1 대한방사선의학회지 1994 : 31( 5) : 제 2 새열낭종의전산화단층촬영소견 : 병변의위치를중심으로 1 한덕섭 김병근 서정진 2 박병란 김세종 고강석 오종섭 목적. 이연구의목적은저 12 새열낭종의 CT소견을알고자함이다. 대상및방법 : 수술과병리학적으로확진된제 2 새열낭종 9예의 CT영상을후향적으로분석하였다. 저자들은심경근막에의하여나뉘어지는근막공간에따른병변의위치를중점적으로분석하였다. 결과. 제 2 새열낭종 9예모두에서종괴가악하선공간과경돔맥공간에위치하였고, 이들중 6 여 ( 67 %) 에서는다른인접공간을동시에침범하였다. 9예모두는원형혹은난원형의단방성낭성종괴이었고, 일부혹은전체적인테두리조영증강이있었으며, 석회화는없었다.8예 (89%) 에서평활한밟은벽을관찰할수있었고, 1 예에서는두쩌운벽과중격을볼수있었다. 낭성종괴의내용물은비교적균 질하였고, CT 번호는 Hounsfield unit(hu) 으 분포를보였고, 평균 28.4HUO I 었다. 결론 : 제 2 새열낭종은전형적위치, 특징적형태와더불어, 흔히다른인접공간을동시에침빔할수있다는점등으로쉽게 CT진단을할수있다. 서 경부제 2 새열낭종 (seco nd branchial cleft cyst, 이하 2nd BCC 로줄임 ) 은 CT 소견상특징적형태와전형적위 치, 그리고인접구조물의전위에의하여수술전에진단을 할수있다. 이들소견중가장중요한것은전형적위치이 다 ( 1-7 ). 그렇지만버전형적위치에생기거나, 인접한다른 근막공간을함께침범한경우에는진단에어려움이었다 (2, 3, 8). 저자들은 2nd B CC 의발생위치를중심으로 CT 소견을분석하여진단에도움이되는소견을얻고자하였 다. 대상및방법 년부터 년까지지난 4 년간수술과병리학적으 로확진된 9 예의 2nd BCC 을대상으로하여 CT 소견을후 향적으로분석하였다. 2nd BCC 환자의연령분포는 18세부 터 55 세 ( 평균 3 1 세 ) 다. 론사이였고, 남자가 3 명여자가 6 명이었 사용된 CT 기기는 GE 9800 scanner(ge Medical Systems, Milwaukee, USA) 이었다. 모든환자에게이온성조영제 (Conray, 1 광주기독병원방사선과 2 전남대학교의과대학방사선과학교실 이논문은 1994 년 4월 16일접수하여 년 11 월 9일에채택되었음 ebec, Canada) 를사용하였는데, 성언에서는 150ml를, 어린이는체중 kg당 3ml를정맥주사해조영증강을시행했고, 스캔간격은 5-lOmm, 두께는 5-1 0mm이었다. 분석한 CT소견은위치, 크기, 전체적인모양, 내용물, 벽두께, 조영증강, 중격, 석회화와내 외경동맥사이로종괴 가끼이는모양등이었다. 저자들은심경근막 (deep 60% iothalamate meglumine: Mallinckrodt Institute Canada, Qu- cervical fas c ia) 에의하여나뒤어지는근막공간 (fascia l space) 에따른병변의위치를중점적으로분석하였다. 결과 2nd BCC 9예모두에서종괴는악하선공간 (subm andibular space) 과경동맥공간 (carotid space) 에위치하였다 (Fig. 1). 이들중 6예 (67%) 에서는다른인접공간을통시에침범하였는데, 전경공간 (anterior cervical space) 을침범한경우가 2예 (Fig. 2), 후경공간 (posterior cervical space) 을침범한경우가 2예 (Fig. 3), 그리고전 후경공간을함께침범한경우가 2예 (F i g.4) 였다. 종괴의크기는최대직경이 4cm에서 7cm까지로평균 5.2cm였고, 크기와인접공간의동시침범여부와는상관관계가없었다 (Table 1). 2nd BCC 9예모두는원형혹은난원형의단방성 (unilocular) 냥종이었고, 일부혹은전체적인테두리조영증강이있었으며, 석회화는없었다. 8 예 (89%) 에서평활하고않은벽을관찰할수있었고, 1 예에서는비교적두꺼운벽 (4mm) 과중격을볼수있었다 (Fig. 5). 종괴내부는비교적균질하였고, CT번호는 H U의분포를보였 - 807

2 대한방사선의학회지 1994 : 31(5) : 고, 평균 28.4 HU이었다. 1 예에서는냥성종괴가내 외경동맥사이로끼이는모양이관찰되었다 (Fi g. 1b). 고찰 2nd B CC는엄상적으로보통젊은성인에서하악각 (mandible angle) 근처의종괴로말현한다 (1-4, 7, 9). 태생기에배아새열 (embr yo nic branchial cleft) 의정상적인완전폐쇄가이루어지지않고잔류새열길 (residual branchial cleft tr act) 이형성되면새열기형 (branchial cleft anomaly) 이발생하는데, 낭종, 동 (sinus), 누공 (fistula) 의형태로나타난다. 이들중 2nd BCC가가장흔하다 (2-4, 6,7). 경부종괴의 CT진단에있어서섬경근막에의하여나뉘어지는근막공간에따라종괴의위치를정하는것이중요한데, 이와마찬가지로 2nd BCC 의 CT소견중가장중요 한컷은종괴의위치이다 (1-7, 10, 11). 2nd B CC 의전형적 CT소견은냥성종괴가악하선공간과경동맥공간에걸쳐있으면서, 흉쇄유돌근 (s ternoclei d omastoid muscle) 의내측, 경동맥공간의외측, 악하선의후방에위치하고, 이들인접구조물을전위시킨다 (Fig. 2a ) (1-7 ). 그렇지만 2nd BC C는잔류새열길을따라서어디서나생길수있다. 즉잔류새열길의인두부위 (pharyn geal portion) 에서는부인두공간 (parapharyngeal space) 에, 인두말단에서는외측인두에생기며, 악하선높이에서는악하선공간과경동맥공간사이에, 설골하경부에서는전경공간에낭성종괴로나타나므로진단이어려울때도있다 (1-3, 6, 11). 저자들은 l 예의 8 세남아에서제 2 새열동 (second branchial cleft s inus) 을 CT영상으로관찰할수있었는데, 동로 (sinus tract) 가구개편도에서쇄골상부피부구 (cutaneous opening in the supraclavicular region) 까지뻗어있었으며, 구연두에서는구개편도에, 악하선높 Table 1. Salazar 등 (5) 은잔류새열길이경동맥의분기점 (bifurcat i o n) 상부에서내경동맥과외경동맥사이를지나가기때문에낭종이내 외경동맥사이로파고들어가면확실한진단을할수있다고하였고, 본연구에서도 l 예에서관찰하였c]-( Case 1, Fig. 1b). 드물게는 2nd BCC가후경공간을단독으로침범하여생긴다고하나 (2, 5), B e ns o n등 ( 6) 과 Park er등 (1 2) 은이를제 3 새열낭종 (third branchial cleft cyst) 이라하였다. 두꺼운낭종벽의조영증강은감염된 2nd BC C를시사하는소견으로알려져있고 (2-4), 본연구에서도 l 예 ( Case 5, Fig.5) 에서통일한소견을관찰하였다. 감염된 2nd B CC 의내용물이감염이안된 2nd BCC 에비하여 CT번호가높을것으로생각되였으나, 본연구에서는 2예의감염안된 2nd B CC (Case 6, 9) 에서오히려더높은 CT번호가측정되어, CT번호만으로감염을판단할수는없었다. 2nc B CC 의전형적위치에발생한다른냥성병변, 즉냥성수종 (cystic h ygroma), 농양, 괴사성종양, 괴사성럼프절, 방정중의갑상설관낭 (paramedian thyroglossal duct cyst), 유피종 ( d e rmoid), 경정맥혈전증등과감별해야한다 (2-4, 7). 평활한앓은벽을가진낭성종괴가전형적위치에있는경우는 2nd B CC 의진단이비교적용이하였으나, 냥성종괴가다른인접공간, 특히후경공간을크게침범한경우 (F ig. 4) 는냥성수종과의감별이문제가되었는데, 이는냥성수종이주로후경공간에생기면서주위로스며 드는양상으로나타나기때문이다 ( 3, 4, 7). 그러나낭성수 종은약 90% 가 2세이전에발현하고, 다방성인경향이많 고, 또한주위연부조직을전위시키기보다는눌리는양상 으로보이기때문에감별할수있었다 (3, 4, 7).1 예의감염 된 2nd B CC 는두꺼운냥종벽을가지고있어서농양, 괴사 성종양흑은괴사성럼프절등과감별이어려웠고, 임상소 견과환자병력이도움은되었으나, 완전한감별은안되었 다 (2-4). 방정중의갑상설관낭은갑상설골막 ( thyrohyoid membran e) 에붙어있어서감별이가능하고 (3), 유피종은 지방성분이았으면진단가능하나, 구별이안되는경우도많다 ( 3, 4). 경정맥혈전증은조영증강후 CT영상에서위 아래의연속적영상을주의갚게관찰하면감별이된다 (4). 결론적으로 2nd BCC 는전형적위치, 특정적형태와더 불어, 흔히다른연접공간을동시에침범한다는점등으로 쉽게 CT 진단을할수있다. ~~ C 그 고 C 그 ïr 1, Fruin ME, Smoker WRK, Harnsberger HR, The carotid space 1 the inlrahyoid neck, Semin Ultrasound CT MR 1991 ; , Miller MB, Rûo VM, Torm BM, Cystic masses 01 the head and neck : pitalall s in CT and MR interpretation, AJR 1992 ; , Harnsberger HR, Mancuso AA, Muraki AS, et al. Materials and Methods: We retrospectively analyzed the computed tomographic images in nine cases of second branchial cleft cyst which was confirmed path 이 ogically. Emphasis was on localization of the masses to fascial spaces as defined by the deep cervical fasica. Results: In all nine cases, the lerions were located in the submandibular and carotid spaces. Among these cases, six(67 %) had simultaneous involvement of the other contiguous spaces, such as anterior and posterior cervical spaces. AII cases had round or oval, unilocular, cystic masses with partial or complete rim enhancement. In eight cases(89%), smooth and thin walls were observed. In one case, thick wall and septations were noted. No definite calcifications were noted in all cases. The internal contents of cystic masses showed relatively homogeneous appearance, and CT number ranged from 20 to 35.2 Hounsfield unit(hu)(mean, 28.4HU).
