Methods : There were, transplant recipients between 969 and at Kangnam St. Mary's hospital. The mean followup period was 8 77 months. We retrospectively analyzed the incidence, clinical course, treatment and prognosis of malignancy in the kidney transplant recipients. Results : The incidence of malignancy after transplantation was 7.% (. cases out of patients). The incidence of malignant lymphoma, thyroid cancer, renal cell carcinoma and Kaposi's sarcoma were higher in the renal transplanted patients than in the general population. The cancer incidence for women was higher than that for men, with stomach cancer being the most common in males and uterine cervix cancer the most common in females. The cumulative incidence of posttransplant malignancy at 99, 99, and were.7%,.9%,.6% and 7.%, respectively. The cancer incidence with the use of initial immunosuppressive agents was 8.% for azathioprine, 7.6% for cyclosporine, and.% for tacrolimus. The mean times for making the diagnosis of malignancy after transplantation were 7±6 months for azathioprine, 9±9 months for cyclosporine, and 7±8 months for tacrolimus, respectively. During the observational period, 7 patients died ( patients died of cancer) and patients were still alive (7 grafts failed). Conclusions : The incidence of malignancy after renal transplantation increases according to the longer followup period. 2 The Korean Journal of Medicine : Vol. 7, No., 7 서론면역억제요법의향상은급성거부반응의발생율을꾸준히감소시키고이식신생존기간을연장시켰지만, 이와반대로이식후악성종양이증가하는원인이되고있다. 신이식후발생한악성종양을최초로보고했던 Penn은악성종양의발생률이약 ~6% 에이르며, 종양의종류에따라일반인구보다 배의발생위험도를가진다고하였고, ), Gaya 등은악성종양의누적발생율이시간이경과할수록증가하여이식후 년뒤 8.%, 년뒤 9.6% 까지증가한다고하였다 ). 최근이식신및환자생존율이증가되면서면역억제기간이장기화되고, 신이식환자들의연령이점차높아지면서악성종양의위험성인식이확대되고있다. 이전의보고에서일본과국내악성종양발생율은약 ~% 로보고되어왔다 6). 저자들은 99년과 년도에각각신이식후종양발생율을각각.% 와.% 로보고하였다 7, 8). 이는신장이식후추적기간이길어질수록종양의발생은증가추세를보인다는이전보고와일치하였다, ). 저자들은이전보고를토대로최근까지발생한악성종양환자를정리하여신이식후악성종양의발생율변화와발생시기, 종양의임상특징과생존율을분석하고일반인구에서발생한악성종양의발생율과비교하고자하였다. 대상및방법 969년부터 년까지가톨릭대학교강남성모병원에서신장이식을시행받은,명을대상으로하였다. 남녀비는 :, 이식시나이는 7세 (~7) 세, 이식신의종류는생체혈연공여자가 9예 (6.%), 비혈연간공여자가 9예 (6.%), 사체신이식은 6예 (.%) 였으며, 원인신질환은만성사구체신염이,9예 (7.%) 이었으며고혈압 9예 (6.%), 당뇨병 79예 (.%), 만성신우신염이 예 (.%), 다낭성신증 9예 (.%), 전신성홍반성루프스 6예 (.%), 기타 예 (.6%), 원인불명 예 (9.6%) 였다. 초기면역억제제로서 Azathioprine ( 이하 AZA로약함 ) 을사용한환자는 7예 (.7%), Cyclosporine A ( 이하 CsA로약함 ) 를사용한환자는,7예 (79.%), FK 6을사용한환자는 7예 (8.7%) 이었다. 대상환자의평균추적관찰기간은 8±77 개월이었다. 본연구에서는이식환자에서발생한종양의빈도를 일반인에서발생한연령표준화암발생율로기대치를계산하여발생비율을구하였다. 국내의연령표준화암발생율은 년 IACR (International Association of Cancer Registeries) Vol. VIII ) 과 년한국중앙암등록본부보고서에기록된 999 한국암발생등록자료 ) 를이용하였다. 다변량 Cox 비례위험분석을통하여암발생율에영향을미치는위험인자 ( 수여자의성별과나이, 면역억제제수, 공여자유형, 이식횟수, 원인신질환, 신대체요법종류, 거부반응 ) 들을평가하였다. 결과,명의신장이식환자중에서악성종양은 명 (7.%) 의환자에서 예가발생하였다. 개종류의암이발생했던환자중 예는이식 개월후우측신장에 Wilms 종양이발생하여우측신절제술을하고 8개월뒤좌측신장, 요관, 방광에이행상피세포암이발생했었고, 다른 예는이식 9개월후방광에카포시육종이발생한지 개월뒤우측슬관절부위에피부암 ( 편평상피세포암 ) 이발생하였다.. 악성종양의발생빈도악성종양은 98년에처음발생한이후 년간 예만발생하였으나 99년부터 년까지매년 8예씩발생하였고, 년이후에는매년 예씩발생하고있으며, 연도별악성종양의누적발생률은.7% (명/9 명, 99년 ),.9% (9예/997명, 99년 ),.6% (9/76 예, 년 ), 7.% (/예, 년 ) 로증가하였고, 추적기간별누적발생률도 년째.9%, 년째 8.8%, 년째 7.%, 년째.6% 로점진적으로증가하였다 ( 그림 ). 악성종양자유생존율은이식후 년째 97%, 년째 9%, 년째 8%, 년째 79% 로급진적으로감소하였다 ( 그림 ). 남녀에따른악성종양의발생은남자 999 명중고형암 명 (.%), 비고형암 명 (.%) 에비해여자 명중고형암 6명 (9.%), 비고형암 명 (8.%) 으로여자의암발생율이상대적으로높게나타났다 (p<.).. 악성종양의발생시기악성종양진단시환자의평균연령은 (~7) 세로, 신이식후평균 8.±.8년경과한후발생하였다. 가장빨리발생한암은카포시육종으로신이식후 개월후 68

Age at the time of diagnosis of malignancy for the transplant recipients 8% of malignancy occurred in kidney transplant recipients who were older than years at the time of diagnosis. 신이식후연간암발생율은일반인에비해대체적으로높았고, 특히임파종, 피부암, 갑상선암, 카포시육종, 비뇨생식계암의일반인대비발생비율이가장높았다 ( 표 ). 암발생율의위험인자로서수여자의성별과나이, 공여자유형, 이식횟수, 원인신질환, 이식전투석종류, 거부반응여부, MMF 사용여부등을분석하였을때신수여자의성별과나이, MMF의사용여부가유의한영향을미쳤다. 암발생위험은신수여자가 세이상인경우 세이하보다약.8배, 여성이남성에비해약.6배증가 7

7 Jin Young Kim, The type and incidence of malignancy in renal transplant recipients at Kangnam St. Mary's Hospital 과악성종양진단시평균연령은 세였고, 신이식후평균 8년경과후발생하였다. 발생시기는카포시육종이이식수술 개월후가장빨리발견되었고, 대장암이 6개월후가장늦게발생하였으며, 림프종, 자궁경부암, 신장암, 카포시육종, 폐암이조기에발생하였고, 소화기계암이상대적으로후기에발생하였다 ( 표 ). 이식후발생하는악성종양의종류는이식장기의종류, 지역, 인종, 환경등에따라차이가있으나 ) 일반인구보다피부암, 악성임파종, 카포시육종, 비뇨생식기암등의발생률이높은것으로알려져있다, ). 본연구에서가장높은빈도를보인종양은남자에게서위암, 여자에게서자궁경부암으로일반인에서가장흔한암과일치하였고 ( 표 ), 일반인대비암발생비율은남자의경우카포시육종, 림프종, 내분비계암이, 여자의경우카포시육종, 림프종, 비뇨생식계암이높았다 ( 표 ). 서구인에서이식후가장흔히발생하는종양으로알려진피부암은, 6) 동양인에서는이보다낮은빈도로보고되고있는데본연구결과편평상피암 예, 기저세포암 예가있었다. 내분비계암은갑상선 7예, 부신암 예, 흉선종 예가있었고, 비뇨생식계암은신장 예, 방광 8예, 자궁경부 예, 기타 예가있었다. 면역억제제가악성종양에미치는영향은 제요법보다 제요법을사용하였을때암발생률이증가하고, CsA 의용량이증가할수록더욱증가하는것으로알려져있으며 7, 8), 면역억제제의종류에따른암발생률은논란의여지가있지만대부분차이가없다고보고되었다 9, ). 본연구에서도악성종양발생은 AZA를사용한경우 8.%, CsA은 7.6%, FK6은.% 로나타나주면역억제제종류에따른차이를보이지않았으며, FK6은사용예가적어의미를두기어려웠다. 발생시기는 AZA는 7±6개월, CsA는 9±9개월, FK6은 7±8개월로 AZA보다 Calcineurin inhibitor 를사용한경우암이조기에발생하여 (p<.) 이전의보고와부합하였다 ). 그러나 calcineurin inhibitor는후기이식시기에주면역억제제로사용하기시작하여, 초기이식시대부터사용했던 AZA군보다추적기간이상대적으로짧아평균발생시기가단축되어나타났을가능성도고려해야겠다. 면역억제제에따른종양의분포는 AZA 사용군에서피부암, 자궁경부암, 회음부암이호발하고, CsA 사용군은카포시육종, 임파종, 신장암이많다고보고되고있는데 ), 본연구결과에서는 AZA 사용후발생한악성종양은 소화기계암이 8%, 자궁경부암이 % 였고, CSA 사용군에서발생한암은림프종 %, 소화기계암 %, 비뇨기계암 %, 간암 9%, 자궁경부암 9% 로분포하였다. 본연구에서암발생율의위험인자로서수여자의성별과나이, 공여자유형, 이식횟수, 원인신질환, 이식전투석종류, 면역억제제수 (제/제), 거부반응여부등을분석하였을때암발생위험은신수여자가 세이상인경우약.8배, 여성인경우약.6배증가하였고, 제요법을사용한경우약.7배상승하였다 ( 표 ). 세이상환자에서종양발생이증가하는것은환자의연령이가장강한영향을미친다는보고와일치하였으며 ), 여성의경우.6배증가한것은우리나라에호발하는자궁경부암의빈도가높은데기인하며이는면역억제로인한 HPV의활성화가주원인으로추정된다. 최근 MMF가여러가지부착분자들의당화와발현을억제하고, 증식활성을방해하여대장암, 전립선암, 백혈병, 림프종에대한항종양효과가있다고보고된바있다 6). 본연구에서 제요법을시행한경우 제요법보다종양의발생위험도가.7 배증가한것은면역억제기능의향상으로인하여종양발생에대한면역감시기능의약화및종양유발성바이러스의활성화등이관여했을것으로이해되나, 7) 초기이식시대에는 제요법을사용하다가후기이식시대에 제요법을사용하기시작하여 제요법을사용한군의평균추적기간이비교적짧아통계적으로위험율이증가되어나타났을가능성도배제할수없다. 동시대간의비교가필요할것으로판단된다. 이식후악성종양의명확한치료지침은없으나우선, 면역억제제를감량하거나중단하고암의종류, 치료종류, 이식신기능에따라추후면역억제제의사용을결정하며, 국소고형암의경우수술적절제를하고암의종류에따라항바이러스요법, 항암요법, 혹은방사선요법을시행할수있다 ). 본연구에서도수술적치료후항암요법혹은방사선요법등을병행하였고, 면역억제제중 MMF는중단하고, 환자의상태와신기능에따라주면역억제제의용량을감량하였으며최근에는 Everolimus로변경하고있다. mtor 투여가종양발생에미치는영향에대하여서는아직까지잘알려져있지않으나최근일련의보고에의하면 Sirolimus 투여는피부에발생한카포시육종을관해시켰다는보고 8) 와함께일반고형암환자에서도그효과가보고되고있다 9). 아직까지국내에는 mtor 가도입되지않았으나향후신이식후 7

8 대한내과학회지 : 제 7 권제 호통권제 6 호 7 종양발생환자에게권장할만한면역억제제로판단된다 ( 표 ). 년국립암센터는일반인에서호발하는 대암검진권고안을발표하였으나 ) 일반인과악성종양의분포가다른이식환자들에대한명확한암검진지침은아직까지제시되지않았다. 본연구에서조기에진단되었던자궁경부암, 유방암, 갑상선암등이예후가좋았고, 대장내시경으로용종절제술후 년뒤말기대장암이진단되었던 예를감안할때, 적극적검진이중요하다고생각된다. 결론적으로, 신이식환자의암검진은일반인보다조기에실시하고, 자주추적하며, 비뇨생식계암, 카포시육종, 피부암, 림프종에대한철저한진찰이포함되어야겠다. 또한, 이러한검진프로그램실행후암발생율의변화에대한전향적연구가필요하다. 요약목적 : 새로운면역억제제의도움으로급성거부반응의발생률이꾸준히감소되고신이식환자의수명을증진되면서이식후발생하는악성종양이사망의중요한원인이되었다. 저자들은신이식환자의추적기간별악성종양발생률의변화를알아보고자하였다. 방법 : 969년부터 년까지가톨릭대학교강남성모병원에서신장이식을시행받은 예를대상으로하였다. 평균추적관찰기간은 8±77 개월이었다. 신이식환자의악성종양발생률, 임상경과, 치료및예후를후향적으로조사하여일반인과비교하였다. 결과 : 7% (명에서 예 ) 의환자에서악성종양이발생하였다. 남자보다여자의암발생률이높았고, 가장흔한암은남자의경우위암, 여자의경우자궁경부암이었다. 99년, 99년, 년, 년누적발생률은.7%,.9%,.6%, 7.% 로점진적으로증가하였다. 신이식환자의암발생률은일반인보다높았고, 특히악성임파종, 피부암, 갑상선암, 카포시육종, 비뇨생식계암이높은발생률을보였다. 초기면역억제제에따른악성종양의발생은 AZA 8.%, CsA 7.6%, FK 6.% 이었으나이식후발생시기는 AZA 7±6개월, CsA 9± 9개월, FK 6 7±8개월로 AZA, CsA에비해 FK6 을투여받은환자에서악성종양이보다일찍발생하였다. 임상적경과는 7명이사망하고 (명은종양으로인한사망 ), 명은현재까지생존하고있다 (7명은이식신기능상실 ). 결론 : 추적기간이길어지면서악성종양의발생률이점진적으로증가하고있다. 암발생률을감소시키기위한적극적인암검진프로그램이필요할것으로생각된다. 중심단어 : 신장이식, 악성종양, 발병률 REFERENCES ) Penn I, Hammond W, Brettschneider L, Starzl TE.
