Hypereosinophilia is a rare cause of thrombosis that usually involves the heart and small vessels like such as retinal vessels. We report a case of massive deep vein thrombosis that developed in a young patient with hypereosinophilia who was successfully treated with continuous local infusion of urokinase. 기생충 입원 당일 헤파린을 투여하기 시작하였고 초음파 유 감염, 알레르기 질환, 약진 등의 병력은 없었고 가족력 도하에 양측 오금정맥(popliteal vein)을 천자하여 정 에서도 특이사항이 없었다. 내원 당시 환자는 급성병색 맥조영술을 시행하였다.

2 내원하였다. 환자는 평소 건강하였으나 내원 1주일 전 심전도 및 흉부 X-선 사진은 정상이었다. 경흉부 부터 좌측 하지부위의 부종이 생겨 점차 심해졌고 내원 심장초음파검사상 방실의 크기나 수축기능은 정상이었 4일 전부터는 우측 하지에도 부종과 압통이 발생하여 고 심실의 비후나 심실내의 혈전, 심낭삼출은 관찰되지 타 병원에 내원하여 시행한 컴퓨터 단층촬영상 심부정 않았다(Fig. 1). 맥 혈전증으로 진단받고 본원으로 전원되었다. 기생충 입원 당일 헤파린을 투여하기 시작하였고 초음파 유 감염, 알레르기 질환, 약진 등의 병력은 없었고 가족력 도하에 양측 오금정맥(popliteal vein)을 천자하여 정 에서도 특이사항이 없었다. 내원 당시 환자는 급성병색 맥조영술을 시행하였다. 정맥조영술상 하대정맥 원위부 을 보였으며 혈압 120/70 mmhg, 체온 36, 맥박수 로 부터 양쪽 오금정맥까지 이르는 거대한 혈전으로 의 86/분, 호흡수 18/분이었다. 경정맥 확장 소견은 없었 심되는 광범위한 음영결손이 관찰되었다(Fig. 2). 이어 고 흉부청진상 심음과 호흡음은 정상이었다. 양측하지 sheath를 통하여 Multi-sideport(Cook Bloomington, 에서 압통과 부종이 관찰되었으나 하지의 동맥들은 잘 USA)카테터를 양측 표재대퇴정맥에 위치시키고 혈전 촉지되었다. 용해제를 병변부위에 국소적으로 투여하는 방법으로 각 3 말초혈액검사상 백혈구 21,500/mm (호중구 35.5%, 3 각의 도관을 통해 urokinase를 30,000 U/hr의 속도로 림프구 16.2%, 호산구 45.7%(9,800/mm ), 단핵구 지속적으로 투여하였다. 입원 2일째 시행한 추적 정맥 2.3%), 혈색소 11.4 g/dl, 혈소판 337,000/mm3였고 조영술상 좌측 표재대퇴정맥의 혈전은 거의 소실되었으 ESR 26 mm/hr였다. P time(inr)은 1.09, aptt 나 골반정맥과 공통대퇴정맥의 혈전은 여전히 관찰되었 38.1초, thrombin time 23.4초로 정상이었다. 간 및 폐 으며 우측 심부정맥의 혈전은 크기는 줄었으나 여전히 흡충에 대한 피내반응검사는 모두 음성이었고 기생충에 관찰되었다. 매일 정맥조영술로 추적 관찰하면서 혈전 대한 ELISA검사도 모두 음성이었다. 면역혈청학 검사 이 용해되는 부위까지 조금씩 카테터를 진행시키는 방 상 rheumatoid factor, VDRL도 음성소견을 보였다. 법으로 혈전 용해제를 국소 투여하였으며, 입원 4일째 간염 검사에서 HBsAg, Anti-HBc, HBeAg이 양성이 까지 urokinase 480만 단위가 주입되었고 양측 표재 었고 HBV-DNA는 pg/ml였다. ANA, ANCA, 대퇴정맥의 혈전은 거의 다 소실되었으나 양측 공통장 anti-phospolipid Ab IgG 및 IgM 모두 음성이었으나 골정맥에서는 혈전에 의한 부분폐색이 관찰되었다. 입원 lupus anticoagulant는 양성이었다. Protein C 73%, 5일째 양측 공통장골정맥의 혈전들이 더 많이 소실된 protein S 84%, anti-thrombin Ⅲ 62%였고 factor V 것을 확인하고 도관을 제거하였으며 입원 6일째 부터 는 정상이었다. 는 coumadin으로 전환하였다. 입원 7일째 폐관류 스캔 Fig. 1.
