These may be helpful for clinical differentiation between Alzheimer's disease (AD) and vascular dementia (VD). Most previous studies have reported the prevalence and severity of BPSD regardless of severity of progression. However, we evaluated the differences of BPSD by grading dementia progression and including severe cases only. Methods: 141 dementia outpatients from clinics of Busan-Gyeongnam Dementia Association in Korea were analyzed. All patients were administered the Korean version of the Neuropsychiatric Inventory (K-NPI), the expanded version of Korean Clinical Dementia Rating Scale (CDR), and the Korean version of Mini-Mental State Examination (K-MMSE). Results: There were 95 patients with AD and 46 patients with VD. AD patients revealed higher frequency of manifested BPSD symptoms and most of subscales showed higher composite scores also. But apathy was more severely and frequently manifested in VD. AD patients had more frequent night-time behavior and aberrant motor symptoms above 3 point in CDR 0.5 and VD patients had more frequent apathy symptom above 3 point in CDR 1. Conclusions: These results suggest that generally AD patients revealed more frequent and severe BPSDs than VD patients. Apathy was a more dominant symptom in VD. It seemed that some of the symptoms were valuable clues for the differential diagnosis even though a better designed study was required. 2 서론 치매의중요한원인인알츠하이머병과혈관성치매의임상적특징중하나인행동심리적증상은감별진단뿐아니라예후판정의요소로임상적중요성이높다. 1-3 알츠하이머병에서나타나는행동심리적증상의발생기전은특정뇌부위의신경전달물질장애에의한것으로추정되고있다. 4 발병기전이다른혈관성치매에서도행동심리적증상이자주동반되는데, 원인이정확히밝혀지지는않았지만뇌병변에의한것으로보고되고있다. 5,6 치매의원인에따른행동심리적증상은임상적중요성이높아, 원인질환별차이에대한연구가많았지만, 연구자마다결과가달랐다. 7,8 국내에서도이에대한연구가있었지만대상군의수가적었고, 9 치매의진행정도에영향을받음에도 10 불구하고진행정도를구분하지않고비교하여결과를해석하는데제한점이있었다. 11 따라서저자들은진행정도를구분하여알츠하이머병과혈관성치매환자의행동심리적증상차이를알아보고자하였다. 행동심리적증상이대상군의구성과연구방법 12 및평가자의특성에영향을받기때문에, 13,14 이들영향을줄이고자저자들은다기관연구로조사를하였다. 행동심리적증상은 Korean Neuropsychiatric Inventory (K-NPI) 15 를사용하여조사하였고, K-NPI 총점과발현된행동심리적증상의수, 그리고세부증상의정도, 유병률을구분하여비교하였다. NPI 총점은보호자나의료인이체감하는증상의심각도와일치하지않는경우가있다. 따라서, 증상으로인한고통이나심각성을고려해서두질환간의차이를알아보는것도임상적의미가있다고판단하여, 부양가족에게스트레스를줄정도의증상을가진경우만포함시켜알츠하이머병과혈관성치매의차이를알아보았다. 대상과방법 1. 대상 2004 년 3월부터 2004 년 5월까지지역사회에거주하는환자로서, 부산 경남지역의 9개의료센터에서 DSM-IV (Diagnostic and Statistical Manual of Mental Disorders 4th edition) 16 진단기준하에치매환자로등록된 150 명을대상으로하였다. National Institute of Neurological and Communicative Disorders and Stroke-the Alzheimer's Disease and Related Disorders Association (NINCDS-ADRDA) 17 의 probable 진단기준에맞는 95 명의알츠하이머병환자와 National Institute of Neurological Disorders and Stroke and the Association International pour la recherche et l'enseignement en Neurosciences (NINDS-AIREN) 18 진단기준의 probable 또는 possible 에맞는 36명의혈관성치매환자를대상으로하였다. 치매환자와적어도 3개월이상동거한주부양자가질문을이해하고, 응답이가능하면서동의서에서명한 141 명을연구대상으로하였다. 2. 방법환자와부양자의나이, 학력, 성별을조사하였고, 전반적인치매상태를평가하기위해치매선별검사인한국판간이정신상태검사인 Korean version of Mini-Mental State Examination (K-MMSE), 19 전반적인치매의진행정도를평가하기위해 Korean version of expanded Clinical Dementia Rating scale (CDR), 20 행동심리적증상평가를위해 Korean version of Neuropsychiatric Inventory (K-NPI) 12 를사용하였다. K-NPI 는세부증상으로구분하여조사하였다. K-NPI 총점, 발현된세부증상수, 각세부증상점수및유병률을구분하여조사하였다. 대상환자들은알츠하이머병군과혈관성치매군으로나누어, 환자와부양자의사회인구학적차이를비교하였다. 학력은무학인경우 1, 초등졸인경우는 2, 중졸인경우는 3, 고졸인경우는 4, 대졸이상인경우는 5로구분하여조사하였다. K-MMSE, CDR, K-NPI 총점, K-NPI 각세부증상점수및유병률의기술통계를구하고, 두군간의차이를 t-test 로검증하였다. 또한치매의진행정도에따른행동심리적증상의차이가있는지를알아보기위하여전체환자및 CDR 등급에따라나누어서도조사를하였다. 증상이심하여부양가족에게스트레스를줄정도의기준은, 문제를유발하는정도의증상 을기준으로행동심리적증상을고찰한 Ballard 등 21 의방법을사용하여, 증상은있으나임상적으로는의미가없음 (K-NPI 의빈도와정도를곱한점수가 1 이상 3 미만인경우 ), 임상적으로의미있음 (K-NPI 의빈도와정도를곱한점수가 3 이상인경우 ) 의두군으로나눈후, 임상적인의미를보인환자들만을대상으로 K-NPI 세부증상의유병률차이를조사하였다. 특히임상적인의미가있는증상이나타나는세부증상과이들의유병률을전체환자와 CDR 등급, 치매의원인별로나누어조사하였다. 성비는 Chi-square test 로검정하였다. 모든통계는 SPSS 10.0 version 통계프로그램을이용하였으며, p 값이 0.05 미만인경우를통계적으로의미있는결과로판단하였다. 459

3 김태유김수영김재우김응규박경원성상민손태홍안경숙유봉구윤수진윤성민이상찬정해관최문성홍태용부산 경남치매학회 결 과 차이가없었다. 보호자의나이, 성비와 (χ 2 =-1.449, p=0.147) 는차이가없었으나학력은알츠하이머병군환자보호자에서높았다 (p<0.01)(table 1). 1. 인구사회학적특성 전체대상환자중여자는 87 명 (61.7%), 남자는 54 명 (34.3%) 이었으며, 평균연령은 72.9±8.4 세 (mean±sd), 평균학력은 2.3±1.3단계, K-MMSE 는 16.7±6.2 점, CDR 은 1.4±0.7, K- NPI 총점은 22.7±22.7 점, 발현된 K-NPI 증상수는 4.9±2.8 개였다 (Table 1). 알츠하이머병군환자는 95명 (69.4%), 혈관성치매군환자는 46명 (30.6%) 이었다. 전체환자의부양자특성은여자가 99명 (63.5%), 남자가 42명 (33.5%) 이었고, 평균연령은 54.3±14.5 세, 평균학력은 3.6±1.3단계이었다 (Table 1). 알츠하이머병군과혈관성치매군의특성을비교해보면알츠하이머병군에서나이가많았으나 (p<0.01), 성비와 (χ 2 =2.604, p=0.107), 학력, 치매의진행정도, 행동심리적증상의총점은 2. 전체환자를대상으로한행동심리적증상차이발현된증상수는알츠하이머병군에서 5.1±3.0 개로혈관성치매군의 4.3±2.2개에비해많았다 (p<0.05)(table 1). 망상 (p<0.05), 탈억제 (p<0.01), 비정상적인운동행동 (p<0.01) 점수가알츠하이머병군에서높았고, 무감동 / 무관심은혈관성치매군에서높았다 (p<0.05). 그리고망상 (p<0.01), 환각 (p<0.01), 불안 (p<0.05), 탈억제 (p<0.01), 비정상적운동행동 (p<0.01), 수면중행동의 (p<0.05) 유병률은알츠하이머병군에서높았고, 무감동 / 무관심의유병률은혈관성치매군에서높았다 (p<0.01) (Table 2). 4 3. CDR 등급별행동심리적증상차이 CDR 등급별로구분하여, 세부증상의평균점수와유병률의차이를살펴보면, CDR 0.5 에서는불안의평균점수가알츠하이머병군에서높았고 (p<0.05), 망상 (p<0.05), 불안 (p<0.01), 탈억제 (p<0.01), 비정상적운동행동 (p<0.01), 수면중행동의 (p<0.05) 유병률은알츠하이머병군에서높았다. CDR 1에서는무감동 / 무관심점수와 (p<0.05) 유병률이 (p<0.01) 혈관성치매군에서높았고, 비정상적운동행동의 (p<0.01) 유병률은알츠하이머병군에서높았는데, 이외에는통계적차이가없었다. CDR 2에서는탈억제 (p<0.05), 비정상적인운동행동 (p<0.01) 점수와망상의 (p<0.01) 유병률이알츠하이머병군에서높았고, 무감동 / 무관심의 (p<0.01) 유병률은혈관성치매군에서높았다. CDR 3 에서는환각 (p<0.05), 탈억제의 (p<0.01) 유병률이알츠하이머병군에서높았다. CDR 0.5 에서알츠하이머병군의수면중행동의유병률은 60% 로혈관성치매군의 20% 보다의미있게높았고 (p<0.05), 점수가 3점이상인경우는각각 27%, 0% 로알츠하이머병에서높 Table 3. 5 김태유김수영김재우김응규박경원성상민손태홍안경숙유봉구윤수진윤성민이상찬정해관최문성홍태용부산 경남치매학회 았다 (p<0.05). 비정상적운동행동의유병률도알츠하이머병군에서 33.3% 로혈관성치매군의 10% 보다높고 (p<0.01), 점수가 3점이상인경우도각각 13%, 0% 로차이를보였다 (p<0.05). CDR 1에서는혈관성치매군의무감동 / 무관심의유병률이 75% 로알츠하이머병군의 47.8% 보다높았고 (p<0.01), 점수가 3점이상인경우가 55%, 21.7% 로혈관성치매에서높았다 (p<0.05). CDR 2에서무감동 / 무관심의유병률은혈관성치매군에서 100% 였으며, 점수가 3점이상인경우도 88.9% 였다. CDR 3에서는환각의유병률이혈관성치매군에서 50% 로알츠하이머병군에서 14.3% 보다혈관성치매군에서높았지만 (p<0.05), 임상적인의미가있는경우는혈관성치매군에서 0%, 알츠하이머병군에서 33% 로차이를보였다. 4. 임상적의미가있는정도의증상을보인환자들만을대상으로한두군간의차이환자를치료하는임상의사의입장에서는행동심리적증상의유무뿐아니라, 심한정도의행동심리적증상이어떤유형의치매환자에게나타나는지도중요하다. 그래서평균점수 (composite score, 정도 빈도 ) 가 3점이상으로임상적인의미가있는증상을기준으로조사를하였다. 알츠하이머병군에서는 CDR 0.5~1 까지는여러가지행동심리적증상의유병률은높았으나, 정도는심하지않았다. 그러나혈관성치매군에서는 CDR 0.5 인경우임상적의미가있는정도의행동심리적증상이나타나지않았지만, CDR 1에서는무감동 / 무관심의평균점수가 3점이상이고, 유병률도높아알츠하이머병군과차이를보였다. 알츠하이머병군과혈관성치매군모두 CDR 2에서대부분의행동심리적증상이임상적인의미를가질정도로심해지고유병률도높아졌는데, 알츠하이머병군에서발현된행동심리적증상수와유병률그리고증상의심한정도가높았다 (p<0.05). 전체환자에서는망상 (p<0.05), 탈억제 (p<0.05), 비정상적운동행동이 (p<0.01) 알츠하이머병군에서많았고, 무감동 / 무관심이 (p<0.01) 혈관성치매군에서많았다 (Table 3). CDR 0.5에서는수면중행동이 (p<0.05) 알츠하이머병군에서많았다. CDR 1에서는무감동 / 무관심이 (p<0.05) 혈관성치매군에서많았다. CDR 2에서는탈억제가 (p<0.05) 알츠하이머병군에서많았다. CDR 3에서는초조 / 공격성이 (p<0.05) 혈관성치매군에서많았다. 알츠하이머병군에서는치매의진행정도에따른, 행동심리적증상의유병률이다양하게나타났다 (Fig 1). 그러나혈관성치매군에서는 CDR 0.5 를제외하면, 진행정도에따른각증상들의유병률분포의형태가유사한양상을보였다. 망상, 환각은비슷한유병률을보이고, 무감동 / 무관심은제일높 은유병률을보이고, 탈억제는낮은유병률을보이며, 화를잘냄 / 불안정의유병률은비정상적운동행동보다높은형태를보였다. 고찰 국내에서도알츠하이머병과혈관성치매에서의행동심리적증상에대한비교연구가있었지만, 연구자마다결과가달랐다 본연구도이전의연구와차이를보였는데, 이는다음과같은원인에의한것으로추정된다. 첫째, 행동심리적증상이환자를대상으로직접검사하는것이아니고, 보호자의관찰을통해간접적으로평가됨으로써보호자의특성이나상황에따라영향을받을가능성이높다. 둘째, 치매의진행정도에따라행동심리적증상의특성이다를수있기때문의대상군의중증도에따른구성비와같은특성에영향을받는다. 셋째, 혈관성치매의아형에따라서도특성이다를수있는데이를정확히구분하지못하였을가능성이높다. 넷째, 대상군이순수한혈관성치매와알츠하이머병환자가아니고, 혼합성치매환자가포함되었거나진단이잘못되었을수있다. 본연구에서는알츠하이머병에서행동심리적증상이빈발하고정도가심했는데, 이것은혈관성치매와알츠하이머병의행동심리적증상차이를비교한다른연구 9 와상반되었다. 하지만일부의연구에서는두군간의차이가없다는보고도있어, 2 연구자마다차이가있다. 이러한차이는대상군의경증과중증환자의비율과연구방법의차이로생각되는데, 다른연구와공통되는점은무감동 / 무관심이혈관성치매군에서심했다는것이다. 인지기능을포함한혈관성치매의임상적특징은주로전두엽혈류량의감소나, 22 포도당대사저하로인한 23 전두엽기능장애로설명하기도하지만, 전두엽기능장애와는관계가없다는보고도있다. 24 혈관성치매에서나타나는행동심리적증상도전두엽기능장애로설명하지만, 전두엽기능장애로나타나는다른증상인탈억제와비정상적운동행동의발현률은혈관성치매군에서높지않아, 이러한기전으로는충분히설명하기어렵다. 따라서최근에는전두엽과연결된안와전두회로 (orbitofrontal circuit), 전대상회로 (anterior cingulate circuit) 등여러종류의뇌경로에대한가설이더정확한기전으로언급되기도한다. 25 알쯔하이머병의경우측두엽, 두정엽에서병리적과정이시작되어전두엽으로진행하기때문에, 무감동 / 무관심은어느정도진행한후에나타나는것으로알려져있지만, 본연구에서는초기부터나타나행동심리적증상이뇌병변이외의요소에도영향을받거나, 우울증상을무감동 / 무관심으로보호자가잘못평가하였을수도있다고생각된다. 462

6 대부분의행동심리적증상은중기부터발현되는것으로알려져있는데, 26 본연구에서도 CDR 2에서부터증상이심해졌다. 2 정도는심하지않았지만, CDR 0.5 인환자에서여러가지행동심리적증상이높은빈도로발생하였다. 이것은이러한증상으로병원을찾은환자를주대상으로했기때문일가능성이높다. CDR 0.5 에서수면중행동과비정상적운동행동이 3점이상인경우는알츠하이머병에서많았고, 무감동 / 무관심, 우울 / 낙담이 3점이상인경우는혈관성치매에서많았다. CDR 1의상태에서무감동 / 무관심이 3점이상인경우는혈관성치매에서많아, 치매의진행정도에따라서도원인에따른행동심리적특성차이가있었다. 임상적인의미가있는정도의행동심리적증상은 CDR 2부터대부분발현되었다. CDR 2 이상에서는무감동 / 무관심이혈관성치매에서높아이에대한처치나치료가중요하다. CDR 3에서는환각에대한치료나처치가알츠하이머병에서중요함을유추할수있다. 앞서의결과에서보듯이치매의진행정도에따라서행동심리적증상의차이가심해, 치매의진행정도를구분하여비교하는것이중요함을확인하였다. 행동심리적증상을 NPI 의총점으로비교한연구가많았는데, 점수가높다고행동심리적증상의심각성이높은것으로판단하기는힘들다. 이것은 NPI 총점이여러증상의합으로이루어져있고, 임상적으로크게문제되지않는증상도포함되어있는구조적특성때문이다. 따라서증상을구분하여연구할필요도있고, 혈관성치매를아형과, 진행정도를구분하여많은대상자를장기간추적관찰하는연구가필요하다. 따라서앞서의조건을충족하지못하는연구결과의해석에있어서는, 대상군의특성이나연구방법에대한고려가필요하다. 혈관성치매의경우진단기준의확립및분류방법이아직정립되지않아연구에걸림돌이되고있다. 따라서최근에는혈관성치매를아형으로구분하여연구를진행하는경우가많지만, 같은아형의혈관성치매라도뇌병변의위치, 크기, 병리적변화의정도에따라서도임상양상이다를수있기때문에, 이에대한구분도앞으로는추가되어야할것으로생각된다. 본연구의제한점으로는증상이심한입원환자가제외된, 외래환자를대상으로하였고, 약물치료를하고있어증상이경했을수도있다. 그리고 CDR 등급별환자수에서차이가있고, 대상군의수도적었으며, 혈관성치매의아형을구분하지못하였다. 결론적으로알츠하이머병군에서행동심리적증상의발현이높았으며, 무감동 / 무관심은혈관성치매군에서보다자주나타나고증상도심하였다. 그리고비정상적운동행동, 수면중행 동, 무감동 / 무관심, 우울 / 낙담이초기에알츠하이머병과혈관성치매의감별에도움을줄것으로판단되지만보다면밀한연구가필요하다.
