1 Journal of Soonchunhyang Medical Science 15(1) p.67~74 June 백혈구수와대사증후군의구성요소및군집성과의연관성 김군홍, 이승훈, 이경식, 김태연, 유병욱, 조용진, 오정은, 홍성호, 조주연 순천향대학교의과대학가정의학교실 Correlation of Erithrocyte Sedimentation Rate(ESR) and Metabolic Syndrome Kun Hong Kim, Kyoung Shik Lee, Seung Hoon Lee, Tae Youn Kim, Byung Wook Yoo, Yong Jin Cho, Jung Eun Oh, Sung Ho Hong, Choo Yon Cho Department of Family Medicine, College of Medicine, Soonchunhyang University, Seoul, Korea Background : Several researches have shown that the risk of cardiovascular disease could be elevated by the metabolic syndrome and increased WBC counts have been associated with obesity, hypertension, hyperlipidemia and impaired glucose tolerance, different components of metabolic syndrome (MS). Therefore, the aim of the present study was to examine the association of WBC counts with components and clustered features of MS. Methods : A total of 1800 subjects visited the health promotion survey of un university hospital from March 2005 to February 2006, we excluded those who did not have records of physical parameters or blood test results or those who did not complete the questionnaire. We also excluded acutely ill patients. They were evaluated and divided according to their WBC counts. Age, sex, body mass index(bmi), blood pressure(bp), fasting glucose, total cholesterol, triglyceride and high-density lipoprotein cholesterol(hdl-c) were compared with WBC counts. Among the subjects, Logistic regression analyses were used to evaluate the association between WBC counts and features of the MS, with an adjustment for age, sex. Results : The relative risks for obesity, hypertension, hypercholesterolemia, low HDL cholesterol levels and hypertriglyceridemia compared with WBC counts < cells/ μl, increased as much as WBC counts increased. The relative risks for the presence of result as 1, 2, 3, 4 features of the MS also increased as much WBC counts increased. Conclusions : An increased, albeit normal, WBC counts associates with the different components and clustered feature of MS. 2 68 Journal of Soonchunhyang Medical Science Vol.15 No.1 June 2009 최근우리나라는 서론 생활수준의향상과의학의발달 등으로평균수명이늘어나면서, 노인인구가빠르게 증가하고있다. 또한, 식습관및생활습관의서구화로 인해허혈성심질환에의한사망률이급격히증가하면 서, 1,2) 심혈관질환의위험요인에대한관심과사회경 제적중요성도높아지고있고, 대사증후군에대한관 심도고조되고있다. 1988년 Reaven 3) 은심혈관질환의주된위험인자인 고혈압, 고지혈증, 복부비만, 고혈당등이동시에발생 하며, 동반되는인슐린저항성을특징으로하는질환 군을 Syndrome X 또는 인슐린저항성증후군 이 라고명명하였고, 그후 1998 년세계보건기구(World Health Organization, WHO) 에서는이것을 대사증후 군 으로정의하였고대사증후군의진단기준을확립하 였다. 4-7) 2001년 5 월미국국립보건원(National Institutes of Health, NIH) 의보고서(National Cholesterol Education Progam-Adult Treatment, NCEP-ATP III) 에서는 WHO 에서보다단순화된새로운진단기준을발표하고, 대사증후군을심혈관질환의위험요소로강조하였다. 8) 백혈구수가증가하면, 비록이때백혈구수치가정상 범위에있을지라도, 9,10) 관상동맥질환의위험인자인대 사증후군의각구성요소와도연관성이있는것으로밝 혀지고있다 ) 현재까지백혈구수와대사증후군과 의관계에대한국내보고는부족한실정이고, 서구와 질병유병률및임상적특징에서의차이점을고려할 때, 백혈구수와대사증후군의관련성에대한확인이 필요하다고생각된다. 또한한개인에서대사증후군의 구성요소들은두개이상이동반되는경우, 즉군집되 는경우가많으므로, 대사증후군의각각의구성요소 뿐만아니라, 구성요소들의군집성과백혈구수와의비 교도필요하다고생각된다. 따라서본연구에서는일 개대학병원건강증진센터수진자를대상으로백혈구 수가대사증후군의구성요소의발생위험과연관성을 갖는지를조사하였으며, 한개인에서대사증후군의구 성요소의군집성과백혈구수증가와의연관성에대하 여알아보고자한다. 1. 대상 대상및방법 2005년 3월부터 2006년 2월까지지방의대학병원 건강증진센터를방문한개인수진자를대상군으로설 정하고, 신체계측치, 검사수치가누락되었거나설문 을작성하지않은자, 급성질환자를제외한 1800명 ( 남자 953 명, 여자 847 명) 을대상으로자료를분석하 였다. 2. 1) 방법 신체계측및채혈 내원당시측정한대상자들의신장과체중으로 [ 체 중( kg)/ 신장(m 2 )] 의식을이용하여체질량지수(body mass index, BMI) 를계산하였고, 고혈압의기왕력에 대한설문을시행하였다. 수축기및이완기혈압은최 소한 10분이상의안정상태를유지한후자동혈압측 정계로우측상완에서측정하였다. 채혈은 8시간이상 의금식후공복상태에서한차례시행한후혈중총 콜레스테롤, 중성지방, 고밀도지단백콜레스테롤, 혈 당, 백혈구수를측정하였다. 2) 대사증후군(Metabolic syndrome) 의진단기준 대사증후군의진단기준은 NIH(National Institutes of Health) 의 NCEP-ATP III(National Cholesterol Education Progam-Adult Treatment) 보고서 8) 를근거 로하여다음의요건중 대사증후군으로진단하였다 가지이상을만족할경우 허리둘레가남자 102cm, 여자 88cm을초과 중성지방이 150 mg/dl 이상 고밀도지단백콜레스테롤이남자 50 mg/dl 미만, 수축기혈압 mm Hg 경우 40 mg/dl, 여자 130 mmhg 이상혹은이완기혈압 85 이상혹은항고혈압치료의근거를충족하는

3 김군홍 : 백혈구수와대사증후군의구성 69 5 공복혈당 본연구에서는 체하되, 110 mg/dl 이상 ➀번의허리둘레는체질량지수로대 세계보건기구및대한비만학회에서제시한 비만의정의에의거하여아시아인의기준에맞는 25.0 kg/ m2이상을기준점으로하였다. 3. 통계분석 17-20) 백혈구수의 5분위수를구하여다음과같이 5개의 군으로나누었다. <4.6 x 103 cells/ μl 4.6 ~ 5.3 x 103 cells/ μl 5.4 ~ 6.1 x 103 cells/ μl 6.2 ~ 7.1 x 103 cells/ μl 7.2 x 103 cells/ μl 백혈구수에따른수진자의특징가운데통계학적차 이점을분석하기위해일원배치분산분석법 (Oneway analysis of variance(anova)) 을시행하고, 사후분 석(Tukey 의방법) 을시행하였다. 수진자들을대사증후 군의구성요소들을갖는개수에따라분류하고, 다시 1, 2, 3, 4, 5 으로분류하였다. 그리고백혈구수와대사증후군의구성요소와의연관 성, 대사증후군의군집성과의연관성을조사하기위해 나이(years), 성별을보정하여로지스틱회귀분석을시 행하여교차비를산출하였다. 통계분석은 Statistical Package for the Social Science(SPSS) 12.0 for Windows 프로그램을사용하 였고신뢰수준은 95% 로하여, P값이 0.05 미만일때 통계학적으로유의한것으로판정하였다. 결과 1. 연구대상자의일반적특징 (Table. 1) 대상군은모두 1800명으로남자는 953 명, 여자는 847 명이었고, 대상자의평균연령은 48.29±11.63 세, 평균체중은 63.88±11.34 kg, 신장은 ±3.20 cm, 체질량지수는 24.51±6.83 kg/m 2, 수축기혈압, 이완기혈압의평균치는각각 ±18.47, 77.19± mmhg 이었다. 레스테롤 검사실검사의평균치는총콜 ±37.60 mg/dl, HDL-콜레스테롤 55.09±14.05 mg/dl, 중성지방 ±91.64 mg/dl, 공복혈당 93.90±22.37 mg/dl, 백혈구수는 ± cells/ μl이었다. 연구대상자들의특징가운데나이를제외한, 체중, 신장, 체질량지수, 수축기및이완기혈압, 공복혈당, 총콜레스테롤, 중성지방은백혈구수에의해분류된 5 개의집단에서백혈구수치가높은군에서낮은군에 비해상대적위험도는증가하는추세를보였으며, HDL- 콜레스테롤은감소하였고, 모두통계적으로유의 한차이를보였다(P<0.05). 연구대상자들의나이는 백혈구수에따라유의한차이가없었다. 2.2 백혈구수와대사증후군의구성요소들과의교차분 석 (Table. 2) 백혈구수에따른대사증후군의구성요소의위험도를 산출할때, 나이와성별의영향을보정하였다. 백혈구 수가 4.6 x 103 cells/ μl미만인집단과비교한비만, 고혈압, 고중성지방혈증, 저HDL-콜레스테롤혈증의상 대적위험도는백혈구수가증가함에따라증가하는추 세를보였으며통계적으로유의하였다(P<0.05). 혈당은통계적으로유의한결과를보이지않았다. 공복 3.2 백혈구수와대사증후군의구성요소의군집성과의 교차분석 (Table. 3) 백혈구수에따른대사증후군의군집성의위험도를 산출할때에도같은방법으로, 나이와성별의영향을 보정하였다. 백혈구수가 4.6 x 103 cells/ μl미만인 집단과비교한, 대사증후군의구성요소들이한개이 상, 두개이상, 세개이상, 네개이상으로군집될 상대적위험도는백혈구수가증가함에따라모두증가 하는추세를보였고, 통계적으로유의하였다(P<0.05). 구성요소들이다섯개이상을통합한상관관계에서는 유의한결과를보이지않았다.

6 72 Journal of Soonchunhyang Medical Science Vol.15 No.1 June 2009 영향을줄수있는것을보정한이후에도백혈구수와 대사증후군의구성요소( 비만, 고혈압, 고중성지방혈증, 저HDL- 콜레스테롤혈증) 사이에는통계적으로유의한 연관성이있었으며, 백혈구수가증가함에따라대사증 후군의군집성에대한위험도도증가하였다. 그러나 대사증후군의구성요소중공복혈당은백혈구수와유 의한상관관계를나타내지않았다. 본연구에서비만, 고혈압, 고중성지방혈증, 저HDL- 콜레스테롤혈증과같은요소들은대사증후군의구성요 소에포함되는것이므로, 5-7) 백혈구수의증가는대사증 후군즉, 인슐린저항상태를반영한다고말할수있 을것이다. 11) 21) 또, 국내의최태순 서도, 등에의한연구에 백혈구수의증가와대사증후군의구성요소들이 연관성이있으며, 도연관성이있다고보고하였다. 대사증후군구성요소들의군집성과 최근 American Heart Association/Centers for Disease Control and Prevention scientific statement 에 의하면, 26,31) 심혈관계위험인자들의평가를위하여 염증반응의지표들의잠재적인임상적유용성에대한 많은연구들이진행되고있다. 이런지표들이새로운 marker 로선택되기위하여고려되어져야할중요한 요소로는다음과같은것들이있다. 지표를표준화시 킬수있어야하며, 측정결과의다양성을조절할수 있어야하며, 기존의정립된위험요소들에대하여독 립성을지녀야하며, 연구와임상실험들에서심혈관계 질환과연관성이입증되어야하며, 여러인구집단을 통하여결과를일반화시킬수있어야하며, 비용면 에있어서도유용성이받아들여질수있어야한다. 이 러한척도들을기초하여, C-reactive protein (CRP) 는 잠재적인염증및심혈관계위험의평가에있어서임 상적으로유용한검사로입증되었다. 26,31) 따라서심혈 관계질환의위험과연관된요소들의신중한평가들이 이루어지고있는시점에서, 현재까지심혈관계질환의 발생의위험인자로서 CRP와백혈구수를직접적으로 비교한연구들은부족하지만, 백혈구수도 CRP와비교 될수있는심혈관계위험에대한지표로여겨질수 있으리라생각된다. 그러나, 백혈구가심혈관계질환의병태생리에직접 적으로포함이되는지, 질병을유발시키는다른요소 들과비교할때유일한위험요소인지는정확히알려 지지않았으며, 백혈구수의증가가대사증후군의위험 도를증가시키는기전에대하여도앞으로밝혀져야할 부분이많이남아있다. Monocytes는 화물질들을증가시켜서 foamy magrophage와반응성의산 atherogenesis에기여를하 여, 심혈관계질환과연관된백혈구유형의하나로여 겨진다. 27,28,31) 이들 macrophage와 lymphocyte는전 염증성사이토카인들은분비하며, mast cell은 matrix metaloprotease를활성화시키는 serine protease를 분비한다. 29) Monocytes는또한혈소판과상호작용을 하여혈관의혈전형성에작용을하며, 이는높은혈전 성의 tissue factor 의풍부한원천이되기도한다. 30) 지방조직에서분비되는사이토카인에는 IL-6, TNF-ɑ 와 leptin 등이있다. IL-6은식후에혈당과인슐린수 치에비례해서증가한다. 22) TNF-ɑ와 leptin은지방조 직에서분비되어인슐린기능을방해하거나, 억제하거나, 분비를 수용체의기능에영향을미쳐서인슐린 저항성을유발하며, β- 세포의기능을변화시킨다 ) 이들전염증성사이토카인들에의하여유발된염증은 내피세포의혈관확장기능, 항혈전기능을소실시키며, 백혈구들이자극되어백혈구수와혈관저항이증가한 다 ) 그러나본연구는단면연구이기때문에그인 과관계를명확히구분하기란쉽지않으며, 백혈구수의 증가와대사증후군의연관성에관한정확한기전및 인과관계에대하여서구명하기에는어려움이많은것 이사실임으로앞으로많은연구가시행되어져야할 것이다. 이상의결과로보아백혈구수의증가는대사증후군 구성요소들과연관성이있으며, 특히비만, 고혈압, 고 중성지방혈증, 저HDL-콜레스테롤혈증과의연관성이 두드려졌고, 한개개인에서대사증후군구성요소들의 군집성과도연관성이있음을알수있었다. 비록정상 범위일지라도증가된백혈구수는대사증후군의또다 른특징이될수잇다는것을보여주었다.
