This syndrome of prostate cancer is rare and has been reported in only a few cases. We report a patient with ectopic ACTH production associated with metastatic prostate cancer. A 70-year-old patient with metastatic prostate cancer was admitted to our hospital with septic shock. He had a history of hormonal therapy and transurethral prostatectomy. Adrenocortical function was checked due to consistent fever and poor general condition, which revealed markedly increased levels of basal plasma ACTH and serum cortisol. The patient did not present typical signs of the Cushing s syndrome, however, and a history of hypertension were found. He died in days as a result of multi-organ failure. On pathology, the prostatectomy specimen showed a tumor composed of mixed populations of adenocarcinoma and small cell carcinoma. The tumor cells in the small cell component were positive for chromogranin and ACTH. Although neuroendocrine differentiation in prostate cancer is rare, etopic ACTH production should be considered in patients with prostate cancer as well as in clinical features of ACTH hypersecretion. 238 Kim EK, et al. 본원응급실에내원하였다. 환자는 4년전전립선비대증에대한검사로시행한전립선특이항원 (prostate specific antigen, PSA) 의상승소견을보여전립선조직검사및복부전산화단층촬영후전립선암의골전이로진단받았다.

배뇨장애에대한증상완화를위하여경요도전립선절제술 (transurethral prostatectomy) 을받았으며, 당시지속적인혈압의상승이관찰되어고혈압으로진단받고 amlodipine 을 1일 10 mg 복용하기시작하였다. 수술후합병증없이경과관찰중 4주전부터좌하지의근력저하가심해짐을호소하였다.

척수압박증후군의치료를위하여덱사메타손 (dexamethasone) 을 5일간 6시간마다 4 mg, 이후 5일간은 6시간마다 2 mg 투여받았으며, 당시혈당이상승하여스테로이드투여에따른고혈당으로진단받고인슐린을투여받았으나덱사메타손치료가종료된후에는점차인슐린요구량이감소하여매식전인슐린글루리신 (insulin glulisine) 6단위, 취침전인슐린글라진

기관내관을통해 10 L/min의산소를공급하는상태에서맥박산소측정기 (pulse oximeter) 로측정한산소포화도는 76% 로, 즉시기계적환기를시작하였다. 환자의활력징후를안정시킨후측정시신장 168 cm, 체중 72.6 kg으로지난 3년간의체중변화는알수없었으나 6주전과비교하여 1.5 kg 정도의체중감소가있었다.

2 238 Kim EK, et al. 본원응급실에내원하였다. 환자는 4년전전립선비대증에대한검사로시행한전립선특이항원 (prostate specific antigen, PSA) 의상승소견을보여전립선조직검사및복부전산화단층촬영후전립선암의골전이로진단받았다. 이에수술적치료없이 goserelin, bicalutamide로호르몬치료를받던중전립선암의진행과배뇨장애의악화로 6주전본원비뇨기과에방문하였다. 전립선자기공명영상및골스캔에서전립선암의정낭침범및다발성골전이가관찰되었고, 흉부전산화단층촬영에서는폐와간의전이가발견되었다 (Fig. 1A, B). 배뇨장애에대한증상완화를위하여경요도전립선절제술 (transurethral prostatectomy) 을받았으며, 당시지속적인혈압의상승이관찰되어고혈압으로진단받고 amlodipine 을 1일 10 mg 복용하기시작하였다. 수술후합병증없이경과관찰중 4주전부터좌하지의근력저하가심해짐을호소하였다. 당시신체검진에서우 / 좌하지근력이각각 grade 4+/4-로확인되어시행한척추자기공명영상에서제9흉추암전이병변의경막외침범및척수압박소견 (Fig. 1C) 이나타나해당부위에대하여 2주간총 36 Gy의고식적척추방사선치료를받았다. 함께시행한복부전산화단층촬영에서는새로양측부신및췌장에전이가의심되는병변이발견되었다 (Fig. 1D). 척수압박증후군의치료를위하여덱사메타손 (dexamethasone) 을 5일간 6시간마다 4 mg, 이후 5일간은 6시간마다 2 mg 투여받았으며, 당시혈당이상승하여스테로이드투여에따른고혈당으로진단받고인슐린을투여받았으나덱사메타손치료가종료된후에는점차인슐린요구량이감소하여매식전인슐린글루리신 (insulin glulisine) 6단위, 취침전인슐린글라진 (insulin glargine) 16단위를투여중이었다. 과거력 : 전립선암치료도중진단받은고혈압과당뇨병이외에다른병력은없었다. 가족력 : 부모, 형제자매중특이병력을가진사람은없었다. 신체검진소견 : 내원당시의혈압은 78/40 mmhg이었으며체온이 38.3 로상승되어있었다. 기관내관을통해 10 L/min의산소를공급하는상태에서맥박산소측정기 (pulse oximeter) 로측정한산소포화도는 76% 로, 즉시기계적환기를시작하였다. 환자의활력징후를안정시킨후측정시신장 168 cm, 체중 72.6 kg으로지난 3년간의체중변화는알수없었으나 6주전과비교하여 1.5 kg 정도의체중감소가있었다. 양측하폐야에서거친수포음이청진되었으며, 경도의양하지부종이관찰되었다. 복부검진에서는특이소견없었으며, 복부압통및늑척추각압통은환자의의식저하로명확히평가할수없었다. 간기능검사에서콜레스테롤 85 mg/dl, 빌리루빈 0.8 mg/dl, 총단백 4.2 g/dl, 알부민 2.2 g/dl, 아스파르트산아미노전이효소 69 IU/ L, 알라닌아미노전이효소 58 IU/L, 알칼리인산분해효소 130 IU/L로현저한저콜레스테롤혈증, 저단백및저알부민혈증이있었다. 혈중요소질소 28 mg/dl, 혈청크레아티닌 1.

3 A Case of Ectopic ACTH Syndrome in Prostate Cancer 239 검사소견 : 일반혈액검사 (complete blood cell count) 에서백혈구 8, /μl ( 호중구, 94.6%), 혈색소 9.9 g/dl, 적혈구용적률 29.5%, 혈소판 /μl였고 C-반응성단백 (C-reactive protein, CRP) 은 14 mg/dl 로상승되어있었다. 혈액응고검사는정상이었으며 D-dimer 3.49 μg/ml로측정되었다. 혈청전해질검사는나트륨 139 mmol/l, 칼륨 3.2 mmol/l, 염소 104 mmol/l로저칼륨혈증의소견을보였다. 간기능검사에서콜레스테롤 85 mg/dl, 빌리루빈 0.8 mg/dl, 총단백 4.2 g/dl, 알부민 2.2 g/dl, 아스파르트산아미노전이효소 69 IU/ L, 알라닌아미노전이효소 58 IU/L, 알칼리인산분해효소 130 IU/L로현저한저콜레스테롤혈증, 저단백및저알부민혈증이있었다. 혈중요소질소 28 mg/dl, 혈청크레아티닌 1.01 mg/dl, 무작위혈당 86 mg/dl로측정되었다. 혈청 PSA는 ng/ml로 4주전 ng/ ml와비교하여상승되어있었다. 영상의학적소견 : 단순흉부방사선촬영에서광범한음영의증가 (increased opacity) 를보였으며 (Fig. 2A), 흉부전산화단층촬영에서폐전반의경화 (consolidation), 간유리음영 (ground glass opacity), 공기- 기관지조영 (air-bronchogram) 및다발성종격동림프종대, 양측성흉수소견이관찰되었다 (Fig. 2B). 폐혈전색전증의소견은없었다. 치료및경과 : 상기의소견들을종합하여폐렴에따른패혈성쇼크로진단하였으며, 기계적환기를유지하면서확인한동맥혈산소분압 (arterial oxygen pressure, PaO 2) 과흡입산소농도 (fraction of inspired oxygen, FiO 2) 의비가 135 mmhg로 200 mmhg 미만의값을보여급성호흡곤란증후군 (acute respiratory distress syndrome, ARDS) 에도합당하였다 [11]. 중환자실에서기계적환기및 norepinephrine 정맥주입을유지하면서항생제로 piperacillin/tazobactam 과 levofloxacin 을투여하기시작하였다. 경기관흡인법 (transtracheal aspiration) 으로얻은호흡기검체및혈액을배양한결과호흡기검체에서는모든약제에감수성이있는 Pseudomonas aeruginosa 가 동정되었으며혈액에서는균이자라지않았다. 이에투여중인항생제를유지하면서혈압및 CRP는호전추세를보여 6일동안 norepinephrine을 16 μg/min에서 2 μg/min까지단계적으로감량하였으며, CRP는최고 mg/dl에서 mg/dl 까지감소하였다. 그러나 0.6 이상의높은 FiO 2 및 39 이상의고열은호전없이지속되어반복적으로배양검사를하면서, 4주전시행한복부전산화단층촬영에서양측부신의전이가의심되는상황으로부신기능저하증이동반되었을가능성을고려하여제7병일에부신피질기능검사를시행하였다. 오후 4시경측정한혈장기저 ACTH 31,500 (10-60) pg/ml, 혈청기저코르티솔 87.8 (5-25) μg/dl로 ACTH 및코르티솔모두매우상승되어있었으며, 익일정오에재측정한혈장 ACTH 농도또한 4,110 pg/ml로상승된소견을보였다. 이에다른기저호르몬검사를시행한결과성장호르몬 (growth hormone) < 0.05 ng/ml, 인슐린유사성장인자 (insulin-like growth factor-1) 33.1 (71-290) ng/ml, 프롤락틴 (prolactin) 36.3 ( ) ng/ml, T3R 40 (60-195) ng/dl, 유리T ( ) ng/dl, 갑상선자극호르몬 (thyroid stimulating hormone) 0.12 ( ) μiu/ml이었다. 환자의신체검진상월상안 (moon face), 물소혹 (buffalo hump), 복부선 (abdominal striae) 등쿠싱증후군을의심할만한외형적특징이나과다색소침착 (hyperpigmentation) 의소견은없었으나, 내원당시의저칼륨혈증및매우상승되어있는혈장 ACTH로전립선암에의한이소성 ACTH 분비를의심하였다. 그러나확진하기위해필요한터키안 (sella) 자기공명영상및하추체정맥동채혈검사 (inferior petrosal sinus sampling, IPSS) 는환자의전신상태를고려할때시행할수없었으며, 전립선암수술조직에서면역조직화학검사를시행하여 ACTH이발현되면이소성 ACTH 분비종양을진단할수있으므로 [3] 결과에따라치료를진행하고자계획하였다. 갑상선호르몬의이상소견에대하여는심각한전신질환에따른변화로판단하여특별한투약은하지않았다. 있는데, 이후로도 전 세 부전으로 제11병일에 사망하였다. 계적으로 보고된 예가 적으며[4-10] 아직까지 우리나라에는 보고된 병리 소견: 수술 검체의 병리검사 결과는 선암(adenocarcinoma) 바가 없다.

전립선암의 이소성 관지 카르시노이드(carcinoid) 및 폐소세포암이 대부분이며[2-4] 전 ACTH 분비를 의심하여 다시 검토한 결과 선암 이외에 일부 소세포 세계적으로 보고된 이소성 ACTH 분비 전립선암의 증례는 다음과 암이 혼재되어 있는 것이 확인되었다(Fig. 3A). 면역조직화학검사 결 같은 정도이다(Table 1).

4 Kim EK, et al. 240 고 찰 한편 제6병일에 접수한 호흡기 검체 및 혈액배양 1쌍에서는 penicillin, ampicillin, ciproﬂoxacin에 저항성이 있는 Enterococcus faecium이 동정되어 감염내과와 상의 후 cefepime, amikacin 및 levo- 본 증례는 급격히 진행하는 다발성 전이성 전립선암 환자에서 이 ﬂoxacin의 3제 병용 투여로 항생제를 변경하였다. 그러나 이후의 배 소성 ACTH 분비가 발견된 매우 드문 경우로, 1992년 di Sant Agnese 양검사상 호흡기 검체에서는 지속적으로 같은 균이 동정되었으며, [12]는 1955년부터 1991년까지의 혈중 ACTH가 상승한 신경내분비 환자는 폐렴에 의한 ARDS의 급격한 진행 및 이에 따른 다발성 장기 성 전립선암 환자 7예의 보고들을 정리한 바 있는데, 이후로도 전 세 부전으로 제11병일에 사망하였다. 계적으로 보고된 예가 적으며[4-10] 아직까지 우리나라에는 보고된 병리 소견: 수술 검체의 병리검사 결과는 선암(adenocarcinoma) 바가 없다. 으로 종양이 전립선 총 부피의 80%를 차지하였고, Gleason score가 이소성 ACTH 분비는 여러 신경내분비 종양과 연관이 있는데, 기 8 (3 + 5)/10로 매우 높은 악성도를 보였다. 전립선암의 이소성 관지 카르시노이드(carcinoid) 및 폐소세포암이 대부분이며[2-4] 전 ACTH 분비를 의심하여 다시 검토한 결과 선암 이외에 일부 소세포 세계적으로 보고된 이소성 ACTH 분비 전립선암의 증례는 다음과 암이 혼재되어 있는 것이 확인되었다(Fig. 3A). 면역조직화학검사 결 같은 정도이다(Table 1). 전립선암의 발생이 우리나라 남성 전체 암 과 소세포암의 부분에서 신경내분비 표지자인 chromogranin이 전 발생 순위에서 5위(2008년 기준 총 6,471명, 전체 암 발생의 7%) [13], 체적으로 발현되며(Fig. 3B), 일부 세포에서는 ACTH가 발현되는 반 전 세계적으로는 2위임(2008년 기준 총 914,000명) [14]을 고려하면 면(Fig. 3C), 전립선암의 표지자인 PSA는 발현되지 않는 것이 관찰되 이소성 ACTH 분비 전립선암은 매우 드문 질환임을 알 수 있다. 었다(Fig. 3D). 악성종양에서의 이소성 ACTH 분비에 따른 쿠싱증후군의 증상 및 발현의 속도는 부분적으로 기저 종양의 악성도와 관계가 있다[7]. 5 A Case of Ectopic ACTH Syndrome in Prostate Cancer 241 기관지카르시노이드와같이서서히자라는종양은대개전형적인쿠싱증후군의증상을나타내는반면, 폐소세포암과같이공격적인종양에서는임상적으로피부의과다색소침착, 부종, 저칼륨혈증등이우세하게나타난다 [1,3]. 따라서전자의경우주로전형적인쿠싱증후군의감별진단과정에서악성종양이발견되는경우가대부분이나 [1-3,15], 후자와같이급격하게진행하는악성종양에동반된이소성 ACTH 증후군은오히려 ACTH 과다분비에의한고코르티솔혈증상태를발견하는것이어려운경우가흔하다. 그러므로급속도로진행하는악성종양에서다른이유로설명할수없는과다색소침착, 부종, 저칼륨혈증등이나타나면감별진단시드물더라도종양의이소성 ACTH 분비여부의확인을고려하여야할것이다. 그간보고된전립선암과연관된증례에서도부종, 고혈당, 저칼륨혈증등의증상이동반되어있었음을확인할수있다 [4-10] (Table 1). 본증례의환자에서는고혈압은있었으나과다색소침착은없었으며, 경도의하지부종은응급실내원당시저알부민혈증에따른것일가능성을우선적으로생각하였다. 저칼륨혈증또한 4주전부터인슐린치료를받으면서나타나과량의인슐린투여에따른혈중칼륨농도의저하일가능성을고려하였으나 [16], 해당기간동안경구및정주로반복적인칼륨보충을했음에도 mmol/l의낮은수 치를보인것과환자의급격한진행경과를고려하면하지부종및저칼륨혈증모두이소성 ACTH 증후군과연관될가능성도충분하다고사료된다. 혈장 ACTH와혈청코르티솔의수치가상당히높은것에비하여증상이뚜렷하지않았던점에대해서는, 암이최근수개월내에매우빠른속도로진행하면서 ACTH를분비하는신경내분비세포분화소세포암은비교적최근에발생하였을가능성을생각해볼수있겠다. 이소성 ACTH 증후군은고코르티솔혈증등에의하여종종면역저하의원인이되는데 [17], 본증례의환자또한 Pseudomonas, Enterococcus 폐렴에의한패혈성쇼크가발생하여항생제의투여에도호전되지않고다발성장기부전으로급격히진행된점은이러한면역저하상태와연관되어있을가능성이있다. 이소성 ACTH가의심되면먼저혈장기저 ACTH를측정하여볼수있는데, 15 pg/ml 이상일경우거의대부분의경우 ACTH 의존성쿠싱증후군이므로이에해당하는또다른질환인쿠싱병과의감별을위해고용량덱사메타손억제검사를시행하며, 가능하면터키안자기공명영상, IPSS 등다른검사방법을추가적으로시행하는것이바람직하나 [1,2], 수술혹은조직검사로얻은종양조직에서 ACTH의염색을확인한다면바로진단하는것도가능하다 [3]. 이소성 ACTH Table 1. 6 242 Kim EK, et al. 증후군에서신경내분비성종양조직의 ACTH 염색정도는중등도이며악성의범위와 ACTH에대한면역반응정도는음의상관관계를보인다. 이러한현상에대해악성도가높은종양은급격히 ACTH를분비하여, 현재의가능한염색기법으로확인하기에저장된양이부족한상태일것으로설명할수있다. 또한이러한종양에서는 ACTH 의여러전구물질형태로분비되기도하므로, 이소성 ACTH 분비조직에서 ACTH의면역조직화학검사결과가음성으로나오는또다른이유로들수있다 [7]. 본증례에서도혈장 ACTH 농도는매우높았으나 ACTH 면역염색에양성을보인세포는그수가많지않았는데, 이는환자의전립선암의악성도가높고상기에언급한바와같이급격히 ACTH를분비하였기때문일것으로설명할수있으며신경내분비표지자인 chromogranin 이양성이면서 PSA가음성인결과는해당전립선암의소세포암부분이이소성 ACTH 분비종양의특징인신경내분비적성격을가짐을보여준다. 그러므로이소성 ACTH 분비가의심되는악성종양환자에서는종양조직내에소세포암형태로신경내분비적분화를보이는부분이있는지를면밀히검사하고, ACTH 또는전구물질의염색및신경내분비표지자염색소견, 혈중호르몬농도를상대적으로해석하여진단하는것이중요하다. 이소성 ACTH 증후군의일차치료는원발종양의외과적절제이다. 종양의절제가불가능할경우고코르티솔혈증은 ketoconazole, metyrapone 등의항아드레날린성약물 (adrenolytic agents), 도파민작용제 (dopamine agonists), 소마토스타틴유사체 (somatostatin analogues) 등의신경조절물질또는 mifepristone 등의글루코코르티코이드수용체차단제로조절하거나양측부신절제술로정상코르티솔농도를이루도록할수있다 [1-3]. 본증례에서는이소성 ACTH 분비가확인된직후전립선암의진행과이에따른급성합병증으로환자가사망하여이에대한치료를시행하지못하였다. 현재까지진행된전립선암의주된치료법은안드로겐박탈치료 (androgen deprivation therapy) 이다. 그러나대부분의종양은호르몬치료에결국불응하게된다 [18]. 이전연구들에따르면, 장기간의안드로겐박탈치료는신경내분비세포의과증식을향상시킨다고보고하였다. 전립선암의신경내분비분화는앞서설명한대로종양의진행및불량한예후와연관되어있다. Taplin 등 [19] 은호르몬불응성전립선암에서 chromogranin 의혈청농도와생존기간은음의상관관계가있음을보고하였으며, 최근의연구들은신경내분비분화가호르몬불응성전립선암의진행에역할을할것으로제안하고있다 [18,20]. 전립선의신경내분비세포는안드로겐수용체가결핍되어있어안드로겐박탈치료에불응성을보이며, 자가분비 (autocrine) 및측분비 (paracrine) 을통해전립선암의성장및증식에역할을할것이라는내용이다. 또한, 악성신경내분비세포에서세포자멸 (apoptosis) 에의저항및혈관내피성장인자 (vascular endothelial growth factor) 와같은혈관신생촉진인자의발현이관찰된바있다 [20]. 증례의환자에서도이러한설명이가능한데, 전이성전립선암으로 3년 간호르몬치료를받은후전립선암에국소적신경내분비분화를보 이는소세포암이발생하고, 이것이전립선암과함께급격히진행하 면서 ACTH 를분비하면서암이더욱빠르게진행하도록유도하였을 가능성을고려해볼수있다. 이러한빠른진행으로인하여환자에 서전형적인쿠싱증후군의임상상이나타나지않았을것으로유추 된다. 결론적으로급격히진행하는전립선암환자에서, 드물기는하나 동반될수있는이소성 ACTH 증후군을적절히진단하여치료하는 것은환자의전립선암의진행속도를완화하고면역억제에의한합 병증의발생을예방하여, 예후를향상시키는데에매우중요하다고 할수있다. 요약 이소성부신피질자극호르몬 (adrenocorticotropic hormone, ACTH) 분비는주로신경내분비종양, 폐소세포암과연관되나드물게전립선 암과연관되어불량한예후를보인다고보고된다. 저자들은전이성 전립선암환자에서의이소성 ACTH 분비를발견하였다. 3 년간의호 르몬치료에도진행한전이성전립선암으로고식적전립선수술을 받은 70 세환자에서패혈성쇼크가발생하였다. 적절한치료에도지 속되는발열및전신상태악화로측정한혈중기저 ACTH 및코르 티솔의상승으로이소성 ACTH 분비가의심되었으나급격한다발성 장기부전으로사망하였다. 환자는쿠싱증후군의외형은뚜렷하지 않았으나고혈압의병력과저칼륨혈증이있었다. 전립선수술조직 에서선암과소세포암이혼재되어있었으며소세포암은 chromogranin 발현증가, ACTH 의국소적양성, 전립선특이항원 (prostate specific antigen) 음성을보였다. 이에전립선암환자에서임상적특 징에따라이소성 ACTH 분비를드문감별진단으로고려하여야할 증례로보고하는바이다. 참고문헌 1.
