Methods: This study retrospectively reviewed the medical records, from January 1 st 2015 to March 31 st 2016, of type 2 diabetic patients who were older than 18 and were prescribed with dapagliflozin or sitagliptin in combination with metformin. Hemoglobin A 1c (HbA 1c ) levels and weights were measured every 3 months. Results: The dapagliflozin group showed a greater decrease in HbA 1c levels after 3 months (-0.75% vs. 0.01%, P<0.001), 6 months (-0.36% vs. 0.08%, P=0.029), and 9 months (-0.53% vs. 0.08%, P=0.046) compared to the sitagliptin group. Also, the dapagliflozin group showed a greater significant decrease in the rate of change in HbA 1c levels after 3 months (-0.09 vs. 0.01, P<0.001), 6 months (-0.04 vs. 0.01, P=0.031), 9 months (-0.07 vs. 0.02, P=0.029), and 12 months (-0.05 vs. 0.05, P=0.047). Furthermore, the dapagliflozin group showed a greater decrease in amount of weight change after 3 months (-2.46 kg vs kg, P<0.001), 6 months (-3.02 kg vs kg, P<0.001), and 9 months (-2.27 kg vs kg, P=0.002). Finally, the dapagliflozin group showed a greater decrease in the rate of change in weight after 3 months (-3.10% vs. 0.52%, P<0.001), 6 months (-3.83% vs. 0.21%, P<0.001), 9 months (-2.84% vs. 0.79%, P=0.002), and 12 months (-4.91% vs. 0.44%, P<0.001). Conclusions: It was concluded that dapagliflozin is more effective than sitagliptin for type 2 diabetic patients. KEY WORDS: Dapagliflozin, sitagliptin, metformin, glycemic control, weight loss 제 2형당뇨병은세계적으로유병률이증가하고있는만성대사성질환으로, 우리나라도사회경제적인발전으로과식, 운동부족, 스트레스증가등의원인으로인해유병률이급격히상승하였다. 또한한국인사망원인중당뇨병이차지하는비율도점차증가하여 2014년통계에따르면한국인사망원인비율 6위에오르기도하였다. 1) 당뇨병은특히중증의저혈당, 케톤산증등과같은급성합병증으로인한입원과만성합병증에의 한심혈관질환으로인해삶의질저하에큰영향을미칠수있다. 따라서합병증예방을위해철저한혈당조절이요구되며, 2-4) 현재우리나라는 HbA 1c (hemoglobin A 1c ) 수치를 6.5% 로설정하여, 중증의저혈당혹은다른부작용이없다면정상수치근처로개별적으로조절할것을권장하고있다. 5) 혈당조절을위한 1 차약제로는 metformin 이권장되고있으며조절이어렵거나필요시 2차, 3차약제를추가하도록권장된 *Correspondence to: Hye Sun Gwak, College of Pharmacy & Division of Life and Pharmaceutical Sciences, Ewha Womans University, 52 Ewhayeodae-gil, Seodaemun-gu, Seoul 03760, Republic of Korea Tel: ; Fax: ; hsgwak@ewha.ac.kr # The authors equally contributed to this work. 99

2 100 / Korean J Clin Pharm, Vol. 27, No. 2, 2017 다. 2차, 3차약제로는다양한기전을가진혈당강하제가쓰여져왔고, 최근에도새로운혈당강하제의개발이이루어져왔다. 그중 dapagliflozin은 sodium-glucose co-transporter subtype 2 (SGLT2) 를억제, 혈중포도당을뇨로배설시켜혈당강하작용을나타낸다. Dapagliflozin 은 sulfonylurea계약물과달리인슐린에의존하지않고혈당을조절하며소변으로당을배출시켜체중감소효과도나타낸다. 6-8) 여러연구에따르면기존의경구용혈당강하제와 dapagliflozin을병용하였을때, 공복혈장혈당 (fasting plasma glucose, FPG) 감소, 경구당부하검사 (oral glucose tolerance test, OGTT) 수치감소, 당화혈색소 (hemoglobin A 1c, HbA 1c ) 감소, 체중감소를확인할수있었다. 9-15) 약물치료와더불어, 당뇨병환자들에게합병증예방을위한운동요법또는식이요법등의생활습관개선은필수적이므로, 표준체중을유지하는것은중요하다. 따라서 dapagliflozin은체중감소효과가있다는연구결과로인해새로운경구용혈당강하제로서주목받게되었다. 6-8) 마찬가지로 2006년개발된 dipeptidyl peptidase-4 (DPP4) 저해제는저혈당, 체중증가부작용이다른혈당강하제에비해적게발생한다고보고되어유사한혈당강하효과를보인 sulfonylurea 계약물에대체되어선택되어왔다. 16,17) 하지만우리나라에서제 2형당뇨병환자에게서 dapagliflozin 과 DDP4 저해제의효능대한직접비교연구는부족하다. 본연구에서는 dapagliflozin과여러종류의 DDP4 저해제중에서도사용량이가장많은 sitagliptin을복용하고있는환자를대상으로 metformin과병용시혈당강하효과와체중감소효과를비교하여제 2형당뇨병환자들에게최적의약물요법의근거를마련하고자하였다. 연구방법 연구대상본연구는제 2형당뇨병환자중 2015년 1월 1일부터 2016년 3 월 31일까지서울 A병원에서 metformin과 dapagliflozin 또는 metformin과 DPP4 저해제인 sitagliptin을처방받은만 18세이상인환자를대상으로하였다. 본연구의약물용량과용법은 dapagliflozin 10 mg 하루한번복용, sitagliptin 100 mg 하루한번복용이고제외기준은다음과같다 : 연구기간동안 metformin을제외한다른혈당강하제를복용한환자, insulin 제제를투여받는환자, 항암치료를받았거나이식을받은환자, 추적관찰기간이 3개월미만이거나추척관찰이불가능한환자. 본연구는임상연구심의위원회 (Institutional Review Board, IRB) 의승인을받은시험계획서 ( 과제번호 ) 에따라진행되었으며, 식품의약품안전처및의약품규제조화국제회의 (International Conference on Harmonization, ICH) 에서정한임상시험관리기준 (Good Clinical Practice, GCP) 에따라 수행되었다. 자료수집자료는전자의무기록을바탕으로복용시작시점부터추적관찰기간종료시점까지수집하였다. 추적관찰종료시점은해당약물복용중단, 다른계열의혈당강하제로변경또는추가, metformin 용량증량또는감량, 연구기간종료일인 2016년 3 월 31일중마지막 HbA 1c 를측정한시점으로하였다. 본연구는후향적연구로포함기준에해당하는환자에대하여다음의자료를수집하였다 : 성별, 나이, 키, 체중, 체질량지수 (body mass index, BMI), metformin 용량, 연구기간동안약물복용시작날짜, 최종처방날짜, 추적관찰기간, 매외래진료시의혈액검사수치 (HbA 1c, glucose, cholesterol, serum creatinine (Scr)), Cockroft-Gault 공식을이용한크레아티닌청소율 (creatinine clearance, CrCl), 흡연, 음주, 고혈압, 고지혈증동반여부, 심뇌혈관계질환과거력, 병용약물 (angiotensin converting enzyme (ACE) inhibitor, angiotensin receptor blocker (ARB), calcium channel blocker (CCB), β-blocker, 이뇨제, statin 등지질저하제, nitrate 계열약물, digoxin, warfarin, aspirin, 항혈소판제 ). 자료분석수집한자료들은 metformin과 dapagliflozin을처방받은환자군과 metformin과 DDP4 저해제중 sitagliptin을처방받은환자군으로나누어분석하였다. 두군의약물효능비교를위해연구기간동안 HbA 1c 의첫측정시점의값을기본값으로하여 3개월간격으로측정된 HbA 1c 를바탕으로각기간별 HbA 1c 와기본값과의차이 ( 변화량 ) 를비교하였다. 이때, 정확한측정시점이아닌경우, ±1개월은가까운개월수에포함시켰다. 기본수치보정을위해기본값으로부터의변화율을비교하였다. 변화량 = 각기간에서의 HbA 1c HbA 1c 기본값 변화율 = 각기간에서의 HbA 1c HbA 1c 기본값 HbA1 c 기본값 또한환자의체중을비교하여연구기간동안체중의첫측정시점의값을기본값으로하여 3개월간격으로측정된체중을바탕으로각기간별체중과기본값과의차이 ( 변화량 ) 를비교하였다. 마찬가지로정확한측정시점이아닌경우, ±1개월은가까운개월수에포함시켰다. 기본값으로부터의변화량및변화율을아래식에따라비교하였다. 변화량 = 마지막측정시점의체중 - 체중기본값 변화율 (%)= 마지막측정시점의체중 - 체중기본값 X 100 체중기본값

3 제 2 형당뇨병환자에서 metformin 과병용시 dapagliflozin 과 sitagliptin 의효능비교 / 101 Randomization 방법및통계분석 Sitagliptin/metformin군에포함되는환자는 stratified random sampling 방법을사용하여 dapagliflozin/metformin군환자와성별, 나이, 복용하는 metformin 용량을바탕으로 randbetween 함수를이용하여무작위선정하였다. 연구대상자의일반적인특성을알아보기위하여빈도분석과기술통계량을비교하였다. 두군의차이를분석하기위해서연속형변수는 unpaired t-test를이용하였고, 범주형변수는 chisquared test를이용하였다. 본연구의분석은유의확률 (p-value) 이 0.05 미만일때통계학적유의성이있는것으로판단하였으며, 통계처리는 SPSS software package (version 23) for windows 를사용하였다. 연구결과 제 2형당뇨병환자중 2015년 1월 1일부터 2016년 3월 31일까지서울 A병원에서 metformin과 dapagliflozin을처방받은 (dapagliflozin군) 만 18세이상인환자는총 222명으로연구기간동안 metformin을제외한다른혈당강하제를복용한환자 2명, 추적관찰기간이 3개월미만인환자 104명, 추적관찰이불 가능한환자 24명, sitagliptin 병용환자 9명을제외하고연구대상자는 83명이었다. 또한 metformin과 sitagliptin을처방받은 (sitagliptin군 ) 환자들은만 18세이상환자중에서나이, 성별, 복용하는 metformin 용량을바탕으로계층화하여 stratified randomization을하여선정되었다. Randomization 과정에서두군간 1:1 matching 되지않는환자가 8명발생하여제외하여최종연구대상자는각군당 75명으로총 150명이었다 (Figure 1, Table 1). 각연구대상군에서나이, 키, total cholesterol, Scr, CrCl 수치, 흡연, 음주여부는평균에유의한차이가없었으나체중, BMI, HbA 1c, glucose 농도, 추적관찰기간, 동반질환에서는고혈압을동반한환자, 병용약물중에서는 ARB, β-blocker, 이뇨제, statin 등지질저하제를복용하고있었던환자에게서유의한차이가있었다 (Table 2). 연구기간동안 HbA 1c 의첫측정시점의값을기본값으로하여 dapagliflozin군과 sitagliptin군의 3개월간격으로측정된 HbA 1c 를분석하였다. 각기간별로비교하였을때, dapagliflozin군에서는기본값의평균 HbA 1c ( 표준편차 ) 가 7.5(0.8)%, 3개월후7.0 (0.5)%, 6개월후 6.9(0.9)%, 9개월후 6.9(0.6)%, 12개월후7.0 (1.1)% 로나타났다. Sitagliptin군에서는기본값의평균 HbA 1c ( 표준편차 ) 가 6.8(0.7)%, 3개월후7.2(1.0)%, 6개월후 6.8(0.7)%, 9개월후 7.1(0.9)%, 12개월후7.1(1.0)% 로나타났다. 각기간별로 Table 1. Stratified randomization based on sex, age, and metformin dose. (A) Amount of weight change from baseline, (B) Rate of weight change from baseline. 비교했을때, 평균 HbA 1c 기본값에는유의한차이가있었지만 (P<0.001), 3개월, 6개월, 9개월, 12개월의평균 HbA 1c 에는유의적차이가없었다 (Figure 2). 각기간별기본값으로부터 HbA 1c 변화량은 dapagliflozin 군에서는 HbA 1c 변화량 ( 표준편차 ) 이 3개월후 -0.75(0.79)%, 6개월후 (1.19)%, 9개월후 -0.53(0.91)%, 12개월후 -0.47(1.25)% 로 나타났다. Sitagliptin군에서는 HbA 1c 변화량 ( 표준편차 ) 이 3개월후 0.01(0.86)%, 6개월후 0.08(0.55)%, 9개월후 0.08(1.06)%, 12개월후 0.33(0.83)% 로나타났다. HbA 1c 변화량은 3개월후 (P<0.001), 6개월후 (P=0.029), 9개월후 (P=0.046) 에는유의적차이가있었지만 12개월의 HbA 1c 변화량에는유의적차이

5 제 2 형당뇨병환자에서 metformin 과병용시 dapagliflozin 과 sitagliptin 의효능비교 / 103 가없었다 (P=0.057). 각기간별기본값으로부터 HbA 1c 변화율은 dapagliflozin에서는 HbA 1c 변화율 ( 표준편차 ) 이 3개월후 -0.09(0.09), 6개월후 -0.04(0.15), 9개월후 -0.07(0.10), 12개월후 -0.05(0.14) 로나타났다. Sitagliptin군에서는 HbA 1c 변화율 ( 표준편차 ) 이 3개월후 0.01 (0.11), 6 개월후 0.01(0.08), 9 개월후 0.02(0.14), 12 개월후 0.05 (0.11) 로나타났다. HbA 1c 변화율은 3개월후 (P<0.001), 6개월후 (P=0.031), 9개월후 (P=0.029), 12개월후 (P=0.047) 로모든기간에서유의적차이가있었다. 각기간별기본값으로부터체중변화량은 dapagliflozin군에서는체중변화량 ( 표준편차 ) 이 3개월후 -2.46(2.33) kg, 6개월후 (2.73) kg, 9개월후 -2.27(2.71) kg, 12개월후 -3.66(1.95) kg 으로나타났다. Sitagliptin군에서는체중변화량 ( 표준편차 ) 이 3개월후 0.37(1.76) kg, 6개월후 0.13(2.20) kg, 9개월후 0.50(2.80) kg, 12개월후 0.26(1.65) kg으로나타났다. 체중변화량은 3개월후 (P<0.001), 6개월후 (P<0.001), 9개월후 (P=0.002), 12개월후 (P<0.001) 로모든기간에서유의적차이가있었다 (Figure 3(A)). 각기간별기본값으로부터체중변화율은 dapagliflozin 군에서는체중변화율 ( 표준편차 ) 이 3개월후 -3.10(2.78), 6개월후 (3.13), 9개월후 -2.84(3.53), 12개월후 -4.91(3.05) 로나타났다. Sitagliptin군에서는체중변화율 ( 표준편차 ) 이 3개월후 0.52 (2.47), 6개월후 0.21(3.01), 9개월후 0.79(3.73), 12개월후 0.44(2.40) 로나타났다. 체중변화율은 3개월후 (P<0.001), 6개월후 (P<0.001), 9개월후 (P=0.002) 12개월후 (P<0.001) 로모든기간에서유의적차이가있었다 (Figure 3(B)). 고 본연구는 dapagliflozin 과 sitagliptin을복용하는만 18세이상의당뇨환자를대상으로혈당강하효과와체중감소효과를비교하였다. 혈당강하효과에서는 dapagliflozin군이 sitagliptin군에비해 HbA 1c 가더많이감소한것으로나타났다. 연구기간동안 HbA 1c 평균변화량은 dapagliflozin 군에서 3개월후 -0.75%, 6개월후 -0.36%, 9개월후 -0.53% 로이기간들에서유의적으로더크게감소하였다. 그리고 HbA 1c 평균변화율도 dapagliflozin 군에서 3개월후 -0.09%, 6개월후 -0.04%, 9개월후 -0.07%, 12개월후 -0.05% 로모든기간에서유의적으로더크게감소하였다. 그러나 dapagliflozin 군의HbA 1c 수치의기본값이통계적으로유의하게큰값을가지고있었기때문에 HbA 1c 수치의변화량이더크게감소한것으로해석될수있다. 실제로 HbA 1c 의기본값이높을수록치료후 HbA 1c 수치가더많이감소한다는결과가보고된바있고, 18,19) HbA 1c 기본값이높은환자에게 dapagliflozin을사용하였을때, HbA 1c 수치가 placebo군에비 찰 해유의적으로크게감소되었다는연구결과도보고되었다. 20) 기본값의차이를보정하기위해변화율을산출하였고그결과 dapagliflozin군에서의감소율이모든기간에서유의적으로큰것으로나타나 dapagliflozin은 HbA 1c 수치를낮추는데있어서더효과적인것으로해석되었다. 반면 sitagliptin군의경우에는 dapagliflozin군에비해 HbA 1c 기본값이낮아추적기간동안오히려상승하는경향을나타내었다. 향후전향적연구를통해정확한기전을밝히는것이필요하다고사료된다. 체중도기본값에서 dapagliflozin군이유의한차이로높아 (79.4 kg vs kg, P<0.001) 직접적인비교가어려웠다. 이는체중이큰환자군에서체중감소효과가있다고알려진 dapagliflozin을 2차약제로선택한비율이크기때문이라고추정된다. 21) 체중변화량은모든기간 dapagliflozin군에서유의적으로더크게감소되어, 체중감소효과에서도 dapagliflozin 군이 sitagliptin군에비해더효과적인것으로해석되었다. 이전연구들에서 SGLT2 억제제계열약물들은체중감소효과가있다고보고되었고, 6-8) 본연구에서도 dapagliflozin 군에서확실한체중감소효과를보이므로, dapagliflozin이가진체중감소효과는당뇨병환자의합병증예방을위한당뇨병치료의약물선택에고려될장점임은분명하다. 본연구는후향적연구로서동일한간격으로동일한검사를실시할수없어 HbA 1c 수치도정확한시기에측정하지못했다. 또한환자들의추적관찰기간이일정하지않아결측값이발생하였고, 그로인해모든환자에게서원하는기간의검사수치를얻을수없었다는한계가있다. 연구자료가기록에의존했으며연구대상자를비교한수가적었고단일센터연구라는한계를가진다. 하지만제 2형당뇨병한국인환자에게서 dapagliflozin과 sitagliptin의혈당강하효과와체중감소효과를비교한연구라는부분에의미가있으며, 연구대상자수가적지만나이, 성별, 복용하는 metformin 용량에따라 stratified randomization을실시하여두군간 1:1 matching 시킨것은검정력확보에기여했다고볼수있다. 결론 본연구에서는 dapagliflozin을복용한경우 sitagliptin에비해 HbA 1c 변화량과변화율은더큰유의적감소가있었고, 체중변화량과체중변화율도더큰유의적감소가있는것으로나타났다. 따라서 dapagliflzin을 2차또는 3차약제로병용하였을때, 혈당감소효과와체중감소효과가있으므로체중조절이필수적인제 2형당뇨병환자에게있어 dapagliflozin은최적의약물로사료되었다. 그러나시판된지얼마되지않은약물로서장기적안전성에관한정보가부족하기때문에관련된모니터링이나각별한주의가필요하겠다.
