Unlike penicillin, cephalosporin haptens and determinants have not yet been fylly identified. Therefore, usually the whole antibiotics are usually used as skin test reagents, diluted at nonirritating concentration in attempts to detect the presence of IgE antibodies to cephalosporins. Skin testing with the reagents above has not yet been proven to be reliable for predicting hypersensitivity reactions. Thus, the usefulness of skin testing before administration of the drugs is controversial. In Samsung Medical Center (SMC), skin tests have been generally performed before administrating the cephalosporins. Therefore, this study is aimed to investigate current statuses of cephalosporin skin testing and supplementary clinical evidences. 2 JKSHP, VOL.30, NO.4 (2013) ting were included and electronic medical records (EMR) of them were retrospectively reviewed. 13 injected cephalosporins were involved. Cephalosporininduced reactions were distinguished based on three causality assessment criteria, and IgEmediated reactions were defined as reactions developed within 72 hours after administration of the drugs. Among the total of 1462 skin tests performed, 21 cases were reported to be skin test positive and 1441 cases were reported to be skin test negative. In 21 cases of positive skin tests, all the patients didn't receive cephalosporins according to the skin test guidelines in SMC. Among skin test negative cases, the patients were challenged with the drugs in only 1413 cases. And it was found that 32 (2.26%) of them presented possible IgEmediated reactions despite the negative skin test results. Symptoms of IgEmediated reactions were usually mild and selflimited or subsided promptly without stopping administration of the drugs; no cases of lifethreatening anaphylactic reactions had occurred. Only in 5 cases, cephalosporins were being discontinued for treating allergic reactions, and 2 (0.14%) of them were assessed to be specifically highly probable. In conclusion, the present study described the prevalence of cephalosporin skin test results and suggested the incidence of false negative reactions in skin testnegative group. Unfortunately, we could not obtain the information on false positive reactions in the skin testpositive group. If it was feasible to administer drugs after positive skin tests and follow up on whether the hypersensitivity reactions really occur, additional conclusions could be derived. However, in respect of patient's consents, it may still be difficult to administer drugs in those patients. Therefore, at this point of time skin tests have been performed routinely, the reported data would help to enhance the less known clinical information on cephalosporin skin tests. [Key words] Cephalosporin, Skin test, Hypersensitivity Cephalosporin은 βlactam계에속하는항생제로그람양성균과그람음성균을모두포함하는광범위한항균효과를가지며, 효과대비약물이상반응이적어널리사용되고있다. 흔히호흡기감염치료나수술시감염예방을목적으로사용되며, 그약물사용량은증가하는추세이다. 1)3) 모든 βlactam계항생제는 βlactam 구조에대한특이적면역반응에의해알레르기반응을유발할수있는데, 4)8) 항생제알레르기에대한진단적도구는제한적이다. 1) 병력청취는단순질문에그치는경우가많아신뢰도가떨어질수있고, 항생제특이적 immunoglobulin E(IgE) 에감작된비만세포의존 재를확인하는검사인 skin test는아직 penicillin 에대해서만표준화되어있다. 1) Penicillin은체내분해후안정한포합체 (conjugate) 를형성하므로알레르기의유발원이되는주결정기와부결정기가규명되어있으며, 9),10) 이에따라적절한 skin test용시약과이들을이용한 skin test 시행의유용성이잘정립되어있다. 11) Cephalosporin도과민반응을유발하는것으로보고되어있으나, 그발생률은 penicillin에의한경우의약 1/10 정도로비교적흔하지않다. 11),12) 그러나 penicillin과마찬가지로치명적인 IgE 매개반응을유발하는것으로알려져있으며, 13) % 의 344

3 박지은 : 세팔로스포린계항생제에대한 Skin Test 현황분석 anaphylaxis 발생률이문헌에의해보고된바있다. 14),15) Cephalosporin에대한알레르기반응의발생위험은 13% 정도로측정되며, 12),13) penicillin에대한알레르기병력이있는환자에서는그발생위험이훨씬높다. 13) Penicillin에대한알레르기병력이없는환자에서 cephalosporin 알레르기발생 (primary cephalosporin allergy) 의정확한빈도는알려져있지않으며, 16) cephalosporin의사용이증가함에따라 cephalosporin에의한알레르기발생빈도도증가하고있다. 2) Cephalosporin에서면역반응을일으키는알레르기유발원은 penicillin에비해다양하다. Penicillin과 cephalosporin의교차반응에서알수있듯이 βlactam 고리에의해알레르기반응이일어나기도하지만, 1),2),12),16) 대부분의알레르기반응은 R group 곁가지 (side chain) 에의해발생한다. 11) 또한 cephalosporin은체내분해후생성되는포합체 (conjugate) 가구조적으로안정하지않아다양한면역원성의합텐 (hapten) 을형성한다. 9)11),17) In vitro 연구에의하면곁가지 (side chain) 의일부분에서부터 cephalosporin 전체분자에이르기까지 cephalosporin과반응하는 IgE의광범위한특수성 (specificity) 이보고되어있다. 2) 이와같이 cephalosporin은 penicillin에비해알레르기유발원에대한이질성 (heterogeneity) 이크며, 이들알레르기유발원의대부분은규명되어있지않다. 따라서원약을비자극적인농도 (nonirritating concentration) 로희석한용액을이용하여 skin test를실시한다. 2),9),18)20) 원약을희석한 skin test의양성결과가약물특이적 IgE 항체의존재를시사할수있음이여러문헌에언급되어있으며, 13),17),21) skin test 양성결과가즉시형반응 (immediate reaction) 진단에유용할수있음이보고된바있어 2) 일부전문가들은 skin test 양성결과의유용성에대해동의하고있다. 22,23) 그러나 skin test 시약으로사용되는 cephalosporin 희석액은그자체가완전한항원이아니므로 21) 시약의적절성에대해논란이있으며, 2) 원약을희석한 skin test의음성결과를통해과민반응발생의위험을완전히배제할수없으므로 skin test의임상적가치 가제한된다. 17),21) 또한 skin test의양성예측치 (positive predictive value) 및음성예측치 (negative predictive value) 에대해잘알려져있지않으며, 21) 검사의민감도에대한대규모연구나확립된자료가없다. 3) 삼성서울병원에서는현재 cephalosporin 투여전 skin test를일반적으로실시하고있으며, 이는원내전산프로그램에등록된 protocol에따라시행되고있다. 본연구는과민반응발생예측에대한 skin test의유용성이정립되어있지않은현실정 11) 에서삼성서울병원입원환자에게실시된 skin test를대상으로그현황을파악하여본원의실증적자료를확보하고, 이를 skin test 시행지침정립에이용하고자하였다. 연구방법 1. 연구대상및기간 1) 대상약물현재삼성서울병원에서사용하고있는주사용 cephalosporin 항생제는총 13가지로 cefazedone sodium, cefazolin sodium, cefoxitin sodium, cefminox sodium, cefotiam hydrochloride, cefotetan, cefuroxime, ceftazidime, ceftriaxone sodium, cefotaxime sodium, ceftizoxime sodium, flomoxef sodium, cefepime hydrochloride를포함한다. 이약물에대해투여전 skin test가시행된건을대상으로하였다. 2) 연구기간설정을위한사전조사본연구에앞서전자의무기록 (EMR, electronic medical record) 을통해과민반응증상발생여부를검토하기에적절한기간을설정하고자 1년간의자료에대해사전조사하였다. 2011년 6월 1일부터 2012년 5월 31일까지삼성서울병원입원기간중주사용 cephalosporin계항생제에대해실시된 skin test를대상으로실시건수및양성 음성결과건수 345

4 JKSHP, VOL.30, NO.4 (2013) 를분석하였다. 21) 1년간 Skin Test 실시건수및양성 음성결과건수사전조사결과현재삼성서울병원에서사용중인 13가지주사용 cephalosporin계항생제를대상으로 1년간의사전조사기간동안총 29,063건의 skin test가시행되고있었으며, 이중 551건 (1.9%) 에서양성결과를나타내었다. 13가지성분중에서는 cefotiam hydrochloride가 7.2% 로양성결과의빈도가가장높았으며, flomoxef sodium이 0.38% 로양성결과의빈도가가장낮았다 (Table 1). 3) 연구대상및기간사전조사결과를토대로 2012년 5월 1일부터 5월 18일까지삼성서울병원입원기간중 cephalosporin skin test를시행한 1,474건을대상으로하여전자 의무기록을확인하였다. 연구대상인 1,474건중다음의제외기준에포함된 12건을제외하고, 총 1,462건을대상으로연구를진행하였다. 가. 1회의 skin test 결과를 2회기록한경우나. 동일한투여건에대해 skin test를 2회실시한경우다. 응급실을경유하여입원한후, 응급실에서실시한 skin test 결과를단순재기록한경우라. 정보보호요청으로인해전자의무기록확인이불가한경우 skin test 시행여부는전자의무기록의사처방화면의시행기록을토대로하였으며, 각건에서 skin test 후약물투여시과민반응발생여부를조사하기위해 skin test를시행한입원기간동안의경과기록및간호기록을검토하였다. 과민반응의과거력여부에대해서는 skin test 시행환자의모든입원기록을검토하여가능한정보를최대한수집하고자하였다. 5 박지은 : 세팔로스포린계항생제에대한 Skin Test 현황분석 2. 연구방법 1) 자료수집연구대상의전자의무기록 (EMR, electronic medical record) 을후향적으로검토하였으며, skin test 결과, skin test 시행후 cephalosporin의실제투여여부, 투여시과민반응증상의발생여부및해당환자의과거력기록여부에대한자료를수집하였다. 과거력여부의판단은 βlactam계항생제 에대한과민반응병력의기록이있거나 βlactam계항생제에대한 skin test 양성결과의기록이있는경우를기준으로하였다. βlactam계항생제 : penicilin, cephalosporin, carbapenem, monobactam 2) Cephal ospor i n Ski n Test 현재삼성서울병원에서시행중인 cephalosporin skin test는원내의약품집에수록된 skin test protocol에따라시행되고있다. Cefotiam의경우 skin test kit가상품형으로존재하므로이 kit를사용하거나상응하는희석농도인 300 mcg/ml로원약을희석하여 skin test를실시하며, 나머지 12가지성분의경우 cephalosporin의일반적인비자극농도로알려진 3 mg/ml 17) 로약물을희석하여검사한다 (Table 2). 3) 과민반응증상과 Cephalosporin 의인과성평가각 skin test건에대한전자의무기록검토결과, cephalosporin 투여후과민반응증상발생에대한기록이있었던경우에는인과관계를보다명확히하기위해인과성평가를실시하였다. WHO 약물부작용평가기준 (Fig. 1), Naranjo Adverse Drug Reaction Probability Scale (Fig. 2), 한국형알고리즘 ver 2.0 (Fig. 3) 의세가지기준을적용하였으며, 이는현재삼성서울병원에서약물유해반응평가 Table 2. WHO 부작용평가기준에의해확실함 (certain) 또는상당히확실함 (probable/likely) 또는가능함 (possible) 으로평가된경우나. Naranjo ADR Probability Scale에의해명 백히관련이있음 (definite) 또는상당히관련이있음 (probable) 또는관련이있을가능성이있음 (possible) 으로평가된경우다. 한국형알고리즘 ver. 2.0에의해확실함 (certain) 또는가능성높음 (probable/likely) 또는가능성있음 (possible) 으로평가된경우또한다음의세가지조건을모두충족하는경우인 348

WHO 부작용평가기준에의해확실함 (certain) 또는상당히확실함 (probable/likely) 으로평가된경우나. Naranjo ADR Probability Scale에의해명백히관련이있음 (definite) 또는상당히관련이있음 (probable) 으로평가된경우다. 한국형알고리즘 ver. 2.0에의해확실함 (certain) 또는가능성높음 (probable/likely) 으로평가된경우 시형 (immediate type), 1시간초과 72시간이내에발현한경우를가속형 (accelerated type), 72시간초과후발현한경우를지연형 (delayed type) 과민반응으로정의하였다. 이중 skin test를통해예측가능할것으로기대되는 IgE 매개과민반응은즉시형 (immediate type) 과가속형 (accelerated type) 을포함하는것으로, 24),25) 즉약물투여후 72시간이내증상이발현된경우로정의하였다. 25) 결과적으로, 본연구에서 cephalosporin에의해발생한 IgE 매개과민반응이란 cephalosporin과의인과관계가능성이있는것 (A) 으로평가되면서약물투여후 72시간이내에증상이발현된과민반응을일컫는다. 4) I ge 매개과민반응발생의정의 5) 음성예측치 (NPV, Negative Predictive Value) 인과관계의가능성이있는것 (A) 으로평가된과민반응발생건을증상발현시점에따라세종류로분류하였다. 증상발현시점은구체적언급이없는한, 증상이최초로기록된시점을기준으로하였다. 약물투여후 1시간이내에과민반응증상이발현한경우를즉 음성예측치를구하기위해아래식을이용하였다. 349

Previous history of allergic reactions to βlactam antibiotics or skin test positive results with these drugs. * Whole antibiotics were used as skin test reagent, diluted in saline solution at nonirritating concentrations. 연구결과 1. 연구대상의특성 2012년 5월 1일부터 5월 18일까지삼성서울병원입원기간중시행된 cephalosporin skin test 1,474건중제외기준에포함된 12건을제외한 1,462 건이최종적으로연구에포함되었다. 연구대상으로포함된 skin test 시행건에서환자의 성별분포는남자 758명 (51.85%), 여자 704명 (48.15%) 이었다. 연령의중앙값은 54세 (range: 0 91) 로, 모든연령층을포함하였으며, 이중 30건 (8.89%) 은만 18세미만의환자에게시행된경우였다. 앞서연구방법에서정의한과거력에대한기록이있는경우는단지 32건 (2.19%) 이었다. 투여된 cephalosporin 성분은 cefotetan (403건, 27.56%), cefazedone (241건, 16.48%), ceftriaxone (224건, 15.32%) 순으로많은것으로분석되었 350

Skin Test 결과및투여여부 1) Skin Test 결과총 1,462건의 skin test 시행건중 21건 (1.44%) 에서양성결과가나타났다. 21건의양성결과에포함된 cephalosporin 성분은 cefazedone, cefotiam, cefotetan, ceftizoxime 이었으며, 이들의성분별 351

Skin test 음성결과를보인 1,441건의경우, 투여전치료계획변경이나, 환자거부등의사유로투여되지않은 28건 (1.94%) 을제외하고 1,413건 (98.06%) 에서약물의투여가진행되었다 (Fig. 4). 3. Skin Test 음성결과후투여한경우에대한결과분석 352

Skin test 음성결과후약물의투여가진행된 1,413건을대상으로과민반응발현여부와그양상을전자의무기록을통해후향적으로검토하였으며, 이를토대로음성예측치에대한결과를도출하였다. 항생제의투여기간은평균 3.7일이었다. 1) 과민반응증상발현및인과성평가 Skin test 음성결과후약물의투여가진행된 1,413건의전자의무기록을검토한결과, 61건에서 약물투여시점이후의의무기록에과민반응증상발현과관련된내용이기재되어있었다. 따라서 61건에대하여증상의초기발현시점과병용약물및기저질환등을추가적으로검토하였고, 이를토대로세가지기준에근거하여투여된 cephalosporin과기록된과민반응증상의인과성평가를실시하였다 (Table 5). 인과관계의가능성이있는것 (A) 으로평가된경우는 32건 (2.26%) 이었으며, 이중인과관계의가능성이높은것 (B) 으로평가된경우 2건 (0.14%) 이었다 (Table 6). 353

12 JKSHP, VOL.30, NO.4 (2013) 2) Cephalosporin 에의한과민반응발현양상인과성평가를통해, cephalosporin에의한인과관계의가능성이확인된과민반응발생건은모두 32 건 (2.26%) 이었다. 32건모두약물투여후 72시간이내에증상이발생하여 IgE 매개과민반응으로분류되었다. 이중 3건 (0.21%) 은약물투여후 1시간이내에증상이발현하여즉시형 (immediate type) 과민반응으로분류되었으며, 나머지 29건 (2.05%) 은약물투여후 72시간이내에증상이발현하여가속형 (accelerated type) 과민반응으로분류되었다. 가속형 (accelerated type) 과민반응으로분류된 29건중26건은약물투여후1시간초과24시간이내에증상이발현한경우였다. 나머지 3건은명확한증상발현시점 ( 혹은증상의최초기록시점 ) 에대한정보없이발생일에대한기록만포함하고있었으므로, 72시간이내증상이발현되었다는사실만확인가능하여가속형과민반응으로분류하였다. IgE 매개과민반응 32건중 29건은피부증상으로 urticaria, itching, rash 등의증상을포함하고있었다. 증상은대부분경미했으며, anaphylaxis와같은심각한과민반응은보고되지않았다. 대다수의경우에서약물투여를유지하였고, 적절한처치후증상이회복되거나증상이자연소실된것으로보고되었다. 32건중증상에대한조치로약물의투여를중단했던경우는 5건이었다. 5건중 3건은약물에 의한이상반응이진단명에도명시되어있었던경우로증상회복에비교적많은시간이소요된경우였지만, 병용약물의영향도의심되어 cephalosporin과의인과성은비교적높지않았다. 나머지 2건은세가지인과성평가기준에의해인과관계의가능성이높은것 (B) 으로평가되었던경우였으며, 이 2건중 1 건에서는증상발현약 2주후재실시한 skin test( 동일성분 ) 에서양성결과를보였다. 3) Skin Test 음성예측치 Skin test 음성결과에도불구하고약물의투여후인과관계의가능성이있는 IgE 매개반응이발생한 32건 (2.26%) 을위음성 (falsenegative) 으로정의하여음성예측치를구하였다. 이에따라 cephalosporin skin test 음성결과는과민반응발생에 97.74% 의예측치를가지는것으로도출되었으며, 이를과거력의유무에따라나누면과거력의기록이있는경우 93.55%, 과거력의기록이없는경우 97.83% 의음성예측치를가진다 (Table 7). 4) Cephalosporin 성분별 IgE 과민반응발생연구방법에서정의된 IgE 매개반응발생건에포함된 cephalosporin 성분에는 cefazedone, cefazolin, cefoxitin, cefotiam, cefotetan, cefurox Table 7. Cefminox, cefepime 등과같이위음성이보고되지않거나, cefoxitin, cefminox, ceftazidime 등과같이성분별 skin test 시행건수가적었던성분의경우, 이결과만으로위음성률을판단하기에는제한점이있었다 (Table 8). 고찰 βlactam계항생제인 cephalosporin은비교적흔 하지않게과민반응발생이보고되어있으며, 사용량증가에따라과민반응발생빈도도증가하고있는추세이다. 2),11),12) Cephalosporin의과민반응유발원은다양하고대부분규명되어있지않기때문에적절한 skin test용시약이없으며, 1),2),9),10),12),13),16),17) 원약을희석하여 skin test를실시하고있다. 9),18)20) 그러나 skin test의과민반응예측에대한유용성이아직정립되어있지않아그시행에논란이있으며, 기관마다상이하긴하지만 anaphylaxis와같은치명적 IgE 매개과민반응의발생을우려하여관행적으로항생제투여전 skin test를실시해오고있는실정이다. 26) 현재삼성서울병원에서는일반적으로 cephalosporin 투여전 skin test를시행하고있다. Skin test의유용성은 penicillin에대해서만정립되어있으며, 1) cephalosporin의경우 penicillin외 βlactam계항생제중에서는비교적연구결과가많 355

Penicillin 과민반응의병력이있는환자 101명을대상으로했던한연구에서 cephalosporin skin test 음성결과후약물을투여했을때과민반응이한건도발생하지않았으며, 이는음성예측치 100% 에상응하는결과였다. 27) 또다른연구에서는민감도 % 의결과가보고되었다. 28),29) 그러나이들은모두소규모연구였으며, cephalosporin skin test의양성 음성예측치및민감도에대한대규모연구나확립된자료는현재부족한실정이다. 26) 또한 skin test 양성결과가약물특이적 IgE 항체의존재를의미할수있고, 13),17),21) 즉시형과민반응의진단에유용할수있다는보고가있는반면, 2) skin test 음성결과는과민반응발생의위험을완전히배제하지못하므로임상적가치가제한됨이알려져있다. 21) 현재 cephalosporin의국내허가사항에따르면 쇽등의반응을예측하기위해충분히문진하고사전에피부반응시험을실시하는것이바람직하다. 는일반적주의사항이기재되어있으나, cephalosporin 과민반응을예측하는피부검사나혈청검사는없다고언급하는문헌도있다. 1) 본연구결과 skin test 음성결과후약물이투여된 1,413건중 32건 (2.26%) 에서 cephalosporin에의한인과관계의가능성이있는 IgE 매개과민반응이발생했다. Cephalosporin에의한피부과민반응은 13% 의발생률이보고되어있으며, 12) IgE 매개반응중 anaphylaxis는 % 의발생률이보고되어있다. 14),15) Cephalosporin에의한 IgE 매개과민반응의발생률은별도로보고되어있지않으며, 이에따라기존의문헌에보고된과민반응의발생률을참고하여본연구의결과에대해고찰하였다. 본연구에서발생한 32건의 IgE 매개과민반응중 29건이피부증상이었던점과 Norrby SR. 의연구 12) 에서보고된 cephalosporin에의한피부과민반응발생률 (13%) 을고려해볼때, 본연구에서 skin test 음성결과후발현된 IgE 매개과민반응의발생률 (2.26%) 은 cephalosporin에의한 IgE 매개과민반응의유병률 (skin test 시행과무관 ) 과유사한수준인것으로판단된다. 즉 skin test 음성결과후약물을투여하더라도유병률보다낮은 IgE 매개과 민반응의발생을보장하지못할것으로생각되며, 이는 skin test 음성결과가과민반응발생의위험을완전히배제하지못한다는기존의문헌보고 21) 내용에부합한다. Penicillin skin test에대한대규모연구에의하면 skin test 음성결과를보인환자에게 penicillin 을투여했을때 13% 에서과민반응이발생하였으며증상은대개경미하고자연회복되었다는결과가있다. 24),25),30)35) 이결과는본연구의결과와유사하지만, 여기서 penicillin과 cephalosporin의과민반응유병률이다름을인식할필요가있다. Penicillin은 cephalosporin보다과민반응을약 10배정도더잘유발하는것으로알려져있으며, 치명적 IgE 매개반응인 anaphylaxis에있어서도 % 의발생률이보고되어있다. 15) 본연구에서도출된 cephalosporin skin test 음성예측치 (97.74%) 는 penicillin skin test에대해문헌등에서보고된음성예측치 (9799%) 와그값이유사한것으로보인다. 그러나 cephalosporin의과민반응발생률이 penicillin에비해상당히낮은점을고려할때, 본연구결과에서도출된음성예측치가가지는실제임상적가치는 penicillin skin test의임상적가치에는미치지못할것으로판단된다. Skin test 음성결과후약물투여시발생한 cephalosporin에의한 IgE 매개과민반응 (32건) 은대부분경미한피부증상이었다. 과민반응증상으로인해약물투여를중단했던경우는단지 5건이었으며, anaphylaxis와같은치명적인반응은나타나지않았다. 즉, 본연구에서 skin test 음성결과후약물투여시발생한 IgE 매개과민반응의증상은대부분심각도가낮았다. 여기에는 cephalosporin에의한 anaphylaxis 발생률이매우낮은점이영향을미쳤을가능성이있다. 만일 skin test 양성결과후약물이투여된군과 skin test 음성결과후약물이투여된군에서발생한과민반응증상을비교하는연구가가능하다면, 과민반응심각도에대한 skin test 음성결과의기여도를명확히판별하는데도움이될것이다. 본연구에서 21건 (1.44%) 의 skin test 양성결과가있었지만, skin test 양성결과후약물이투여되지 356

15 박지은 : 세팔로스포린계항생제에대한 Skin Test 현황분석 않았으므로투여후발생할수있는과민반응에대한정보는확인할수없었다. 그러나 skin test 양성결과의발생빈도에대한자료를확보할수있었다는점에는의미가있다고하겠다. 본연구는연구기간이짧아 13가지성분중 4가지성분에서만 skin test 양성결과가보고되었으므로, 성분별 skin test 양성결과의발생빈도를포괄적으로파악하기위해서는 1년간의자료를포함하고있는사전조사의내용을참고할수있다 (Table 1). 흥미롭게도현재유일하게 skin test kit가존재하는 cefotiam에서연간 7.02% 의높은양성결과발생이보고되었는데, kit 사용이 skin test 시약조제과정상의오차를감소시킴으로써높은발생률에영향을미쳤을가능성등을생각해볼수있다. 연구결과에서 skin test 음성결과후약물투여시 IgE 매개반응이발생한건에대하여 2주후다시 skin test를실시하였을때양성결과가보고된 1건의사례가있었다. 이과민반응사례는인과성평가에의해 cephalosporin과인과관계의가능성이높은것 (B) 으로평가되었고, 24시간이내증상이발현하여 IgE 매개반응으로분류되었다. 따라서본사례는 skin test 양성결과가즉시형과민반응의진단에유용할수있다는기존의보고 2) 를뒷받침한다고할수있겠다. 본연구에서양성결과에대한결론을도출할수는없었지만, 현재까지알려진양성결과의유용성에대한보고를수용하여양성결과의유용성을가정한다면, 다음과같은분석을고려할수있다. 각 cephalosporin 성분별 skin test 양성결과의발생률 (Table 1) 을, skin test 음성결과에도불구하고약물투여시발현된과민반응의발생률 ( 성분별위음성률 )(Table 8) 과비교함으로써, skin test의유용성을판단하는것에대한가능성을생각해볼수있다. 예를들어, cefotiam의경우 skin test 양성률은비교적높은반면 (7.02%), skin test 음성결과후약물투여시과민반응발생률 ( 위음성률 ) 은비교적낮아 (1.22%) skin test 시행의이점이좀더클것으로보인다. Cefazolin의경우 skin test 양성률 (0.96%) 은비교적낮은반면, skin test 음성결과후약물투여시과민반응발생률 ( 위음성률 ) 은비교적 높아 (3.81%) skin test 시행의유용성이감소한다고볼수있다. 하지만이러한분석은모든성분에대해충분한건수가확보되지않은상태에서가능성만제시한것으로명확한결론도출을위해서는추가적연구가필요할것으로판단된다. 본연구에서 skin test 양성결과를보인 21건 (1.44%) 은 skin test 양성결과가나올경우약물을투여하지않는원내지침에따라모두투여되지않았다. 따라서 skin test 양성결과후약물이투여될경우발생할수있는과민반응의정보는확인할수없었다. Penicillin skin test의유용성을논한대규모연구에서는 skin test 양성결과를보인환자중대체약이없는불가피한경우에대해 penicillin을투약한후과민반응발생여부를조사하여, 이를 skin test 음성결과후약물이투여된군과비교한바있다. 24),25) 그러나본연구에포함된대상중에는위연구처럼예외적인사례가없었으므로 skin test 음성결과후약물이투여된군과 skin test 양성결과후약물이투여된군을비교할수없었다. 이와같은비교를통해 skin test 유용성에대한종합적결론이도출될수있다면, cephalosporin skin test 시행여부가의료기관마다상이한현실정에서 skin test 시행과관련된업무개선에활용할수있는객관적근거자료가될수있을것이다. 또한이미작성된기록에산발적으로존재하는자료를후향적검토를통해수집하였으므로정확한정보획득에제한이있었다. 과거력의유무는실제임상에서 cephalosporin에의한과민반응위험의판별에중요한부분을차지한다. 그러나본연구에서실제로 βlactam계항생제에대한과민반응과거력의기록이있었던경우 (βlactam계항생제에대한 skin test 양성결과의기록이있는경우포함 ) 는전체 1,462건중 32건 (2.19%) 에불과했으며, 이는기록누락의가능성을시사한다. 따라서과거력기록의유무가실제과거력유무를의미하지않을가능성을고려하여, 과거력과관련된위험인자분석을시행하지않았다. 또한증상에대한최초기록이있던시점을증상발현시점으로간주했지만, 투여시점부터추적관찰하여작성된기록이아니므로여기에도누락의가능성이있다. 일반적으로 IgE 매개과민반응은 357

16 JKSHP, VOL.30, NO.4 (2013) 투여후 1시간이내발현된과민반응증상으로정의되어있지만, 본연구는이같은제한점을염두에두고가속형 (accelerated type) IgE 매개반응의범주도포함하여 72시간이내발현한증상을기준으로하였으며, 실제로 1시간이내에최초증상발현의기록이있었던경우는 3건에불과했다. 또증상에관한기술은주로간호기록에만포함되어있었으므로정확한진단정보가불충분하였기에, maculopapular rash 등의특징적증상으로지연형 (delayed type) 과민반응을분류하지는못했다. 향후의료진과협력하는전향적연구를통해투여시부터추적관찰하여정확한증상발현시점및진단에대한정보를얻고, 병력기록의검토뿐만아니라환자본인으로부터병력을청취함으로써이부분에대해보완할수있을것이다. 본연구는시행여부에대한논란이있고정립이부족한실정에서실시되고있는 cephalosporin skin test의시행건수및결과등의원내현황을파악했다는점에의미가있다. 현재삼성서울병원에서는원칙적으로 cephalosporin 투여전 skin test를시행하고있으므로, 사전조사및본연구에서보인항생제성분별 skin test 시행건수의차이는각항생제의처방률에따른것이다. 따라서연구결과에서보인전체 cephalosporin skin test에대한결과는실제적현황을반영한다고할수있다. 수술과관련된진료과에서의 cephalosporin 처방률이높았던점은수술전예방적목적으로사용된경우가많았음을반영한다. 아울러 skin test 음성예측치에대한결과를얻고, 음성결과후투여되었을때발생한증상의심각도가낮음을확인함으로써, 잘알려지지않은 cephalosporin skin test에대한임상적자료를본원의실증적연구결과로써보완할수있었다. 본연구결과가비록음성결과에대한자료만을포함하고있으나, skin test가시행되고있는현상황에서는 skin test 결과해석에유용한참고자료가될수있을것으로판단된다. 향후의료진의집중관찰하양성결과후에도투여하고관찰하는전향적연구를수행한다면 skin test 유용성에대한추가적판단이가능할것으로판단된다. 결론 Cephalosporin은과민반응발생이흔하지않게보고되어있으며, 과민반응의유발원이규명되지않아원약을희석하여 skin test를실시하고있다. 그러나검사결과와과민반응예측의연관성에대한정립이부족하여 skin test의효용성에대해논란의여지가있다. 이같은상황에서실시되고있는 cephalosporin skin test의현황을파악함과동시에부족한임상적자료를본원의실증적자료로써보완하고, 현시점에서 skin test 시행의유용성을재조명하고자하였다. 총 1,462건의 skin test 시행건중 21건 (1.44%) 에서양성결과를보였으며, skin test 양성결과를보인경우원내지침에따라전부투여하지않았다. Skin test 음성결과를보인 1,441건중 1,413건에서 cephalosporin이투여되었으며, 1,413건중 32 건 (2.26%) 에서 cephalosporin에의한인과관계의가능성이있는 IgE 매개과민반응이보고되었다. 인과관계는 3가지인과성평가기준을통해확인되었고, IgE 매개과민반응은약물의투여후 72시간이내증상이발현한경우로정의되었다. 문헌에보고된 cephalosporin에의한피부과민반응의유병률 (skin test와무관 ) 이 13% 임을고려해볼때, skin test를시행하여음성결과후약물을투여하는것이 cephalosporin 과민반응으로부터의안전을보장하지않는다는것을알수있었다. 연구에서도출된 cephalosporin skin test 음성예측치 (97.74%) 는유일하게 skin test가정립된것으로알려진 penicillin skin test에대해보고된음성예측치와유사한것으로나타났다. 하지만 cephalosporin 과민반응의유병률이 penicillin에비해상당히낮은점을고려할때, cephalosporin skin test의음성예측치의의미는 penicillin skin test의음성결과가가지는임상적가치에는미치지못할것으로판단된다. 또한 skin test 음성결과후약물투여시발생한 IgE 매개과민반응의증상은대부분심각도가낮은피부반응이었다. 본연구는 skin test 시행의원내현황을파악함과동시에 skin test 음성결과에대한임상적자료를확보한데의의가있다. 하지만 skin test 양성결과 358
