Materials and Methods: Twenty patients with shoulder impingement syndrome received an injection of 60 mg ketorolac and were evaluated in terms of visual analogue scale (VAS), range of motion (ROM) and Constant-Murley score. The outcomes are compared with the data of patients treated by 40 mg triamcinolone injection, retrospectively. Results: There was no significant difference in the demographics, VAS, ROM, and Constan-Murley score between the two groups before the injection. At the 4 weeks follow-up, pain improvement was significantly greater in the corticosteroid group (2.7±1.5) than in the ketorolac group (4.9±2.08; p=0.001). However 12 weeks after the injection, there was no significant difference in pain improvement between the two groups (ketorolac: 2.9±2.2, corticosteroid: 2.6±1.82; p=0.707). The Constant-Murley score at the final follow-up improved from.5 to 52.1 in the corticosteroid group, and from 9.0 to 56.6 in the ketorolac group (p=0.677). ROM was increased in both groups, and external rotation was significantly greater in the ketorolac group than in the corticosteroid group at the final follow-up (ketorolac: 29. ±9.90, corticosteroid: 20.8 ±7.99 ; p=0.005). Conclusion: In this study, ketolorac provided an effect equivalent to triamcinolone in the treatment of subacromial shoulder impingement syndrome at 12 weeks after the injection. This result could offer better opportunities to manage patients with diabetes or local and systemic side effects of repetitive use of corticosteroids. 2 171 Effectiveness of in Shoulder Impingement Syndrome 방법으로는휴식, 경구용소염진통제, 주사치료, 운동치료등의보존적치료와견봉성형술과같은수술적치료가있다. 1-8) 대부분의 SIS는보존적치료에효과를보이는데, 그중에서도견봉하스테로이드국소주사는널리사용되고있는치료방법으로여러연구에서효과가증명된바있다. 4,7,8) 그작용기전은아직명확히밝혀져있지않으나스테로이드의항염작용 (anti-inflammatory effect) 이견봉하공간의염증반응을감소시켜진통효과를보이는것으로생각되고있다. 7,9) 그러나스테로이드국소주사는건파열, 피하지방의위축, 감염및전신부작용등의합병증을유발할수있고, 당뇨병환자에서혈당을증가시킨다는연구결과도보고된바있다 ) 이에, 스테로이드국소주사대신사용할수있는비스테로이드성소염진통제 (non-steroidal anti-inflammatory drugs, NSAIDs) 의사용이제안되었으나그임상적효용성에대해서는논란이있는상태이다. 1-17) 저자들은 NSAIDs 제재인 ketorolac 이 SIS 환자에서스테로이드의일종인 triamcinolone과유사한치료효과가있을것이라는가설을세웠다. 이를증명하고 SIS 환자에서 NSAID의임상적효용성을알아보고자 ketorolac을주사한군과 triamcinolone을주사한군사이의임상결과를후향적으로비교, 분석하였다. 대상및방법 국립경찰병원기관생명윤리위원회 (institutional review board, IRB) 에서승인을받아연구를진행하였다 (IRB number: HR-002). 2014년 월부터 2015년 12월까지견관절통증을주소로내원한환자중 1) 최소 1개월이상증상이지속되고, 2) 이학적검사상견봉의전방또는외측부에명확한압통점이존재하며, ) 상완을전방굴곡시켰을때 60 o -120 o 에서통 증을호소하고, 4) 상완을 90 o 외전, 내회전시켰을때통증이악화되어 SIS로진단된환자 40명을대상으로하였다. 병력상 1) 개월이내에환측견관절에주사를맞은병력이있거나, 2) 전신의염증성질환 (inflammatory disease) 또는국소염증소견이있는경우, ) 환측견관절의수술병력이있는경우, 4) 이학적검사상견관절불안정성이있는경우, 5) 단순방사선사진상관절와상완관절의관절염이있는경우, 6) 초음파또는자기공명영상 (magnetic resonance imaging) 소견상회전근개파열 ( 고도의부분파열및전층파열 ) 소견이있는경우는제외하였다. 1) 전체환자군은총 40명 ( 남자 4명, 여자 6명 ) 이었으며평균연령은 55.4세 ( 범위, 42-69세 ) 였다. 평균증상발현기간은 9.7주였고, 첫방문시시각적통증점수 (visual analogue scale, VAS) 는평균 7.16점이었다 (Table 1). 군에해당하는 20명의환자는전향적인방법으로모집되었으며, 해당되는환자에게 1% lidocaine 5 ml와 60 mg ketorolac을혼합주사하고자료를수집하였다. 그에상응하는 corticosteroid군에대한자료는의무기록을활용한후향적인방법으로수집되었으며, 1% lidocaine 5 ml와 40 mg triamcinolone을혼합주사한 20명의환자에대한자료를수집하여 ketorolac군과비교, 분석하였다. 각각의주사는환자를등받이가없는의자에앉힌후환측견관절을신전, 내회전하게하여대결절이견봉의앞쪽에위치하도록하는 modified Crass position에서초음파유도하에시행되었으며, 초음파선형탐촉자를전방에위치하여극상건 (supraspinatus) 상방의점액낭 (bursa) 이부풀어오르는것을관찰하여정확한위치에주사되었음을확인하였다. 18) 주사의용량은 corticosteroid 군의경우, 본기관에서통상적으로사용하는용량을적용하였으며, ketorolac군의경우, 기존의연구를참고하였고, 이러한용량과용법으로견봉하공간내에주사시, 유해하지않 Table 1. 3 172 Jieun Kwon, et al. 음을확인하였다. 1,17,19) 주사전, 모든환자에대하여한명의견관절분야의정형외과 전문의가각측정기 (goniometer) 를이용하여견관절의전방거상 (forward flexion), 내회전 (internal rotation at back), 외회전 (external rotation arm at side) 운동범위를측정하였다. 통증의정도와견관 절의기능적평가를위하여 VAS, Constant-Murley score 를측정 하였다. Constant-Murley score 는자가설문으로시행되었으며, 강도 (strength) 항목의 25 점을제외한총 75 점만점으로측정되었 다. 주사 4 주, 12 주후에각각의평가항목을재측정하였다. 모든 환자에게동일한운동치료를교육하고, 자가시행하도록하였다. 운동치료는견관절의전방거상, 내전 (adduction), 내회전, 외회전 에대한수동적관절운동으로구성되어있으며, 교육자료를통 해설명하고시범을보인후하루 회, 각각 15 세트씩시행할수 있도록하였다. 추가적인경구소염진통제는복용하지않도록하 였다. 통계적인분석은 IBM SPSS Statistics ver (IBM Co., Armonk, NY, USA) 을사용하였으며, p 값이 0.05 미만인경우통계적 인유의성이있는것으로해석하였다. 나이, 이환기간, 견관절운 Table 2. VAS 는주사 4 주째 ketorolac 군에서 4.9±2.08 점, corticosteroid 군에서 2.7±1.5 점으로 corticosteroid 에서유의하게더많이호전 된것으로나타났으나 (p=0.001), 주사 12 주째에는두군간에 VAS 의차이나호전정도의차이가없었다 (ketorolac 군 : 2.9±2.2 점, corticosteroid 군 : 2.6±1.82 점 ; p=0.707). RM ANOVA 에서시간에 따른통증은두군모두에서유의하게호전되는것으로나타났으 며 (p=0.001), 시간에따른통증변화양상이두군에서차이가있 는것으로나타나 (p=0.00), 주사후 corticosteroid 군에서조금더 빠른통증의호전을보이지만 12 주째에는두군모두비슷한정 도의호전이되는것을알수있었다 (Table 2, Fig. 1). Constant-Murley score 는두군모두에서시간경과에따라유 의하게호전되었으며, 시간경과에따른변화양상도유의한차 이가없는것으로나타났다 (ketorolac 군 : 56.6±7.92 점, corticosteroid 군 : 52.1±8. 점 ; p=0.677). 견관절운동범위도두군모두 에서시간경과에따라유의하게향상된것으로나타났다. 이중 견관절의외회전은두군에서시간경과에따른변화양상은비 Pain VAS Figure 1. Differences in Constant-Murley score over time between the two groups. 한차이가있는것으로나타났다 (p=0.02, between subjects effect) (Table 2, Fig. 2, ). 고찰 본연구에서가장중요한점은 SIS 환자에서 NSAIDs 제재인 ketorolac이스테로이드제재인 triamcinolone과유사한치료효과가있음을보였다는것이다. 은 1989년개발되어사용하기시작하여 1992년미국 Food and Drug Administration 승인을받은약제로 heterocyclic acetic acid 유도체이며 NSAIDs의일종이다. Cyclooxygenase inhibitor로작용하여체내 prostaglandin의형성을억제하며, 이러한기전을통해항염, 진통작용을보이는것으로알려져있다. 간에서대사되며, 90% 가량은신장을통해서배설되고, 나머지는담도계를통하여배설되고, 반감기는대략 시간에서 9시간정도이다. 사용방법은경구, 정주, 근주, 국소용법등다양한경로로사용되고있다. 20) 국소주사용약제로스테로이드대신 NSAIDs가제안된것은, 크게두가지이유에서비롯된다. 첫째, 스테로이드가국소적인항염작용에의해통증을완화하고기능적회복을돕는다면국소 A 150 B FF ( ) o 10 ER ( ) o C IR ( ) o Figure. 5 174 Jieun Kwon, et al. 적인항염작용을보이는다른약제의사용역시가능하다는관점이다. NSAIDs는 prostaglandin의형성을억제함으로써항염작용을보이고, 정주, 경구, 국소주사등여러경로로투약이가능한약제이다. 또한충분히많은연구가선행되어그작용기전과체내흡수, 배설경로, 부작용과사용금기등이널리알려져있다는장점이있다. Itzkowitch 등 16) 은이러한점에착안하여견관절운동범위의제한이없는회전근개염환자 80명을무작위배정하여 NSAIDs의한종류인 tenoxicam 20 mg을주사한군과위약을주사한군의임상결과를분석하였다. 그결과 tenoxicam을주사한군이위약을주사한군에비하여유의하게통증이호전되고, 운동범위가향상된다는것을보고하였다. 이연구는 SIS에서 NSAIDs 제재가국소주사제로사용될수있다는가능성을제시하였다. 그러나그이후 NSIADs 제재가 corticosteroid 제재에비하여국소주사시통증경감을비롯한임상적호전측면에서효과가떨어진다는연구들이발표되었다. Karthikeyan 등 17) 의이중맹검무작위배정임상연구에서는 tenoxicam과 methylprednisolone을견봉하주사하고 Constant-Murley score, disability of arm, shoulder and hand score, Oxford shoulder score를분석하였는데, 주사 2주후, 4주후, 6주후결과가모두스테로이드주사군에서우월한것으로보고하였다. 그외에도스테로이드와 NSAIDs 를혼합주사할경우스테로이드단독제재에비하여임상적우월성이있는지비교하기위한연구가있었으나, NSAIDs 병합투여와스테로이드단독투여사이에는유의미한임상적차이가없는것으로밝혀졌다. 15) 반면 Min 등 1) 은 2명의환자를무작위로두군으로분류하고, 한군은 triamcinolone을, 나머지한군에는 ketorolac을주사하여 4주후 VAS와견관절운동범위, UCLA score를분석한결과, ketorolac을주사한군에서모든임상적지표가유의미하게더많이호전되었음을보고하였다. 이처럼견봉하공간 NSAIDs 제재국소주사의치료적효과에대해서는성분별로아직논란이있는상태이다. 국소주사용약제로서 NSAIDs에대한더많은연구가이루어져야하는또다른이유는현재국소주사용약제로널리사용되고있는스테로이드의크고작은유해성또는주사후발생할수있는부작용때문이다. 건또는힘줄자체 (tendon substance) 에스테로이드제재를직접적으로주사할경우, 건파열이발생할수있으며, 조직학적으로건위축이생긴다는것은널리알려진사실이다. 21,22) 또한최근발표된동물실험에서는스테로이드가회전근개를약화시킬뿐만아니라골질 (bone quality) 을저하시키고, 골-건접합부 (osteotendinous junction) 에도악영향을준다는것이증명되었다. 2) 반복적인스테로이드주사는회전근개봉합술후의결과에도영향을미치는데, 술전 회이상의스테로이드주사는봉합뽑힘강도 (suture pull-out strength) 를감소시키고재파열률을높이는것으로보고되었다. 4,24) 또한국소스테로이드제재주사는당뇨환자의혈당을상승시키는것으로알려져있다. 25,26) 또한가지고려할점은장기적치료의관점에서스테로이드국소주사가도움이되는가의문제이다. SIS 환자에서스테로이드국소주사가단기적측면에서도움이된다는것은많은연구에서증명된바있다. 그러나 Blair 등 4) 은 0주이상의장기적측면에서는효과가없다고하였고, Gaujoux-Viala 등 27) 에의한메타분석에서도 8주이상의장기적측면에서는물리치료, 경구소염진통제등의다른치료수단에비하여효과가떨어진다고보고하였다. 한편 Smidt 등 28) 은주관절외상과염 (lateral epicondylitis) 에서환자스스로 완전히회복되었다 또는 상당히호전되었다 라고답한경우를치료성공으로평가하였는데, 국소스테로이드제재를주사한경우주사 6주후에는치료하지않고경과관찰한군 (waitand-see) 에비하여치료성공률이높았지만 12주후에는비슷한성공률을보였고, 그이후에는오히려치료의성공률이떨어지는것으로보고하였다. 즉스테로이드제재의국소주사가장기적으로는아무것도하지않고경과관찰한것보다도나쁜결과를보인다는것이다. 이러한기존의연구결과들은스테로이드국소주사는단기적측면에서치료효과가입증되었으나장기적인측면에서는그치료효과가불분명하며오히려질병의경과를악화시킬가능성이있음을시사하고있다. 본연구의결과, 주사 4주후두군모두에서통증이호전되었으나 corticosteroid군에서더많이호전되는양상이었고, 12주후에는두군사이에유의한차이가없었다. 이는 SIS 환자에서 ketorolac 국소주사가스테로이드제재와유사한치료효과를보일수있음을증명한것으로보인다. 이러한결과로미루어혈당이조절되지않는당뇨환자, 이미수차례스테로이드국소주사를시행하여건의약화가염려되는경우, 회전근개파열이있으나수술이불가능한상태로통증조절이필요한경우등스테로이드국소주사를사용하기어려운환자에서안전한대체재로사용될수있을것으로기대된다. 본연구는전향적인자료의수집과후향적분석으로인하여본질적인제한점을안고있다. 그러나 Table 1에서제시한바와같이나이, 성별, 이환기간등, 각각의군의특징을조사하여분석하였으며, 그결과두군간의특징에유의미한차이가없는것으로나타났고이러한결과는각군의특징에의해결과가편향되는위험성을감소시켜줄것으로생각된다. 결과평가에영향을미칠수있는다른요인을통제하기어려운점이또하나의중요한제한점으로생각되나본연구에서는모든환자에대하여동일한치료방침을적용하고추가적인보존적치료수단을통제하여이러한제한점을최대한극복하고자하였다. 그러나각군의환자수가충분하지않아통계적인신뢰성이확보되지않은바, 결과값을받아들임에있어신중해야하며결과의신뢰도를좀더높이기위해서는향후추가적인무작위배정전향적연구가이루어져야할것으로생각된다. 또한 ketorolac군과 corticosteroid 군사이의단기적치료효과의차이가불분명하다는단점이있

6 175 Effectiveness of in Shoulder Impingement Syndrome 다. 본연구에서는주사 4주후 VAS 외의다른임상지표에서는유의미한차이가보이지않아이러한차이를단기적치료효과의차이로규정하기어려운점이있었다. 보다짧은기간에추가적으로추시를하였더라면보다명확한단기적치료효과의차이를확인할수있었을것으로생각되어전향적인연구가필요할것으로보인다. Constant-Murley score의항목중강도의항목을제외하고평가한것역시제한점이될수있을것으로생각된다. 저자들은 SIS 환자의치료결과를평가함에있어서가장중요한항목은통증의호전이라고판단하였다. 통증이있을경우이로인한강도의감소가 Constant-Murley score의결과값을편향시킬수있고, 점수값의신뢰도를떨어뜨릴수있을것 29) 으로판단하여강도항목을제외한바있다. 그러나기존연구에이러한선례가없으며사용가능한다른기능적점수체계를사용하지않았다는점에서아쉬움으로남는다. 또한관절운동범위의제한이경미한초기유착성관절낭염의경우 SIS와감별이어려워연구대상중이러한초기유착성관절낭염환자가혼재되어있었을가능성이있다. 0) 그러나현재널리사용되고있는스테로이드제재의제한성을알리고이를대체할수있는약제로서 NSAIDs를재조명했다는점에서본연구의의의가있다고생각된다. 결론 본연구는 SIS에서 ketorolac의견봉하국소주사요법이스테로이드제재와유사한치료효과를보임을증명했다는점에서의의가있다고보인다. 반복적으로스테로이드제재를국소주사할경우발생될수있는부작용들을감안한다면 ketorolac의견봉하국소주사는 SIS 환자에서스테로이드사용에대한하나의대안이될수있을것으로생각된다. 8 177 pissn : , eissn : Original Article J Korean Orthop Assoc 2017; 52: 충돌증후군에서 국소주사요법의단기효과 : 스테로이드국소주사요법과의비교연구 권지은 이예현 김해민 김종민 정현석 이승림 국립경찰병원정형외과 목적 : 견봉하국소주사요법으로 ketorolac 과 corticosteroid 의효과를비교분석하고자하였다. 대상및방법 : 견관절충돌증후군으로진단된환자 20명에게 ketorolac 60 mg을, 또다른 20명에게 triamcinolone 40 mg을주사하고, 시간에따른시각적통증점수 (visual analogue scale, VAS), 관절운동범위 (range of motion, ROM) 및 Constant-Murley score 를측정하여후향적인방법으로비교, 분석하였다. 결과 : VAS는주사 4주후, 두군모두에서감소된소견을보였으나 corticosteroid 군에서 ketorolac 군에비하여유의하게더감소된소견을보였고 (p=0.001), 12주후에는두군간의차이가없었다 (p=0.707). Constant-Murley score 는두군모두에서향상되었으며유의한차이가없었다 (p=0.677).
