Coronary artery aneurysm is an uncommon disease which is defined as coronary dilatation which exceeds the diameter of normal adjacent segments or the diameter of the patient s largest coronary vessel by 1.5 times. In young ages, symptoms are unusual, but significant symptoms and complications such as congestive heart failure, subacute bacterial endocarditis, coronary steal syndrome, aneurysm formation, rupture, and pulmonary hypertension may appear among the older age group. We report a case of giant aneurysm of a congenital coronary arteriovenous fistula between left coronary artery and right ventricular outflow tract with significant left to right shunt confirmed in a 84-year old female with a brief review of literature. 검사실 소견 내원 당시 혈액 검사상 혈색소는 10.3 g/dl, 헤마토크리트치 30.8%, 백혈구수 8000/mm3이 증 례 었고, 혈소판은 144 10 3/μL 였다. 적혈구 침강속도 는 17 mm/hr이었고 CEA는 4.3 ng/ml, IgE는 8.4 환 자 하 숙, 여자 84세.

2 저자들은 흉부 X선 검사상 우연히 발견된 심장주위 박은 분당 75회, 호흡은 분당 20회였으며 체온은 정상 종물에 대하여 심초음파, 흉부 전산화 단층촬영, 심도 이었다. 경정맥압은 상승되지 않았고 흉부에서는 정상 자술 그리고 대동맥 조영술 등을 시행한 결과 좌우 관 호흡음이었고 심박동은 규칙적이었고 III/VI의 지속성 동맥이 확장되고 우심실 유출로로 개구한 관동정맥루 심잡음이 좌측 전흉벽에서 들렸다. 복부 진찰상 특이소 가 관동맥류를 형성한 것으로 확인된 1례를 경험하여 견은 없었다. 문헌고찰과 함께 보고하는 바이다. 검사실 소견 내원 당시 혈액 검사상 혈색소는 10.3 g/dl, 헤마토크리트치 30.8%, 백혈구수 8000/mm3이 증 례 었고, 혈소판은 /μL 였다. 적혈구 침강속도 는 17 mm/hr이었고 CEA는 4.3 ng/ml, IgE는 8.4 환 자 하 숙, 여자 84세. IU/ml, 말초혈액 호산구수는 120/mm3로 모두 정상범 주 소 우연히 발견된 좌측흉부에 위치한 심장주위 위였다. 총 콜레스테롤치는 184 mg/dl, 중성지방이 95 종물. 현병력 평소 건강하게 지내던 환자로 내원 당일 미 mg/dl, HDL-콜레스테롤이 36 mg/dl, LDL-콜레스테 롤이 129 mg/dl였으며 간기능 검사는 정상이었다. 끄러지면서 생긴 좌측 대퇴골 경부 골절을 주소로 본원 흉부 방사선 소견상 좌측 심장 경계에 연해서 폐 중 정형외과에서 진료중 흉부 X선 검사상 좌측흉부에 5 간 부위에 말초 석회화를 동반한 5 5 cm정도의 뚜렷 5 cm 크기의 심장주위 종물이 발견되어 내과로 진료의 한 원형 종물이 관찰되었다(Fig. 1). 흉부 전산화 단층 뢰 되었다. 촬영에서는 cm정도의 종물이 심장의 좌 과거력 특이사항 없음. 측에 연해 있었다. 이 종물은 심장과 같은 정도로 조영 가족력 특이사항 없음. 되었고 말초의 선상 석회화를 동반하였으며 우심실과 이학적 소견 내원 당시 혈압은 160/90 mmhg, 맥 연결된 것처럼 보여 우심실의 동맥류를 의심하게 하였다 A Fig. 1. (Fig. 2). 심전도는 분당 82회의 정상 율동이었고 1도 방실차단의 소견을 보였고 비특이적인 T파의 변화를 보였다(Fig. 3).

Color Doppler echo에서는 작은 구멍을 통해서 낭종으로부터 우심실 로 지속성 난류가 관찰되었다(Fig. 4B). 심도자술 소견 상 우심실과 폐동맥압은 정상이었으며 우심실 유입로 에 비하여 유출로에서 20%의 산소함량의 증가를 보였 Fig.

3 (Fig. 2). 심전도는 분당 82회의 정상 율동이었고 1도 방실차단의 소견을 보였고 비특이적인 T파의 변화를 보였다(Fig. 3). 심초음파상에서는 좌심실의 크기는 정상이었고, 좌심 구혈율은 60%, 좌심실의 이완기 장애, 좌측 관동맥의 동맥류형 확장과 우심실의 유출로에 붙은 4 4 cm의 낭종성 종물의 소견을 보였다(Fig. 4A).
