Department of Neurological urgery, Asan edical enter, ollege of edicine, University of Ulsan, eoul, Korea Objective:To access the risk factors of posterior cerebral artery(pa) occlusion during endovascular treatment of basilar bifurcation aneurysm and importance of collateral circulation through posterior communicating artery(poa) after PA occlusion, the authors review the results of 2 patients with basilar bifurcation aneurysms treated with Guglielmi detachable coils(gds). ethods:ale to female ratio was 1:3.2(6:19) and the mean age of this group was 4. Nineteen patients were presented with subarachnoid hemorrhage, 1 patient had neurologic deficits of hemiplegia and dysarthria, and patients had incidental aneurysms with headache and dizziness. All patients underwent GD embolization and their clinical results were analyzed. Results:even of nine patients who resulted in PA occlusion had a collateral flow from the anterior circulation through PoA. ix patients had a PoA larger than 1mm and they had no symptoms of ischemia after PA occlusion. One patient who had a PoA with 0.3mm in diameter had transient ischemic symptoms. The PA arouse from the aneurysm neck in seven patients, and six of them resulted in PA occlusion. our of five patients who had aneurysms with broad neck(>4mm) resulted in PA occlusion. onclusion:the aneurysm with either broad neck or PA arising from the aneurysm neck have a high risk of PA occlusion. After PA occlusion, significant blood flow from anterior circulation through the PoA larger than 1mm in diameter can minimize the ischemic result in PA territories. GD of Basilar Aneuryms 병증이 올 수 있다. 본 연구에서는 기저동맥분지부에 발생한 뇌동맥류를 GD 를 이용하여 혈관내 수술법으로 치료한 임상경험을 분석하여 후뇌동맥폐쇄와 관련이 있는 기저동맥분지부 동맥류의 특징 을 알아보고 후뇌동맥폐쇄 후 혈류역학적 변화와 치료에 대 해서 알아보고자 하였다.

2 GD of Basilar Aneuryms 병증이 올 수 있다. 본 연구에서는 기저동맥분지부에 발생한 뇌동맥류를 GD 를 이용하여 혈관내 수술법으로 치료한 임상경험을 분석하여 후뇌동맥폐쇄와 관련이 있는 기저동맥분지부 동맥류의 특징 을 알아보고 후뇌동맥폐쇄 후 혈류역학적 변화와 치료에 대 해서 알아보고자 하였다. 대상 및 방법 1997년 3월부터 2001년 1월까지 4년동안 2례의 기저동 맥분지부 뇌동맥류를 GD를 이용하여 색전술을 시행하였다. 모든 시술은 국소마취 하에 시행되었고, 환자와 협조가 안되 는 경우 진정제를 투여하여 시술 중 수면을 유도하였다. 시술 전에 혈관조영상을 면밀히 분석하여 동맥류의 직경 대 경부의 비율(dome-to-neck ratio, DNR)과 후교통동맥을 통한 측 부순환의 정도, 후뇌동맥이 태생기 순환(fetal circulation)인 ig. 1. Both posterior cerebral arteries are at high risk of occlusion because of broad neck. 지의 여부를 확인하였다. 1999년부터 동맥류의 3차원적인 해부학적 특성을 자세히 알기 위해 필요한 경우 T angio- 세(32세~77세)였다. 파열성 동맥류가 19례였고 나머지 6 graphy(ta)를 시행하였다. 뇌동맥류의 크기는 혈관조영 례는 비파열성 동맥류였다. 비파열성 동맥류의 증상은 반신 상에서 가장 긴 직경을 찾아 small(2~6mm), medium(6~ 부전마비 및 언어장애 1례, 두통 및 현훈이 례였다. 지주막 1mm), large(1~2mm), 그리고 giant(2mm이상)로 하출혈로 내원한 19례 중 내원시 Hunt and Hess Grade가 나누었다. 혈관조영상 동맥류와 경부가 가장 잘 보이는 각도 4나 인 poor grade가 4례였으나, 이중 3례가 시술 전에 신 를 선택하여 DNR을 계산하였고, 경부의 크기는 4mm를 기 경학적 호전을 보여 Hunt and Hess Grade가 한 단계씩 상 준으로 small과 large로 나누었다. 시술 후에 microcatheter 승하였다. 를 제거하고 혈관조영을 하여 동맥류의 폐쇄정도를 compl- 2례 중 9례에서 색전술 중 후뇌동맥(P1)의 폐쇄가 발생 ete, residual neck (dog ear), 그리고 residual aneurysm 하였으며, 이중 7례는 혈관조영상 전방순환계에서 동측의 후 으로 나누었다. 시술 중 첫 번째 코일을 삽입한 후 1000IU~ 교통동맥을 통한 측부순환이 있었다. 이 7례 중 6례는 후교 3000IU의 heparin을 정주(Intravenous bolus injection)하 통동맥의 직경이 1mm 이상이었는데, 시술 중 후뇌동맥이 폐 였고 코일을 분리하기 전에 모혈관으로 코일이 돌출되거나 쇄된 후에도 신경학적 장애가 발생하지 않았다(ig. 2). 나 후뇌동맥이 폐쇄되는지 혈관조영을 하여 확인하였다. 시술 중 머지 1례는 후교통동맥의 직경이 0.3mm이었는데 시술 중 후뇌동맥 폐쇄는 코일 분리 후에 일어났으며, 환자는 갑작스 후뇌동맥이 막히자 구토와 어지러움을 호소하여 시술 후 3 런 의식저하를 보이거나 심한 두통과 오심, 구토증세를 보였 일째 까지 전신적인 heparin을 사용하였고, 증상이 소실된 후 고 경미한 두통과 어지러움만 보이는 경우도 있었다. 환자의 에 투여를 중단하였다. 의식이 저하되거나 심한 증상을 보이는 경우 필요한 응급처 전방순환계로부터 측부순환이 없었던 2례 중 1례는 시술 치를 한 후에 동맥내 혈전용해술을 시행하여 폐쇄된 동맥이 전부터 동측 내경동맥이 완전히 막혀있어 후뇌동맥에서 후교 재개통 되도록 시도하였으며, 시술이 끝난 후 최대 72시간까 통동맥을 통해 전방순환계로 혈액공급을 하고 있었고, 동맥 지 전신적인 heparin을 사용하다 증상이 호전되거나 T에 류 완전폐쇄 후 전방으로 혈액을 공급하는 후뇌동맥이 막혔으 서 뇌경색의 소견이 명백한 경우 heparin투여를 중단하였다. 며, urokinase를 사용하여 재개통을 시도하다 동맥류의 sac 밖으로 조영제의 누출이 관찰되었다. 나머지 1례는 후교통동 결 과 맥을 통한 측부순환이 없었으며, 후뇌동맥 폐쇄 후 두통과 오 심 및 구음장애와 복시를 호소하여 일간 전신적인 heparin 2례의 기저동맥분지부 뇌동맥류 환자의 남녀비는 을 사용한 뒤 증상이 호전되었고, 3주 후 혈관조영상 후뇌동 (남자 6례, 여자 19례)로 여자가 많았으며, 연령은 평균 4 맥이 재개통된 것을 확인하였으며, 합병증은 발생하지 않았다. 176 J Korean Neurosurg oc/ Volume 33/ ebruary, 2003

3 YI hun, et al. 서 혈관조영상 추가로 완전 폐쇄가 된 경우가 3례 있었고, 남아있는 동맥류가 다시 커진 경우가 1례 있었다. 관찰기간 중 추가로 완전폐쇄가 된 3례(ase, 8, 2)는 모두 dometo-neck ratio(dnr)이 2 이상이고 경부의 직경이 각각 2, 4, 6mm였다. 불완전 폐쇄된 동맥류가 다시 커진 1례(ase 18)는 경부의 크기가 10.4mm이고 DNR이 1.4였다. 불완전 폐쇄 후 관찰기간 중 변화가 없던 3례(ase 2, 6, 13)는 경 부의 직경이 mm 이하이고 DNR이 1례(ase 13)를 제외 하고는 2.0 이상이었다. 결과를 분석해 보았을 때 DNR이 2 미만인 경우 후뇌동맥 폐쇄율이 26.7%(1례 중 4례)이나 2 A 이상인 경우 0%(10례 중 례)에 달해 차이를 보였다. 경부 크기에 따른 비교에서는 small인 경우 20례 중 6례(30%), large인 경우 례 중 4례(80%)의 시술 후 후뇌동맥 폐쇄율 을 보여 뇌동맥류의 경부가 넓은 경우 후뇌동맥 폐쇄율이 높 았다. DNR이 2 이상인 경우 상대적으로 후뇌동맥 폐쇄율이 높다는 결과는 지금까지 알려진 바와 상반된 결과이기 때문에 좀더 세밀한 분석이 요구되었다. 시술 전 시행한 혈관조영상 후뇌동맥이 동맥류의 경부에서 기시하는 것처럼 보였던 경 우가 7례가 있었고, 이중 6례에서 후뇌동맥 폐쇄의 합병증이 발생하였고, 나머지 1례는 DNR이 2.9였고 동맥류의 크기는 mm, 경부의 크기는 2.9mm였으며 혈관조영상 후뇌동맥혈 B ig. 2. ollateral flow via posterior communicating artery(poa) after ipsilateral posterior cerebral artery(pa) obstruction, A Right PA occlusion by coils(arrow). This patient(ase 2) was also good after the procedure. ystemic heparinization was maintained for 48 hours. 류에 이상이 없었으나, 시술 후 일시적으로 어지러움과 경 미한 구음장애를 호소하다가 완전히 회복되었다. 이 6례 중 3례에서 DNR이 2 이상이었고 나머지 3례도 1. 이상이어 서 DNR과 상관없이 후뇌동맥기시부와 동맥류의 경부와의 관계가 시술에 의한 후뇌동맥 폐쇄의 위험인자로 여겨졌다 (Table 1, 2). 후뇌동맥 폐쇄의 원인으로는 9례 중 8례에서 Table 1.
