The aim of this study was to determine whether polymorphism in NRAMP1 (natural resistance associated macrophage protein 1) gene is associated with the susceptibility or resistance to tuberculosis infection for patients with drug-sensitive pulmonary tuberculosis (DS-TB) and multi-drug resistant pulmonary tuberculosis (MDR-TB). Methods: Eight genetic polymorphisms of the NRAMP1 gene were investigated in patients suffering with DS-TB (n=100) or MDR-TB (n=102), and in healthy normal controls (NC, n=96). The genetic polymorphisms of NRAMP1 were determined by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). Results: The frequency of D543N A/G heterogygotes was significantly higher in the DS-TB subjects than the NCs (OR=2.10, 95% CI: 1.00 to 4.41, p=0.049). The frequency of 823C/T T/C heterozygotes was significantly higher in the DS-TB subjects (OR=2.79, 95% CI: 1.11 to 7.04, p=0.029) and the MDR-TB subject (OR=3.30, 95% CI 1.33 to 8.18, p=0.010) than in the NCs. However, the frequency of these genotypes was not different between the DS-TB and MDR-TB subjects. Conclusion: A significant association was found between NRAMP1 823 C/T polymorphism and pulmonary tuberculosis. This result suggests that NRAMP1 polymorphism may be involved in the development of pulmonary tuberculosis in Koreans. (Tuberc Respir Dis 2007; 62: ) Key words: NRAMP1, Polymorphism, Tuberculosis. 서 론 * 본연구는부산대학교자유과제학술연구비 (2 년 ) 의지원으로연구되었음 Address for correspondence: Min Ki Lee, M.D. Departments of Internal Medicine, College of Medicine, Pusan National University, 1-10 Ami-dong, Suh-gu, Busan , Korea Phone: , Fax: leemk@pusan.ac.kr Received: Apr Accepted: May 결핵은진단기법이나치료에있어서많은발전을이루면서환자가처방된약제를복용한다면거의 95% 이상에서치유할수있는여건이되었다. 그러나이러한발전에도불구하고전세계적으로지난수년간결핵신환자의수는점차증가하는추세이다 1. 전인류의약 1/3에서결핵균 (Mycobacterium tuberculosis) 에감염되지만그중약 10% 만이임상적질환으로발병한다고알려져있으며 2, 결핵균감염자중일부만이임상적인질환으로발병하는것은숙주의유전적인자가관여하기때문으로생각된다 3,4. 이러한숙주반응은결핵에대한선천적내성과 bacille Calmette Guérin(BCG) 접종과같은획득면역으로결정된다. 이중선천적내성은결핵균에대한감수성을결정하는유전적요인으로이에기여하는유전자및단백질등의발현에대한연구는결핵균에대한인체방어기전을이해하고, 결핵을예방하고치료하는데있어서유용한정보를제공할것으로인식되고있다. 실제로백서를이용한동물실험에서결핵균을포함하는몇몇세포내병원체에대한감수성및내성이 1번염색체에존재하는 Bcg 유전자에의해조절되는것으로밝혀졌다 5-7. 이후 Vidal 등 8 은 Bcg에대한후보유전자를유전자클로닝 (cloning) 방법을이용하여 492

No.6, Jun 분리하고, 이를 natural resistance associated macrophage protein 1(Nramp1) 으로명명하였으며, 이유전자로부터합성되는 Nramp1 단백질의기능에이상이있는경우결핵균과같은세포내병원균에대한감수성이증가하는것으로보고하였다. 한편 Cellier 등 9 은인간의염색체 2q35에서 16개의액손 (axon) 으로구성된백서 Nramp1 유전자와상동체인 NRAMP1 유전자를밝혀내었으며, Liu 등 10 은이유전자에는약 9가지의유전자다형성 (genetic polymorphism) 이존재하며, 이러한다형성은이유전자에의해합성되는 NRAMP1 단백질의합성또는기능에이상을초래하여결핵균에대한감수성을증가시킬수있음을보고하였다. 또한, Bellamy 등 11 은서아프리카지역의폐결핵환자에서이유전자다형성이결핵의감수성과관련성이있음을보고하였다. 결핵은발병뿐만아니라치료에대한반응과경과에도숙주의유전적요인이관여할가능성이존재한다고알려져있으며, 특히다제내성결핵은부적절한치료와순응도부족으로인한획득내성이주요원인으로알려져있지만숙주에침입한결핵균을적절히제거하지못하는유전적소인이관여할가능성도제기되고있다 12. 최근에재치료군과치료실패군에서보다더강한연관관계를보이는유전자 13,14 와다제내성폐결핵과연관된유전자 12,15 에대한보고가있었다. 이에본연구에서는 NRAMP1 유전자다형성이폐결핵의발생에미치는영향을평가하기위해한삼차병원과결핵전문병원의결핵환자를대상으로다형성의빈도를확인하였으며, 약제내성유무와의관계를알아보기위해약제감수성및다제내성폐결핵환자로나누어서다형성의빈도를비교하였다. 대상및방법 1. 대상 2002년 5월부터 2004년 6월까지부산대학교병원과국립마산병원을내원하여폐결핵으로진단된 202명을환자군으로하였고, 동일기간에부산대학교병원 건강검진센터를방문한사람중폐결핵의과거력이없고흉부 X-선상정상소견을보이며다른동반된질환이없는성인 96명을대조군으로하였다. 폐결핵은세균학적, 임상적기준을이용하여진단하였다. 세균학적기준은결핵균배양검사에서양성을보이는경우로하였고, 임상적진단기준은폐결핵을시사하는증상 ( 기침, 객담, 발열, 발한, 체중감소등 ) 이있으면서흉부 X-선이나고해상도흉부전산화단층촬영에서활동성폐결핵에합당한소견이있으면서항결핵제투여후임상적, 방사선학적호전을보이는경우로하였다. 약제감수성검사결과에따라약제감수성군과다제내성군으로구분하였으며, 다제내성군은약제감수성검사결과에의해최소 isoniazid와 rifampicin에동시내성을보이는경우로하였다. 약제감수성검사를시행하지않았거나배양음성으로약제감수성검사를시행할수없었던경우는초치료환자로서단기표준요법으로치료를종결한경우에만약제감수성군에포함시켰다. 이연구는부산대학교병원임상시험위원회의승인을받았으며, 참여하는환자로부터고지된승낙을얻어시행하였다. 2. 방법 1) Genomic DNA 의추출환자군과대조군의상완정맥에서전혈 3 ml를 EDTA가함유된시험관에채혈하여실온에서 2,500 rpm으로 20분간원심분리하였다. 원심분리후상층의연막 (buffy coat) 을생리식염수에희석한뒤 Ficoll- Paque TM PLUS (Amersham Biosciences, Sweden) 에중층시켜 2,500 rpm으로 20분간원심분리하여림프구를분리하였다. 분리된림프구는 DNA를추출하기전까지 -20 에보관하였다. DNA 추출은 Wizard Genomic DNA Purification Kit (Promega Co, Madison, USA) 를이용하였다. 림프구를진탕하여재부유시킨뒤핵융해완충액 300 μl를첨가한다음단백질침전용액 100 ul를넣어서강하게진탕한후 12,000 rpm으로 5분간원심분리하였다. 회수한상층액에 isopropanol 300 μl를첨가하여섞은뒤원심분리하여얻어진 DNA 압착결정에 70% 493

PCR 산물에각각의다형성확인을위해제한효소 2 unit를넣어반응시킨후 G/C와 del4 는 3% agarose gel에서 274C/T, G/A, 823C/T, A318V, G/A 그리고 D543N은 7.5% polyacryamide gel에서전기영동하여 ethidium bromide 로염색한후자외선조사기에서결과를확인하였다. 3. 통계분석각군의 NRAMP1 유전자형과각대립형질의빈도는카이제곱검정 (Chi-square test) 을통해분석하였고, p 값이 0.05 미만인경우를통계적으로유의한것으로판정하였다. 통계학적의미가있는유전자형에대해위험도를교차비 (odds ratio, OR) 와 95% 신뢰구간 (confidence interval, CI) 으로표현하였다. 모든통계분석은 SPSS for Windows 13.0 (SPSS Inc. Chicago, USA) 을이용하였다. 2) NRAMP1 genotyping NRAMP1 유전자다형성은기존에보고된 8개의유전자형에대해추출된 DNA를중합효소연쇄반응 (PCR) 과중합효소연쇄반응-제한분절길이다형성법 (polymerase chain reaction-restriction fragment length polymorphism, PCR-RFLP) 을이용하여결정하였다. 각시발체들은기존에보고된염기서열을이용하여제작하였다 (Table 1). PCR 반응은 AccuPower PCR PreMix(Bioneer Co. Seoul, Korea) 에 DNA 0.5 μg, 시발체 (primer) 각각 10 pmol, 그리고증류수로최종 20 μl를맞춘후 274C/T, A318V, D543N 그리고 del4의경우는 94 에서 5분간반응시킨후변성반응은 94 에서 1분, 결합반응은 55 에서 1분, 연장반응은 72 에서 1분씩반복적으로 35회전을실행하여 72 에서 7분간반응시켰다 G/C는 94 에서 1분, 60 에서 1 1. 대상환자의특성 결 약제감수성군은 100명으로 90명은결핵균배양검사에서양성 ( 이중 54명은항산균도말검사상양성 ) 이었고 10명은임상적기준으로진단되었으며, 다제 Table 2. No.6, Jun 내성군은 102명이었고모두도말및배양검사에서양성이었다. 건강대조군, 약제감수성군, 다제내성군의남녀비는각각 1:1, 1:1, 2.4:1이었으며, 평균연령 ( 연령분포 ) 은각각 50.0(15-59) 세, 40.4(15-79) 세, 44.0(21-91) 세였으며연령에따른차이는관찰되지않았다 (Table 2). 2. 건강대조군과약제감수성군에서 NRAMP1 유전자다형성의빈도 NRAMP1 유전자의 D543N 유전형의경우 G/G 동형접합체의빈도가약제감수성군에서 75%, 건강대 조군에서 85.4% 였고, A/G 이형접합체의빈도는약제감수성군에서 25%, 건강대조군에서 13.5% 로약제감수성군에서높았다 (OR=2.10, 95% CI=1.00 to 4.41, p=0.049). 823C/T 유전자형의경우 C/C 동형접합체의빈도가약제감수성군에서 81%, 건강대조군에서 91.7% 였고, T/C 이형접합체의빈도는약제감수성군에서 18%, 건강대조군에서 7.3% 로약제감수성군에서높은빈도를보였다 (OR=2.79, 95% CI=1.11 to 7.04, p=0.029). 그러나 NRAMP1 유전자의 del4, 274C/T, G/C, G/A, G/A, A318V 유전형의빈도는건강대조군과약제감수성군에서유의한차 Table 3. 5 JS Lee et al: Relations between polymorphism of NRAMP1 gene and susceptibility to pulmonary tuberculosis 이를보이지않았다 (Table 3). 3. 건강대조군과다제내성군에서 NRAMP1 유전자다형성의빈도 NRAMP1 유전자의 823C/T 유전형의경우 C/C 동형접합체의빈도가다제내성군에서 78.4%, 건강대조군에서 91.7% 였고, T/C 이형접합체의빈도는다제내성군에서 20.6%, 건강대조군에서 7.3% 로두군사이에유의한차이가있었다 (OR=3.30, 95% CI=1.33 to 8.18, p=0.010). 그러나 NRAMP1 유전자의 D543N, del4, 274C/T, G/C, G/A, G/A, A318V 유전형의빈도는건강대조군과다제내성군에서유의한차이를보이지않았다 (Table 3). 4. 약제감수성군과다제내성군에서 NRAMP1 유전자다형성의빈도 NRAMP1 유전자의 D543N, 823C/T, del4, 274C/T, G/C, G/A, G/A, A318V 유전형의빈도는약제감수성군과다제내성군에서유의한차이를보이지않았다 (Table 3). 고찰결핵의감수성에관여하는숙주의유전인자를밝히려는연구는결핵발병기전을더잘이해할수있게하고이를바탕으로새로운치료법과예방법을고안할수있다는점에서중요하다. 숙주의유전인자와결핵의감수성의관련성을보여주는근거로일란성쌍생아에서이란성쌍생아에비해결핵발병일치율이일관되게높았으며 16,17 인종간결핵발병률에차이가있다는보고 18 가있다. 결핵균감염에대한초기방어는폐포에존재하는대식세포 (alveolar macrophage) 들에의하여이루어진다. 이러한대식세포의기능에이상이발생하면결핵균에대한선천적감수성이증가되는데, 그간의연구들에의하면결핵균에대하여선천적으로감수성및내성을갖는백서로부터얻은대식세포에서선천 적인아포프토시스 19 또는항원표현능력 (antigen presenting ability) 의차이가보고된바있다 20. 최근특히주목받고있는결핵균에대한숙주의방어기전에관한연구는탐식작용후포식소체-용해소체결합체가성숙될때중요한역할을하는 NRAMP1 단백으로그합성과기능에영향을미치는유전자단계에서의이상에관심이집중되고있다 21. NRAMP1 단백은약 60kD 정도의분자량을가진소수성단백으로 NRAMP1 유전자로부터만들어지며, 12개의경막영역 (transmembrane domain) 과당화세포외고리 (glycosylated extracellular loop) 가존재한다 8. 동물을이용한 in vitro 및 in vivo 연구에의하면 NRAMP1 단백은대식세포의초기활성화과정에서중요하고다양한영향을나타내어항암, 항균작용을조절하고또한대식세포의막운반과정에있어서중요한기능을하는것으로알려져있다. 이들작용중특히주요한기능중의하나는감염초기에대식세포세포질내의아질산염 (nitrite) 을보다산성인포식용해소체로이동시켜산화질소 (nitric oxide) 를생성하는것으로이기능에이상이발생하면결핵균과같은세포내병원균에대한감수성이증가하게된다 22. NRAMP1 단백은포식용해소체내부로부터 2가양이온을제거하여 superoxide dismutase의작용을억제하여결핵균을사멸시키는데중요한역할을한다. 그리고포식용해소체에직접또는간접적으로영향을미쳐고도의살균성을갖도록성숙시킨다 23,24. NRAMP1 단백을합성하는인간의 NRAMP1 유전자다형성은현재까지 9가지정도가보고되고있다 10. 결핵균의감염과이에따른결핵의발병과유전자다형성의상관관계에대한임상연구들을살펴보면아직까지 NRAMP1 유전자다형성과결핵발병간의임상적인상호관계는명확치않다. Shaw 등 25 과 Blackwell 26 은브라질의가족집단을대상으로행한대규모의유전학적역학연구에서 NRAMP1 유전자가결핵의발병에영향을주는주된후보유전자는아닌것으로보고하였고, Huang 등 27 도 Mycobacterium avium-intracellulare에의한폐질환에서 NRAMP1 유전자다형성과이균에의한감염증의발생에연관 496

No.6, Jun 성이없다고주장하였다. 그러나서아프리카지역의폐결핵환자들을대상으로시행한연구에서는촉진자 (promotor) 부위의 (CA) n microsatellite 반복의수변화 {5 (CA) n }, 4번인트론의 G/C 치환 (469+14G/C, INT4), 543 코돈의아스파르테이트 (aspartic acid) 에서아스파라긴 (asparagine) 으로의변이 (D543N), 3' 비해독지역 (untranslated region) 의 TGTG 결손 ( del4, 3'UTR) 등의다형들이건강대조군에비하여결핵환자군에서의미있게높은빈도로나타남을관찰함으로써, 이들유전자다형성과결핵에대한선천적감수성과의밀접한상관관계를보고한바있는데 11, 본실험에서도약제감수성군에서 NRAMP1 유전자중 D543N 유전자다형성은건강대조군에비해의미있는차이를보여서아프리카의연구와유사한결과를보여주었으나, G/C와 del4 유전자다형성은차이를보이지않아유전적변이가인종간에차이가있음을보여준다. 또한, 본실험에서는이전의연구들에서조사하지않았던 823C/T 유전자의다형성이약제감수성군에서의미있는차이를보였다. 본연구에서다제내성군에서도정상대조군에비해 823C/T 유전자의다형성이의미있는차이를보여주었으며, 약제감수성군과다제내성군사이에서는유전자다형성의빈도의차이를보이지않아 NRAMP1 유전자와약제내성의발생과는관련성이없을것으로추정할수있다. 본연구에서 823C/T 유전자다형성은약제감수성군과다제내성군모두에서결핵발병감수성과관련이있을것으로추정되며, 추후더많은우리나라정상인구에서 NRAMP1 유전자다형성의빈도를조사하고더많은결핵환자를대상으로한검사가진행되면결핵발병과의관계를규명하는데도움이될것으로생각된다. 결론적으로본연구에서 NRAMP1 유전자중 823C/T변이가폐결핵의발생과유의한상관성을보여 NRAMP1이폐결핵발병에대한숙주의감수성과연관되는후보유전자일가능성이있으며약제내성유무와는관계가없었다. 이러한결과가한국인의유전적감수성을반영하는것인지에대해향후대규모연구를통한검증이필요할것으로사료된다. 요 연구배경 : 결핵균에노출된후임상적결핵으로발병하는데유전적인자가관여할수있으며, 치료반응에도숙주의유전적요인이관여할가능성이있다. 이에저자들은 NRAMP1 유전자다형성을감수성결핵과다제내성결핵환자로나누어서비교하였다. 방법 : 100명의약제감수성군, 102명의다제내성군, 96명의건강대조군을대상으로 274C/T, G/C, G/A, 823C/T, A318V, G/A, D543N, del4 의 NRAMP1 유전자다형성의빈도를중합효소연쇄반응기법과중합효소연쇄반응-제한분절길이다형성법을이용하여분석하였다. 결과 : NRAMP1 유전자의 D543N 이형접합체의빈도는건강대조군에비해약제감수성군에서높았으나 (OR=2.10, 95% CI=1.00 to 4.41, p=0.049) 다제내성군에서는차이가없었다. 823C/T 의이형접합체의빈도는건강대조군에비해약제감수성군 (OR=2.79, 95% CI=1.11 to 7.04, p=0.029) 과다제내성군 (OR=3.30, 95% CI=1.33 to 8.18, p=0.010) 에서유의하게높았으나약제감수성군과다 제내성군사이에는유의한차이가없었다. 결론 : NRAMP1 유전자의 823C/T 이형접합체의빈도는약제감수성군과다제내성군에서유의하게높아, 폐결핵의발생과연관되는후보유전자일가능성이있으며약제감수성결과에따른결핵이환에는차이를보이지않았다. 약 참고문헌 1.
