Materials and Methods: From February 2011 to September 2011, 31 patients with Peyronie s disease received potassium aminobezoate (500 mg Peyron capsules) and were divided into two groups. Group 1 (N=10) received potassium aminobezoate (500 mg Peyron capsule) 3 g four times daily without previous treatment of Peyronie s disease, while group 2 (N=21) received the same drug with previous treatment of Peyronie s disease (10 mg Tamoxifen +300 mg L-carnitine two times daily). Outcomes were assessed by subjective symptom change, pain relief, resolutions of the plaque, and curvature. Results: After 3 months, there were no significant improvements in clinical outcomes of either group and among all the patients, 23 stopped taking potassium aminobezoate (23/31, 74%). The reasons for ceasing the therapy were gastrointestinal trouble (8/23, 35%), too many doses to take (7/23, 30%), ineffectiveness (6/23, 26%), and high price (2/23, 9%). Conclusions: Athough the etiology of Peyronie s disease has not been elucidated, potassium aminobenzoate in therapy of Peyronie s disease has been used. The use of this medication has the limitations of gastrointestinal trouble, ineffectiveness, too many doses, and high price. Further evaluations of the effect and appropriate dosing of potassium aminobenzoate are needed. 2 214 대한남성과학회지 : 제 29 권제 3 호 2011 서 페이로니병은음경백막에발생하는섬유성결절 (fibrotic nodule) 로인해발기시음경의만곡과통증등을유발하는질환이다. 1 페이로니병은주로중년남성에서많이발생하며, 유병률은 % 정도이며연령이증가함에따라빈도가증가하는것으로보고되고있다. 2,3 이러한높은유병율에도불구하고페이로니병의정확한원인은불분명한상태로음경외상, 성관계시미세손상으로인한백막내섬유소의과다침착, 혈관손상에의한자가면역, 비타민 E 결핍, 당뇨병, 파제트병 (Paget s disease), 뒤퓌트랑구축 (Dupuytren s contracture) 등이원인으로생각된다. 4,5 페이로니병은 1561년처음보고된이후병의경과와진행을뚜렷이호전시키는내과적치료법은없는질환으로경구약물치료제로는 Potassium aminobenzoate (Potaba), 비타민 E, tamoxifen, colchicines, Acetyl-L-carnitine, Omega-3 등이사용되고있다 이러한다양한경구약물치료제중에서 Potassium aminobenzoate (Potaba) 는 1959년에 Zarafonetis와 Horrax 11 에의해페이로니병에대한치료효과가최초로보고되었으며, 조직내산소포화도를높이고, Glycosaminoglycans과 monoamine oxidase의활동을증가시켜항섬유화작용을하는것으로알려져있다. 11,12 Potaba는페이로니병의초기에증상을안정화시키고, 음경만곡의진행을억제하는데효과적인것으로보고되며, 이약물을사용할수있는여건이되는국가에서는페이로니병의진행을억제하기위해사용할것을권장하고있다. 13 최근국내에서페이로니병의경구약물치료제로서 Potaba (Peyron capsule 500 mg) 이시판되었고이 론 에저자는이약물의사용경험을바탕으로 Potaba (Peyron capsule 500 mg) 의효과와개선점을파악하고자하였다. 대상및방법 2011 년 2 월부터 2011 년 9 월까지본원비뇨기과에내원하여페이로니병으로진단받은환자중 Potaba (Peyron capsule 500 mg) 처방받은 31 명을대상으로치료전, 후의효과와더불어치료약물의부작용, 순응도등을추적관찰하였다. 전체 31 명의환자중, 초진군 (Group 1) 은 10 명의환자로이전에페이로니병의치료없이 Potaba 를처방받은환자들이었고재진군 (Group 2) 은 21 명의환자로 Potaba 복용전에페이로니병의치료로 tamoxifen 과 L-carnitine 을복용했던환자들이었다. 전체 31 명의환자는 Peyron capsule 500 mg 을 1 회 6 capsule, 1 일 4 회복용하였고, 재진군의환자들은 Peyron capsule 복용전에 tamoxifen 10 mg 과 L-carnitine 330 mg 을 1 일 2 회복용하였던과거력이있었다. 전체대상환자 31 명의평균연령은 55.2±7.9 세였고, 초진군 (Group 1) 의평균연령은 59.3±6.8 세였으며재진군 (Group 2) 의평균연령은 53.2±7.8 세였다. Peyron capsule 평균복용기간은 79.5 일 ( 일 ) 이며, Peyron capsule 복용전페이로니병의치료를받았던 21 명의환자의평균복용기간은 일 ( 일 ) 이었다 (Table 1). 약물복용전, 문진을통해환자의임상증상, 과거력, 가족력등을파악하였고, 음경결절의크기, 발기시동통, 음경만곡등을조사하였다. 음경결절의크기는초음파를통해결절의최장길이를측정하였고, 발기시동통은통증수치표 (0 10 점 ) 를통해점수로기록하였다. 음경만곡의정도는발기 Table 1. Clinical characteristics of patients after the treatment Group 1 Group 2 p-value Decreased size of plaque (cm) Improved pain scale (score) Decreased curvature (degree) Stopping medication (number) 0.52± ± ±3.6 8/10 (80%) 0.64± ± ±5.6 16/21 (76%) 유발후사진촬영을한후, 각도기를이용해만곡부위와만곡각도를측정하였다. 전체환자의음경결절의크기는 1.9±0.6 cm 였고, 초진군 (Group 1) 의결절크기는 1.5±0.6 cm, 재진군 (Group 2) 의결절크기는 2.1±0.5 cm 였다 (p=0.099). 전체 31 명의환자중발기시동통을호소하는환자는 15 명 (15/31, 48%) 이었고, 초진군 5 명 (5/10, 50%) 과재진군 10 명 (10/21, 48%) 의환자가발기시동통을호소하였으며통증수치표값은각각 2.54±1.03 점, 2.74±1.12 점이었다 (p=0.905). 그리고전체환자중발기시음경만곡이관찰된환자는 21 명 (21/31, 68%) 이었고, 초진군 7 명 (7/10, 70%) 과재진군 14 명 (14/21, 67%) 의환자의음경만곡의정도는각각 21.4±11.0 도, 20.4±6.6 도였다 (p=0.819, Table 1). 치료효과는 Peyron capsule 복용 3 개월후치료전과동일하게문진과음경결절의크기, 발기시동통, 음경만곡등을재측정하였고, 더불어치료약물의부작용등을조사하여치료전결과와비교하였다. 두군간의차이는독립표본 t- 검정을실시하여 p 값이 0.05 미만인경우를유의하다고판정하였고, 통계프로그램은윈도우용한글 SPSS 12.0 을이용하였다. 결 과 Fig. 1. (Table 2). 이에대한원인으로는위장관장애 (8/23, 35%), 너무많은약물복용량 (7/23, 30%), 효과없음 (6/23, 26%), 비싼가격 (2/23, 9%) 으로파악되었다 (Fig. 1). 또한전체 31 명의환자중치료를더이상원하지않은 2 명의환자를제외하고두군간의치료후의음경결절의크기감소, 발기시동통의호전, 음경만곡의호전을비교분석한결과두군간의유의한차이를보이지않았다 (Table 2). 3 개월간의약물치료후시행한추적관찰에서전체 31 명의환자중, 23 명의환자가 Peyron capsule 복용을중단하였고 (23/31, 74%), 8 명의환자가 Peyron capsule 복용을유지하였다 (8/31, 26%). 초진군 (Group 1) 10 명의환자중, 8 명의환자가 Peyron capsule 복용을중단하였고 (8/10, 80%), 이중 6 명의환자가 Tamoxifen 과 L-carnitine 치료로전환하였고, 2 명의환자는더이상의치료를원하지않았다. 재진군 (Group 2) 21 명의환자중, 16 명의환자가 Peyron capsule 복용을중단하고기존의 Tamoxifen 과 L-carnitine 치료로전환하였다 (16/21, 76%) 고 페이로니병은정확한병인이규명되지않았으나, 음경백막의손상후섬유화, 자가면역질환또는유전적요인으로인하여백막내에과도한섬유조직이침착되어발기시동통과음경만곡등을유발한다고알려졌다. 4 주로 50 대중반의연령층에서발병하나, 연령별유병률은 30 대 1.5%, 40 대와 50 대 3.0%, 60 대 4.0%, 70 대이상이 6.5% 정도로연령에따라증가하는양상을보인다. 14 페이로니병의치료는크게비수술적치료법과 찰

4 216 대한남성과학회지 : 제 29 권제 3 호 2011 수술적치료법이시행되고있으나, 환자나의사모두에게만족할만한근치적치료법은없는실정이다. 페이로니병의경구약물치료로서비타민 E (Tocopherol), Potaba, colchicines, tamoxifen, acetyl-l-carnitine, Omega-3 등이사용되고있으나, 이러한다양한약물치료에도불구하고효과적이고명확한기전이확인된약물은현재까지미비한실정이다. 15 이러한경구약물치료제중, Potaba는 1959년 FDA (Food and Drug Administration) 에의해페이로니병, 피부경화증, 피부근염에효과가있다고보고된이후, 페이로니병의치료에사용되고있다. Potaba는조직내의산소이용도를증가시키고 monoamine oxidase의활동성을증가시켜이로인해섬유화를유발한다고알려진물질인세로토닌 (Serotonin) 의농도를감소시킨다고알려졌다 년 Carson 16 은 32명의페이로니병환자를대상으로최소 3개월간하루 12 g의 Potaba 복용후, 44% 의환자에게서주관적증상의호전, 56% 의환자에게서결절크기의감소, 58% 의환자에서음경만곡의호전을보였다고보고하였다. 또한 2005년 Weidner 등 13 은 75명의페이로니병환자를대상으로 Potaba 군과위약군으로나누어전향적, 맹검연구를시행하였는데, 이연구에서발기시동통과음경만곡은두군간의유의한차이가없었고, Potaba 군에서위약군보다결절의크기가감소했다고보고했으나, 이러한결절의크기감소가음경만곡의호전에영향을미치지않았다고보고하였다. 최근국내에도 Peyron capsule 500 mg로 Potaba가시판되어저자들도 2011년 2월부터 31명의페이로니병환자에게복용시킨결과, 앞서결과에서언급하였듯이하루에 24 capsule을복용해야하는너무많은양의복용량과이로인한위장관장애, 환자의주관적증상의호전이없는점, 상대적으로비싼약값등이페이로니병환자에게이약물의치료를유지하는데걸림돌이되었다. 또한, 페이로니병에서 Potaba 치료후발생할수있는급성간염이올수있기에시행한간수치검사에서는 31명의환자모두특이소견을발견할수없었다. 17 본연구는짧은기간과적은인원, 74% 에이르는실험군의탈락으로인한제한점이있지만국내에서페이로니병의치료의 Potaba에대한초기경험을보고한첫연구로향후 Potaba의효과와적절한약물용량을조사하는것이필요할것이다. 결 페이로니병은아직까지뚜렷한병인이밝혀지지않은질환으로서경구약물치료와더불어수술적치료에대해서도논란이많다. 페이로니병의 Potaba 의효과에대한기존의보고처럼위장관장애, 많은약물용량, 효과의불명확성, 비싼가격등은이약물의사용에제한점을보이고있다. 이에페이로니병의 Potaba 의효과와적절한약물용량에대한연구가필요할것으로생각된다. 론 REFERENCES 1) Larsen SM, Levine LA.
