However, the differences between these two diseases have not been clearly defined. The aim of this study was to more clearly define the clinical characteristics of EORA. Methods : We retrospectively reviewed 50 EORA and 58 YORA patients who met the classification criteria established by the American College of Rheumatology (ACR). The two groups (EORA and YORA) were compared by three criteria. First, we considered the patterns of the joints involved and the presence of rheumatoid nodules. Second, we compared the disease activity indices and the level of auto-antibodies. Finally, we compared the use of medications. Results : The mean age-of-onset and the women-to-men ratio in the EORA group was 66.2±5.5 years and 2.1:1, respectively. There was more large joint involvement seen in the EORA group. The titer of disease activity indices (ESR, CRP) and positive rate of auto-antibodies (rheumatoid factor, ANA, but not anti-ccp antibody) were also higher in the EORA group. We found no differences in the prescribed medications between the two groups. Conclusions : From these studies, we believe that EORA has higher disease activity indices at onset and greater joint involvement, along with higher titers of auto-antibodies as compared to YORA.(Korean J Med 72:62-67, 2007) Key Words : Rheumatoid arthritis, Aging 서론 최근우리나라는평균수명의연장으로 65세이상노인인구가 1960년에는전체인구의 3. 에불과했으나 2002년에는 7.9% 를차지하여유엔이정한고령화사회 (aging society) 인 7.0% 를넘어섰다 1). 유엔개발계획 (UNDP) 이발표한 인간개발보고서 2004 에따르면한국인의평균수명이 75.4세에달한다. 따라서만성노인성질환의증가가큰사회적, 경제적부담으로대두되고있다. 2 - Jung-Hyun Park, et al : The clinical characteristics of elderly onset rheumatoid arthritis - 노인성질환중에는관절염이가장많으며그중대부분은골관절염이고, 류마티스관절염및다른근골격계질환이나머지를차지한다. 노인에서류마티스관절염은유병률이 정도에불과하지만, 다른질환과동반되는경우가많고신체장기의전반적인기능이저하되어있기때문에관절염치료와회복이어려운경우가많다 2). 특히 60세이후에류마티스관절염이처음발생한경우를 elderly-onset rheumatoid arthritis (EORA) 라고정의하며임상적특징, 질병의진행과정이 60세이전에발생한성인형류마티스관절염 (younger-onset rheumatoid arthritis, YORA) 과다른것으로알려져있다. EORA는피하결절형성과류마티스인자혈청양성률이낮고급성활막염과전신증상이심하며작은관절보다견관절등큰관절을잘침범하는특징이있다 3). 국내에서도류마티스관절염에대한많은연구가이루어져성별에대한차이점, 유전자에대한연구, 검사실검사소견의비교, 여러약제에대한임상연구, 증례보고등다양한연구가진행중이지만 EORA의임상적연구는미미한것이현실이다. 이에저자등은 EORA의관절침범, 이환기간, 검사실검사소견등임상적특징을중심으로 YORA와비교하였으며향후 EORA의진단과치료에도움이되고자한다. 대상및방법 1. 대상 2001년 1월부터 2005년 6월까지본원에서 1987년개정된미국류마티스협회의진단기준에의해류마티스관절염으로진단받은환자 108명을대상으로의무기록및검사결과를후향적으로조사하였다. 60세이상에서류마티스관절염으로진단되었을때를 EORA로정의하였고, 60세미만인경우를 YORA로정의하였다. 이것은이전연구자들이 60세를기준으로구분하였기때문이다. 2. 방법 1) 연구방법류마티스관절염의이환기간이 10년이경과된경우는제외하였는데, 이것은병력청취의충실도와치료의정확도를확인하기어렵기때문이다. 108명의환자들의의무기록을후향적으로나이, 성별, 이환기간, 내원당시검 사실검사소견, 침범관절, 치료약제등을중심으로조사하였다. 견관절을포함한큰관절염은활막비후가명확하거나관절천자에서염증성관절염이확인된경우로제한하였다. 2) 검사실방법적혈구침강속도 (ESR) 은 modified westergren 법을사용하였으며, 류마티스인자는면역비탁법 (Terbidimetry) 으로, C-반응단백 (CRP) 도면역비탁법으로측정하였다. 류마티스인자는 20 IU/mL 이상인경우를양성으로판정하였다. 항핵항체는간접형광항체법 (Indirect fluorescence antibody method) 으로, 항CCP항체 (anticyclic citrullinated peptide antibody) 는효소면역측정법 (ELISA, enzyme-linked immunosorbent assay) 으로모든환자에서측정하였으며검사방법의변화는없었다. 항핵항체는 1:80 이상을기준으로양성으로판정하였으며항CCP항체는 25 U/mL 이상을양성으로판정하였다. 3) 통계방법통계적분석은 t-test와 Chi-square test를이용하였으며 p-value 가 0.05 이하인경우에통계적의미를갖는것으로정의하였다. 통계프로그램은 Minitab-release 14 한글판을이용하였다. 결과 1. 역학대상환자 108명중 EORA는 50명, YORA는 58명이었고, 그중여자는각각 34명 (68%), 45명 (78%) 이었다. 평균연령은 EORA 70.3±5.6세, YORA 45.1±8.3세였고, 발생연령은 EORA 66.2±5.5세, YORA 40.4±8.6세였고, 이환기간은 EORA 4.08±2.3년, YORA 4.67±2.2 년으로양군간의통계적차이는없었다 ( 표 1). 2. 임상증상침범관절수는 EORA 5.56±3.1, YORA 4.88±3.0으로통계적차이는발견할수없었으나견관절, 고관절등의큰관절침범은 EORA 15명 (36%), YORA 11명 (19%) 으로통계적으로의미가있었다. 피하결절양성률은 EORA 18%, YORA 10% 로 EORA에서높게나타났

Ab. is antibody. 으나통계적으로의미는없었다 ( 표 2). 3. 검사실검사소견내원당시측정한적혈구침강속도 (ESR) 는 EORA 68.2±35.6 mm/hr, YORA 46.4±34.5 mm/hr로 EORA 에서통계적으로의미있게증가하였고, C-반응단백 (CRP) 또한, EORA 5.19±5.7 mg/dl, YORA 2.09±3.07 mg/dl로의미있게증가하였다. 류마티스인자의양성률은 EORA 44명 (88%), YORA 39명 (67%) 이었고, 류마티스인자의역가는 EORA 262±226 IU/mL, YORA 131 ±134 IU/mL로 EORA가통계적으로의미있게높게나 타났고항핵항체의양성률은 EORA 36%, YORA 14% 로의미있게 EORA가높게나타났다. 항CCP항체는 EORA 77%, YORA 75% 로양군의차이는없었으며항 CCP항체의역가는 EORA 738±748 U/mL, YORA 1087 ±1407 U/mL로 YORA가높게나타났으나통계적의미는없었다 ( 표 3). 4. 치료약제환자치료에있어 EORA와 YORA는차이를두지않고같은방식으로치료하였다. 양군에있어이차약제로치료한비율은동일하였다. 그러나 EORA에서스테

Hydorxychloroquine, methotrexate, cyclosporin, etanercept는 YORA에서조금더많은비율로사용하였다. 그러나이러한차이가미미하거나서로비슷하므로통계적인의미는없었다 ( 표 4). 고찰류마티스관절염은어떤연령에서도발생할수있지만특히 60세이상에서발생한 EORA에서는임상증상과혈청학적검사등에서차이가있는것으로알려져있다. 일반적으로류마티스관절염은 40, 50대에서흔하고빈도는나이와함께증가하여 65~80세경에최고조에이른후감소한다 2). 미국에서 60세이상노인에서류마티스관절염의유병률은 2.03~2.34% 이고, 남녀비율은 1.6~1.8:2.4~2.7로여성, 멕시칸, 저학력, 70세이상에서빈도가높았다 4). 류마티스관절염환자중에 60세이상이 30~40% 를차지하고그가운데 60세이후새롭게발생하는 EORA의발생빈도는보고자에따라차이가있지만약 20% 를차지한다 4). 남녀비율은연령군에따라차이가있지만남성보다여성에서 3~4배정도더많이발생하는것으로알려져있으며 EORA와 YORA의남녀비는 Deal 등 3) 은 EORA 1:1.4~2.2, YORA 1:3.5~3.6이었으며 Papadopoulos 등 5) 도유사한결과를보고하였다 6, 7). 본결과에서도 EORA 에서여성의비가 YORA보다적어나이가들수록그차이가줄어들지만통계적의미는없었다. 임상양상에서 EORA인경우에피하결절형성이적고급성활막염과전신증상이심하며손의작은관절보다견관절을포함한큰관절에침범하는비율이높다고연구자들에의해알려져있다 3, 8, 9). 본연구에서도 EORA 에서견관절을포함한큰관절의침범이다른연구자들의결과와같이의미있게많았다. 본연구에서 EORA에서 ESR과 CRP가다른연구자들의결과와같이통계적으로의미있게증가하였다 3, 8, 9). 그러나 ESR은연령증가와여성에서폐경기이후호르몬의영향등으로상승할수있고, CRP는다른전신적인염증반응에도증가하기때문에관절염활성도평가에있어서이런요소들의보정이필요하다 12). 아직 EORA와 YORA간의면역학적변화및차이에대해서는잘밝혀지지않은상태이나 EORA의관절활액에서사이토카인연구에서 IL-6는 YORA에비해현저히증가하는반면, IL-1 beta와 IL-8 은차이가없었다 10). 이러한염증성사이토카인의차이가 EORA에서 YORA 에비해 CRP와 ESR의증가, 관절염의급성발생과같은임상적인차이를설명하기도한다. 노화과정에서 B세포기능과체액성면역기능저하로연령의증가와함께정상인에서류마티스인자양성의빈도가상승함에도불구하고 EORA에서 YORA에비해류마티스인자의빈도가낮거나같은정도로보고하고있으나실제연구결과는연구자마다다르게나타났으며일부에서는높은빈도로보고하고있다 9, 11, 12). 본연구에서도 EORA에서 YORA보다의미있게류마티스인자가높은비율로나타났다. 각연구마다류마티스인자양성률이다른이유는 EORA가노인에서많이발생하는류마티스성다발성근육통 (Polymyalgia Rheumatica), 부

5 -The Korean Journal of Medicine : Vol. 72, No. 1, 종양성관절염, 결정성관절염, 미란성골관절염과의감별이어려워이러한질환들이포함되어낮게나올수있고 11, 12), 류마티스인자가연령이증가함에따라위양성률이증가하여양성률이높게나올수있다 12). 대개류마티스인자는정상성인의약 2~5% 정도에서양성이지만 65세이상에서는 5~45% 까지양성으로나타난다. 노령인구에서면역체계의이상의빈도가높기때문에비교적낮은역가의혈청류마티스인자와항핵항체가양성으로나타날비율이증가한다. 항핵항체는정상성인의 1~4% 에서양성이지만노인에서는 10~18% 에서양성이기때문에그판정에주의를요한다. 본연구에서항핵항체양성률은 EORA에서통계학적으로의미있게높았지만임상적의의를둘수없었다. 항CCP항체는통계적의미는없지만 EORA에서 77% 로 YORA 75% 보다양성률이높게나타났다. 항CCP항체 (anti-cyclic citrullinated peptide antibody) 는류마티스관절염에있어임상적유용성에한계가있는류마티스인자보다더효과적인진단과예후예측을위하여개발된감수성과특이도가더뛰어난검사이다 13-15). 항CCP항체의의미는 EORA와 YORA의검사소견의차이에있어하나의감별점이되지는않지만류마티스관절염에서 EORA, YORA 양군모두조기에진단을가능하게함으로써다른질환과의감별에있어중요한역할을한다. 특히류마티스인자가음성인혈청음성 EORA는발병초기에류마티스성다발성근육통과감별하기가어렵고초기에는두질환이혼재하여나타나거나류마티스성다발성근육통으로시작하여류마티스관절염으로진행하는경우가있다 16). 이런경우항CCP 항체가감별에도움이되며 Lopez 등 17) 은 EORA의 65% 가 anti-ccp항체가양성인반면류마티스성다발성근육통에서는모두음성이라고보고하였다. 본연구에서혈청음성 EORA와 YORA에서항CCP항체양성률은 40% 와 60% 였다. EORA와 YORA의치료에있어동일한방식으로치료할경우에대부분의약제들이비슷한비율로사용되며효과에있어서도비슷한것으로알려져있다. Deal 등 3) 에따르면 EORA에서스테로이드의사용비율이높았으며본연구에서도통계적의미는없지만동일한결과가나타났다. 이는스테로이드가골다공증및다른부작용을감안하더라도노인성류마티스관절염에가장선호되는약제이며특히 EORA에서치료효과가아주 좋았고, 그중에서혈청음성 EORA에효과가있는것으로알려져있기때문이다 18, 19). 또한노인에서안전한약물로알려져있는 sulfasalazine이 EORA에서많이사용되는것으로본연구에서나타났지만사용빈도가적기때문에임상적의미를부여할수없었다. 결론적으로본연구를통해 EORA는 YORA에비해임상적으로견관절을포함한큰관절을침범하는특징을보였으며검사실검사소견상으로나이와연관된 ESR과 CRP의증가소견을보였다. 류마티스인자와항핵항체양성률은높았으나항CCP항체는차이가없었다. 치료약제또한차이가없었다. 이를통해저자등은 EORA가 YORA와임상적차이가엄연히존재하지만아직까지명확하게구분되어지는검사소견이나진단방법은부족한것으로판단되어지며앞으로더오랜기간의추적관찰을통해광범위한역학적인연구들이더진행되어야한다고생각된다. 또한 EORA의이러한특징을중심으로다른질환과의감별을정확하게함으로써조기진단과조기치료가가장중요한과제로생각되어진다. 중심단어 : 류마티스관절염, 노인성 REFERENCES 1) 통계청. 통계청자료 ) Laiho K, Tuomilehto J, Tilvis R.
