Department of Orthopedic Surgery, St. ary s Hospital, Catholic University edical College, Seoul, Korea Purpose: We wanted to evaluate the results of nonoperative treatment for acute, undisplaced, stable scaphoid fracture. aterials and ethods: We retrospectively reviewed cases of acute undisplaced scaphoid fracture that were treated at our department from ay to January 00. The immobilization method was a long or short arm cast. lexion-extension exercise was permitted during the period of the short arm cast, but pronation-supination motion was prohibited. After the removal of the cast, active and passive range of motion exercise was started immediately. The result was assessed by the audsley scale. Results: The twenty four cases were undisplaced or minimally displaced fracture (0 cases of Herbert type A, cases of A and cases of B) according to the ayo classification. The average duration of immobilization was 7. weeks. According to the audsley scale, cases had an excellent result and cases had a good result. One case had a complication of scapho-lunate dissociation. Conclusion: We recommend conservative treatment with cast immobilization as the first treatment of choice for acute undisplaced scaphoid fracture. 70 이상욱ㆍ송석환ㆍ이승구외 인 결과 명의환자중남성이 명, 여성이 5명이었고평균연령은 세 (-0) 였다. 수상기전으로는낙상이 예, 교통사고에의한손상 예, 스포츠손상이 예였다. Herbert 분류상 A형이 0예, A형이 예이었고, B형이 예이었다. ayo 분류상비전위요부골절 예및원위부골절이 0예로모두안정형골절이었다. 골유합의기간은평균 7. 주 (-0 주 ) 로이기간동안석고고정을시행하였으며, 추시관찰은 개월에서 개월로평균 7.7개월이었다. 합병증은 예에서주상-월상골간해리소견이보여수술적치료권하였으나환자사정으로시행하지못하였다. 평가결과는합병증이있는경우를제외하고는우수가 예였고, 관절운동이제한된소견을보이는 예가양호로나타나대부분좋은결과를보였다 (Table ). A, B) 장상지무지수상석고고정및단상지무지수상석고고정을각각 주간시행하였다. 보존적치료 주째시행한단순방사선사진상 (ig. C, D) 골유합소견이관찰되고관절운동시작후 5주째운동범위를회복하였고, audsley ) 의평가기준에따른임상적, 직업적활동, 방사선적평가에서모두우수한결과를보였다.. 증례 0 세남자가넘어진후발생한좌측수근관절의통증을주소로내원하였다. 단순방사선검사와컴퓨터단층촬영검사상좌측주상골에 Herbert B 형의골절소견이보여 (ig. A, B) 무지수상석고를고정하여 7주간치료하였다. 석고고정제거후촬영한방사선사진상골절부에골소주가관찰되어골유합으로판정하고 (ig. C, D) 일상생활및직업활동을허용하였다. 관절운동시작후 5주째에시행한 audsley ) 의평가기준에따른임상적, 직업적활동, 방사선적평가에서모두우수한결과를보였다. 고찰비전위된안정형주상골의골절은약 0% 이상비수술적치료방법으로골유합이된다.,,,7,) 이러한비수술적방법을시행하는적응증은안정형, 비전위된골절로임의의단순방사선사진상골절선간격이 mm 미만이거나주상골내각형성 (intrascaphoid angle) 이 0도미만인경우이다.,,0) 특히 Cooney 등 ) 은안정형주상골골절로주상- 월상골각도가 0도미만이거나월상- 유두골각도가 5도미만인경우, 주상골의원위부 (distal pole) 골절을규정하였고이분류가임상적으로많이이용된다. 주상골의장축을따라시행하는컴퓨터전산화단층촬영을통해좀더쉽게골절의전위여부및정도를확인할수있다. ) 비수술적치료방법인석고고정은고정의형태와길이에대하여논란이있지만, 몇몇저자들은수근관절에서 0-0도의신전, 척측변위, 전완부의회외전이생역학적으로주상골의장축배열및요측측부인대의긴장
