This study was performed to identify genetic variations in the proximal promoter region of OCT2. Methods:The promoter region of OCT2 was amplified and directly sequenced from genomic DNA samples from individuals of diverse ethnicities(n=272). The promoter activity of OCT2 was measured using a luciferase reporter assay in two cell lines ; ACHN, and HCT-116. Results:There were four polymorphic(minor allele frequency >1% in at least one of the four ethnic groups) variants in the OCT2 promoter region(-250/+60 from the transcription start site). One(V2, g.-246c>t) of them showed decreased reporter activity by 38%(p < 0.001), whereas another one(v4, g.-47c>t) showed increased activity by 10%(p < 0.05), compared to the reference in HCT-116 cell line. Conclusion:This study revealed that novel promoter variants of OCT2 results in changes in transcriptional activity of this gene. These variants can potentially affect the pharmacokinetics or drug response of many drugs that are substrates of OCT2. KEY WORDS: OCT2 ㆍ Genetic variation ㆍ Promoter ㆍ Transporter. 서 론 신장은간장과더불어약물의체외배출에중요한역 교신저자 : 최지하, 서울양천구목동 이화여자대학교의과대학약리학교실전화 :(02) ㆍ전송 :(02) Jihachoi@ewha.ac.kr 할을하는기관으로양이온을띠는약물들은신장에서여과및능동수송의과정을거쳐서제거된다 1). 능동수송이일어나기위해서는약물의혈액에서신장세뇨관쪽으로의이동이필요한데, 이때의약물이동은농도및전기화학적경사에반하는것이므로수송체의도움이필요하다. 유기양이온수송체 (organic cation transporter, OCT) 중의하나인 OCT2는주로신장의근위 - 9 -

2 세뇨관에발현되어있으면서이와같은약물의능동수송, 궁극적으로는약물의신장을통한배설에중요한역할을한다 2)3). 선행연구에따르면 metformin, amantadine, cimetidine, quinine 등의다양한약물들이 OCT2 의기질 (substrate) 로, 이수송체의도움을받아체내에서제거되는것으로알려졌다 2). 같은약물을동일한용량으로투여하는경우대부분의사람들에서는비슷한치료효과를얻을수있지만일부사람들에서는효과가나타나지않거나심각한부작용이나타난다. 예를들어미국의경우입원환자의약 5% 에서약물부작용이나타나며연간 10만명이상의사람들이이로인해사망한다 4). 이와같이약물에대한반응이개체간에다르게나타나는이유로는환자의나이, 몸무게, 성별, 영양상태등의환경적인요인과더불어유전적인요인을고려할수있겠다. 최근인간게놈에대한광범위한염기서열분석진행으로많은약물수송체의단일염기다형성 (single nucleotide polymorphism, SNP) 이밝혀지고있다. SNP는수송체의발현, 기질친화도등에영향을줄수있고이로인해수송체의기능은 wild type과비교했을때감소하거나증가할수있다 5). OCT2는신장에서약물을운반하는역할을하므로이수송체의기능변동은약물의약동학에변화를일으킬수있고, 치료효과, 부작용등에도의미있는영향을줄수있다. 또한 OCT2는체내신경전달물질운반에관여하므로항상성유지에도중요한역할을할것이다 6). OCT2의유전자변이에대한연구는최근에활발히이루어져현재까지약 40여개의유전자변이가밝혀졌다 7). 이중 coding 부위에서는 16개의유전자변이가발견되었는데 8개의유전자변이가아미노산의변화를초래하는것이었다 8). 이러한 nonsynonymous 변이중에서도 OCT2의기능에미치는영향에대하여가장많은연구가진행된유전자변이는 OCT2 p.a270s이다. Chen Y 등 8) 에의한연구에따르면 270S 변이유전자의경우 in vitro 실험에서 OCT2의수송능을증가시키며, 건강한사람들을대상으로한임상시험에서도 270S 변이유전자가있는개체군 ( 아프리카인또는유럽인 ) 의경우 OCT2의기질중하나인 metformin 의신장내제거율이증가한다는것이밝혀졌다. 반면 Song IS 등 9)10) 은이와는반대로 270S 변이유전자는 in vitro 상에서 OCT2의수송능을감소시키며, 한국인을대상으로한 임상시험을통하여이변이유전자를가진군에서 metformin의신장제거율이감소한다고보고하였다. 같은변이유전자라도어떤변이유전자와일배체형 (haplotype) 을구성하고있느냐에따라궁극적으로유전자의기능에다른영향을초래할수있기때문에위와같이인종에따라임상시험결과가상반되게나오는것은가능한일이다. 하지만동일한조건에서시행된 in vitro 결과가상반된것에대한이유는아직까지밝혀지지않고있다. Coding 부위에대한유전자변이에대한연구는비교적활발하게진행되고있는반면, OCT2 포로모터부위의유전자변이에대해서는아직까지연구된바가거의없다. Ogasawara K 등 11) 은일본인을대상으로 OCT2 프로모터유전자변이를검색한결과 -578_-576delAAG 변이를찾았고 in vitro 실험결과이변이유전자가 OCT2의프로모터활성을감소시킨다는것을밝혔다. 본연구에서는다양한인종 ( 아프리카, 유럽, 아시아, 멕시코 ) 에서 OCT2 프로모터부위의새로운유전자변이를찾고변이유전자가 OCT2의프로모터활성에미치는영향에대하여알아보고자하였다. 대상및방법 1. 유전자분석 272명의혈연관계가없는건강한사람들로부터 (4군의인종 : 아프리카인, 유럽인, 아시아인, 멕시코인, 각각 68명 ) 얻은게놈 DNA을 autonomic sequencer 방법을이용하여 OCT2 근위프로모터부위 (-250부터 +60까지의염기쌍, 전사시작점기준 ) 의유전자변이를분석하였다. 각유전자변이위치번호는 OCT2 mrna(gen- Bank accession number;nm_003058) 서열을기준으로전이시작점으로부터나타냈으며, Antonarakis SE 등 12) 이제안한유전자변이명명법을따랐다. 2. OCT2 프로모터부위의변이유전자벡터제작 OCT2 프로모터를포함하는리포터플라스미드 (reporter plasmid) 를제작하기위하여게놈 DNA을주형으로중합효소연쇄반응 (PCR) 을시행하여 437 염기쌍의 OCT2 유전자를증폭하였다. 여기서얻은생성물을 NheI, HindIII효소로자른후 pgl4.11b [luc2] 벡터 (Invitrogen) 에삽입하였다. 변이유전자를포함하는클론은 wild type 클론을주형으로site-directed muta

3 genesis kit(stratagene) 을이용한중합효소연쇄반응을통하여얻었다. 본연구에서제작한모든클론은직접적인시퀀싱방법을통하여 DNA 서열을재확인하였으며, 각중합효소연쇄반응에사용된프라이머목록은 Table 1에명시하였다. 로부터얻어진평균 ± 표준편차로나타내었다. 이때각값은공벡터인 pgl4.11b [luc2] 의 luciferase 활성정도를 1로보았을때 reference(wild type, OCT2- PGL4.11b) 혹은변이클론의활성정도를상대적으로나타내었다. 통계분석은 Dunnett s multiple comparison test을이용하였으며, p값이 0.05보다작은경 3. Wild type 클론을두종류 (ACHN, HCT-116) 의세포에 transfection 시킨후 luciferase 활성을측정했을때 ACHN 세포에서보다 HCT-116 세포를이용했을때가 2.0배정도의높은활성을보였다 ((a), Fig. 1). 따라서변이클론에대한 luciferase 활성측정은 HCT-116 세포를이용하여시행하였다. 그결과본연구에서가장높은빈도를보인 g.-246c>t 변이유전자의경우 reference 와비교했을때 luciferase 활성이 38% 정도감소되었고 (p=0.0001), 또다른변이유전자 g.-47c>t는 10% 증가된 (p= ) luciferase 활성을나타내었다 ((b), Fig. 1). 고찰 OCT2는신장세뇨관세포의기저측면에많이발현되어있으면서다양한약물들의수송에관여한다 2)3). 따라서 coding 혹은 non-coding 부위의유전자변이에의한 OCT2의기능혹은발현변화는이수송체기질약물의농도변화를초래하게될것이다. 본연구는 OCT2 근위프로모터부위에서의새로운유전자변이를찾고각변이유전자의기능변화를알아보기위하여시행되었다. 다양한인종군의 DNA 검색을통하여총 4개의유전자변이를발굴하였다. 그러나최근 Ogasawara K 등 11) 이일본인을대상으로찾은 OCT2 프로모터유전자변이인 -578_-576delAAG 은본연구에서는발견되지않았다. 이는본연구에서사 용된아시아인 DNA는중국인으로구성된것으로, 유전자변이빈도는같은아시아인이더라도중국인과일본인간에차이가있을수있음을말해준다. 본연구에서가장높은빈도를나타낸변이인 g.-246c >T는 reporter assay 결과유의하게감소된프로모터활성을보였다. TFSearch 3.1(Real-World Company Partnership) 소프트웨어를이용한전사인자결합부위 (transcription factor biding site, TFBS) 분석결과 g.-246c>t 부위에 XFD-2라는전사인자가결합하며유전자변이유무에따라결합정도가다를것으로예측되었다. XFD(Xenopus fork head domain related)-2 는 Xenopus laevis embryo에서처음발견되었으며 HNF(hepatocyte nuclear factor)-3 와아미노산서열및 DNA 결합부위가유사한것으로알려졌다 13)14). 그러나아직까지이전사인자의기능에대해서는연구된바가없다. Wild type과비교했을때유의하게높은프로모터활성을보인 g.-47c>t 변이유전자의경우 TFBS 분석결과유전자변이유무에따라결합정도가달라지는전사인자가없었다. 이와같은소프트웨어를통한전사인자예측을검증하기위해서는젤지연분석법 (electrophoretic mobility shift assay, EMSA) 과같은추가적인분자생물학적실험이뒷받침되어야할것이다. 결론적으로본연구에서는새롭게발견된 OCT2 프로모터부위의유전자변이 g.-246c>t, g.-47c>t 가 OCT2 유전자의전사조절에관여한다는것을밝혔다

5 이와같은역할을하는변이유전자는궁극적으로 OCT2 기질약물들의약동학혹은약력학에영향을줄수있을것이다. 요 목적유기양이온수송체 2(OCT2, SLC22A2) 는신장의근위세뇨관세포에많이발현되어있으면서내인성또는외인성 ( 약물포함 ) 물질들의신장내제거에중요한역할을한다. 본연구는 OCT2의근위프로모터부위의유전자변이를찾기위하여시행되었다. 방법다양한인종으로구성된 272명의게놈 DNA을이용한직접적인시퀀싱방법을통해 OCT2 프로모터부위의유전자변이를검색하였다. 프로모터활성은 ACHN, HCT-116 세포를이용한 dual luciferase assay 기법을통하여측정하였다. 결과 OCT2 근위프로모터부위에서총 4개의유전자변이를발견하였으며모든유전자변이는다형성 ( 적어도한인종군에서마이너대립유전자빈도>1%) 을나타냈다. 프로모터활성을측정한결과 g.-246c>t 변이는유의하게감소된활성 (38%, p<0.001) 을보였고, 또다른변이인 g.-47c>t 는유의하게증가된활성 (10%, p< 0.05) 을나타냈다. 결론본연구는 OCT2유전자의전사를조절하는새로운유전자변이를밝혀냈다. 이러한유전자변이는궁극적으로 OCT2의기질인많은약물들의약동학혹은약물반응에영향을끼칠수있을것이다. 약 중심단어 : OCT2 유전자변이 프로모터 수송체.
