M encephalopathy, 이하 TSE)은 프리온 단백질에 의해 발생하 며, 사람을 포함한 여러 포유 동물의 중추 신경계에 나타나는 일단의 신경 변성 질환이다(1). 사람에게서 나타나는 TSE는 CJD, Gerstmann-Straussler-Scheinker syndrome, fatal familial insomnia, 쿠루(kuru) 등이 포함되며, 비교적 오랜 역 사를 지닌 산양의 스크래피(scrapie)에 비해, 소 해면양 뇌증 (bovine spongiform encephalopathy), 변성(variant) CJD 등 은 최근에 주목되어지는 TSE 이다(1). CJD는 역학적으로 원인 규명이 불가능한 산발성 CJD(8590%), 유전학적으로 규명이 가능한 가족형 CJD(familial type, 8-13%), 의인성 CJD(iatrogenic type)로 분류된다(1). 산발성 CJD는 세계적으로 한 해에 백만 명 중 한 명 정도 의 발생율을 보이며, 발병시의 평균 연령은 60±9세 이며, 발 병 후 1-2년 이내에 사망하는 치명적인 질환이다. 프리온 단 백질 염색체 검사, PrPres 전기 영동 검사, western-blot 검 사등에 의해 분류되어, 진행성 치매, 근간대성 발작, 특징적인 뇌파 소견을 보이는 전형적 산발성 CJD(classic type, 70%) 와 나머지 30%의 아형으로 분류할 수 있다(1). 산발성 CJD 환자들은 대부분 서서히 시작하여 수 주 내지 수개월에 걸쳐 점차 진행하는 신경학적 증상들을 보인다. 임 상적으로 2년 이내의 이환기간을 가지는 진행성 치매와 함께 근간대경련, 시각 장애 혹은 소뇌 증상, 추체 또는 추체외로 증상, 무동성 무언증을 보이면 가정 진단이 가능하며, 이상의 임상증상과 함께 뇌파 검사상 전형적인 간헐성 극파를 보이 거나 뇌척수액 검사상 단백질이 양성일 때는 추정 진단에 속한다(1, 2). 확진(definite CJD)은 뇌 생검이나 부 검(90%의 확진)에서 해면양 뇌병증, 신경세포의 소실, 성상 교세포증을 확인 하거나 말초 혈액의 백혈구에서 면역 세포 화학 검사 혹은 Western blot에서 변종 프리온 단백질을 발 견(CJD의 10%)하는 것이다. 다른 병리 소견으로는 뇌실질의 공포화 및 매우 드물게 아밀로이드 플라크(amyloid plaque) 를 발견하는 경우도 포함된다(1-3). 일반적으로 산발성 CJD 환자에서 67-79%가 MRI(T2 강 조영상, 양자밀도영상, FLAIR, 확산 강조영상)에서 미상핵과 피각에 대칭적인 고신호 강도가 나타난다. 뇌의 다른 부위의 회색질에도 고신호강도를 보이는데 해마, 중뇌, 수도관 주위 (periaqueductal) 회색질, 시상등의 회색질에 고신호 강도를 보일 수 있다. It is a rare and fatal progressive neurodegenerative disease that affects the middle and old aged. There are a number of subtypes of CJD, one of which is the sporadic type characterized by rapidly progressing clinical symptoms, including progressive dementia, myoclonic jerk, and pyramidal or extrapyramidal syndrome. Patients usually end up dying within 1 to 2 years of contacting the disease. We report an autopsy-proven case of sporadic CJD with clinical symptoms that progressed within several days, along with dramatic changes on diffusion weighted magnetic resonance images.
