Materials and Methods: The subjects included in this study were 20 patients, matching the inclusion criteria of the current study, who received bone grafting treatment at Yonsei University Dental Hospital department of oral and maxillofacial surgery between march 2010 till September Postoperative cone beam CTs (Ray Co., Ltd./ Rayscan symphony, Seoul, South Korea) were taken for each patient at 1 and 6 months. All CBCT slices were fixed at 1.0mm intervals. The thickness of the grafted bone was measured at 1 and 6 months, by converting these images into Dicom files and using the Simplant software program (Materalise, Leuven, Belgium). Results: As the result, 85.45% of transplatanted biphasic calcium phosphatemass is maintained until six months and the average volume loss from 1-month to 6-month is 0.24 cc. Conclusion: We conclude that, the biphasic calcium phosphate has enough short-term stability as a scaffold until the ingrowth of new bone apposition. 2 I 서론 구강내골결손부에대한골이식시 gold standard는자가골이식임은주지의사실이다 1. 하지만추가적인수술의부담, 공여부의감염, 이식골의흡수는자가골이식의단점으로생각되어져왔다 2,3. 또한동종골, 이종골의이식도활발히연구되었으나, 종간의교차감염, 개체간감염전파, 항원-항체반응에대한우려를불식시키지는못하였다 1,4. 따라서이러한이식재들의대안으로합성골, 특히 calcium phosphate 제재는 1980년경부터임상에서많이쓰이고있는제제이다 5. Calcium phosphate는인체의골미네랄과같은화학적성분이며, 뛰어난생체적합성, 국소적 이온용출을통한골과의계면부착성을가져골이식재로써뛰어난장점이있다 1,6,7. 이러한 calcium phosphate 는그화학적성상, 즉 Ca/P의비율에따라여러가지종류를가진다. 가장널리알려진것은 Hydroxyapatite (HA) [Ca 10 (Po 4 ) 6 (OH) 2 ] 와 Beta-tricalcium phosphate (β-tcp) [Ca 3 (PO 4 ) 2 ] 이며, 화학적성상에따라체액용해도, 흡수율, 압축강도등의물리-화학적성질도달라짐이여러기초학연구들에서기술되고있다 7-9. Calcium phosphate에의한골생성기전이골전도임을고려할때, 신생골의생성시기까지비계역할을충분히할수있는체적안정성은골생성의중요한요소이다. HA는여러기초학적, 임상연구에서골이식후체내에서오랜기간흡수되지않고잔존되어이물반응, 만성염증등의부작용을가져온다고알려져있다 3,10. 반대로 β-tcp는신생골생성시까지비계의역할을하지못하고빠른시간에붕괴되는현상을보여주었다 3. 따라서최근위의두가지재료를화학적으로결합하여각각의장점을살릴수있는 Biphasic calcium phosphate가개발되었다. 즉 β-tcp의친수성과단기간에다량의칼슘이온 Fig. 1. 따라서본저자등은본과에내원한 20 명의환자의다양한구강내골결손부에국내에서개발되어식약청허가를획득한 BCP인 Osspol을이식하여단기간의체적안정성을검증하였다.

연구대상본연구는 2010년 3월부터 2010년 9월사이본대학병 원구강악안면외과에내원한환자중피험자선정기준에합당한 20명의환자를대상으로진행되었다.

임상시험의절차는연세대학교치과대학병원의료기기임상시험심사위원회 (IRB) 의승인을받아윤리적으로진행되었다. 2.

3 Clinical Articles 을용출하여골계면에서의부착을용이하게하지만빠른붕괴의단점을 HA와결합을통해보완하는개념이다. 하지만 HA, β-tcp의체적안정성에대한여러보고들이있었지만 BCP의체적안정성에대한임상보고는전무한실정이다. 따라서본저자등은본과에내원한 20 명의환자의다양한구강내골결손부에국내에서개발되어식약청허가를획득한 BCP인 Osspol을이식하여단기간의체적안정성을검증하였다. II 연구재료및방법 1. 연구대상본연구는 2010년 3월부터 2010년 9월사이본대학병 원구강악안면외과에내원한환자중피험자선정기준에합당한 20명의환자를대상으로진행되었다. 환자군의구성및수술명은다음과같다 (Figs. 1~3). 환자들은임상시험에참여함을고지받은후동의한자들로 Osspol 을이용하여인공뼈이식시술을받았다. 임상시험의절차는연세대학교치과대학병원의료기기임상시험심사위원회 (IRB) 의승인을받아윤리적으로진행되었다. 2. 연구방법수술당일국소마취후낭종의적출술, 상악동거상술후 200~500 micron의 Osspol granule 1.0~3.0 cc를골결손부에충전후 water-tight suture 를시행하였다. 수술후다음날상처부드레싱을시행후일주일뒤발사를진행하였다 (Figs. 4~6). 이후술후 1개월, 6개월후에내원하여 Conebeam CT (Ray Co., Ltd./Rayscan Fig. 4. 두번째방법은 Simplant progrm의 OMS module 의이용하여 1개월 CT 와 6개월 CT를동일한 CT 감쇠계수값으로 Segmentation 한후, 해부학적구조를중첩하여각각의 3차원모델의부피를 Voxel 을이용하여부피차를측정하는방법 (Method B) 으로진행되었다 (Fig. 10).

(Method A) 이사용되었다 (Figs. 7~9). 두번째방법은 Simplant progrm의 OMS module 의이용하여 1개월 CT 와 6개월 CT를동일한 CT 감쇠계수값으로 Segmentation 한후, 해부학적구조를중첩하여각각의 3차원모델의부피를 Voxel 을이용하여부피차를측정하는방법 (Method B) 으로진행되었다 (Fig. 10). 측정된부피계측치는 SPSS 18.0 프로그램을이용하여동일집단내의두시기간의모평균차이의유의성을검증하기위하여대응표본 t-검정통계분석을시행하였다. III 연구결과 Method A의방법으로 20명의환자의 CT 데이터를 Simplant progrm의 tracing tool을이용하여분석하였다 (Fig. 11). 분석된이식재의부피는통계처리하였다 (Table 1). 술후감염은 20명중 2명에서나타났으며, 그외특기할합병증은병발치않았다. Method B의경우중첩의정확도에따라부피차데이터의오차가크게발생하여본연구의통계처리에포함되지않았다. 158 Implantology Vol. 15 No. 2, 2011

IV 총괄및고찰 Calcium phosphate는골미네랄과같은화학적성분으로인하여검증된생체적합성, 국소적이온용출을통한 골과의계면부착성을가진다 1,6,7. 이식시골대체의기전은일반적으로골전도의기전을따른다고알려져있다 3.

(OssPol) in Jaw Bone Defect: 3-Dimensional Volumetric Study by Simplant Software. * 잔존이식재백분율 (%)= 6개월이식재부피평균 /1개월이식재부피평균 100 자료를분석시 20명의환자에서 6개월까지평균적으로 85.45% 의이식재가붕괴되지않고, 유지되고있었다. 흡수된이식재의부피는평균 0.24 cc였으며, 유의수준 (p-value) 은 0.05이하로두시기간의평균차는통계적으로매우유의하였다 (Table 1, Fig. 12). IV 총괄및고찰 Calcium phosphate는골미네랄과같은화학적성분으로인하여검증된생체적합성, 국소적이온용출을통한 골과의계면부착성을가진다 1,6,7. 이식시골대체의기전은일반적으로골전도의기전을따른다고알려져있다 3. 골전도의기전을따른다고했을때, 이식재가필수적으로갖추어야할요건을 Lichte 등은생체적합성, 생체배출성, 골유도성등의조건도중요하지만쉽게간과되는조건으로이식재의흡수역학이있음을논문에서기술하였다 1. 즉이식재의흡수속도는주변부에서이식재를대체하는신생골의생성속도와유사하여야한다. 그렇지못하여흡수속도가느릴경우오히려신생골의생성을저해하게되고, 반대로흡수속도가너무빠를경우비계의역할을하지못하여신생골의생성이이루어지지않는다고여러논문에서기술하고있다 1,9,11,12. 하지만여러종류의 Calcium phosphate의흡수역학에대한연구는동물실험에제한적으로있었던것이사실이며, 임상연구또한증례보고에한정되어체계적인흡수역학에대한 대한구강악안면임프란트학회지 15 권 2 호,

8 임상연구가부족했던것이사실이다. Hydroxyapatite는신체내에이식시대부분흡수되지않고, 이물반응을일으킨다고여러논문들에서보고하고있다. Fujita 등은 2003년 hydroxyapatite와 β-tcp를쥐의두개골과골막사이에온레이이식한후 24주까지진행한실험에서 hydroxyapatite의경우, 거의붕괴되지않고유지되었으나, β-tcp 경우 4주뒤부터붕괴가진행됨을조직학적으로관찰하여보고하였다 3. 또한 Ricci 등과 Klein 등도역시 Hydroxyapatite를이식한동물실험에서흡수되지않고잔존한다는사실을보고하였다 8,9. 골전도시골대체재표면에신생골이침착되고, 신생혈관을통해유입된대식세포에의해이식재가흡수되는 resorption 기전과국소환경내의체액에의한 degradation 기전을통해이식재가붕괴되면, 그공간을통해신생골이침착됨을상기할때, 이식재의만기잔존은신생골로의대체를불가능하게한다 13. 또한이러한만기잔존은지속적인붕괴산물의방출로인해만성적인염증반응을유발한다고보고되고있다 14. 이러한염증반응의부산물들로인해결국이식재주변의주변정상골조직마저괴사되는합병증을야기할수있다 15. β-tcp 경우는 Hydroxyapatite와다른흡수역학을보인다. 다수의연구들에서 β-tcp는친수성과화학적조성으로인해빠른속도로붕괴됨을보고하고있다 9,13. Koerten과 van der Meulen 13 은쥐의복강에 β-tcp 입자를주입한후주사현미경관찰을통해용해된정도를관찰한동물실험에서한달후약 74% 의이식재가용해되었음을보고하였다. 따라서 β-tcp는신생골형성시까지공간을차지할수있는체적안정성이매우떨어진다. 따라서 β-tcp의화학적조성, 입자크기, Pore size 의조절을통하여흡수역학의개선을보고한논문들이발표되었지만 7,10, 여전히어떠한조성, 입자크기가효과적인지에대한합의점은없다. 하지만 Kühne 등은쥐에시행한동물실험에서 300~500 μm 의 pore 크기는신생 골침투에용이할수있다는결과를보고하였다 16. 따라서이러한두재료의최적혼합을통해서로다른흡수역학을이용하여신생골의생성속도와이식재의흡수속도간의균형을맞추려는개념으로 biphasic calcium phosphate (BCP) 가임상에소개되었다 17,18. Cheng 등은 2010년쥐의비골에 BCP를식립한후 3 개월뒤조직학적으로관찰한실험에서성공적인골대체와신생골형성을확인하였다. 6 또한조직면역염색을통해대조군에비하여 BMP-2의발현이유의미하게증가하였다고보고하였다. 이현상을 intrinsic osteogenesis 로정의하였으며, BCP의골대체제로써의장점을역설하였다. 또한 Schwarz 등은 2007년개의하악골의결손부에 BCP를이식한후 9주뒤관찰한논문에서성공적인신생골형성이상품화된이종골에비하여비교우위에있음을주장하였다 19. 하지만아직까지는 BCP의흡수속도, 흡수역학에대한임상연구는부족하다 4. 본저자는술후골부피의계측을위하여 computed tomography 를이용하였다. 그동안통상적인연구들에서는이식골의흡수여부를 2차원방사선사진상에서 tracing을통하여판단하였다. 하지만최근장기, 이식재, 병소등의부피를 3차원 CT를촬영후평가하는방법에대한논문들이보고되고있다 4, 년 Wang 등은두개골의안와의부피를본저자와같은 simplant program의 tracing tool을이용한방법으로나온데이터와직접안와의부피를측정하여비교한실험에서 Simplant program을이용한방법의임상적정확성및유용성을증명하였다 20. 본저자는 CBCT dicom data를축상단면에서관찰하여주변골과이식재와의경계부위를 manual tracing 하였다. 지정된구역내의 Pixel 을통하여면적을계산하게되고, 각각의단면의높이 1.0 mm와곱하여부피를계산하게된다. 이방법은 Wang 등이발표한방법과일치하였다. 즉 Simplant의프로그램을이용한부피측정은현재재현성있고정확한부피측정방 대한구강악안면임프란트학회지 15 권 2 호,

9 Clinical Articles 식으로인정받고있다. 하지만본저자가 Method B 방식을시도한것처럼좀더정확한부피측정방식에대한연구가이루어져야할것으로보인다. 저자등은이러한방법을통하여 Biphasic calcium phosphate의인체내에서 6개월내체적안정성은 85.45% 라는결과를도출하였다. 또한 BCP의흡수역학에대한연구에부가하여일정시점에서골생검을통하여신생골형성이이루어졌는지에대한추가후향연구도필요할것으로보인다. 현재 BCP의구강내적용후체적안정성에대한보고는현재없어본논문의결론은추후동재료의체적안정성에대한여타의임상연구시비교의기준이될수있을것으로생각된다. V 결론 저자등은 20명의지원자에대하여진행된구강내이식된 BCP의단기간체적안정성에대한본임상실험을통하여다음과같은결론을도출하였다. 이식된 BCP는특기할합병증이없이치유가이루어졌으며, 1개월과 6 개월사이의이식재의단기간체적안정성은 Simplant program을이용하여분석시 85.45%, 흡수되어소실된골의부피는평균 0.24 cc로나타났다. BCP, HA, β-tcp를인체내적용후체적안정성을분석한임상시험은전무하여본연구결과는추후연구들의기준이될수있을것으로생각된다. 임상적으로는구강내다양한결손시 BCP를이용한수복은경제적인측면에서다른이식재와비교하여장점이있으며, 또한 β-tcp의단점이었던빠른흡수역학을 HA와의최적혼합을통하여극복하여골전도의측면에서임상가에게큰장점이될것으로기대된다. 참고문헌 1.
