2 당 28회, 체온은 36.5였다. 외견상 급성 병색소견이었 tting edema)이 촉지되었으며, 양측 정강이에 보라색의 으며 흉부 청진상 심음은 규칙적이었으며, 심잡음(mu- 반점이 있었다(Fig. 1). 신경학적 검사는 정상이었다. 심 rmur)이나 분마음(gallop)은 들리지 않았다. 폐음은 양 전도 검사상 사지유도 Ⅱ, avf 및 전흉부유도 V3-6 측에서 깨끗하였고, 수포음(crackles)도 들리지 않았다. 에서 3 mm의 ST분절 상승을 보였으며(Fig. 2) 심초 복부 촉진상 압통이나 간, 비장 종대는 없었고, 복부 청 음파검사에서는 심첨부의 무운동소견을 보였다. 유로키나 진상 혈관 잡음도 들리지 않았다. 하지에 함요 부종(pi- 제를 double bolus로 투여 후 심실세동이 나타나 세 차 례 제세동 후 정상 동율동으로 회복되었다. 흉부 X-선 단순촬영에서 특이 소견은 없었으며 혈액학검사는 백혈 구 16,400/mm3, 혈소판 228,000/mm3였으며 혈액화 학검사에서는 AST 40 IU/L, CPK 302 IU/L, LDH 414 IU/L, Troponin-I 0.5 ng/ml, BUN 18.8 mg/dl, Cr-eatinine 1.0 mg/dl, 총콜레스테롤 248 mg/dl, 중 성지방 135 mg/dl, HDL 콜레스테롤 73 mg/dl, LDL 콜레스테롤 141 mg/dl, 프로트롬빈 시간(PT) 10.3초 (Int-ernational Normalized Ratio(이하 INR) 0.838), 부분활성프로트롬빈 시간(aPTT) 22.5초였다. 추적 혈액 화학검사에서 Troponin-I는 100 ng/ml(정상 0 1) 까지 상승하였고 CK-MB는 279 ng/ml(정상 0 5.8) 까지 상승하였으며 CPK 최고치는 9790 IU/L, LDH 최 고치는 2187 IU/L였다. 심부정맥 혈전증의 과거력이 있어 전신 색전증의 고위험군에 속하는 것으로 판단하여 헤파린을 3일간 사용하였다. 내원 6일째 추적관찰 심초 음파상에서 좌심실 심첨부에 cm 크기의 혈전이 관찰되어(Fig. 3) 헤파린 및 coumadin을 사용하여 항응 고요법을 실시하였다. 내원 20일째 실시한 추적 심초음파 검사에서 좌심 며 protein C 92.0%(73 142%), protein S 99.

5 정도 lupus anticoagulant 0.9 LAC(0.8 1.2), homocys- 로 유지 중이다. teine 12.1 ng/dl(4.45 12.4)으로 antithrombin Ⅲ 결핍증으로 진단하였다.

Left (A) and right (B) coronary angiograms show no significant stenosis in coronary arteries. 소견이 없었다(Fig. 4). 유전성 과응고혈증을 감별하기 thrombin Ⅲ 측정결과 6.9 mg/dl이 측정되었다(Fig. 위해 실시한 검사에서 factor V Leiden은 음성이었으 6). 내원 20일째 실시한 추적 심초음파 검사에서 좌심 며 protein C 92.0%(73 142%), protein S 99.0% 실내 혈전이 더 이상 관찰되지 않아 퇴원하였으며 현재 (60 150%), antithrombin Ⅲ 8.0 mg/dl(22 35), 외래에서 coumadin을 사용하여 INR을 정도 lupus anticoagulant 0.9 LAC( ), homocys- 로 유지 중이다. teine 12.1 ng/dl( )으로 antithrombin Ⅲ 결핍증으로 진단하였다. 정맥조영술상 하지심부정맥에 고 찰 다발성 폐색소견이 관찰되었으며(Fig. 5) 난원공개존을 확인하기 위해 경식도심초음파 및 contrast 검사를 실시 유전적인 원인으로 혈전증의 위험이 높은 유전성 혈 하였으나 난원공개존은 관찰되지 않았다. 가족에 대한 전 질환은 여러 가지가 알려져 있는데 그 중 가장 빈도 antithrombin Ⅲ 검사를 실시하려고 했으나 다른 가족 가 많은 것은 Factor V Leiden으로 알려져 있으며 그 들은 비협조로 실시하지 못하였고 환자의 누나의 anti- 다음으로는 C 단백 결핍, S 단백 결핍, 그리고 antithr523

7) AT Ⅲ 결핍증은 유전성과 후천성으로 구분되며 유전 성 AT Ⅲ 결핍증은 AT Ⅲ 항원이 감소하거나(type Ⅰ), 항원 농도는 정상이지만 antithrombin 및 헤파린 cofactor actvity가 모두 감소된 경우(type Ⅱ), 그리고 Fig. 6. Family study.

Other family members were not tested because of poor cooperation. 헤파린 cofactor activity만 감소된 경우로 나눌 수 있 다.8) 본 증례의 경우 AT Ⅲ 항원이 감소하여 type Ⅰ AT Ⅲ 결핍증으로 진단할 수 있었다.
