제환준 외: 좌내유동맥-관상동맥 우회술 환자에서 동반된 좌측 쇄골하동맥 협착에 대한 중재적 치료 로가 있거나, 좌내유동맥을 이용한 관상동맥 우회술을 시행 받 을 예정인 환자 22명을 대상으로 하였다. 대상환자는 남자 15명, 여자 7명이었고 연령분포는 55-77 세, 평균연령은 69.3세였다.

천자부위는 총대퇴동맥을 주로 사용하였으나, 쇄골하동맥 의 완전 폐색이나 총대퇴동맥을 통한 협착 부위의 접근과 통 과가 어려울 경우 필요에 따라 상완동맥이나 액와동맥을 천자 하였다.

5 F 돼지꼬리(pigtail) 카테터를 삽입하고 디지털감산 혈관 촬영기를 이용하여 대동맥궁 혈관조영상을 얻어 쇄골하동맥의 협착 부위와 정도를 확인하였다. 혈관의 협착이나 폐쇄 부위 통과는 적절한 모양의 5 F 카 테터와 0.035 inch 유도철선을(Terumo, Tokyo, Japan) 이용 하였다.

2 제환준 외: 좌내유동맥-관상동맥 우회술 환자에서 동반된 좌측 쇄골하동맥 협착에 대한 중재적 치료 로가 있거나, 좌내유동맥을 이용한 관상동맥 우회술을 시행 받 을 예정인 환자 22명을 대상으로 하였다. 대상환자는 남자 15명, 여자 7명이었고 연령분포는 세, 평균연령은 69.3세였다. 시술 방법 쇄골하동맥 협착에 대한 중재적 시술은 방사선과 내의 혈관 조영실에서 국소 마취 하에 시행되었으며, 디지털감산 혈관촬 영기(ngiostar; Siemens, Erlangen, Germany)를 사용하였 다. 천자부위는 총대퇴동맥을 주로 사용하였으나, 쇄골하동맥 의 완전 폐색이나 총대퇴동맥을 통한 협착 부위의 접근과 통 과가 어려울 경우 필요에 따라 상완동맥이나 액와동맥을 천자 하였다. 모든 예에서 7 F 피포(sheath)를 사용하였으며, 총대 퇴동맥을 통해 접근 시에는 80 cm 길이의 피포(Shuttle introducer sheath; Cook, loomington, U.S..)를 사용하였다. 동맥 천자 후에는 3,000-5,000 단위의 헤파린을 투여하였다. 5 F 돼지꼬리(pigtail) 카테터를 삽입하고 디지털감산 혈관 촬영기를 이용하여 대동맥궁 혈관조영상을 얻어 쇄골하동맥의 협착 부위와 정도를 확인하였다. 혈관의 협착이나 폐쇄 부위 통과는 적절한 모양의 5 F 카 테터와 inch 유도철선을(Terumo, Tokyo, Japan) 이용 하였다. 유도철선이 병변 부위를 통과한 후 5 F 카테터를 유 도철선을 따라 통과시켜 병변의 전후 혈압차를 측정하였다. 병 변의 위치와 형태, 협착 정도를 고려하여 혈관성형술이나 스 텐트 삽입술을 시행하였으며, 협착 병변이 쇄골하동맥 기시부 위이거나, 편심성의 협착, 석회화 플라크가 동반된 경우에는 스 텐트 삽입술을 선택하였다. 혈관성형술은 병변 주위에 정상으로 보이는 혈관부위를 기 준 직경으로 하여 풍선과 혈관의 비율이 1 1 내지 1.1 1이 되게 풍선 카테터(Ultra-Thin; oston Scientific, Natick, U.S..)를 선택한 다음 inch Extra-stiff 유도철선(Cook, loomington, U.S..)을 통해 병변 부위를 통과시킨 후 팽창 기(Inflator; Guidant, Indianapolis, U.S..)를 이용하여 9-10 기압으로 확장하였다(Fig. 1). 스텐트 삽입시에는 병변의 길이, 혈관의 크기를 파악한 후 각 환자에서 적절한 크기와 길이의 스텐트를 선택하였다(Fig. 2). 원위부 색전이나 내막 박리 등의 합병증을 줄이기 위해 일 차적 스텐트 삽입술을 주로 사용하였으나, 완전 폐색 병변이 거나 협착 정도가 심해 스텐트가 협착 부위를 통과하기 어려 울 것으로 예상되는 경우에는 4 mm 직경의 풍선 카테터 (Ultra-Thin; oston Scientific, Natick, U.S..)를 이용하여 스텐트 삽입 전에 혈관 성형술을 먼저 시행하였다. 스텐트 삽 입 후에는 혈관 조영을 시행하여 잔류협착이 남아 있는 경우 추가로 풍선 확장을 하여 20% 미만의 협착을 얻으려고 하였 다. 평가방법 및 추적 관찰 쇄골하동맥 협착에 대한 중재적 치료의 기술적 성공의 기준 C Fig year-old woman presented with recurred angina due to subclavian artery stenosis. 결 과 대상환자 및 시술의 특징 대상환자 중 좌내유동맥을 이용한 관상동맥 우회술을 시행 받을 예정인 환자가 16명(관상동맥 우회술 전 그룹)이었으며, 이전에 관상동맥 우회술을 시행 받아 좌내유동맥과 연결된 관 상동맥 우회로가 있었던 환자는 6명이었다(관상동맥 우회술 후 그룹).

관상동맥 우회술 후 그룹에서 관상동맥 우회술과 좌측 쇄골 하동맥협착의 중재적 시술 사이의 기간은 평균 18개월이었으 나, 그 중 4명은 10일 미만이었으며 1명은 9개월, 1명은 103 개월이었다.

3 대한영상의학회지 2006;55: 은 혈관조영술상 잔존 협착이 20% 이내이거나 병소 전후의 수축기 압력차가 10 mmhg 이내인 경우로 하였다. 진료기록 을 통해 시술 전후의 합병증을 조사하였다. 추적 관찰은 외래진료를 통해 하였으며 필요할 경우 CT 혈 관조영술이나 추적 심도자 검사 등이 시행되었다. 추적 관찰 기간 중 쇄골하동맥의 재협착 여부나 그로 인한 흉통 등의 증 상 재발에 관해서도 조사하였다. 결 과 대상환자 및 시술의 특징 대상환자 중 좌내유동맥을 이용한 관상동맥 우회술을 시행 받을 예정인 환자가 16명(관상동맥 우회술 전 그룹)이었으며, 이전에 관상동맥 우회술을 시행 받아 좌내유동맥과 연결된 관 상동맥 우회로가 있었던 환자는 6명이었다(관상동맥 우회술 후 그룹). 관상동맥 우회술 전 그룹에서 쇄골하 동맥의 협착 은 심도자 검사(n=6)나 수술 전 CT 혈관촬영술(n=10)에서 발견되었으며, 임상적으로 뚜렷한 좌측 상지의 파행을 호소한 환자는 없었다. 관상동맥 우회술 후 그룹에서 관상동맥 우회술과 좌측 쇄골 하동맥협착의 중재적 시술 사이의 기간은 평균 18개월이었으 나, 그 중 4명은 10일 미만이었으며 1명은 9개월, 1명은 103 개월이었다. 좌측 쇄골하동맥의 협착의 원인은 모든 예에서 동맥경화성 병변이었으며, 50% 이상의 쇄골하동맥 협착성 병변이 21예였 고, 쇄골하동맥의 완전 폐색이 1예 있었다. 병변의 위치는 쇄 골하동맥의 기시부가 8예 있었고, 쇄골하동맥의 근위부가 14 예 있었다. 천자부위는 상완동맥이 2예, 대퇴동맥이 18예였다. 나머지 2예에서는 대퇴동맥과 상완동맥 양쪽을 모두 천자하였는데, 그 이유는 5 F 카테터도 통과하지 못할 정도의 심한 쇄골하동맥 협착이었기 때문에 상완동맥을 통해 협착 병변을 통과시킨 유 도철선을 대퇴동맥을 통해 낚아채어 양쪽에서 지지한 후 카테 터를 통과시킬 수 있었다. 스텐트 삽입술은 18예에서 시행되 었고, 나머지 4예에서는 경피적 혈관성형술만 시행되었다. 경피적 혈관성형술만 시행된 경우에는 모두 8 mm 직경의 풍선 카테터가 사용되었으며, 1예에서 혈관성형술 후 국소적 내막 박리가 발생하였으나 박리가 더는 진행하지 않고 혈류의 장애를 초래하지 않아 스텐트 삽입 등의 추가 시술은 시행하 지 않았다. 스텐트를 삽입한 18예 모두에서 풍선확장형 스텐트를 사용 하였다. 사용한 스텐트는 Jostent (Jomed, Helsinborg, Sweden) 6예, Palmaz stent(cordis, Miami, U.S..) 2예, Genesis stent(cordis, Miami, U.S..) 10예였다. 스텐트의 직경은 6-10 mm였고, 길이는 mm였다. 쇄골하동맥의 완전 폐색이 있었던 1예와 95% 이상의 협착이 있었던 1예에서 4 mm 직경의 풍선 카테터를 이용하여 스텐 트 삽입 전 풍선확장술을 시행하였고 나머지 16예에서는 일 C Fig year-old woman who was going to have left internal mammary artery-coronary artery bypass grafting.. 7 Endovascular Therapy of Left Subclavian rtery Stenosis in Patients with Left Internal Mammary-Coronary rtery ypass Graft 1 Hwan Jun Jae, M.D., Jae Hyung Park, M.D., Cheong Il Shin, M.D., yung Jin Lee, M.D., Young Kwon Cho, M.D., Jin Wook Chung, M.D. 1 Department of Radiology, Seoul National University College of Medicine, Institute of Radiation Medicine, Seoul National University Medical Research Center, and Clinical Research Institute, Seoul National University Hospital Purpose: We wanted to evaluate the clinical usefulness of percutaneous transluminal angioplasty (PT) and stenting of left subclavian artery (LS) stenosis in the patients with a left internal mammary artery (LIM)- coronary artery bypass graft. Materials and Methods: From September 1998 to November 2005, significant proximal LS stenoses were treated with PT and stenting in 22 patients (15 men and 7 women) who had a prior LIM-coronary artery bypass graft or who were willing to undergo LIM-coronary artery bypass grafting. The technical success rates, complications and restenosis during the follow-up period were retrospectively evaluated. Results: Six patients had a prior LIM bypass graft and 16 patients were treated before their coronary artery bypass surgery. The etiology of the LS stenosis was atherosclerosis in all patients. Four patients were treated with PT only, and stents were placed in 18 patients. Technical success was achieved in all patients. n embolism in the proximal LIM occurred after stenting in one patient, and the LIM was recanalized with transcatheter thrombolysis. During a mean follow-up of 30 months, only one patient was found to have recurrent LS stenosis 29 months after stenting and this patient was successfully managed with angioplasty. Conclusion: Endovascular therapy is useful and efficacious for the treatment of LS stenosis in patients with a LIM-coronary artery bypass graft.
