Objectives: The purpose of this study was to investigate the parent perception of their typically developing children s language development and language therapy. Methods: A questionnaire was developed to examine the parent perception of their typically developing children s language development and language problems, and of speech-language therapy. One hundred and ninety-two parents of children in early childhood participated in the survey. Results: First, results indicated that parents perceived that language development was most important among early childhood developmental domains and interaction with caregivers was the most effective way to promote their children s language development. Second, although most parents have heard of the term speech-language therapy, their perception of detailed intervention areas in speech-language therapy is not relatively high. Third, they perceived that speech-language therapy was most needed when children s language development was delayed and the communication breakdown occurs. Conclusion: The study results need to be considered to develop parent education program in speech-language pathology areas for accurate information on language development and language problems in early childhood and on how to use the speech-language therapy services. Keywords: Parent perception, Early childhood, Language therapy 언어치료는말이나언어장애로인하여의사소통에문제를가진이의잠재되어있는언어능력을최대한개발시키며의사소통기술을습득하게하여일상생활에서적절한의사소통을할수있도록정상적인언어발달을도모하는것이다. 현재한국에서는매우빠른속도로언어치료에대한관심이늘어나고있고, 언어치료에대한수요도더욱증가하고있는추세이다. 장애재활서비스중부모, 특수학교교사, 특수학급교사모두언어치료에대한요구가가장높은것으로알려져있으며 (Hwang & Kim, 2002; Kim et al., 2015), 실제언어치료서비스가가장많이이용되고있다 (Byeon et al., 2009). 2011년장애인복지법에 언어재활사 가법제화되었으며, 소정의시험을거쳐국가공인자격증을부여하여언어장애인의진단및치료를담당하는언어재활사의전문성을검증할수있는자격기준이마련되었다 (Kim et al., 2015). 그러나언어치료의필요성에대한높 은인식과는별개로 (Kim, 2008), 언어치료서비스에대한이해는여전히낮은것으로나타나고있다 (Park, 2015; Park, 2013). 의사소통장애에서아동기언어장애는비교적많은부분을차지하며, 언어중재에있어서매우중요한시기라고할수있다 (Kim, 2007; Paul, 2007). 0세에서 3세까지를영유아기 (infant & toddler), 3세이후부터취학전시기를학령전기 (preschooler), 그리고취학이후를학령기로구분할때, 영유아기는전체인구에비하면크지않은비중을차지하지만, 조기평가와중재의예방적개념을고려하면, 영유아기언어장애에대한인식과지원확대는장애를미연에방지하거나최소화하여, 장애아동과가족의삶의질을향상시키고사회적부담을경감시키는효과가크다고할수있다 (Cho, 2002; Kim & Hong, 2008; Lee & Cho, 2004; Shelov, 1998). 학령전기는언어의모든측면에서엄청난성장을이루는시기로, 새로운단어와 Copyright 2018 Korean Academy of Speech-Language Pathology and Audiology This is an open-access article distributed under the terms of the Creative Commons Attribution Non-Commercial License ( by-nc/4.0) which permits unrestricted noncommercial use, distribution, and reproduction in any medium, provided the original work is properly cited. 31

Parent Perception of Language Therapy 문법을배우는일외에도진정한대화상대방으로서의역할을부모와다양한사회적맥락을통해익혀가게된다 (Owens, Metz, & Haas, 2007). 따라서영유아기와학령전기를아우르는이시기에부모는아동의정상적인언어발달을도울수있는기회를제공해야하고아동의발달적인측면에민감하게반응할수있어야한다. 이렇듯취학전자녀의주양육자인부모의발달에대한인식과이해는아동의발달문제를조기에발견하고중재하는데에아주중요하다. 우리나라의모든아동들은 영유아건강검진 을통해건강이나발달상태를일차적으로확인할수있으나, 발달지연이의심될경우확진을받을수있도록하는제도나대처방법에대한후속조치가제한적이다. 실제로발달장애가있는아동중거의절반은소아과의사에의해발견되지않는다고보고되고있다 (Glascoe & Dworkin, 1995). 따라서부모가자녀의정상적인발달에대한지식이필요하며, 발달을촉진하는지원서비스를제공받고자하는적극적인노력을하지않으면조기중재시기를놓칠위험이있다. 조기중재전문가들은 5-8세가될때까지도거의중재를받지못하고, 때로는부모및전문가들이장애가있음을인지하지못하는 경도장애 (less severe disabilities) 아동들을지원해야한다고주장하고있는데 (Fewell, 1996), 이를위해서는부모의정상발달및장애에대한지식이선행되어야하며이는언어발달및언어치료영역에서도마찬가지일것이다. 부모는언어재활사보다일상생활에서더자주, 더다양한상황에서아동과접촉하게되므로, 훈련된부모는아동이의사소통에대한높은동기나욕구를보일때그순간을잘포착하여상황에적절한중재를즉각적으로제공할수있기때문이다 (Dale, Crain-Thoreson, Notari-Syverson, & Cole, 1996). 최근까지연구된부모를대상으로한언어발달및언어치료와관련된인식조사들은대부분언어치료대상자를중심으로이루어졌다 (Cho, 2004; Hwang, 2013; Jeong, 2003; Kim & Hwang, 2013; Lee, Kang, & Kim, 2013). 언어치료를경험하지않은대상자에게실시한연구는학령전유아에대한교사및부모의언어발달관련인식조사 (Paek, 2016) 와영유아의언어발달및언어치료에대한예비부모들의인식 (Ji, 2017) 등이있다. 그러나 Paek (2016) 의경우언어장애나언어치료에대한인식은다뤄지지않았고, Ji (2017) 의경우아직육아를경험하지않은 2, 30대예비부모들을대상으로연구가이루어졌다. 이에본연구에서는언어치료의경험은없으나학령전기아동을양육하는부모를대상으로언어발달및언어치료에대한인식을살펴보고자한다. 또한선행연구에서는장애를가진자녀의경우연령이증가함에따라부모요구가달라진다는점을보여주었다 (Wikler, 1981). 국내에서도영유아기장애아동을둔부모의경우아이의장애를받아 들이기위한심리적인과정을겪고있는반면, 학령전기장애아동을둔부모는이미자녀의장애를수용하고있기때문에장애서비스에대한요구가더높다고지적했다 (Kim, 2002). 이에본연구에서는취학전아동을영유아기와학령전기로나누어이들부모를대상으로언어발달및언어치료에대한인식을살펴보려고한다. 취학전아동의언어장애진단평가는주로보호자의의뢰 (84.2%) 로이루어지고 (Seo, Yoo, & Jeong, 2015), 언어치료는아동본인의의사가아닌부모의요구에의해시작된다는점을고려할때, 취학전자녀를둔부모의언어발달과언어장애에대한인식및언어치료에대한인식을알아보면부모들이생각하는아동언어문제의인식요인들과이들자녀에게요구되는언어치료및부모교육서비스제공유형파악에도움이될것이다. 이에따른연구문제는다음과같다. 첫째, 언어발달및언어장애에대한영유아기및학령전기아동부모의인식은어떠한가? 둘째, 언어치료에대한영유아기및학령전기아동부모의인식은어떠한가? 연구방법연구대상본연구에서는취학전자녀를둔부모의언어치료에대한인식을알아보기위해서울경기, 전라도, 경상도지역에거주하는취학전자녀를둔부모를대상으로설문지를배부하였으며, 그중 192명이회신하였다. 불충분하게작성한설문지가없음이확인된 192부를모두분석하였다. 연구대상의구체적정보는 Table 1과같다. 연구도구설문내용은 (1) 언어발달및언어장애에대한인식 (4문항), (2) 언어치료에대한인식 (13 문항 ) 의두가지측면으로구분하여작성하였다. 조사대상자의인구사회학적특성및기본정보를묻는 7문항을설문지가장앞부분에삽입하였으며, 반응유형은주로번호선택형과 5점척도형으로구성하였다. 설문문항중언어발달및언어장애에대한인식을알아보기위하여 Kwak (2016), Kim (2014) 등을, 언어치료에대한인식을알아보기위하여 Cho와 Kim (2010), Kim과 Hwang (2013), Park (2015) 의선행연구를참고하여본연구의목적에맞도록수정 보완하여 1 차문항을구성하였다. 이를언어치료학과교수 2인에게내용의타당성과소요시간을검증받았으며, 이과정을통해불필요한문항을삭제하고, 용어와난이도, 문항제시순서등의수정및보완을거 32

Participants characteristics Characteristic No. (%) Parents group Relationship Father 33 (17.2) Mother 159 (82.8) Age (yr) (13.0) (60.4) (25.5) (1.0) Occupation Professional 46 (24.0) Office 35 (18.2) Production 3 (1.6) Sales and service 22 (11.5) Housewife 81 (42.2) Etc. 5 (2.6) Education High school graduates 30 (15.6) College graduates 140 (72.9) More than MA 22 (11.5) Residence Seoul and Gyeonggi 34 (17.7) Metropolitan city 89 (46.4) Small city 60 (31.3) Rural area 9 (4.7) Children group Sex Male 98 (51.0) Female 94 (49.0) Age (mo) (10.9) (13.5) (17.7) (10.9) (20.8) (26.0) 연구절차본연구는 2017년 11월부터 12월까지전화및구두로연구의취지에대한사전설명, 설문에협조한기관및개인에게 URL로설문지배포, 그리고설문지회수 ( 총 192부 ) 순으로진행되었다. 자료분석본연구에서수집된자료는 IBM SPSS Statistics version 21 (IBM, Armonk, NY, USA) 을이용하여분석하였다. 연구대상자의인구사회학적특성과기본정보에대한응답분포를파악하기위하여빈도와백분율을산출하였으며, 응답의차이를분석하기위하여카이제곱 (chi-square) 검정과 ANOVA 검정을실시하였다. 복수응답을허용한경우케이스백분율을산출하였다. 연구결과언어발달및언어장애에대한부모의인식자녀의연령에따른언어발달및언어장애에대한부모인식의차이를알아보기위하여, (1) 아동기발달영역중가장중요하다고생각하는영역, (2) 언어발달에가장효과적인방법, (3) 아동기에흔히나타나는언어문제, (4) 자녀의언어문제로전문가의도움이필요하다고느끼는경우등에대하여살펴보았다 (Appendix 1). 먼저, 아동기발달영역중가장중요하다고생각하는영역은영유아기아동과학령전아동집단모두 언어발달 이 40% 이상으로가장많았으며, 다음으로 사회 정서발달, 대소근육발달, 적응 자조기술발달 순이었다 (Table 2). 연령집단간유의미한차이는나타나지않았다. 언어발달을위해가장효과적이라고생각하는방법에대하여영유아기아동과학령전아동집단모두 양육자와의상호작용 이 60% 이상으로가장많았으며, 다음으로 책읽어주기, 또래친구와의접촉 순이었다 (Table 3). 연령집단간유의미한차이는나타나지않았다. 취학전아동들에게흔히나타나는언어문제에대하여모두선택하도록한결과 ( 복수선택가능 ) 는 Table 4와같다. 두집단모두 발음이불분명하여알아듣기어렵다 가가장높았고, 다음으로영 Table 2. 5 취학전아동부모의언어치료에대한인식 김다영외 이해하고표현하는것이전반적으로늦다 (27 명, 24.3%) 가다음으 로많았으며, 말이너무빠르거나더듬는다 (25 명, 22.5%) 가뒤를 이었다. 마지막으로자녀의언어문제로전문가의도움이필요하다고느 끼는경우가있다면언제인가에대해영유아기 (43.2%) 와학령전기 (31.5%) 모두에서 아이가원하는것을알수없을때 가가장높았 고, 다음으로영유아기의경우 또래에비해느릴때, 그런경우가 없다, 규칙이나필요한지시를따르지않을때 순으로나타난반 면, 학령전기의경우 그런경우가없다, 규칙이나필요한지시를따 르지않을때, 또래에비해느릴때 순으로나타났다. 그러나연령 집단간유의미한차이는나타나지않았다 (Table 5). 언어치료에대한부모의인식 자녀의연령에따른언어치료에대한부모인식의차이를알아보 기위하여, (1) 언어치료라는용어에대해알고있고접근경로는무 엇인지, (2) 언어치료의중재영역은무엇인지, (3) 어떤문제가있을 때언어치료서비스를받는지, (4) 언어치료서비스는누가제공해 야하는지, (5) 언어치료제공자의어떠한면이가장중요한지, (6) 언 어치료가아동의말언어의사소통문제에효과적이라고생각하는 지등에대하여살펴보았다. SLT= speech and language therapy; SLP= speech-language pathologist. 먼저언어치료라는용어에대해서들어본적이있느냐는질문에 그렇다 응답이두집단모두에서 90% 이상으로높게나타났다. 그러나구체적으로언어치료관련서적을읽어본적이있느냐, 혹은강의를들어본적이있느냐, 언어치료를경험한적이있느냐는질문에두집단모두 아니오 응답이높았다 (Table 6). 그러나주변에언어치료서비스를받는아이를알고있느냐는질문에영유아기아동은아니오 (64.02%) 응답이높은반면, 학령전기아동은그렇다 (52.3%) 응답이많았으며, 이는연령집단간유의미한차이를보였다 (p =.024). 다음으로언어발달장애, 유창성장애, 말소리장애, 음성장애가각각언어치료의중재영역이라고생각하는지에대하여부모의인식을살펴보았다. 전혀그렇지않다 (1 점 ) 에서 매우그렇다 (5점) 에이르기까지 5점척도를이용하여응답하도록한평균과표준편차는 Table 7과같다. 두집단모두말더듬, 발음, 언어발달지연, 목소리순으로나타났으며목소리를제외하고 사이의점수를보였다. 그러나언어치료가말 언어 의사소통에구체적도움을주는가에대하여 5점척도를이용하여응답하도록한결과, 두집단모두 4.4 이상의높은점수를주었고, 언어발달및언어장애는언어재활사의전문영역인가에대하여 5점척도를이용하여응답하도록한결과도두집단모두 4.0 이상의높은점수를주었다. 언어치료를받는다면어떤문제로가장많이받는다고생각하는지를살펴본결과, 두집단모두 언어발달지연, 의사소통단절, 발음문제, 말더듬 순으로나타났다 (Table 8). 언어치료서비스는누가제공해야한다고생각하는지를살펴본결과, 두집단모두 언어재활사 가 80% 이상으로가장많았다 (Table 9). 다음으로영유아기의경우 심리치료사 (4.9%), 교사 (1.2%) 순으로나타난반면, 학령전기의경우 심리치료사 (12.6%) 가많았고다음으로 교사 와 작업치료사 가각각 2.7% 로나타났다. 마지막으로언어치료를위해언어치료제공자의어떠한면이가장중요하다고생각하는지를자녀의연령에따라살펴본결과, 두집단모두 임상경력 이 50% 이상으로가장많았으며, 다음으로 인 Table 8. Important characteristics for speech-language pathologists as perceived by parents Clinical experience Age Sex Major Character χ² (p-value) Infant & toddler (N= 81) 52 (64.2) 1 (1.2) 1 (1.2) 8 (9.9) 19 (23.5) (.535) Preschooler (N= 111) 62 (55.9) 2 (1.8) 0 (.0) 12 (10.8) 35 (31.5) Total (N= 192) 114 (59.4) 3 (1.6) 1 (0.5) 20 (10.4) 54 (28.1) 성, 전공여부 순으로나타났다 (Table 10). 논의및결론본연구는취학전자녀를둔부모를대상으로 (1) 언어발달및언어장애에대한인식과, (2) 언어치료에대한인식을조사하여, 이들부모들이생각하는아동언어문제의인식요인들과이들자녀에게요구되는언어치료및부모교육서비스제공유형을파악하고자하였다. 이를위해취학전시기를영유아기와학령전기로구분하여자녀의연령에따라부모의인식에차이가있는지를알아보았다. 설문조사를통하여양육자들의인식을분석한결과를요약해보면다음과같다. 첫째, 취학전자녀를둔부모들은이시기자녀의발달영역중언어 의사소통발달이가장중요하다고인식하고있었으며, 아동의언어발달을촉진시키는데에효과적인방법으로영유아기아동 (64.2%) 과학령전아동 (60.4%) 의부모모두양육자와의상호작용을가장많이꼽았다. 이는부모들이자녀의언어발달을위하여양육자의역할, 특히상호작용을중요하게인식하고있다는것을보여준다. 이러한상호작용의중요성에대한부모들의인식은이들이인식하는자녀의언어문제에서도드러났다. 자녀의언어문제와관련하여영유아기아동의경우 43.2% 가, 학령전기아동의경우 31.5% 의부모가 자신의자녀가원하는것을알수없을때 전문가의도움이필요하다고느끼는경우가있다고응답했다. 또한영유아기아동의부모의경우, 또래에비해느릴때 (19.8%) 가다음으로높게나타났는데, 이는아동들의언어발달의개인차가매우큰 (Fenson et al., 1994) 영유아기에부모의염려를보여주는것으로보인다. 이는학령전기아동의경우 도움이필요하다고느낀경우가없다 (25.2%) 는응답이다음으로높게나타난것과다른양상이었다. 둘째, 취학전자녀를둔부모들은 언어치료라는용어를들어본적이있다 라고응답한비율이 96.1% 로압도적으로높았다. 그러나언어치료에대한접근경로를세부적으로살펴보았을때책이나강의, 실제경험여부등에서 아니오 응답이훨씬높은모습을보였다. 이는본연구에서언급한외에부모들이언어치료서비스에대한접근경로가있을가능성을보여주는것으로추후연구에서는이에대한추가적인탐색이필요할것으로보인다. 한편주변에언어치료를받은아이가있는가에대한문항에서만학령전기아동의경우영유아기아동과달리 그렇다 답변이높았는데이는아동의연령이증가함에따라언어문제가두드러지면서언어치료서비스를이용하는아동이늘어남에따른현상으로보인다. 또한언어치료의세부적인중재영역에대한인식을 5점척도로알아보았을때, 언어치료는 말늦은아이를치료하는것이다 에대한인식은 3.36점, 말더듬는아이를치료하는것이다 는 3.54점, 발음을치료하는것이다 는 3.41 점, 목소리를치료하는것이다 는 2.46 점으로중간정도의점수를나타내고있어, 언어치료의구체적인중재영역에대하여제한된지식을보여주었다. 그러나 언어치료가말, 언어, 의사소통에구체적도움을주는가 에대한인식은 4.58점으로, 언어발달및언어치료는언어재활사의전문영역인가 에대한인식은 4.04점으로매우높은점수를나타내어실제언어치료의효과에대한신뢰와언어발달및언어장애전문가로서의언어재활사에대한신뢰를보여주었다. 이는선행연구에서특수교사및일반 36

7 취학전아동부모의언어치료에대한인식 김다영외 교사를대상으로실시한연구 (Park, 2015) 와같은결과를보여주어가정이나교육현장에서언어발달및언어장애전문가로서언어재활사의입지를보여주었다. 셋째, 취학전자녀를둔부모들은언어발달지연 (49.5%) 과의사소통단절 (42.2%) 이있을때언어치료서비스가필요하다고인식하고있었으며, 이러한언어치료서비스는언어재활사가제공하는것이가장바람직하다 (87.0%) 고인식하고있었다. 또한언어치료서비스에가장영향을미치는서비스제공자의역량으로임상경력 (59.4%) 과인성 (28.1%) 을들었다. 이는 Park (2015) 에서특수교사와일반교사의경우언어재활사가갖추어야할요건중인성은 5% 로가장낮게응답한것과는대조를보였다. 이러한응답의차이는본연구에서는조사대상자가아동의부모라는점과최근유아교육기관내아동학대에대한인식이증가하는현실과무관하지않을것으로사료된다. 마지막으로본연구에서는연령에따른부모의언어치료인식에큰차이가나타나지않아, 자녀의연령이증가할수록부모의서비스에대한요구가증가한다는선행연구 (Kim, 2002; Wikler, 1981) 와는다른결과를보였다. 이에대한한가지가능한설명은선행연구에서는장애자녀를둔부모를대상으로한반면본연구에서는장애가없는자녀를둔부모를대상으로하였다는점이다. 즉장애가있는자녀를둔부모의경우자녀가어린시기에는자녀의장애를받아들이기위한심리적갈등단계를거치고, 자녀의연령이증가할수록자녀의장애를수용하여정보와서비스에대한요구가계속적으로늘어나는반면 (Kim, 2002), 본연구의응답자들은이러한과정이없기때문에차이가났을수있다. 그러나이에대한정확한분석을위해서는추후연구가필요할것이다. 결론적으로, 본연구의결과는취학전자녀를둔부모들이자녀의발달에있어언어발달을중요하게생각하고있으며이를위해양육자로서의책임을강하게인식하고있다는것을보여주었다. 한편언어능력향상을위한언어치료의효과와언어발달및언어장애의전문가로서의언어재활사에대한신뢰를보이면서도실제자녀의언어발달을돕기위해이러한전문성을이용하기위한실질적인접근경로는많지않은것으로드러났다. 아동은부모및다른사람들과의상호작용속에서언어를습득한다 (Girolametto, 1988; MacDonald, 1990). 취학전자녀를둔부모및주양육자들을대상으로언어발달및언어치료에대한실질적인교육이이루어질수있는체계적인방법들이언어치료분야에서모색되어야할것이다.
