In contrast to other options, stent-assisted aneurysm embolization has a distinct feature in which the stent is permanently implanted in the cerebral vessels. Therefore, we must consider the short-term effect of stent-assisted aneurysm embolization in the prevention of coil migration and adverse thrombogenicity, but also the permanent mechanical, hemodynamic, and biological influence on the parent vessels and the aneurysm. We have reviewed the current experimental and clinical data on stent-assisted coil embolization of cerebral aneurysms. (Kor J Cerebrovascular Surgery 13(1):5-14, 2011) KEY WORDS : Intracranial aneurysms Endovascular treatment Stent 서론 뇌동맥류에대한코일색전술은 1995 년미국 FDA 에서 Guglielmi detachable coil system 을승인한이후본격적으로시작되어시술빈도가점차늘어나다가, 논문접수일 : 심사완료일 : 교신저자 : 이상형, 신경외과, 서울대학교보라매병원 서울특별시동작구보라매길 41 SMG-SNU Boramae Medical Center Tel : (02) Fax : (02) nslee@snu.ac.kr 최근에는전세계적으로그시술빈도가빠르게증가하고있다. 현재전세계적으로뇌동맥류환자의약반수가코일색전술로치료받은것으로믿어진다. 코일수술에대한임상경험이늘어남에따라점차그적용범위또한확대되어가고있다. 특히, 시술초기에는넓은경부를가진 (wide neck) 뇌동맥류와방추형 (fusiform), 박리형 (dissecting) 뇌동맥류의경우코일색전술이불가능하다고생각하였는데, 이는코일을동맥류낭안에분리시킨후코일이동맥류낭 (aneurysm sac) 을빠져나와원위부로날아가거나, 모혈관안에혈전을형성하여폐색을초래하는문제가발생할수있기때문이었다. 이러한한계를극복하기위하여다양한모양의코일 (complex-shaped coil) 이개발되었고, 43)49)52) 5

It shows a typical strut change of a open cell type stent. 두개이상의미세도관을동맥류낭안에넣어여러개의코일을동시에집어넣는방법 (multiple catheter technique) 을사용하기도하고, 19)20) 임시적으로뇌동맥류의입구를막기위해풍선 (balloon-assisted) 을이용하기도하였다. 41) 이러한방법들과함께많이사용된또다른방법은뇌동맥류입구에스텐트 (stent-assisted) 를설치하는방법이었다. 뇌혈관에스텐트사용은작고중요한혈관에영구적이식물을넣는다는점에서초기에는많이사용되지않다가, 뇌혈관전용스텐트들이개발, 도입되면서, 그리고점차스텐트사용이그리위험하지않다는것이알려지면서최근급격히많이시술되고있다. 스텐트의사용은여타의방법들이일시적으로도구를사용하고제거하는것과달리모혈관안에설치하기때문에, 영구적으로뇌동맥류의경부와인접모혈관의변화를초래할수있다는뚜렷한차이점이있다. 특히이러한변화는단지구조적이나혈류역학적인측면에국한되지않고, 생물학적인측면에도영향을줄수있다. 긍적적인면으로는스텐트의존재가혈관벽조직을강화시켜병적인혈관의치유에기여할수있다는점이있으며이는곧혈관내코일색전술의최대약점인동맥류재개통을줄이는데기여할수있다고믿어지기도한다. 32)50)51) 이러한기대속에 Turjman 등 47) 에의해이미 1994년동물실험결과가보고된바있다. 부정적인면으로는스텐트로인한혈전형성, 혈관벽조직의과다성장, 추가적약물복용과관련한문제, 예측가능하지않은혈류역학적변화등을들수있다. 본논문에서는스텐트보조코일색전술 (stent-assisted coil embolization) 의이론적, 실험적근거를고찰하고현재까지보고된결과들을검토하고자한다. 현재사용중인뇌동맥류색전술보조를위한스텐트들 뇌혈관전용스텐트가개발되기전에는풍선을부풀려설치하는관상동맥용스텐트를사용하였는데, 이스텐트는매우뻣뻣하여구불구불한뇌혈관을지나기가매우힘들어설치지점까지올리기어려운문제가있었다. 그래서의도한위치까지스텐트를올리는것을실패하거나, 여러번시도하는과정에서혈관의손상이일어나는경우도있었다. 또한원하는위치까지삽입시켜도, 풍선을부풀려스텐트를펴는과정에서뇌동맥류가파열될위험이있었다 (Fig. 1). 최근뇌혈관전용으로개발되어동맥류치료를위해사용되는스텐트는모두 nitinol 이주재료인자가팽창형 (self-expanding) 스텐트들로심장용스텐트에비해훨씬부드러워뇌혈관원위부까지진행하는데거의문제가없다. 현재까지개발된 4 개스텐트의특징은다음과같다. Neuroform (Boston Scientific, USA) 유일하게 open-cell 구조로되어있으며, nitinol 봉을 laser로깎아만들었다. 2002년에처음출시되었다. 이후 Neuroform II와 Neuroform II Treo를거쳐현재의 Neuroform III로개선되면서일부 closed cell 구조의장점을채용하여목표부위도달능력과동맥류경부를 cover하는정도가향상되었다. Open cell 구조는스텐트가열리는순간부터 strut의 cell이열리는구조이므로일단열리기시작하면다시뒤로뺄수는없다 (Fig. 2, 3). 6 Kor J Cerebrovascular Surgery 13(1):5-14, 2011

3 권현조 권오기 고현송 이상형 Leo (Balt, France) 가는 nitinol 선을엮어서만들며, closed cell 구조로되어있다. 부분적으로스텐트를다시뒤로빼는것이가능하다. 방사선으로잘보이는백금선이스텐트전장에들어가있어설치전후스텐트의위치를쉽게확인할수있다. 36) 현재우리나라에는들어와있지않다 (Fig. 4). Enterprise (Codman, J&J, USA) Neuroform 스텐트처럼 laser로깍아만든 nitinol 스텐트이며 closed cell 구조이다. 전체길이의 70% 가펼쳐지기전에는다시카테타속으로접어넣고, 다시스텐트를펼칠수있는장점이있다. 10) 스텐트가부드러워뇌혈관에잘올라가고, 다시접을수있어위치조정이용이한점때문에최근가장많이사용된다 (Fig. 5). Solitaire (ev3, USA) Laser-cut 제작이고, closed-cell 구조인것, 그리고기존의마이크로카테타를이용하여올릴수있는것은 Fig. 4. A figure of Leo stent. It is a closed cell type self expandable stent. Enterprise 스텐트와유사하다 (Fig. 6). 하지만이스텐트는 strut 사이의간격이 3~4 mm 정도로조금더크고, 스텐트근위부가 introducer에금속으로붙어있다. 그러므로스텐트를최종적으로혈관에두고나오려면스텐트를 introducer로부터분리시켜야한다. 분리시키는방법은전기분해를이용한다. 이방법은초기 GDC를분리하던방법과동일하다. 스텐트와 introducer의접합력이매우좋으므로스텐트전체를펼친후에도거두어수차례다시스텐트를펼칠수있는장점이있다. 24) 스텐트의구조적효과 입구가넓은뇌동맥류에서스텐트는코일이빠져나와원위부로이동하는것과모혈관을침범하여좁게만드는것을방지할뿐만아니라, 좀더많은코일을동맥류안에채울수있도록도와준다. 이러한효과는결국수술결과를향상시키고, 동맥류의파열이나재파열을줄이고, 코일이압축 (compaction) 되어재개통되는것을막는데에도움이된다. 스텐트의설치순서는먼저코일삽입을위한마이크로카테타를초선택 (microselection) 하여동맥류안에넣고, 스텐트를이후에설치하여기존의마이크로카테타가스텐트에의해고정되어안정적인코일삽입이가능하도록하는 Jailing 방법과먼저스텐트를적절하게설치하고이어스텐트의 strut들사이로마이크로카테타를통과시켜동맥류안에집어넣고코일을채우는방법이있다. 최근에는다시거두어들일수있는스텐트 (Solitaire와 Enterprise) 가보급되면서스텐트를완전히펼치지않고, 일시적혹은부분적으로스텐트를설치하여도움을받는방법 (semi-jailing) 도발표되었다. 12) 이러한스텐트를동맥류입구주변의모혈관에설치하는형태도여러가지가보고되었는데, Basilar tip과 MCA bifurcation, A-com aneurysm 등에서두개의스텐트를서로교차시키는 Fig. 5. 4 스텐트 보조 뇌동맥류 색전술 - 종설 - Y-stenting 과3)35)39) X-stenting 38) 방법과 P-com artery과 스텐트의 혈류역학적 효과 A-com artery를 통해 건너가 다른 혈관의 말단부에 생 42) 동맥류의 입구에 설치된 스텐트가 동맥류 안의 혈류 동맥류의 입구와 낭 안에 스텐트의 원위부 끝의 일부 패턴을 변화시키고, 전단응력(shear stress)에 영향을 긴 뇌동맥류 입구에 설치하는 Horizontal technique, 13) 양 쪽 M2에 각각 스 준다는 것은 많은 실험을 통하여 입증되어 왔다. Ohta 텐트를 펼치고 그 근위부가 동맥류의 입구에서 만나게 등 은 두개강내 혈관에 설치된 스텐트가 동맥류벽, 를 넣는 Waffle-cone technique, 26) 33) 등이 대표적이다. 술자가 원하 특히 동맥류낭 안에 작용하는 Shear stress를 줄이고 는 위치에 스텐트를 성공적으로 설치하는 비율은 새로 동맥류 내 혈액의 점성도(viscosity)를 높여 뇌동맥류의 운 Neuroform 스텐트와 재설치가 가능한 Enterprise, 2) 폐색을 유도한다고 보고하였다. Canton 등 은 Silicone Solitaire 스텐트가 사용되면서 대부분 95%이상의 성공 으로 만든 Sidewall 동맥류 실험에서 digital particle 율을 보고하고 있으며, 스텐트 설치와 관련한 유병율 image velocimetry를 이용하여 관찰할 때, 여러 개의 도 초기 5% 내외로 보고되다가 현재는 더 감소하였 Neuroform 스텐트를 설치하면서 최대 평균 혈류속도 하는 Linear stenting 18) 하지만 이러한 구조적 효과가 설치 후 모든 환자 와 소용돌이도(vorticity), shear stress가 일관되게 감소 에서 이상적으로 유지되는 것은 아니어서 Enterprise 스 하였으며, 이 효과는 첫번째 스텐트 설치 이후에 가 37) migration이 장 컸다고 보고하였다. 또한 분지(bifurcation) 동맥류 보고되어 있으며, 성공적인 코일 색전술 후 delayed 실험에서는 두 개의 Neuroform 스텐트를 Y자 모양으 다. 텐트의 경우 delayed distal 21) or proximal 9) 로 설치(Y-stenting)한 후 11%의 혈류속도 감소, 5.8% coil migration도 보고된 바 있다(Fig. 7, 8). Thin nitinol wires are used. 의소용돌이도감소, 9% 의 shear stress 감소효과가나타났다고보고하였다. 1) Kim 등 16) 은환자의 CTA image 를이용하여재구성한 Basilar trunk aneurysm에 Neuroform2 스텐트를적용한효과를 CFD (computational fluid dynamics) 를이용하여알아보았는데, 스텐트를여러개설치하면서동맥류안의 wall shear stress가줄었고, 특히 3개의스텐트를펼치고난후혈류의정체가크게늘어난다고보고하였다. Dorn 등 5) 은 sidewall aneurysm 의 pulsatile model에 Laser Doppler anemometry를이용한실험에서 Solitaire 스텐트가 67.59% (inflow zone), 9.65% (dome), 37.94% (outflow zone) 의혈류감소를초래한다고보고하였다. Tremmel 등 45) 은 CFD를이용한 Enterprise설치효과실험에서 Enterprise 설치이후동맥류낭안의소용돌이도가감소하고, wall shear stress 가설치스텐트숫자에따라 14.9% ( 한개 ), 30.8% ( 두개 ), 45.3%( 세개 ) 감소하며, 낭안의혈류속도는각각 15.0%, 26.4%, 38.1% 감소하고, 혈류의정체도각각 13.8%, 27.2%, 41.0% 증가하고, 압력은각각 0.01, 1.47, 1.85 mmhg씩감소한다고보고하였다. Zhang 등 54) 은중국에서개발한 EZ 스텐트의혈류역학적영향을 flow visualization과 CFD를이용하여관찰하여, 스텐트설치후동맥류안의소용돌이 (vortex) 와 wall shear stress 가감소하였고이러한효과는동맥류의형태에따라달라진다고보고하였다. Tateshima 44) 은 Neuroform 스텐트를이용하여실제환자의한개의안동맥과 2개의작은중대뇌동맥뇌동맥류모형에서평균혈류속도가스텐트설치전에비하여 22~64% 감소하였다고 보고하였다. 특이한것은대부분의동맥류낭안의소용돌이들이스텐트설치이후감소하거나사라졌지만안동맥류에서한개의큰소용돌이가설치이후여러개의작은소용돌이로분리되었으며, 이와함께매우작고빠른혈류가새롭게관찰되었다고보고하였다. 이러한혈류역학적효과들은다음에언급할혈관벽에대한생물학적효과와함께뇌동맥류의 progressive thrombosis를유도하고재소통 (recanalization) 을줄여서바람직한장기추적결과를제공하지만, 반면수술후관찰되는급만성의 in-stent thrombosis의원인이되며, 또한이를줄이기위한항혈소판제복용은출혈경향을증가시켜특히, 파열동맥류환자에서색전술이후의수술적처치에큰부담으로작용하게된다. 14)46) 한편으로이러한혈류역학적효과들은, 스텐트설치후중요분지를보존하기위해추가코일색전이불가능한박리성동맥류 (dissecting aneurysm) 등의경우단지스텐트만을설치하고시술을마친경우 (Sole stenting) 의결과들에서, 4)6)34)48)53) 대부분의경우코일을채운수술정도의안정성에는미치지못하지만일반적인예상보다는긍정적인결과를보고하는데기여하였을것으로생각되며, 나아가완전히스텐트만으로뇌동맥류의치유를목표로하는소위 Flow diverter 들의개발을가져왔다 (Fig. 8). 28)29) 스텐트의생물학적효과 스텐트보조색전술을시행하게되는입구가넓은동맥류들은혈관자체만을볼때입구가좁은동맥류들에비하여더심하고광범위한혈관벽의이상을가지고있는상태이며, 이러한상황에서혈관벽에영구적으로남아대개설치후 30일이내에내막세포화 (endothelialization) 가일어나는스텐트는아직까지뚜렷한효과가입증되지못한 Bioactive coil (Matrix, Cerecyte) 들에비하여재개통을줄이는데있어더나은효과를기대할수있을것이다. 하지만, 장기적으로가장우려하는것은바로이러한내막의과증식 (neointimal hyperplasia) 로인하여생기는스텐트설치부위의 in-stent stenosis이다. 기존의협착이있던관상동맥에스텐트를설치한후에는 10~50% 까지재협착이관찰된다고보고되어왔으며, 대개스텐트설치후 3~6개월경에관찰되고있다. Miao 등 31) 은관상동맥용스텐트를 113명의증상있는중대뇌동맥협 Kor J Cerebrovascular Surgery 13(1):5-14,

6 스텐트 보조 뇌동맥류 색전술 - 종설 - 착 환자에 설치한 결과 평균 29개월이 지난 후 20.25% 동맥류에 대하여 Neuroform 스텐트를 설치하고 코일을 8) 의 재협착이 관찰되었다고 보고하였으며, Fiorella 등 은 채우지 못한 환자의 4개월째 부검소견을 보고하였는데 후순환계에 관상동맥용 스텐트를 설치한 결과 12.5%의 스텐트 근위부에 경도의 내막비후가 관찰되고, 스텐트 환자에서 재협착이 나타났다고 보고하였다. 이러한 변화 설치부에는 중도의 내막비후가 관찰되나 유의한 협착은 에는 당연히 스텐트 strut의 구조와 모양, 개수, 밀도 등 관찰되지 않으며, 뇌동맥류의 안과 경부 변두리로 섬유 이 기여하고, 스텐트 자체와 설치 후 생기는 혈관의 변 조직이 관찰되지만 경부의 중심부에서는 관찰되지 않았다 형 등으로 인해 발생하는 해당부위 shear stress의 변화 고 보고하였다. Hoit 등11)은 3D rotational angiography 소견 등도 영향을 주겠지만, 동맥류의 색전술에 이용되는 자 을 이용하여 Neuroform 스텐트 설치 직후에는 변화를 가팽창형 스텐트의 경우에는 관상동맥용 스텐트과 비교 보이지 않았던 모혈관이 추적검사에서 통계적으로 유의 하여 설치시 풍선을 쓰지 않아 혈관벽의 손상이 적고, 한 내경과 단면적의 감소를 보여 임상적으로는 차이가 훨씬 작은 radial force를 가지고 있어 설치 후에도 혈관 없으나 수치상으로 19%의 협착과 52% 혈류역학적 저항 벽에 자극을 덜 줄 것이기 때문에 비교적 그 발생이 적 의 증가를 보였다고 보고하였다. Lee 등22)도 혈관조영소 다고 생각하고 있다. Wanke 등50)은 91명의 Neuroform설 견을 이용하여 Neuroform스텐트를 설치한 뇌동맥류 색전 치 환자에서 단 한 명의 50%이상 협착환자가 발생하였다 술 환자의 18.8% (6/32)에서 혈관내막의 비후가 관찰되고, 7) 고 보고하였으며, Fiorella 등 은 5.8%의 발생율을 보고하 유의한 비후는 3.1% (1.32)였으며, 비후가 관찰되는 모든 였는데, 특히, 증상이 없는 환자의 경우에는 추적검사에 환자에서 동맥류의 폐색도 수술 직후보다 호전되었다고 서 완전하게 또는 부분적으로 호전되는 경우가 많아 주 보고하였다. Neuroform스텐트를 제외하고 장기 추적결과 의깊게 관찰하며 기다리는 것(watchful waiting)이 좋겠다 보고가 많지 않아 스텐트의 차이에 따른 in-stent stenosis 25) 고 보고하였다. Lopes 등 은 원위 내경동맥에 발생한 Fig. 9. (A) A large vertebrojunction aneurysm. But the aneurysm recurred and enlarged just 3 months later. 한 보고들은 뇌동맥류의 코일 색전술에서 함께 사용하 결과를 관찰하였는데, Progressive thrombosis가 스텐트 는 스텐트가 재협착의 위험성을 갖지만 동시에 수술의 보조를 이용한 동맥류들에서 유의하게 많았으며(70 일차목표인 뇌동맥류의 폐색과 더 나아가 비정상 혈관 vs. 25%), 이러한 차이는 특히 작은 동맥류에서 유의 부위의 치유에 기여할 수 있다는 기대의 근거가 된다. 하게 관찰되었다. Sedat 등 은 대부분 1년 이상의 경 40) 과후에 추적검사한 Neuroform 스텐트 사용 동맥류환 스텐트 보조를 이용한 뇌동맥류 자에서 71%의 complete occlusion이 관찰되었고, 수술 코일색전술의 추적검사 결과 이후에는 recanalization이나 regrowth가 없어서 Neuroform 후 최초 검사에서 9.5%의 동맥류 regrowth가 보였지만 스텐트의 경우 설치 후 1년이 경과하면 안정적이라고 27) 30) Lubicz 등 은 Leo와 Enterprise 스텐트를 이용한 코 결론짓고 있다. Maldonado 등 은 Neuroform 스텐트를 일색전술의 결과에서 총 34개의 뇌동맥류 중 수술 직후 이용한 색전술의 장기 추적검사 결과에서 최초 31.6% 9개의 complete occlusion (26.5%), 2개의 neck remnant 에서 관찰되던 complete occlusion이 18개월 째 검사에서는 (6%), 23개의 incomplete occlusion (67.5%)을 보였던 64.7% (44/68), 36개월 째 검사에서는 56.5% (26/46)을 것이 평균 20개월의 경과 후 18 (53%)개에서 추가 보였다고 보고하였다. Liang 등 도 동일한 관찰에서 thrombosis가 관찰되고, 16개는 안정된 소견을 보여, 평균 37.2개월 경과 후 86.3%에서 폐색이 호전되거나 결과적으로 27개의 complete occlusion (79%), 3 neck 안정된 소견을 보였고, recanalization이 13.7%에서 관 remnant (9%), 4 incomplete occlusion (12%)이 되었다 찰되었다고 보고하였다(Fig. 9). 23) 17) 고 보고하였다. Kim 등 은 136개의 동맥류에 대하여 Neuroform stent를 이용하여 색전술을 시행하고 평균 13.7개월이 경과한 후 추적검사를 시행한 77개의 동 맥류에 대하여 분석한 결과 최초 31.7%에서 관찰되던 스텐트 보조를 이용한 뇌동맥류 코일색전술의 미래 complete occlusion이 74%에서 관찰되었으며, 유의한 in-stent stenosis는 없었다고 보고하였다. Kim 등 은 최근에는 혈관내경이 작은 동맥까지 도달 능력이 스텐트 보조 코일 색전술을 시행한 37개의 동맥류에 우수한 스텐트의 개발과 사용의 확대에 따라 점차 중 대하여 파열유무, 위치(sidewall or bifurcation), 수술 대뇌동맥과 전대뇌동맥등 내경이 작고 원위부에 위치 직후 폐색정도, 동맥류 낭의 크기 등이 유사한 스텐 한 혈관에 발생한 뇌동맥류에도 사용하는 경우가 늘 트 보조 없이 시술한 동맥류들을 비교하여 추적검사 고 있으며, 현재까지의 초기 결과와 소수의 장기 결 15) Kor J Cerebrovascular Surgery 13(1):5-14,

8 스텐트보조뇌동맥류색전술 - 종설 - 과들은비교적긍정적이고, 상당한기간의경과후에도우려하는 in-stent stenosis는많지않은것으로보고되고있다. 스텐트보조를이용한뇌동맥류코일색전술은향후사용이간편하고안전하며효율적인스텐트의개발과 in-stent stenosis의발생빈도, Flow diverter 의발달정도등에따라그사용빈도의증감이결정될것으로예상된다 (Fig. 10). 결론 스텐트보조를이용한뇌동맥류코일색전술은긍정적인초기결과를보이고있으며, 향후스텐트의개선에따라사용증가의가능성이높다. 하지만장기적으로 in-stent stenosis 문제는아직남아있어매시술마다신중한선택이필수적이다.
