Recent studies have also shown that finasteride reduce angiogenesis and prostatic bleeding associated with BPH. We evaluated that pretreatment with finasteride could decrease perioperative bleeding associated with transurethral resectrion of prostate (TURP) in this way. Material and methods: A total of 56 patients who underwent TURP due to BPH between January 2004 and August 2006 were evaluated. Of the patients, 30 recieved pretreatment with finasteride 5mg daily (group 1) while 26 did not undergo any pretreatment (control group). In the group 1, 17 had pretreatment period of 3months or more (group 2). In all patients we evaluated the degree of perioperative bleeding, intended as a reduction tendency in hemoglobin (Hb) and hematocrit (Hct) value in the 24 h following TURP. Also, we evaluated the correlation of the preoperative factor and postoperative change of Hb and Hct. Results: Difference of reduction tendency in Hb and Hct between group 1 and control group was not significant (p=0.86, 0.95, respectively). Difference between group 2 and control was not significant (p=0.56, 0.29, respectively). The change of Hb and Hct correlated with prostate volume, significantly (p=0.006, 0.010). Also, operation time was correlated with the change of Hb and Hct (p=0.006, 0.011). Conclusions: There were no significant difference of the perioperative bleeding according to finasteride medication or medication duration. 2 서론경요도전립선절제술은전립선비대증으로인한중등도또는심한하부요로증상으로삶의질에영향을주거나, 급성요폐, 재발성감염, 혈뇨및신기능의악화로고통받는환자들에대한표준적인치료법으로여겨져왔다 (1,2). 경요도전립선절제술의합병증은출혈, 경요도절제술증후군, 부고환염과요로감염, 발기부전과역행성사정, 요실금과배뇨장애, 요도협착과방광경부구축등이있다 (3). 이중에서가장흔한수술후합병증은요폐, 수혈을요하는출혈, 응고된혈액에의한요폐등이있다. Uchida 등 (4) 에의하면 1971년부터 1985년까지수술받은집단과, 1986년부터 1996 년까지수술받은집단의환자 3,861명을대상으로한연구에서, 전자에서는 20.2%, 후자에서는 6.1% 가수혈이필요하였다. Mebust 등 (3) 의보고에의하면수술을시행받은환자의약 3.9% 에서수혈이필요했으며, 그중 75% 에서는 2unit 이하의수혈이이루어졌고, 절제된조직이 45g보다크면출혈의위험이높아졌고, 수술시간이 1.5시간보다긴경우에도출혈의위험이높아졌다. 이처럼경요도전립선절제술에따른술후출혈의위험성을감소시키기위하여간질성레이저응고술 (interstitial laser coagulation; ILC) 과경요도침파괴술 (transurethral needle ablation; TUNA), 경요도에탄올주사법 (transurethral ethanol ablation of the prostate; TEAP) 및 potassium- titanyl-phosphate laser (KTP) 레이저등의덜침습적인수술방법이많이이용되어왔다 (5,6). Finasteride는 5알파환원효소억제제의길항저해제로 testosterone을좀더강력한형태인 dihydrotestosterone (DHT) 으로변환되는것을막는다. 감소된 DHT는 insulin-like growth factor (IGF) 와상호작용하여전립선상피의증식을억제하며 (7), 이로인해요류가개선되고전립선비대증으로인한증상이경감하게된다 (8). Finasteride는요폐를줄이고비대된전립선으로인한수술적치료의필요성또한감소시킨다. Finasteride는전립선비대증으로인한혈뇨에대한치료로도사용된다. Miller 등 (9) 의보고에서 finasteride를복용한환자들에서평균 31개월간의장기추적결과전립선비대증으로인한혈뇨가 16명중에서 14명이개선되었으며, Puchner 등 (10) 은 finasteride를복용한군이비복용군에비하여전립선비대증에의한출혈의재발이의미있게감소되는것을확인하였다. Foley 등 (11) 은 finasteride를복용한군과 복용하지않은군으로나누어복용한군에서육안적인혈뇨가적었다는결과를제시하면서이전의 Puchner (10) 와 Miller (9) 의 전립선비대증에의한혈뇨의예방에 finasteride 가효과가있다 라는연구결과를통한가설을확인하게되는계기가되었다. 이러한 finasteride의혈뇨에대한효과를경요도전립선절제술후출혈에대하여적용한 Sandfeldt 등 (12) 의연구에따르면 3개월이상 finasteride를복용했을때경요도전립선절제술후출혈이비복용군에비하여적었으며, Crea 등 (13) 은 finasteride를 8~10주복용한군이비복용군에비해술후출혈이적었다고하였다. 이에본저자는 finasteride의복용여부및복용기간에따라서경요도전립선절제술후출혈양상에대하여어떤효과가있는지알아보고자한다. 대상및방법 2004년 1월부터 2006년 8월까지본원에서전립선비대증으로인한심한하부요로증상, 급성요폐및약물치료에효과가없었던경우에대하여경요도전립선절제술을시행하였다. 총 101명의대상환자중에서 2차례이상중복하여수술을시행받은 2명, 방광암이있었던 7명, 조직검사상전립선암으로진단된 9명, 수술전경직장초음파를시행하지않았던 2명, 요도협착이있었던 1명, 집도의가달랐던 24명을제외하고주기적인추적관찰이불가능했던 11명을제외한 56명을대상으로하였다. 56명중에서 30명은수술전 finasteride 복용군, finasteride 를복용하지않은나머지 26명은대조군으로하였다. finasteride의복용여부및기간에관계없이일괄적으로모든환자에대해서문진및직장수지검사, 요검사및요배양검사, 혈액검사로는전혈구감별계산검사, 프로트롬빈시간, 활성화부분트롬보플라틴시간, 혈액요소질소, 크레아티닌, 전해질등을기본적인검사실검사로시행하였다. 또한, 전립선비대증에대하여국제전립선증상점수 (International Prostate Symptom Score; IPSS), 요류검사및잔뇨량검사, 전립선특이항원 (prostate-specific antigen total/free; PSA), 경직장초음파검사 (8MHz, Logiq9, GE) 및요역동학검사 (DUETR ver. 8.42, Dantec) 를시행하였다. 과거력에서당뇨및고혈압, 폐결핵및간염등이있는환자에대하여내과적협의진료후에수술을진행하였다. 수술전급성요폐로도뇨관을유치한환자들은도뇨관 Vol. 12, No. 2,

3 조근현 장진석 장영섭 송기학 한동석 삽입후부터수술전까지예방적항생제를사용하였으며, 그외의환자들에대해서는수술전날및수술시작직전에예방적항생제를경정맥투여하였다. 수술은 24Fr. 또는 26Fr. Storz 절제경을사용하여시행하였고, 관류액으로 Urion용액 (d-manntol 0.54g, d-sorbitol 2.7g/100ml, 3,000ml, 중외제약 ) 을사용하였다. 수술후에는 24Fr. 3-way 30cc balloon Foley catheter를삽입하였으며, 관류용생리식염수로지속적인관류를약 2~3일간시행하였다. 수술 24시간후에전혈구감별계산검사, 전해질검사를시행하여술후출혈경향및관류용액에의한혈액희석정도를파악하였다. 수술후에이러한혈액검사에서헤모글로빈수치가수술전에비해최소 1g/dl 감소된환자들은술후 2일째재검사를시행하여지속적으로감소할경우또는수술 1일후헤모글로빈수치가 10g/dl 미만으로떨어지고어지러움증등의빈혈증상을호소할경우수혈을시행하였다. 단, 수술후 24시간이지난후시행한혈액검사에서헤모글로빈수치와 Sodium 수치가모두감소했을경우혈액희석에의한것으로생각하고수액조절을통하여다음날재검사를하였으며위와같은상황에서수혈을시행하였다 (13). 위에서제시한 finasteride 복용군과대조군에대하여수술전후의혈색소및혈구용적의변화양상을비교하였고, 2차적으로 3개월이상복용한군과대조군에대하여같은방법으로비교하였다. 또한, finasteride 복용외에다른수술전또는수술중인자가수술전후의혈색소및혈구용적차이에대한연관성이있는지분석하였다. 이러한통계적분석은 SPSS for windows 13.0을사용하였고, finasteride 복용여부및복용기간에따른각대상군의차이는 Student's t-test 및 Mann-Whitney test를사용하였고, 각군내에서수술전후의혈색소, 혈구용적및전해질수치의변화는 paired sample t-test 및 Wilcoxon signed ranks test를사용하였다. 각각의대상군간의혈색소, 혈구용적및전해질수치의변화양상의차이는 Repeated measures ANOVA를이용하여비교하였다. 혈색소및혈구용적의수술전후차이와상관성이있는수술전또는수술중인자를찾아내기위하여 Pearson 상관분석을이용하여알아보았으며, p값이 0.05미만일경우의의있는것으로간주하였다. 결과 1. Finasteride 비복용군과복용군 (Table 1, 2) Finasteride 복용유무에따라두군으로나누었을때복용군의평균복용기간은 4.98±5.74개월이었으며, 두군간에나이, 과거력, 전립선의크기, PSA 수치는큰차이를보이지않았으나 IPSS의삶의질점수가복용군에서유의하게높았다 (p=0.02, Table 1). 요류검사, 요역동학검사에서의미있는차이가없었고, 수술후조직검사소견, 수술시간, 마취방법, 절제된전립선의용적, 사용한 urion의양, 수술전혈색소및혈구용적, sodium, chloride 수치는두군에서비슷하게나타났으며, 술전 potassium 수치만이복용군에서유의하게높았다 (p=0.02, Table 1). Finasteride 비복용군은수술전후혈색소수치 (p=0.03), 복용군은혈색소및혈구용적모두의미있게감소하였다 (p<0.05, <0.05, Table 2). 두군간에수술전후의혈색소, 혈구용적및전해질의변화양상에있어서는차이를보이지않았다. 2. 비복용군과 3개월이상복용군 (Table 3, 4) 두군간에수술전요류검사의배뇨량이 3개월이상복용한군이더유의하게높았다 (p=0.04, Table 3). 두군사이에수술전후의변화양상에있어서의미있는차이를보이지않았다 (Table 4). 3개월이상복용군의 finasteride 복용기간은 8.24±5.77개월이었으며두군간에나이와과거력및술전전립선비대증에대한검사결과에있어서별다른차이가없었으며, 요류검사에서배뇨량에있어서만유의한차이를보였다 (p= 0.04, Table 3). 3개월이상복용군도비복용군과마찬가지로수술전후의혈액검사수치에있어서혈색소및혈구용적이의미있게감소하는경향을보였다 (p<0.05, <0.05, Table 4). 역시두군간에혈액검사수치의수술전후변화양상은유의한차이를보이지않았다. 3. Foley 등 (11) 은전립선비대증으로인한혈뇨에대하여 finasteride로치료받은환자들에서수술의필요성이매우감소하였다고하였다. 이들의연구에서 57명의만성간헐성혈뇨환자를 finasteride 치료를받는군과대조군으로나누어 12개월동안관찰하였고, 대조군의 17명 (63%) 이혈뇨가재발하였고그중에서 7명 (26%) 가수술이필요할정도였으며, finasteride 치료군은 4명 (14%) 에서재발하였고수술적치료가필요한환자는없었다. 이연구에서 finasteride 치료를시작하고 4주이내에혈뇨가호전되었다. Carlin 등 (14) 은전립선비대증으로인한육안적혈뇨를보이는환자 12명에대하여전향적으로 finasteride 치료를시행하였고, 2주이내에혈뇨가개선되는것을보고하였다. 또한, 혈뇨치료에대한 finasteride의효능은전립선의크기와반비례한다고하였다. Kearney 등 (15) 은급성또는재발성출혈에대해 finasteride 치료받은환자들을후향적으로분석하였으며, 급성출혈이있는군에서출혈이멈추는시간이평균 12일이었고, 전립선크기가 40g 미만일경우최소 2.7일에서 150g 이상일경우 45일이상까지길어진다고하였다. 전립선비대증으로인한혈뇨의치료에이러한 finasteride가효과가있다는것이증명되었지만, 정확한 찰 기전은아직명확히밝혀지지않은상태이다. 전립선비대증에의한혈뇨는전립선의혈관분포의증가와연관되어있을가능성이높다 (10,16). 이러한혈관형성은 finasteride로치료받은개의전립선에서전립선내혈류가의미있게감소하였던결과를토대로하여남성호르몬차단에의해억제될수있다고가정하고있다 (17). Puchner와 Miller (10) 가제안한기전은 finasteride가 testosterone이 DHT로변환되는것을막아서혈관형성을관장하는남성호르몬에의해조절되는성장인자의활성이감소하고, 이로인해전립선출혈이감소하게된다는것이다. 또한, Hagerty 등 (18) 은 finasteride에의한전립선크기감소효과로전립선의대사요구량이감소하여이차적으로혈류량이감소하게된다고하였다. Hagerty 등 (18) 은이러한 finasteride의혈뇨에대한치료효과를경요도전립선절제술을시행받을양성전립선비대증환자에게적용하였다. 이들은 finasteride를 2~4개월간복용한 25명과대조군 50명을대상으로경요도전립선절제술을시행하였고, 환자들을절제량 30g 이상과미만으로나누었을때 30g 미만일경우에는 finasteride의복용여부에관계없이술후출혈이없었으나, 30g 이상일경우 finasteride를복용한환자들은 12명중에서 1명 (8.3%) 만이술후출혈이있었고대조군의경우 19명중에서 7명 (36.8%) 이출혈이있었다고보고하였다. Sandfeldt 등 (12) 은 finasteride를 3 개월간복용한군과대조군에대하여경요도전립선절제술을시행하였으며, 두군간에수술중출혈량, 수액흡수량, 수술시간, 절제량에는차이가없었으나, 절제량이 18.6g 이상일경우에 finasteride가출혈을감소시키는효과가있다고하였다. 또한, Crea 등 (13) 은 finasteride를복용한군에서경요도전립선절제술후출혈에대하여 24시간후의혈색소가감소되는비율을통하여평가하였으며, 이러한감소율은 0.9% 로매우낮았으며, 수혈을필요로하는출혈은없었다고하였다. 그러나, 대조군에서는 12% 가수혈을필요로하였으며, 혈색소의감소율은 2.36% 로 finasteride 복용군에비하여매우높았던결과를토대로하여 finasteride가술후출혈의감소에중요한역할을한다고강조하였다. 저자의예에서는수술전 finasteride의복용여부및복용기간에관계없이술후혈색소의감소양상에는차이가없었다. 본문에제시하지않았지만, Crea 등 (13) 이제시한방법으로술후출혈을평가한경우에도역시각군간에통계적으로유의한차이를갖는결과는없었으며, Hagerty Vol. 12, No. 2,

7 조근현 장진석 장영섭 송기학 한동석 (18) 와 Sandfeldt 등 (12) 이절제량과출혈간에유의한상관관계를갖는다고하였으나본예에서는없었고수술전전립선의크기와수술시간이혈색소및혈구용적의수술전후의차이에대하여의미있는상관성을보였다. Kim 등 (20) 은경요도전립선절제술중의실혈량에관한연구에서실혈량과절제된조직의무게, 수술시간과상관관계가있다고하였으며, Park 등 (21) 은경요도전립선절제술중의실혈량에대한연구결과실혈량은절제시간및사용된관류액의양과통계학적인의의가있었다고하였다. 이러한결과를저자의결과와비교하였을때저자의연구목적은수술후출혈이라는점에서이들의결과와차이가있었지만, 수술시간이출혈과관련된인자라는면에서는유사하였다. 이렇듯, 여러연구자들의결과가각각차이가있었던이유는술기적인측면및출혈분석방법의차이로인하여나타난것으로생각되며, 본예에서분석한출혈경향의분석결과 finasteride의복용여부및기간이경요도전립선절제술후출혈에대하여큰영향을미치지않았으며, 오히려다른인자들이수술후출혈과상관성이있는것으로나타났다. Finasteride의전립선비대증에의한혈뇨치료에대한기전은이미앞에서기술하였다. 이러한기전은여러연구자들이 finasteride를사용한전립선세포내에발현되는 VEGF (Vascular endothelial growth factor) 및미세혈관밀도의변화를확인함으로써뒷받침되고있다 (22-24). 최근연구에서양성전립선비대증으로전립선수술을시행받는환자들대상으로 5알파환원효소억제제를복용한군과대조군을비교한결과결절과다증식조직 (nodular hyperplasia tissue) 은대조군에비해 VEGF와미세혈관밀도가차이가없지만, 전립선의요도하 (suburethral) 조직에 VEGF와미세혈관밀도의발현이실험군에서의미있게감소하는것을발견하였고, 이는 finasteride가 VEGF의발현을감소시켜혈관형성을억제하고미세혈관밀도를감소시켜전립선요도출혈을줄일것이라는기전을주장하였다 (22,23). Donohue 등 (24) 은이러한기전을역시같은방법으로증명하였으며, 특히 finasteride의복용기간이 2주정도만지나더라도전립선의미세혈관에대한감소효과를나타낼수있다고하였다. Lekas 등 (25) 도 finasteride를복용한군에서경요도전립선절제술후출혈의감소효과를보였으며, 요도하전립선조직에서 VEGF 및미세혈관밀도, HIF-1α (hypoxiainducible factor-1 alpha) 의의미있는발현감소가나타나술후출혈의감소효과에대한구체적인증거를제시하였다. 하지만 Hahn 등 (26) 은전립선비대증환자를대상으로 dutasteride를수술전에투여한결과투여군에서출혈양감소가없었으며결절과다증식조직과요도하조직의미세혈관밀도역시변화가없다고보고하였다. 이러한결과는향후 5알파환원효소차단제의술후출혈에대한효과를평가할수있는객관적인기준이될수있을것이며, 임상적인결과와더불어앞으로더욱많은연구가필요할것으로생각한다. 결론기존의연구자들이제시한 finasteride의복용기간과저자의예에서 finasteride의복용기간사이에큰차이가없었지만, 저자의결과는이전과다른결과를나타냈다. 즉, finasteride 가술후출혈에대한영향이없었다는결과를보여주었다. 또한, 절제된전립선의용적보다는오히려수술전전립선의크기, 수술시간이출혈과상관성이있는것으로봐서 finasteride의복용이술후출혈에미치는영향은다른인자에비하여그리높지않은것으로생각된다. 향후이러한결과를뒷받침할수있는세포학적인연구가필요할것이다. 저자는 finasteride의술후출혈에대한예방적차원에서의복용이실제적으로미치는영향은그리크지않을것으로여겨지며, 경요도전립선절제술의여러가지다른수술전인자에대한수술전평가및준비가오히려술후출혈을더욱줄일수있는기반이될것으로생각한다.
