1 대한내과학회지: 제 92 권 제 4 호 Roadmap to diagnosis 말초혈액 호산구증가증의 진단적 접근법 인제대학교 의과대학 해운대백병원 내과, 2한양대학교 의과대학 내과학교실 2 박찬선 김상헌 Diagnostic Approach to Peripheral Eosinophilia Chan-Sun Park and Sang-Heon Kim2 Department of Internal Medicine, Inje University Haeundae Paik Hospital, Inje University College of Medicine, Busan; 2 Department of Internal Medicine, Hanyang University College of Medicine, Seoul, Korea 증 례 감염, 약물 등 이차적 원인에 대한 병력을 자세히 청취하고 기본 검사를 시행한다. 50세 남자가 건강 검진에서 발견된 말초혈액 호산구증가 와 가슴 X선 이상으로 방문하였다. 비흡연자로 3주 전부터 시작된 마른 기침 이외에는 발열, 호흡곤란, 객담, 체중감소 등은 호소하지 않았다. 혈액 검사에서 백혈구 7,00/μL 중, 호산구 3.% (절대 호산구 수 98/μL), 혈색소 4.8 g/dl, 혈 소판 244,000/μL였다. 가슴 X선에서 우측 상엽에 경계가 명 확하지 않은 둥근 음영증가 소견이 관찰되었다(Fig. ). 신체 진찰에서 호흡음은 정상이었다. 주요 소견 - 경증(500-,500 cells/μl)의 호산구증가증 - 마른 기침 - 폐 음영증가 진단의 일차 단계 경증 호산구증가증의 흔한 원인인 알레르기 질환, 기생충 Figure. Diagnostic approach to peripheral eosinophilia - 일차단계이후임상경과삼년전부터고혈압으로복용하는암로디핀 (amlodipine) 일일 5 mg 이외에한약, 건강보조식품등을포함한투약력은부인하였다. 최근여행한적도없다고하였다. 20대부터환절기에비염증상이있었으나, 천식, 음식물, 약물알레르기의과거력은없었다. 간헐적으로육회, 간 / 천엽등생식은한다고하였다. 말초혈액펴바른검사에서백혈구 8,690/μL 중호산구 29% ( 절대호산구수 2,520/μL), 혈색소 4.6 g/dl, 혈소판 25,000/μL였으며혈구세포의형태학적이상은동반되지않았다. 대변기생충검사와기생충특이항체검사 ( 스파르가눔 [sparganum], 폐흡충 [Paragonimus], 간흡충 [Clonorchis]) 에대한 IgG 항체검사는음성이었으나, 개회충 (toxocariasis) 특이 IgG는양성이었다. 총 IgE,073 IU/mL ( 참고치 < 00) 였고, 나무, 잡초, 집먼지진드기에대한특이 IgE가양성이었다. 가슴컴퓨터단층촬영에서는우측상엽에경계가불규칙하고주변에젖빛유리모양음영으로둘러싸인약 3 cm 크기의종괴와약.3 cm 크기의결절이발견되었다 (Fig. 2A). 간기능검사, troponin I, 비타민 B2, 심전도, 폐기능검사, 복부초음파검사는정상이었다. 진단의이차단계혈액검사에서는절대호산구수,500 μl 이상으로증가하여중증도호산구증가증을확인하였다. 폐병변에대하여는호산구증가증과는별개의국소폐렴또는폐암등을고려하여조직검사를시행하는것이원칙이나생식병력과개회충양성소견, 총 IgE의증가등의혈액검사와영상의학적소견을근거로호산구의폐침윤을염두에두고조직검사없이단기추적관찰하기로하였다. 생식력을고려하여프라지콴텔과알벤다졸치료를시행하였다. 최종진단및치료경과한달뒤시행한혈액검사에서백혈구 6,240/μL 중호산구 2.2% ( 절대호산구수 37/μL) 로정상화되었으며, 가슴컴퓨터단층촬영에서폐종괴와결절은모두소실되었다 (Fig. 2B). 상기소견과경과를종합하여개회충감염에의한폐침범이동반된과호산구증가증후군으로최종진단할수있었다 (Table ). 환자에게생식을피하도록교육하였다. 4 - 박찬선외 인. 말초혈액호산구증가증의진단적접근법 - 고찰진료실에서장기침범동반유무와상관없이말초혈액검사에서호산구증가가발견된환자를흔히접하게된다. 호산구증가증은절대호산구수에따라경증 (600-,500 cells/μl), 중등증 (,500-5,000 cells/μl), 중증 (> 5,000 cells/μl) 으로분류할수있다 []. 중등증이상의호산구증가증은호산구침윤에의한조직손상과장기부전이발생할수있어적극적인평가와추적관찰이필요하다. 말초혈액호산구증가증의진단적접근에대한국내가이드라인은없으나, 본저자들은환자의증상과호산구증가증의중증도에따라단계적으로진행할것을추천한다 (Fig. 3). 호산구증가증의가장흔한원인은알레르기질환, 기생충감염, 약물이기때문에이에대한자세한병력청취가진단에가장중요한단계이다. 이밖에독소, 내분비, 자가면역질환, 악성종양에서도호산구증가증이동반될수있으므로기저질환과동반증상등을확인한다 [2]. 기본검사로절대호산구수를포함한일반혈액검사와기생충대변검사및혈청검사를시행하며개회충 ELISA 검사를반드시포함하도록한다. 이밖에총 IgE, 간기능검사, 신기능검사와가슴흉부 X선촬영을시행한다. 만약, 중등증이상의호산구증가증이지속되거나, 장기또는조직에호산구침윤이발견된다면과호산구증가증을고려해야한다. 최근발표된호산구관련질환의정의와분류에따르면호산구수,500/μL 이상의호산구증가증이 4주간격으로 2회이상확인된경우를과호산구증가증 (hypereosinophilia, HE) 으로, 과호산구증가증이있으면서장기손상과기능저하가동반된경우에는과호산구증가증후군 (hypereosinophilic syndrome, HES) 으로정의한다 [3]. 전통적인과호산구증가증후군의기준은 Chusid 등 [4] 에의한 6개월이상지속되는,500/μL의중등도과호산구증가증과장기침범이있는경우이나최근에는 6개월이지속되지않더라도장기손상의증거가있으면과호산구증가증후군으로진단이가능하다 [5]. 따라서과호산구증가증에해당한다면주요장기침범유무를평가하기위해복부초음파 ( 또는컴퓨터단층촬영 ), troponin I, 심전도, 심초음파, 폐기능등을포함하고장기침윤이의심되는경우조직검사를시행하도록한다. 한편이차성원인이배제된과호산구증가증의경우에는말초혈액펴바름검사, 비타민 B2, 트립타아제 (tryptase), 인터루킨 (interleukin)-5, fip like -platelet driven growth factor receptor alpha (FIPL-PDGFRA) 검사를시행한다. 빈혈, 혈소판감소증, 비만세포증, 단핵구증이동반되거나, 미성숙과립백혈구증과백혈구모세포가발견되면초기에골수검사와유전형검사를고려한다. 이차적원인에의한과호산구증가증또는과호산구증가증후군의치료는기저질환을치료하는것이원칙이다. 만약, 심장이나신경계증상이나중증호산구증가증이이는경우에는응급치료를고려하여전문가에게의뢰하는것이바람직하다. 국내호산구증가증의가장흔한원인은개회충증으로보고자와지역에따른차이가있지만, 호산구증가증환자의 6-84% 에서혈청개회충항체양성이확인되었다 [6,7]. 대부분은무증상이지만, 유충이행증 (larva migrans) 으로인한장기침범이가능하며특히, 폐와간이가장흔한장기로알려져있다. 개회충증에의한폐침범은다양하게나타날수있지만, 본증례와같이경계가불분명한단일또는다발성결절또는젖빛유리모양음영이일시적으로나타났다가소실되거나이동하는특징을가지고있다 [8,9]. 개회충에의한과호산구증가증후군의치료에대해서는여전히논란이있으나, 최근의연구에서스테로이드병합치료와상관없이알벤다졸이호산구증가증과간침윤의호전에의미있는효과가있는것으로나타났다 [6,0,]. 따라서, 중등증이상의호산구증가증, 총 IgE 증가, 장기침범이있는경우에는알벤다졸치료가추천된다. 말초혈액호산구증가는혈액검사에서발견되는흔한이상소견중하나이며앞서서술한대로다양한원인이있다. 정확한진단을위해서는자세한병력청취가가장중요하며, 환자의증상과호산구증가증의중증도에따라단계적인감별진단을시행하도록한다. 저자들은건강검진에서우연히발견된말초혈액호산구증가증과폐음영증가를주소로내원한환자에서단계적인진단절차를통해개회충증에의한이차성과호산구증가증후군으로진단할수있었다. 중심단어 : 과호산구증가증 ; 개회충증 ; 호산구증가증 REFERENCES.
