑상선안병증의임상활동도및중증도의연관성에대해알아보고자하였다. 대상환자는진단후 3개월간격으로추적관찰하였고, 외래방문시안과적문진, 시력검사, 시야검사, 정면주시와상, 하방주시시안압검사, 세극등현미경검사, 안구돌출검사, 색각검사, 외안근운동검사, 복시검사, 위눈꺼풀각막반사간거리 (MRD1), 눈꺼풀올림근기능검사, 쉬르머검사 (schirmer test) 등을시행하였다. 안와컴퓨터단층촬영은초진시안증상이있는환자 15명을대상으로시행하였다. 이를바탕으로활동기여부는 Mourits 9 의임상활동도 (clinical activity score) 로측정하였고, 중증도 (severity) 는 modified NOSPECS classification 10,11 으로분류하였다 (Table 1). 각측정값의통계적분석은 SPSS software 프로그램 (SPSS 12.0 K for Windows) 을이용하였다. 연령, 경과관찰기간, 갑상선질환으로부터갑상선안병증의진단까지의기간은 Mann-Whitney U test를통해그레이브씨병과하시모토갑상선염환자군을비교하였고, 진단시갑상선호르몬상태와임상특징은 Pearson s chi-square test를이용하여검정하였다. 갑상선혈청자가항체와갑상선안병증의임상활동도, 중증도의연관성은스피어만상관계수 (Spearman correlation coefficient) 를이용한단순상관분석 (simple correlation analysis) 으로검정하였다. p값이 0.05 미만인경우를통계학적으로의미있는것으로하였다. 결과 환자들의평균나이는 12.9세 (7-19 세 ) 였으며, 남자가 4 명 (11.8%), 여자가 30명 (88.2%) 으로여자대남자의비율은 7.5:1이었고, 평균추적관찰기간은 24.1개월 (2-47 개월 ) 이었다. 소아과에서갑상선질환진단시평균나이는 11.1세 (5-16 세 ) 며, 갑상선질환으로진단받은후갑상선안병증으로진단받기까지평균기간은 18.2개월 ( 즉시-55 개월 ) 이었다. 갑상선질환가족력이있는경우는 8명 (23.6 %) 이며, 흡연자는없었으나간접흡연력이있는경우가 13 명 (38.2 %) 에해당하였다 (Table 2). 34명의갑상선질환환자를그레이브씨병 14명 (41.2%), 하시모토갑상선염 20명 (58.8%) 으로두군으로분류하였 Table 1. CAS = clinical activity score. 다. 그레이브씨병 14명중갑상선기능검사상정상갑상선기능상태 1명을제외한 14명 (93.3%) 의갑상선기능항진상태환자에서는항갑상선약물치료인 Methimazole (Methimazole ) 를시행하였다. 하시모토갑상선염환자 20명중 9명 (45.0%) 은갑상선기능저하상태로갑상선호르몬제인 Levothyroxine sodium (Synthyroxin ) 치료를시행하였으며, 갑상선기능항진상태 4명 (20.0%) 과정상갑상선기능상태 7명 (35.0%) 에서는갑상선기능검사를주기적으로시행하며경과관찰하였다. 34명의갑상선질환환자중안증상이발생한경우는 15 명 (44.1%) 이었고, 이중그레이브씨병은 9명 (64.2%), 하시모토갑상선염은 6명 (30.0%) 이었다. 임상특징으로눈꺼풀부종 14명 (41.2 %), 눈꺼풀후퇴 8명 (23.5%), 눈꺼풀내림지체 6명 (17.6%) 등눈꺼풀증상이가장많았다. 각막및결막증상으로결막충혈 5명 (14.7%), 결막부종 3명 (8.8%), 표재성점상각막염 3명 (8.8%) 이었다. 안구돌출증상은 3명 (8.8%), 안와단층촬영상외안근비대가관찰된경우는 6명 (17.6%) 이었다. 평균최대교정시력은우안 1.0, 좌안 1.1이며, 시력저하소견은없었다. 그외시야결손, 색각이상, 외안근운동장애, 복시와같은심한안증상은없었다. 그레이브씨병환자에서하시모토갑상선염환자보다증상발현율이높았으나, 두군간의차이는통계적으로유의하지않았다 (p=0.703, Pearson s chi-square test) (Table 3). 평균안구돌출값은우안 15.8 mm ( mm), 좌안 15.1 mm ( mm) 이며, 안구돌출증상이있는 3명에서는평균우안 19.4 mm ( mm), 좌안 19 mm (19-22 mm) 로증가되었다. 위눈꺼풀각막반사간거리 (MRD1) 는평균우안 3 mm (1-6 mm), 좌안 3 mm (2-6 mm), 눈꺼풀올림근기능검사는평균우안 11.8 mm (7-16 mm), 좌안 11.1 mm (6-14 mm) 이며, 아래눈꺼풀위공막노출은평균우안 0.4 mm (0-2 mm), 좌안 0.4 mm (0-2 mm) 이었다. 쉬르머검사 (schirmer test) 는평균우안 17.8 mm (2-30 mm), 좌안 18.6 mm (2-30 mm) 로 10 mm 미만은 8명 (22.9%) 이었다. 평균안압은정면주시시우안 15.8 mmhg, 좌안 15.5 mmhg, 상방주시시우안 16.2 mmhg, 좌안 16.0 mmhg, 하방주시시우안 15.9 mmhg, 좌안 15.8 mmhg 로정상소견으로관찰되었다. 갑상선안병증진단시임상활동도점수결과 19명 (55.9 %) 은 0점으로, 50% 이상에서는안증상이나타나지않았다. 안증상이있는 15명 (44.1%) 의환자에서는안검부종및결막부종등의소견으로 1점 8명 (23.6%), 2점 3명 (8.8%), 3점 3명 (8.8%), 4점 1명 (2.9%) 이며, 5점이상은없었다. 그레이브씨병과하시모토갑상선염환자의임상활동도비교시두군간의통계적유의성은보이지않았다 (p=0.138, Pearson s chi-square test) (Table 4). 4 - 대한안과학회지 2013 년제 54 권제 8 호 - 증상이나타나지않은 19명 (55.9%) 은 0점에해당하였다. 안증상이있는 15명 (44.1%) 의환자에서는안와단층촬영상외안근비대및각막손상소견으로 1점 11명 (32.3%), 2점 4명 (11.8%) 이며, 3점이상은없었다. 그레이브씨병과하시모토갑상선염환자군의중증도분류에따른차이는통계적으로유의하지않았다 (p=0.142, Pearson s chisquare test) (Table 5). 소아과초진시시행한 TSH, free T4, 갑상선자극호르몬수용체, 항갑상선과산화효소항체, 항티로글로불린항체를정량화하여갑상선안병증의임상활동도, 중증도와의연관성을스피어만상관계수 (spearman correlation coefficient) 를 이용한단순상관분석 (simple correlation analysis) 을시행하였다. TSH, free T4, 갑상선자극호르몬수용체, 항갑상선과산화효소항체, 항티로글로불린항체모두갑상선안병증의임상활동도와중증도에대해통계적으로유의하지않았으며 (p>0.05), 낮은상관관계를보여주었다 (-0.1<r<+0.1). 이중 free T4는안구돌출계검사에서우안과좌안모두안구돌출정도에따라의미있는양적상관관계를보여주었다 (r=0.361, r=0.405) (Fig. 1). 대상환자의치료로인공눈물및치료용렌즈처방등보존적치료를시행한경우가 91.2% 로대부분이었으며, 스테로이드치료나방사선치료가필요한경우는없었다. 눈꺼 Table 5. 5 - 배정범외 : 소아갑상선안병증의임상적특징 - 풀후퇴로인한각막노출소견및양안크기차이로인한미용상의목적으로 3명 (8.8%) 에서수술적치료로눈꺼풀후퇴교정술을시행하였다 (Table 6). 고찰 갑상선안병증은 Graves ophthalmopathy로알려질만큼그레이브씨병에서흔히발생하며, 하시모토갑상선염에서의안병증에대한보고는드물었다. 1 본연구에서갑상선안병증환자 15명중그레이브씨병이 9명 (64.2%), 하시모토갑상선염이 6명 (30.0%) 으로기존의보고에비해하시모토갑상선염안병증이많은수에서나타난것을확인하였다. 갑상선안병증은일반적으로여자에서더많이발생하는것으로알려졌는데, Kendler et al 12 은 3.5배, Perros et al 13 는 4.1배, Bartley 14 는 6.1배여성에서남성보다더많다고하였다. 소아를대상으로시행한본연구에서도비슷한결과를보였는데, 여성이 30명 (88.2%), 남성이 4명 (11.8%) 으로여성대남성의비가 7.5:1이었다. 이를통해소아의갑상선안병증에서도여성의비율이높음을확인하였다. Bartley et al 15 은성인갑상선안병증연구에서 140명의환자중 100% 의환자가눈꺼풀후퇴를보였고, 62% 는안구돌출을, 43% 는제한성근병증을, 6% 에서시신경병증을보였으며, 이중상기증상을모두보인경우는 5% 로보고하였다. 국내에발표된연구에서도눈꺼풀후퇴가나타난환자가가장많았으며, 안구돌출, 안와주변조직부종, 복시및외안근운동제한, 노출성각막병증, 시신경병증순으로발현되었다. 16,17 반면소아에서의주된임상양상은경한안구돌출과눈꺼풀부종, 눈꺼풀후퇴등으로알려졌다. 지난 40년간발표된연구결과를비교해보면 Young 18 은 33명을대상으로시행한연구에서 52% 에서무증상, 36% 에서눈꺼풀부종, 9% 에서연부조직침범, 3% 에서안구돌출이있었으며, 시신경손상은관찰되지않았다고하였다. Liu et al 19 은 8명의환자를통해다양한눈꺼풀증상과안구돌출소견을관찰하였으며, 안와컴퓨터단층촬영상안와지방부피증가및경미한외안근침범소견을확인하였다. Goldstein et al 20 의연구에서 26명의환자중안구돌출 10명 (38%), 눈꺼풀후퇴 6명 (23%), 표재성점상각막염 3명 (12%) 이있었으나, 시신경병증은관찰되지않았다고하였다. 본연구에서도눈꺼풀부종 14명 (41.2%), 눈꺼풀후퇴 8명 (23.5%), 눈꺼풀내림지체 6명 (17.6%), 표재성점상각막염 3명 (8.8%), 안구돌출 3명 (8.8%) 으로주로경한증상이발생한것을확인하였다. 소아의갑상선안병증에서는임상양상의발현이성인보다경미하게나타나며, 이는해외의이전연구결과와비교시우리나라환자의임상양상도유사함을확인할수있었다 (Table 7). 안구돌출계측정값은기계의종류나성별, 나이등에의해차이가있지만, 유럽성인의정상범위는 20 mm 보다작거나, 양안의차이가 2 mm 미만으로알려졌다. 21,22 Nucci et al 21 은 3-10세를대상으로한정상안구돌출계측정연구에서평균값을 3세군에서 9.1 mm, 5세군에서 9.9 mm, 7세군에서 11.3 mm, 10세군에서 11.7 mm로보고하였다. Dijkstal et al 22 은 1-4세에서 13.2 mm, 5-8세에서 14.4 mm, 9-12세에서 15.2 mm, 13-17세에서 16.2 mm 로보고하였으며, 11세이상에서는사춘기에따라다양한값을보이기때문에 18 mm를초과하는것을병적인상태로보았다. 본연구에서안구돌출의기준값은 18 mm 이상으로정하여 3명 (12%) 에서안구돌출소견을확인하였다. 안병증의활동기여부를나타내는임상활동도점수는 0점이 19명 (55.9%) 으로가장많은비율을차지하였으며, 1점 8명 (23.6%), 2점 3명 (8.8%), 3점 3명 (8.8%), 4점 1명 (2.9%) 이며, 5점이상은없었다. 소아를대상으로한 Eha et al 6 의연구에서임상활동도점수는 0점이 8명 (44.4%), 1점이 8명 (44.4%), 2점이 2명 (11.2%) 이었으며, 3점이상을보인경우는없어활동성이낮은상태인결과로본연구에서의결과와유사하였다. 우리나라성인을대상으로한 Kim and Lee 23 의연구에서임상활동도점수는 0점, 1점, 2 점, 3점을보였던경우가각각 5명 (4%), 37명 (29.6%), 27 명 (21.6%) 이었으며, 4점이상의활동성이높은경우가 38 명 (30.4%) 인것을통해성인에비해소아에서는안병증의활동성이낮음을확인하였다. Chan et al 7 의연구에서 0점 31명 (37.3%), 1점 21명 (25.3%), 2점 12명 (14.5%), 3점 7명 (8.4%), 4점 1명 (1.2%), 5점 11명 (13.3%) 이었다. 본연구에서 0점 19명 (55.9%), 1점 11명 (32.3%), 2점 4명 (11.8%) 이며, 3점이상은없었으며, 이는기존의연구결과와비슷한양상을나타냄을확인하였다 (Table 8). 안와컴퓨터단층촬영상 6명 (17.6%) 에서외안근비대소견이관찰되었으나, 약물치료를필요로하지않았으며, 경과관찰이가능한상태로소아에서는성인에비해외안근침범의빈도가낮은것을확인할수있었다. 흡연은갑상선안병증의발생과진행의위험인자로조직의저산소증및항염증사이토카인의조절에의해발생하는것으로알려졌다. 27,28 흡연과갑상선안병증의위험성에대한많은연구에서흡연자는갑상선안병증발생의위험성이더높고, 흡연량이많을수록위험성도높아진다고보고되고있다. 29,30 소아에서의증상및경과가성인에비해경한점에대하여흡연의영향이적은것을하나의요인으로들고있으며, 간접흡연도갑상선안병증의염증반응을진행시킬수있으므로흡연을피해야한다고알려졌다 대상환자중갑상선안병증으로약물치료나방사선치료를시행한환자는없었으며, 갑상선질환진단시시행한약물치료로갑상선기능조절이가능한점으로갑상선안병증의치료에서갑상선기능을안정화시키는것이중요한부분으로생각한다. 6,33-35 본연구에서는우리나라소아갑상선안병증의특성에대해알아보는것에의미를두었다. 소아갑상선환자에서안증상이나타난경우는 42.9% 로성인에서처럼연조직침범과눈꺼풀후퇴등의증상의발생빈도가높았으며, 일부에서는경도의안구돌출이나타났다. 3명에서눈꺼풀후퇴 교정술이필요하였으나, 대부분은보존적치료만으로유지되었다. 외국의연구와유사하게성인에비해임상양상발현이경미하고, 보존적치료를주된치료로하며, 심각한합병증이나타나지않는특징을확인하였다. 대상환자들이일정지역에서특정병원으로제한되었다는점과적은환자수, 후향적인연구방법은본연구의한계점으로볼수있다. 최근성인의갑상선안병증연구에서는갑상선자극호르몬수용체항체와갑상선안병증과의연관성에대하여보고되고있으며, 본연구에서도갑상선기능검사를정량화하여평가하였으나통계학적으로유의한연관성은없었다. 적은환자수로인한통계처리의제한점으로생각하며, 추후혈청자가항체와갑상선안병증의임상활동도, 중증도와의연관성에대하여많은수의환자를대상으로전향적인연구가필요할것으로생각한다. 국내에아직보고된바없는우리나라의소아갑상선안병증환자의임상적특징에대한연구를통해향후연구에대한새로운과제를제시하며, 소아갑상선환자의진료및치료향상에도움을줄수있을것이라생각한다. Methods: To determine the prevalence and clinical characteristics of pediatric thyroid-associated orbitopathy, we retrospectively analyzed the medical records of 34 patients admitted to the Pediatrics Department of our institution between September 2010 and September The patients had been diagnosed with autoimmune thyroid disorder and were admitted for treatment of thyroid-associated orbitopathy. Results: In the 24.1 months of follow-up observation, 14 patients were diagnosed with Graves disease (41.2%) and 20 patients with Hashimoto s thyroiditis (58.8%). Of the 34 patients, 15 (44.1%) developed eye symptoms. Among the eye symptoms, lid swelling was the most prevalent in 41.2% of patients, followed by eyelid retraction in 23.5%, lid lag in 17.6%, conjunctival injection in 14.7%, proptosis in 8.8%, and extraocular muscle hypertrophy in 17.6%. Severe impairment of visual acuity, visual field, and ocular motility were not observed in our study. Although eyelid surgery was performed in 3 cases, most patients were treated conservatively. Conclusions: Pediatric thyroid-associated orbitopathy was frequently observed in patients with Graves disease and Hashimoto s thyroiditis. Compared to adults, children showed milder disease manifestation and progression, and the disease could be managed with conservative treatment.
