Methods : Sera from 3,896 persons who had health checkups between December 2005 and November 2006 were included in the evaluation of positive rates and biological false positives (BFP) for Mediace RPR and TPLA. In addition, 134 patients sera positive for automated Mediace RPR or TPLA were tested for VDRL and TPHA. Discrepancies between TPLA and TPHA results were confirmed by the RecomBlot Treponemal IgG/IgM (Mikrogen GmbH, Germany). Automated Mediace RPR and TPLA were performed using the Hitachi 7600 chemistry autoanalyzer (Hitachi, Japan). Samples with positive Mediace RPR and negative TPLA results were defined as BFP. Results : Positive rate of automated Mediace RPR was 0.23% (9/3,896). BFP of the Mediace RPR was 0.18%. Positive rate of automated TPLA was 1.62% (37/2,284). Among the 134 patients sera, 33 (24.6%) showed a discrepancy between conventional VDRL and automated Mediace RPR results: Among 31 Mediace RPR(+)/VDRL(-) sera, 13 were positive and 18 were negative for TPLA. The remaining 2 sera of discrepancy with Mediace RPR(-)/VDRL(+) were all positive for TPLA. There were seven sera that showed a discrepancy between automated TPLA and TPHA results: Two sera with Mediace RPR(+)/TPLA(-)/TPHA(+) showed negative recomblot Treponemal IgG/IgM results, and among five sera with TPLA(+)/TPHA(-), three demonstrated IgG or IgM by recomblot Treponemal IgG/IgM. Conclusions : The results of comparison data demonstrated that automated TPLA results had a high concordance with recomblot Treponemal IgG/IgM results. Moreover, there are additional advantages of automated methods such as quantitative detection, low infection risk, and no influence by human handling. 2 자동화매독검사인 Mediace RPR, TPLA 양성분석 325 률이 0.2% 로감소되었다고보고되고있지만, 질병관리본부의표본감시체계통계자료에따르면그이후매독환자는점차증가추세를보이고있다 [1]. 그러므로공혈자검사나건강검진, 침습적인시술전검사등으로시행되고있는매독검사는여전히중요한검사이다. 비트레포네마검사 (nontreponemal test) 와트레포네마검사 (treponemal test) 두가지검사법으로구분되는혈청학적방법은매독의선별검사뿐만아니라확진검사로가장유용하게이용되고있다 [2]. 매독진단을위한지침은다양해서효소면역법 (enzyme immunoassay) 을선별검사에이용하거나비트레포네마검사와트레포네마검사를동시에선별검사로적용하는경우도있지만 [3], 주로비트레포네마검사인 VDRL, card rapid plasma reagin (RPR) 등을선별검사에트레포네마검사인 FTA-ABS, Treponemal pallidum particle agglutination (TP-PA), T. pallidum hemagglutination assay (TPHA) 등을확진검사에이용하고있다 [4]. 이처럼매독진단을위한검사법이다양하고두방법의검사를진단에이용해야하는이유는, 한가지혈청학적검사만으로는과거감염과현증을구별할수없고, 비트레포네마검사가생물학적위양성과발병초기와말기매독에위음성을보이는문제가있고, 트레포네마검사도위음성과위양성의가능성이있기때문이다 [5]. 또한, 국내에서주로이용되는수기법인 VDRL, TPHA, FTA- ABS 등은검사시간이오래걸리고검사자간결과판독에차이를보이며감염의위험이있는단점이있어, 생물학적위양성의가능성을최소화하고민감하면서간단하게검사할수있는자동화기법이필요한실정이다. 최근라텍스응집비탁법을기본으로생화학자동분석기로검사할수있고정량가능한비트레포네마검사시약인 Mediace TM RPR (Sekisui Chemical Co., Ltd, Osaka, Japan; 이하 Mediace RPR) 과트레포네마검사시약인 Mediace TM TPLA (T. pallidum latex agglutination, Sekisui Chemical Co.; 이하 TPLA) 시약이개발수입되어국내에도입되었다. 이에저자들은 Mediace RPR과 TPLA 시약을국내에서매독진단을위해많이이용되는 VDRL, TPHA와비교하기위해건강검진환자를대상으로 Mediace RPR과 TPLA의양성률과위양성률, 생물학적위양성률을평가하였다. 또, 자동화검사양성으로판정된환자검체를기존의방법으로검사하고결과차이를보이는예들은확인검사를시행하여자동화검사양성결과를분석하였다. 재료및방법 진을위해 Mediace RPR을시행한 3,897예를연구대상으로 Mediace RPR의양성률과위양성률, 생물학적위양성률을산출하였고, TPLA를시행한 2,284예를대상으로 TPLA의양성률과위양성률을산출하였다. 같은기간에 Mediace RPR 또는 TPLA 검사가의뢰된환자검체중하나또는모두에서양성결과를보인 134예를연구대상으로다시전통적인방법의 VDRL와 TPHA 를시행하였다. 134예는남자 62예, 여자 72예였으며, 연령분포는 0-84세 ( 중앙값 49세 ) 였다. Mediace RPR과 VDRL 결과의차이를보인예들은 TPLA와 TPHA 검사를통해확인하였고, TPLA와 TPHA 결과의차이를보인예들은 recomblot Treponemal IgG/IgM 검사를시행하였다. 환자의병록을찾아서임상진단과의상관성을분석하였다. 2. 자동화매독검사법인 Mediace RPR과 TPLA 검사 Mediace RPR 시약과 TPLA 시약을이용하여 Hitachi 7600 automated chemistry analyzer (Hitachi, Tokyo, Japan) 로측정하였다. Mediace RPR의결과치가 1.0 R.U. 이상인경우와, TPLA의결과치가 10.0 T.U. 이상인경우양성으로판정하였다. Mediace RPR 양성인예는지질항원이첨가되어있지않고라텍스만으로구성된시약인 lipid antigen free latex 시약으로검사를시행하였다. 이검사에서양성이라면라텍스에대한비특이적항체의존재때문에결과양성을보인것이므로음성인경우만최종양성으로판정하였다. Mediace RPR lipid antigen free latex 시험전양성이었으나, 최종음성으로판정된경우는 위양성 으로정의하였다. Mediace RPR 최종양성예에서 TPLA 음성인경우 생물학적위양성 으로정의하였다. TPLA 양성인예는매독항원으로구성된 TPLA 중화시약으로검사를시행한후다음의식으로계산하여 0.5 이상인경우최종양성으로판정하였다 : (TPLA 결과 -TPLA 중화시험결과 )/TPLA 결과. 중화시험결과최종음성으로판정된경우는 위양성 으로정의하였다. 3. VDRL, TPHA 검사 Mediace RPR 또는 TPLA 검사에서양성을보인 134예를대상으로기존의검사방법인 VDRL (BD Diagnostics, Franklin Lakes, NJ, USA) 과 TPHA (Asanpharm, Seoul, Korea) 검사를제조사의설명서에따라시행하였다. VDRL은희석하지않은검체에서양성을보이는경우양성으로판정하였다. TPHA는 1:80 희석에서육안관찰하여적혈구의응집이있는경우양성으로판정하였고, 1:40이나 1:20에서만양성인경우는약양성으로판정하였다. 1. 대상 4. 매독항체검출을위한 IgG/IgM immunoblot 검사 2005 년 12 월부터 2006 년 11 월까지동국대일산병원에서건강검 TPLA 와 TPHA 결과의차이를보인예들을대상으로 Re-

3 326 허희진 이교관 김의석외 1 인 comblot Treponemal IgG/IgM (Mikrogen GmbH, Martinseried, Germany) 검사를제조사의설명서에따라진행하였다. RecomBlot Treponemal IgG/IgM는 T. pallidum 항원인 Tp 47, TmpA, TP 37, TP 17, Tp 15에대한 IgG 또는 IgM 항체를검출하는 western blotting 법이다. 판정은각항원별항체유무를점수로환산하여 IgG인경우 2점이하는음성, 3-4점은경계양성 (borderline positive), 5점이상은양성으로판정하였고, IgM의경우 2점이하는음성, 3점은경계양성, 4점이상은양성으로판정하였다. 5. 통계처리 Mediace RPR과 VDRL의결과와 TPLA와 TPHA 결과는 Fisher s exact test로비교하였고, Mediace RPR과 TPLA의결과치는 Mann-Whitney Test로비교하였다. P<0.05를통계학적으로유의한차이가있는것으로간주하였다. 통계는 SPSS 11.5 (SPSS, Chicago, USA) 를이용하였다. 결 1. Mediace RPR, TPLA 의양성률, 위양성률및생물학적위양성률 Mediace RPR 검사를시행한 3,896예에서양성결과를보인 28예중 lipid antigen free latex 검사를통해 19예가위양성으로판정되어최종 Mediace RPR 양성률은 0.23% (9/3,896) 이 과 었다. Mediace RPR 최종양성예에서 TPLA가음성인예는 7예로판정되어생물학적위양성률은 0.18% (7/3,896) 였다. Mediace RPR과 TPLA 검사동시양성률은 0.05% (2/3897) 였다. TPLA의최종양성률은 1.62% (37/2284) 였다 (Table 1). 건강검진으로 Mediace RPR 검사를시행한예의 lipid antigen free latex 검사전양성중 67.9% (19/28) 는위양성이고 TPLA의중화시험전양성중 9.8% (4/41) 는위양성이었다. Mediace RPR 또는 TPLA 양성인 134예중 Mediace RPR lipid antigen free latex 검사전양성은 88예였고이중 40예가위양성으로판정되어 40.9% 는위양성이었다. Mediace RPR 최종양성으로판정된 48예중 19예는 TPLA 음성으로 39.6% 는생물학적위양성이었다 (Table 2). 2. Mediace RPR과 VDRL 결과비교 Mediace RPR 양성예의중앙값은 1.5 R.U. ( ) 였고, 위양성으로판정된 40예의 lipid antigen free latex 검사전중앙값은 1.3 R.U. (1-9.6) 였다 (P>0.05). 위양성으로판정된 40예는모두 VDRL 음성이었다 (Table 3). Mediace RPR 양성이고 TPLA 양성인 29예의중앙값은 1.8 R.U. ( ) 이고, 생물학적위양성을보인 19예의중앙값은 1.3 R.U. (1-2.3) 였다 (P=0.03). 24.6% (33/134) 는 Mediace RPR과 VDRL 결과가일치하지않았다 (P<0.05). Mediace RPR 음성이고 VDRL 양성인 2예는모두 TPLA와 TPHA 양성이었다. Mediace RPR 양성이고 VDRL 음성인 31예중 13예는 TPLA 양성이고나머지는음성이었고, 14예는 TPHA 양성이고나머지는음성이었다. Table 1. TPLA 와 TPHA 결과비교 TPLA 양성예들의중앙값은 73.5 T.U. ( ) 였고, TPLA 중화시험결과위양성으로판정된 8예의중화시험전중앙값은 55.5 T.U. (15-252) 였다 (P>0.05). 위양성으로판정된 8 예는모두 TPHA 음성이었다 (Table 4). TPLA 양성 73예중 5예는매독으로진단된예였고, 8예는치료된매독이었고, 18예는매독이의심되는예였고, 43예는기타진단이었다. 5.2% (7/134) 에서 TPLA와 TPHA 결과가일치하지않았다 (P<0.05). 4. TPLA 와 TPHA 결과에차이를보인예의 IgG/IgM immunoblot 결과 TPLA와 TPHA 검사결과의차이를보인 7예중매독의임상적증거와과거력이있는예는없었다. Mediace RPR 양성, TPLA 음성이고 TPHA 양성인 2예는 recomblot Treponemal IgG/IgM 음성이었다 (Table 5). TPLA 양성이고 TPHA 음성인 5예에서 2예는 recomblot Treponemal IgG 결과양성이었고, 한예는 recomblot Treponemal IgM 양성이었다. 고 찰 국내매독양성률은연구대상에따라다른결과를보이고있는 데, 1997년에서 1998년사이헌혈자의매독항체양성률은 0.08% 이고, 2000년에헌혈자와임산부, 건강검진환자를대상으로한매독검사양성률은 0.2% 로보고되고있다 [1, 6]. VDRL, card RPR 등의시약에이용되는항원의정제기술이발전하였지만, 비트레포네마검사는여러만성감염과자가면역질환등에서 5-30% 의생물학적위양성을보인다 [7]. Mediace RPR 검사의생물학적위양성률은보고된위양성률보다높은 39.6% (19/48) 였다. 이번연구에서매독의유병률이낮은건강검진과수술전매독검사에서양성을보인예가주로연구대상에포함되었고, 이처럼 유병률이낮은대상군에서는양성예측률이낮기때문에생물학적위양성률이높게산출된것으로생각된다 [9]. 건강검진환자전체를대상으로한생물학적위양성률은 0.18% (7/3897) 로이는 VDRL의생물학적위양성률인 0.24% 와크게차이가없었다 [8]. 건강검진환자에서의 Mediace RPR 위양성률은최초양성의 67.9% 인데반해전체양성환자에서의위양성률은 40.9% 였다. 건강검진환자의 Mediace RPR과 TPLA 동시양성률이 0.05% 로낮아, 유병률이낮은대상군에서는양성예측률이낮아지는것과같은의미로해석된다 [9]. 그러므로, 특히유병률이낮은군을대상으로 Mediace RPR 검사로선별검사를시행할때에는라텍스에반응하는비특이적항체에의한검사양성을감소시키기위해 lipid antigen free latex 시약을이용한검사가필요하다. 비트레포네마검사양성환자의확진검사로이용되는트레포네마검사인 TPLA 위양성률도건강검진환자양성중 9.8% 이므로 TPLA 중화시험도필요한것으로생각된다. VDRL 검사의일차매독민감도는 89-87%, 말기매독에서는 37-94% 이고특이도는 % 로보고되고있다 [10, 11]. 본연구에서 Mediace RPR 검사의민감도와특이도를산출하지는못했지만, TPLA를기준으로두검사를비교했을때 Mediace RPR 양성이고 VDRL 음성인 31예중 13예는 TPLA 양성이었고 18예는음성이었다. 연구대상을선택한기준이 Mediace RPR 양성검체였기때문에 Mediace RPR 양성이고 VDRL 음성인예가많은것으로추측된다. 결과에제시하지는않았지만, VDRL 양성인검체를대상으로 Mediace RPR 검사를시행했을때 VDRL 양성인 21예중 15예가 Mediace RPR 음성이었으므로선별검사로이용되는비트레포네마검사인두검사중어떤검사가우수하다고결론내기는어렵고, 양성결과를보였을때트레포네마검사로확진검사를시행하는것이필요하다. FTA-ABS 검사가확진검사의 gold standard로여겨지지만, 위양성의위험이있고 IgG 검사의경우초기매독에서 20% 의위음성가능성이보고되고있어, 또다른확인검사로 western blot법이제시되고있다 [12-15]. 본연구에서는 TPLA와 TPHA 결과의차이를보이는 7예의확인을위해 recomblot Treponemal IgG/IgM를이용하였다. 5 328 허희진 이교관 김의석외 1 인 법의시약은 T. pallidum의항원들중항원성이강해매독진단에있어중요한역할을하는 47, 17, 15 kda의분자량을갖는지단백인 TP 47, TP 17, TP 15와 [10], TmpA, Tp 37 항원들로구성되어있다. Western blot 법의민감도는 93.8%, 특이도는 100% 로보고되고있다 [10, 14]. TPLA 양성이면서 TPHA 음성인 5예중한예는 Tp 47 IgG 항체양성이었고, 한예는 IgM 항체양성이었다. Tp 47에대한항체는초기매독에서양성을보이고치료후음성으로전환될가능성이높은항체로, TPLA 검사는주로 Tp 47을주항원으로이용하고있으므로 TPLA 양성, TPHA 음성의결과를보인 2예의설명이가능하다 [16]. TPLA 와 TPHA 결과차이를보인 7예모두매독의임상적증거와과거력은없었기때문에임상진단을기준으로판단하기는어려웠지만, recomblot Treponemal IgG/IgM 검사결과와비교해보았을때 (Table 5), 5예에서 TPLA와 recomblot Treponemal IgG/ IgM는일치하는결과를보였으므로 TPLA는 TPHA에비해확진검사로써의우수성을갖는것으로생각된다. Mediace RPR, TPLA 검사의결과값들을이용하여위양성과생물학적위양성을구분할수있을지확인하였다. Mediace RPR 과 TPLA의양성군과위양성군의값차이는없었다. Mediace RPR 양성이고 TPLA 양성인예의중앙값은 1.8 R.U. (1-1,177.6), 생물학적위양성을보인예의중앙값은 1.3 R.U. (1-2.3) 로생물학적위양성일때의미있게낮은값을보였다 (P=0.03). 그러므로위양성이배제된다면 Mediace RPR 결과치가높은환자는 TPLA 양성매독의가능성이높음을시사한다. 결론적으로, Mediace RPR의생물학적위양성률은보고된 VDRL의생물학적위양성률과큰차이가없었지만, Mediace RPR은라텍스에대한비특이결합에의해발생하는위양성의가능성이있으므로 Mediace RPR 양성결과를보이는검체에대해서는 lipid antigen free latex 시약을이용해위양성유무를확인하고, TPLA 양성결과를보이는검체에대해서는 TPLA 중화시험을시행해야할것으로생각된다. TPLA와 TPHA 결과차이를보이는예들의확인검사에서새로운자동화검사법이 TP- HA보다 recomblot Treponemal IgG/IgM과일치도가높았다. 또한, Mediace RPR과 TPLA는선별검사목적의대량검사수행이가능하고, 정량적측정이가능하고, 감염의위험이적고, 검사자간결과판독의차이를보이지않는장점을가지고있는유용한검사로생각된다. 요약배경 : 최근자동화장비로검사가능한 Mediace Rapid plasma reagin (RPR), Treponema pallidum latex agglutination (TPLA) (Sekisui, Japan) 법이소개되었다.
