37 결론 After two treatment steps, it appears that over 50% of patients will achieve remission if they stay in treatment (i.e., 36.8% step 1 plus 30.6% of the remaining 63.2% of patients). Thereafter, the chances of subsequent remission are much lower. The theoretical cumulative remission rate after four acute treatment steps was 67%. 두번의치료시기후약 50% 의환자들이 remission 에도달한다 ( 첫번째치료단계에서 36.8%, 두번째치료단계에서 30.6%). 그후 remission 율은낮아진다. 4 번의치료시기후총 remission 율은 67% 이다. *33% 가약물난치성우울증으로나타났다.

38 약물에대한반응 Response to pharmacotherapy in depressive disorders has been divided into three phases: (1) Response, manifested by an improvement of 50% or more in the severity of the depressive symptomatology; this is typically observed in the first 8 weeks of pharmacotherapy. 50% 이상의우울증개선이대개첫 8 주에보인다 Partial response: 25-50% 증상감소, No response: <25% (2) Remission, reflected in a complete symptom remission. Approximately 50% of patients are expected to reach remission within the first 6 months and about two thirds within 2 years of the start of pharmacotherapy. 완전관해 : 100% 증상소멸, 부분적관해 : 잔여증상이있는경우우울증상의완전소멸 (remission) 은 50% 환자는 6 개월이내에보이고, 2/3 는 2 년이내에보인다. (3) Recovery, defined as the state when remission has lasted for a period of 6 12 months. Symptom relapse is considered to occur when symptoms recur before a full remission has been achieved or before a state of remission has turned into a recovery. 우울증상의 remission 이 6-12 개월지속되면회복 (recovery) 로볼수있다.

40 Comparative efficacy and acceptability of 12 new-generation antidepressants: a multiple-treatments meta-analysis. BACKGROUND: Conventional meta-analyses have shown inconsistent res ults for efficacy of second-generation antidepressants. We therefore did a multiple-treatments meta-analysis, which accounts for both direct and indi rect comparisons, to assess the effects of 12 new-generation antidepressant s on major depression. METHODS: We systematically reviewed 117 randomised controlled trials ( participants) from 1991 up to Nov 30, 2007, which compared any of the following antidepressants at therapeutic dose range for the acute tre atment of unipolar major depression in adults: bupropion, citalopram, dul oxetine, escitalopram, fluoxetine, fluvoxamine, milnacipran, mirtazapine, paroxetine, reboxetine, sertraline, and venlafaxine. 43 가능한위험한 Venlafaxine 사용을피할것. A cohort study in about 50 elderly patients and analysis of several hundred reported suicide attempts by venlafaxine overdose demonstrated a risk of QT interval prolongation, which can lead to torsades de pointes, an unusual and potentially fatal type of ventricular tachycardia. In practice, the data available as of mid-2015 from clinical trials and epidemiological studies confirm the harms foreseeable from venlafaxine's pharmacological properties: a higher risk of cardiovascular adverse effects and of fatal overdoses than with most SSRI antidepressants. Since venlafaxine and SSRI antidepressants have similar and limited efficacy, venlafaxine is best avoided. An SSRI anti-depressant is a more reasonable option, with the exception of citalopram and escitalopram which also expose patients to more cardiovascular adverse effects.
