They are classified into four categories based on the etiology and pathological findings. In Korea, epidemiological data on DILD has never been reported in a prospective manner. Method : From May 2002 to April 2004, total 487 patients with DILD were prospectively registered at Samsung Medical Center. The prospective observational analysis of the etiologies, its classification based on 2002 ATS/ERS (American Thoracic Society/European Respiratory Society) guidelines, as well as diagnostic tests and the retrospective analysis of the treatment modalities were carried out. Any infectious and malignant causes were excluded. Results : 1) The patients were classified into idiopathic interstitial pneumonia (IIP) in 269 patients (55.2%), known causes of DILD in 168 patients (34.5%), sarcoidosis in 27 patients (5.5%), other forms of DILD in 14 patients (2.9%), and undetermined DILD in 9 patients (1.9%). 2) The diagnostic test showed that most patients had undergone chest high resolution computed tomography (HRCT) and pulmonary function test (PFT) (97%, 89%). Transbronchial lung biopsy (TBLB) and surgical lung biopsy (SLB) were performed in limited patients (38%, 29%). 3) Among 269 patients with IIP, 220 (82%) had idiopathic pulmonary fibrosis (IPF) while 23 (9%) had nonspecific interstitial pneumonia. SLB was carried out in 36% of patients with IIP. 4) Symptomatic supportive care was given to 67% of IPF, but specific medical treatment including corticosteroids was administered to 89% of non-ipf patients. Conclusion : A nationwide registry of DILD patients is required to determine the annual incidence, etiology, and practice pattern of diagnosis and treatment in Korea. 2 Tuberculosis and Respiratory Diseases Vol. 58. No. 6, Jun 하 TBLB 로약함 ) 을시행할수있고, 외과적폐생검도고려할수있다 1,6,7. 특히, 외과적폐생검은전신마취하에시행되는침습성진단수기로서합병증발생률및사망률로인해일부에서만시행되고있으나, 전체 DILD 환자에서얼마나시행되고있는지에대한국내보고는없는실정이다. DILD 역학에대한국외보고는미국, 벨기에, 독일과이탈리아에서조사된바있는데, 이들모두다기관전향적등록에의해조사되었고유육종증과 IIP가비교적흔한질환이었으나나라마다각질환들의빈도가다르게보고되었다 국내에서는 1990년결핵및호흡기학회주관전국다기관조사보고 11 가있었으나, 조사기간이병원마다서로다른후향적조사이고진단기준도각병원마다다르며속립성폐결핵을비롯한감염및악성종양까지 DILD에포함시킨문제점이있었다. 1996년임등 12 에의한단일기관보고에서도폐조직검사가시행된경우만을조사한한계가있었다. 그외에는대부분산발적인증례보고나일부질환에대한보고만있었고 DILD의전향적등록에의한국내역학연구는없었다. 이에저자들은한 3 차병원에서 2년간전향적으로 DILD 환자들을등록하여각질환의분포와빈도를알아보고진단수기및치료방법의실태에대해파악하고자본연구를시행하였다. 되는것을모두만족하는경우로했고, 감염과악성종양이최종원인으로밝혀진경우는제외하였다. DILD 의분류는직업력, 흡연력을포함한환경인자, 약물복용력, 동반된교원혈관병, 방사선노출력, 이학적소견과진단적검사들 (HRCT, 폐기능검사, BAL, TBLB, 외과적폐생검 ) 의소견에의해 2002년 ATS/ERS 분류에근거하여분류하였다. DILD of known causes 중교원혈관병과연관된 DILD가의심될경우는류마티스내과에협진의뢰하여미국류마티스학회진단기준에따라교원혈관병을진단하였다. 진단방법으로사용된검사는 HRCT, 폐기능검사, BAL, TBLB, 외과적폐생검술시행여부에대해조사하였고, 외과적폐생검술은개흉폐생검과흉강내시경폐생검으로나누어조사하였다. IIP의세부분류는외과적폐생검소견을기준으로하였으나이를시행하지못한환자는 HRCT 진단을기준으로하였다. 치료방법은대증치료와스테로이드단독치료, 스테로이드와세포독성제 (azathioprine, cychlophosphamide) 의병합치료, 세포독성제단독치료, 감마인터페론주사치료, 폐이식으로나누어후향적으로조사하였다. 결과 1. DILD의원인적분류 대상및방법 2002년 5월부터 2004년 4월까지총 2년간삼성서울병원호흡기내과간질성폐질환클리닉및외래 / 입원환자, 그리고다른진료과에서협진의뢰된환자중에, 18세이상이면서아래 DILD 진단기준을만족하는환자 487명을전향적으로등록하여원인적분류및진단수기에대해서는전향적조사를하였고, 치료방법에대해서는후향적으로환자의챠트를검토하여조사하였다. DILD의진단기준은 1) 기침과노작성호흡곤란이주요호흡기증상, 2) 단순흉부방사선검사상미만성간질성침윤이양측폐에서관찰, 3) 폐기능검사상제한적폐기능장애의소견, 4) 폐조직검사가시행된경우에병리소견상염증과섬유화가관찰 총 487명의 DILD 환자의중앙연령은 61세 (18 ~ 87세 ) 이고남녀비는 1.6:1로남자가많았다. DILD의원인적분류는 IIP가 269명 (55.2%) 으로가장많았고, DILD of known causes가 168명 (34.5%), granulomatous DILD가 27명 (5.5%), other forms of DILD가 14명 (2.9%), 분류가불확실한환자들이 9명 (1.9%) 이었다 (Figure 1). DILD of known causes에는교원혈관병이 129명 (76.8%) 으로가장많았고, 직업이나환경적원인에의한경우가 25명 (14.9%), 약물에의한경우가 12명 (7.1%), 과민성폐렴이 2명 (1.2%) 이었다. 원인이되는교원혈관병에는류마티스성관절염이 36명 (27.8%), 피부근염이 23명 (17.8%), 전신성경화증이 22명 (17.1%), 혈관염이 17명 (13.2%) 이었고, 그외쇼그렌씨증후군, 혼합결체조직질환, 전신성홍반성낭창등이있었다 571

IIP의세부분류로는특발성폐섬유화증 (idiopathic pulmonary fibrosis, 이하 IPF로약함 ) 이 220명 (81.8%) 으로가장많았고, 비특이성간질성폐렴 (nonspecific interstitial pneumonia, 이하 NSIP로약함 ) 이 23명 (8.5%), 특발성기질화성폐렴 (cryptogenic organizing pneumonia, 이하 COP로약함 ) 가 8명 (3%), 급성간질성폐렴 (acute interstitial pneumonia, 이하 AIP 로약함 ) 가 4명 (1.5%) 이있었다 (Figure 1). Other forms of DILD 에는랑게한스세포조직구증 (Langerhans cell histiocytosis) 4명, 폐림프관평활근증 (lymphangiolei omyomatosis) 3명, 폐포단백증 (pulmonary alveolar pro teinosis) 3명, 호산구성폐렴 4명이있었다. A. DILD B. Treatment for idiopathic interstitial pneumonia IPF (n=220) NSIP (n= 23) CS alone(%) CS + CTT(%) CTT alone(%) IFN-gamma(%) Supportive case only(%) Abbreviation, AIP; acute interstitial pneumonia, COP; cryptogenic organizing pneumonia, CS; corticosteroid, CTT; cytotoxic agent, IFN-gamma; interferon-gamma, IPF; idiopathic pulmonary fibrosis, NSIP; nonspecific interstitial pneumonia COP (n=8) AIP (n=4) Others (n=14) 2. 진단수기전체환자에서 HRCT와폐기능검사는대부분 ( 각각 97%, 89%), BAL은 214명 (44%), TBLB 가 187명 (38%) 에서시행되었으며, 외과적폐생검은 139명 (29%) 에서시행되었다. 외과적폐생검은 IIP에서 36%(98/269명 ), DILD of kwon causes 에서 17%(29/168명 ), granuloma tous DILD에서 7%(2/27명 ), other forms of DILD에서 57%(8/14명 ) 에서시행되었다. 외과적폐생검의시행은 DILD의진단목적으로 88.5% 에서시행되었으며이들모두흉강내시경폐생검술을이용하였고, 폐암과같은다른동반질환의치료중개흉폐생검이이루어진경우가 11.5% 였다. 외과적폐생검이시행된 IIP 환자들은시행되지않은환자들보다평균연령이더낮고 (57.8 세 vs. 68.2세, p<0.001), 심장질환, 뇌혈관질환, 악성종양등과같은심각한동반질환이더적었으며 (16.5% vs. 69%, p<0.001), HRCT 진단이 non- IPF로진단된경우가더많았다 (33.7% vs 9.2%, p<0.001). 3. 치료방법전체환자의 50% 에서는대증치료만받았고, 28% 에서는스테로이드단독요법, 10% 에서는스테로이드와세포독성제의병합요법, 7% 에서는세포독성제단독요법, 5% 에서는감마인터페론치료, 0.2%(1 명 ) 에서는폐이식을각각받았다. 대증치료이외의다른치료법 ( 스테로이드, 세포독성제, 감마인터페론, 폐이식 ) 은 DILD of known causes의 67%, IIP의 43%, granuloma tous DILD의 53%, other forms of DILD의 36% 에서 받았다. IIP환자중에서는 IPF 환자의 67% 가대증치료만을받았으나, NSIP, COP, AIP 환자에서는스테로이드를포함한다른치료를더많이받았다 (p<0.001) (Table 1). 고찰 DILD는조사대상지역과진단하는의사에따라유병률에많은차이를보일수있다. 외국에서연구된전향적등록에의한 DILD 의역학조사에는나라에따라차이가있으나유육종증이상대적으로많은빈도를보였지만, 유육종증을제외하면 IIP가가장많고교원혈관병과연관된 DILD와과민성폐장염도많은것으로보고되고있다 외국에서시행된연구중미국역학조사는뉴멕시코주에서일차진료의사와호흡기의사에의해환자들을등록하였고병리소견, 퇴원기록, 사망기록, 부검결과등을검토하여정리한비교적정확한연구라고할수있다. 이연구에서는 DILD의유병률이 10만명당 74.1명으로드물지않은것으로조사되었고, 원인적분류에서 IIP가 23% 로가장많은것으로되어있고, 진폐증이 14%, 교원혈관병과연관된 DILD가 13.8%, 유육종증이 12% 의순서로많았다 9. 국내연구는 1990년결핵및호흡기학회에서시행한다기관연구가있으며속립성결핵이원인질환중 38% 로가장많은것으로나타났으나원인으로감염을제외하면 IIP(27%) 와교원혈관병과연관된 DILD(15%) 가많았다 11. 본연구에서도 DILD의원인분류중 IIP가가장많은빈도를보였고, 교원혈관병과연관된 DILD 가다음으로많은것으로되어있어 573

5 YS Kwon, et al.: Etiologies, diagnosis and treatment of DILD in Korea 상기연구들과유사한빈도를나타내었다. 그러나, 전체적인환자분포차이는 3차병원에서의환자등록이라는단점이작용하였을것으로추정된다. 이런 DILD 의역학연구결과는인구집단에따라다르게나타나는데특히직업성폐질환은해당지역사회의주요산업이무엇이냐에따라다른빈도로나타날수있고, 과민성폐장염또한지역마다유병률에많은차이를보이는데기후와계절, 흡연, 직업등에영향을받기때문이다 13. 더구나 DILD의역학연구는등록환자의선택오차를가질수있는데, 후향적등록에서더많이나타날수있다고알려져있다. 실제이탈리아에서연구된역학조사를보면, 후향적조사에서는유육종증이 53% 로가장많은것으로나타났으나전향적조사에서는 30% 로 IIP(37%) 보다적은것으로조사되었다 14. DILD의진단에서 HRCT 는단순흉부방사선검사에비해 DILD 에대한민감도가더높고 15 폐가정상인지아닌지구별하는데더우수하며 16, 폐병소의범위를알아내고 BAL, TBLB 및외과적폐생검의위치를결정하는데중요한정보를제공하므로, 단순흉부방사선검사와병력에서특정진단이강력히의심될경우를제외하고는모든환자들에게시행하는것이좋을것이다. 임상정보와 HRCT 소견을종합하면 DILD 의진단률이 80% 이상이되며, 임상정보와 HRCT 소견이특징적이면폐조직검사없이도진단할수있다고알려져있다 1,5,7. 본연구에서도 HRCT 는 DILD 환자의대부분인 97% 에서시행하였고, 외과적폐생검이이루어지지않은환자들의대부분은임상정보와 HRCT에의해진단되었다. 외과적폐생검은 DILD의원인을확진하여불필요한치료와그에따른합병증을줄일수있다 5. 외과적폐생검의방법으로흉강내시경폐생검이최근주로사용되고있는데, 흉강내시경폐생검은개흉폐생검에비해진단정확도는비슷하지만수술합병증이적고입원기간이짧다는장점이있다 1. 본연구에서는외과적폐생검이 29% 에서시행되어외국연구 (7-25%) 와국내연구 (7%) 에비해비교적많이시행되었으나, 대부분흉강내시경폐생검으로시행되었기때문에개흉폐생검보다안전하였고외과적폐생검이이루어진환자들의 HRCT 소견상 IPF 이외의진단이많았기때문에확진을위해적 절히시행한검사라고사료된다. DILD의치료는각각의질병에따라다르고, 치료하는의사에따라많은차이를보이고있다. 본연구에서의치료는각환자들의챠트를후향적으로분석한것으로 DILD 의치료지침이될수는없다. 다만치료효과가입증된약제가없는 IPF 환자의 2/3는대증치료만받았다. 본연구는단일 3차병원에서조사되었고일정한인구집단에서의발생한환자를조사한것이아니기때문에유병률을알수없으며본연구에서조사된 DILD의원인적빈도가전체인구에서동일하게나타난다고할수는없지만, 국내연구중전향적등록에의한첫연구로서 3차병원에서 DILD의원인분포를알려준데의미가있다고하겠다. DILD의유병률, 발생률, 원인적분포및진단수기, 치료방법의연구를위해서국내에서도전국적인다기관전향적등록사업이필요할것으로사료된다. 요약배경 : 국내에서전향적으로 DILD를등록하여원인분류한보고가없고진단수기및치료에대한보고도전무하여, 한 3차대학병원에서 2년동안전향적으로진단된 DILD의원인, 진단수기및치료현황에대한실태를파악하고자본연구를시행하였다. 방법 : 2002년 5월부터 2004년 4월까지 2년간삼성서울병원에서전향적으로등록된 DILD 환자 487명을대상으로, 2002년 ATS/ERS 분류에따른원인, 진단에이용된수기, 치료방법에대해후향적조사를시행하였다. 결과 : 1) 전체 487명중 DILD of known causes 168명, IIP 269명, granulomatous DILD 27명, other forms of DILD 14명이었고, 분류가불확실한환자 9명이었다. 2) 진단수기는 HRCT 가 97%, 폐기능검사가 89%, BAL이 44%, TBLB가 38%, 외과적폐생검이 29% 에서이용되었다. 3) 269명의 IIP 환자중 IPF가 222명 (82%), NSIP 23명 (9%) 이었고, 진단수기는 HRCT가 97%, BAL이 42%, 외과적폐생검이 36% 에서받았다. 4) IIP 574

No. 6, Jun 환자중폐생검을받은환자는받지않은환자에비해평균연령이더낮고 (57.8 세 vs 68.2세, p<0.05) 심장질환등의동반질환이적었으며 (16.5% vs 69%, p<0.001), HRCT 소견상 non-ipf인경우가더많았다 (33.7% vs 9.2%, p<0.001). 5) IPF 환자는 67% 에서대증적치료만을받았으나 non-ipf 환자에서는 89% 에서스테로이드등의약물치료를받았다. 결론 : 국내에서도 DILD 질환등록사업및적절한검사를통한정확한진단이필요하고이에따른치료가이루어져야할것으로사료된다. 참고문헌 1.
