Whereas static symmetry is often improved, dynamic asymmetry frequently persists because of the imbalance of complex coordinated movements of facial expression. The paralyzed face is often distorted by the excessive pull of the normal contralateral face during emotional expression. We report the use of botulinum toxin in patients with facial paralysis to improve lower facial asymmetry by limiting the excessive pull of contralateral face. Subjects and Method:Ten adult patients with unilateral facial paralysis were included in this study. Patients had injections at contralateral facial muscle which were zygomaticus major, levator labii superioris and depressor angularis orii. The effectiveness of the botulinum toxin injections on facial symmerty and patient appreciation of this were assessed by measuring the level difference of both mouth angles and difference in the distance from the midline to both mouth angles before and 1 month after injection. Also patients were asked to fill up the questionnaire about the magnitude of effect and any related side effects of injection at 1 month follow up. Results:Botox injection decreased the level difference of both mouth angle and difference of distance from midline to both mouth angles in patients with unilateral facial paralysis at full-denture smiling. Five of ten patients were reported as moderate to marked improvement in expressive facial asymmetry after botulinum toxin injection into normal lower face. Conclusion:Botulinum toxin injection is a useful ancillary technique for improving dynamic facial asymmetry of the paralyzed face. (Korean J Otorhinolaryngol-Head Neck Surg 2007;50: ) KEY WORDS:Botulinum toxin Facial paralysis. 서 론 안면신경마비는감염, 외상등다양한원인에의해발생하며경우에따라서는완전회복이어려울수있다. 안면마비중가장흔하며예후도긍정적인벨마비의경우를보더라도인구 10만명당 15~40 명꼴로발생하는것으로알려져있으며, 1) 대부분의환자들이완치되나약 16% 의환자들에서는후유증이남는다. 2) 이러한안면신경마비의 논문접수일 :2007 년 1 월 27 일 / 심사완료일 :2007 년 7 월 9 일교신저자 : 변재용, 서울강동구상일동 149 번지경희대학교의과대학이비인후과학교실전화 :(02) 전송 :(02) otorhino512@naver.com 불완전한회복시영구적으로기능적, 미용적인측면에서변화를초래하여환자의정신적, 사회적인많은문제를일으킨다. 그동안이러한문제로불완전회복된안면신경마비로인한불균형을교정하기위해교차안면신경이식 (crossfacial nerve graft) 이나설하신경 (hypoglossal nerve), 척추부신경 (spinal accessory nerve), 횡경막신경 (phrenic nerve) 등을이용한신경교차술 (nerve cross over), 그리고측두근 (tempralis muscle), 저작근 (masseter muscle) 등을이용한근이식술 (muscle transfer) 등다양한방법들이시도되어왔다. 3,4) 그러나이런방법들은매우침습적이며또한마비된안면의반대측정상부위의과도한당김현상을해결하지는못했다. 특히이러한과도한당김현상 1087

불완전회복된안면마비의교정을위한보톡스의유용성 은웃을때처럼자신의의지와상관없는표정을지을때더욱또렷이나타난다. 그러므로정상부위의과도한당김현상을제한함으로써안면불균형을교정할수있는 Botulinum toxin 은이러한목적에합당한물질이라고할수있다. 현재까지국내에서는미묘한안면불균형을교정할수있는방법이발표된바가없다.

2 불완전회복된안면마비의교정을위한보톡스의유용성 은웃을때처럼자신의의지와상관없는표정을지을때더욱또렷이나타난다. 그러므로정상부위의과도한당김현상을제한함으로써안면불균형을교정할수있는 Botulinum toxin 은이러한목적에합당한물질이라고할수있다. 현재까지국내에서는미묘한안면불균형을교정할수있는방법이발표된바가없다. 이에저자들은안면불균형을교정하는새로운치료방법인 Botulinum toxin A를이용한치료경험과결과를보고하고자한다. 대상및방법 대상 2003 년 3월부터 2004년 12월까지안면마비가발생한 지 6개월이상되고, 웃을때안면하부의비대칭을보이는일측성안면마비가있는환자중보톡스치료를받은환자 10명을대상으로하였다. 연령은 24세에서 59세까지로평균나이는 42.5세였고, 남자 3명, 여자 7명이었다. 안면신경마비가발생한기간은 6개월 1명, 1년 3명, 2년 2명, 3년 1명, 5년 2명, 17년 1명으로평균 3.75 년이었으며, 원인으로는벨마비가 7명, 수술후마비 ( 청신경종양 :2, 만성중이염 :1) 가 3명이었다. 방법냉동건조된 botulinum toxin A(Allergan, Inc., Irvine, CA) 100 unit/vial 을근육주사하여사용하는것으로 Toxin 은영하 4도씨에서보관하였고생리식염수 4 ml로희석하여 25 U/ml 의농도로재구성하여사용하였다. 주사시엔 30-gauge needle의 tuberculin syringe를이용하였다. 환자에게전치미소 (full-denture smile) 를짓게하여정상측안면부위의최대근육당김현상이있는부위에주사하였다. 주사부위마다 3~5 units 씩총 15~30 units 의용량을주사하였다. 주사부위는 Fig. 1에서보는바와같이 3부분이며 A point 는상순거근 (levator labii superioris) 으로코입술주름 (nasolabial fold) 의상방 1/4 지점에서외측으로 5 mm 떨어진지점이며, B point 는큰관자근 (zygomaticus major) 으로코입술주름 (nasolabial fold) 의하방 1/4 지점에서외측으로 5 mm 떨어진지점이고, C point 는입꼬리내림근 (depressor angularis oris) 으로 melomental fold 의하방 1/4 지점에서외측으로 10 mm 떨어진지점이다. 환자는첫주사후 4주간 7일간격으로외래경과관찰하였고, 2주째에불완전한부위를재교정하기위한보톡스추가점적을시행하였다. 그후 1개월간격으로외래추적관찰하였다 안면불균형을평가하기위해서첫방문시와 1개월후추적관찰시휴식 (resting) 시와전치미소 (full-denture smile) 시의양쪽구각 (mouth angle) 의높이차이와정중선 (midline) 에서양쪽구각 (mouth angle) 까지의거리의차이를비교하였다 (Fig. 2). 그리고추적관찰 1개월째주사의효과와부작용에대한설문조사를실시하였다 ( 부록 1). 결 과 환자 10명의보톡스주사전과주사후 1개월째에측정한양측구각 (mouth angle) 의높이차이의평균은휴식 (resting) 시에주사전에 2.0 mm였고주사후 1개월째에 2.1 mm였으며, 전치미소 (full-denture smile) 시에주사전에 3.6 mm였고주사후 1개월째에 1.5 mm였다 (Table 1). 또한보톡스주사전과주사후 1개월째에측정한정중선 (midline) 에서양측구각 (mouth angle) 까지거리의차이는휴식 (resting) 시주사전에평균 1.0 mm였고, 주사후 1개월째에평균 1.7 mm였으며, denture smile 시에는주사전에평균 4.7 mm였고주사후 1개월째에평균 2.2 mm로개선되었다 (Table 2). 보톡스주사후 1개월째에조사한안면불균형의개선정도를조사한설문에서는총 10명중 1명은현저한호전을답했고, 4명은상당히호전되었다고답해 50% 에서상당히만족함을알수있었고, 4명은약간호전되었다고답했으며 1명은호전이없다고답해환자들의만족도도높음을알수있었다 (Table 3). 또한보톡스주사후에생긴부작용 Fig. 1. 3 홍창기외 에대한조사에서침흘림 (drooling), 볼물림 (cheek biting), 언어장애 (speech impairment), 가면양얼굴 (masked face) 같은부작용을호소한환자는없었으며국소반응 (local reaction), 즉주사부위의통증을나타낸환자는 1명이있었다 (Table 4). 또한주사후평균 6.2일경안면비대칭개선효과가나타났으며비대칭개선의지속기간은 2개월에서 5개월정도로평균 3개월이었다. Justinus Kerner 는근육의이완목적으로과자극된 motor system 의질환이있는경우에의학적으로이 toxin 의사용가능성을언급하였다. 5) 1897년벨기에의미생물학자인 Van Ermengem은부패된소시지를먹은환자의배변에서병원균을분리하는데성공하였으며이를 Bacillus botulinus 라고명명하였다. 6) Schantz 는많은양의 toxin 을대량생산하는데성공하였고, Burgen이 1949 년 presynaptic acetylcholine inhibition에의해 botulinum toxin이작용한다는사실이밝혀짐으로써이 toxin 의임상적적용에대한토대가마련되었다. 7) 1973년 Botulinum toxin A는 Scott 에의해사시의치료에처음으로사용되었다. 8) 그후 Carruthers 는사시의임상시험에참여하였다가우연히안검경련환자에게서 Botulinum toxin A를치료하는동안에미간부위의주름이감소한것을발견하였다. 몇달동안피부가팽팽하게유지되는것을관찰하였고이는표정근육의이완에기인함을확인하였다. 이어서눈가주름, 콧등, 턱부위의주름치료에도이용되기시작했다. 9) 특히요즈음엔그이용범위가더욱넓어져서근육긴장이상등다양한분야에서이용되고있다. 4,10,11) 혐기성그람음성간균인 Clostridium botulinum 으로부터생성된 botulinum toxin 은 heavy chain 을통해서운동신경말단에부착하고세포내이입 (endocytosis) 에의해신경세포로합쳐진다. Endosome으로부터세포질 (cytoplasm) 으로분비되는 light chain은 acetylcholine의세포외유출 (exocytosis) 에관여하는 synaptosome-associated protein of 25kDa(SNAP-25) 를절단한다. 결과적으로 botulinum toxin A는신경-근접합부에서절전신경섬유의 acetylcholine 의유리를막아절후종말의탈분극을억제하여근육의수축을막아근육이이완되게한다. 그리고 botulinum toxin A는 acetylcholine의생성을막는것이아니므로수개월후근육은다시수축할수있다. 약 6개월간기능적탈신경 (functional denervation) 을야기시킨다. 이러한 botulinum toxin 은 A, B, Cα, Cβ, D, E, F, G 까지 8가지의 Type 이있으며이중 Type A가임상적으로그유용성과안정성이증명되었고, 12) 그작용부위가특이적이며, 또한원하는효과를얻기위해그농도를조절할수있다. 현재상품화된 botulinum toxin 은 3가지가있는데그중 Botox (Allergan, Inc., Irvine, CA) 와 Dysport(Ipsen Li- 1089

4 불완전회복된안면마비의교정을위한보톡스의유용성 mited, Maidenhead, Berkshire, UK) 는 Type A이고, Myo (neuro)bloc(elan Pharmaceuticals, South San Francisco, CA) 은 Type B이다. 일반적으로 1 U의 Botox 는 3~5 U 의 Dysport, 50~100 U의 Myo(neuro)bloc과같은농도이며, 약 70 kg의사람에서 2,500~3,000 U의 Botox 가치사량으로알려져있고, 또한미용적인측면에서사용할시엔 30 U 이하로사용하는것이안전하다고알려져있다. 13) Bulstrode 등에의하면 23명의지연성불완전회복된안면마비환자에게 Botox 주사후평균 37개월간추적관찰한결과눈썹높이 (brow height) 나치아노출 (teeth exposure) 이 Botox 주사전보다호전됐고, 부작용은없었으며, 주사효과에대한설문조사에서도호전됐다고보고하고있다. 14) 또한 Bikhazi 등에의하면 10명의안면마비환자에게 Botox 주사후 6개월에서 21개월간추적관찰한결과 8명에서상당한정도의호전을보였고, 부작용은관찰되지않았다고보고하고있다. 15) 본저자들의경우에도조사대상수가적어통계처리는하지않았으나 Botox 주사후양측구각 (mouth angle) 의높이차이와정중선 (midline) 에서양측구각 (mouth angle) 까지의거리차이가많이줄어든것을관찰하였으며, 주사효과에대한설문조사에서도 10명중 9명에서호전되었다고답했으며, 특히이중 5명은상당한정도의호전이있다고답했다. Botox 주사로인한부작용으로는통증, 부종, 반상출혈 (ecchimosis), 두통, 안검하수증 (eye brow ptosis), 건성안증후군 (dry eye syndrom), 침흘림 (drooling), 가면양얼굴 (mask face) 같은국소적부작용과오심, 전신피로, 권태감 (malaise), 아낙필락시스 (anaphylaxis) 같은전신적부작용이있을수있다. Scott 등에의하면 botulinum toxin A에의한부작용은 20~65% 정도로보고하고있고, 16) Park 등의연구에의하면국소적인부작용은일시적이고 1~3주이내에모두사라지고전신적인부작용은발생하지않은것으로알려져있다. 17) 그러나 Dutton 등에의하면 botulinum toxin 주입후에전신쇠약, 전신소양감, 구역등의전신부작용이나타났다고보고하고있으나이의정확한원인은밝히지못했다. 18) 가장흔한부작용으로는안건조로서빈도는 4~65% 로보고하고있으며, 19) 안면근육의약화는다양하게보고되고있다. 본저자의경우국소적으로통증을나타낸 1명을제외하고는특별한부작용을호소한경우는없었다. 18,20) 결론 편측안면마비가있는환자에서보톡스주사는전치미소 1090 (full-denture smile) 시에양측구각 (mouth angle) 의높이차이와정중선 (midline) 에서양측구각 (mouth angle) 까지의거리차이를감소시킨다. 또한보톡스주사를맞은 10명중 5명이상당한정도이상의호전을보였고심각한부작용도없었다. 그러므로보톡스주사는안전하고사용방법이간단한물질로마비된얼굴의 dynamic facial asymme-try 를호전시킬수있는방법으로유용하게이용될수있다. 중심단어 : 보톡스 안면신경마비.
