Materials and Methods: We retrospectively reviewed imaging findings of 70 patients with histopathologically proven mediastinal cystic lesions. They were 33 male and 37 female patients. Results: Among 70 cases, 49 cases were in the anterior mediastinum, 12 cases were in the middle mediastinum, and 9 cases were in the posterior mediastinum. 19 patients had symptoms. Chest discomfort was the most common symptom. When the cystic lesion was located in the anterior mediastinum, and unilocular, the possibility of thymic cyst was the most likely (p < ). When the cystic lesion was located in the anterior mediastinum and was multilocular with a relatively thick wall, the possibility of a mature cystic teratoma was the most likely (p < 0.001). When the lesion was a high attenuation cystic lesion located around the air-way, the possibility of a bronchogenic cyst was the most likely (p < 0.001). Conclusion: Chest CT gives information about the location, loculation, wall thickness and internal attenuation of mediastinal cystic lesions. And certain details seen on CT imaging can help with the correct diagnosis, especially in the cases of thymic cyst, mature cystic teratoma and bronchogenic cyst. Tel Fax yhkim001@jnu.ac.kr This is an Open Access article distributed under the terms of the Creative Commons Attribution Non-Commercial License ( which permits unrestricted non-commercial use, distribution, and reproduction in any medium, provided the original work is properly cited. 서론 종격동낭성종양은액체를포함하며경계가잘지워지고, 둥근모양을갖는다. 종격동낭성종양은종격동의어느부위에서나발생할수있으며, 최근국내보고에따르면종격동낭성종양은전체종격동종양의 11.7% 를차지하는흔하지않은종양이다 (1). 종격동낭성종양은기관지원성낭종, 흉선낭종, 심낭낭종, 성숙낭성기형종, 식도낭종, 신경장낭 (neurenteric cyst), 림프관종등이있다. 이러한종격동낭성종양은다른종양에비하여상대적으로드물게발생하며영상소견이서로중복되는경우가많아감별진단이어려운경우가종종있다. 따라서이연구의목적은흉부전산화단층촬영 (computed tomography; CT) 을시행한환자중종격동낭성종양이있으 면서외과적시술을통해조직학적으로증명된경우를후향적으로분석하여종격동낭성종양의감별진단에도움이되는소견이있는지알아보고자하였다. 대상과방법 대상환자 2004 년 1월부터 2012 년 3월까지외과적수술을시행후종격동낭성종양이증명된 90명의환자중의무기록을통한후향적조사가불가능하거나, 흉부 CT를찾을수없는경우그리고흉부 CT에서정확한정보를얻기힘든 20명을제외한 70 명의환자를대상으로하였다. 최종진단은적출된표본의병리조직학소견으로결정하였다. 2 흉부전산화단층촬영을이용한종격동낭성종양의감별 흉부전산화단층촬영술 (Chest Computed Tomography; CT) 흉부 CT 촬영은여러다중검출기 CT (multidetector CT; MDCT) 기계들 (Light speed 16, Light speed VCT, Light speed QX/I: GE Medical systems, Milwaukee, WI, USA; Cardiac 64, Somatom Definition Flash: Siemens Medical Solution, Erlangen, Germany) 을이용하였다. 흉부 CT는모든환자에서조영증강전후영상을얻었으며, 조영증강후영상은비이온조영제인 Omnipaque R (Amersham health, Co., Ireland) 와 Ultravist 300 R (Schering, Berlin, Germany) 120~130 ml를 2 ml/sec 의속도로정맥주사를시작한후 60~70 초후에얻었다. 절편의기준은 5 mm, 두께는 3.8 mm였으며, 촬영조건은 120 kvp 와자동조절 ma였다. 흉부 CT 소견의분석영상은 5년이상의흉부 CT 판독경험이있는 2명의흉부영상의학과전문의가분석하였다. 영상을분석한영상의학과의사에게환자의이름, 나이, 성별, 최종진단명등의임상정보는주어지지않았다. CT 영상은무작위로영상의학과의사에게주어졌으며팩스 (PACS, picture archiving and communication system; MAROSIS, Infinity, Seoul, Korea) 모니터에서분석하였다. 본연구에서는기존에보고된여러분류방법중전체종격동을전종격동, 중종격동, 후종격동으로나누고상중격동을따로 나누지않는방법을사용하였다 (2). 각각의종격동구간에있는개별낭성종양에대해위치, 크기 ( 장경, 단경 ), 모양, 종양의구성요소 ( 액체, 지방그리고석회화 ), 종양벽의두께를분석하였다. 내부감쇠값 (attenuation) 은 region of interest 를이용해측정하였다. 종양벽의두께는낭성종양의벽중가장두꺼운부위를기준으로측정하였다. 결과에대한통계적분석통계학적분석은 SPSS (The Statistical Package for Social Sciences for Windows, version 17.0, Chicago, IL, USA) 를이용하였다. 개별낭성종양의위치, 크기 ( 장경, 단경 ), 모양, 경계, 종양의구성요소, 종양벽의두께등을카이제곱검정 (chisquare test), 이분형로지스틱스, analysis of variance 등을이용하여비교분석하였으며, 유의수준은 p < 0.05 로규정하였다. 결과 본연구의전체적인정보는 Table 1과같다. 대상환자는총 70 명이며남성이 33명, 여성이 37 명으로성별에따른차이는없었다. 평균연령은 44.5 ± 18.8 세 (4개월 ~77 세 ) 였다. 진단당시임상적증상및징후의발현은 19예 (27%) 에서관찰되었다. 비특이적흉부불쾌감이 10명으로가장많았고만성기침 6명, Table 1. 6 cm) 로다른종격동낭성종양에비하여길었으며기관지원성낭종은평균장경 3.6 ± 1.4 cm(1.7~5.7 cm) 로다른종격동낭성종양에비하여짧았다 (p < 0.01). 전종격동에위치하는종양 49 예중단방성낭성종양이 31 예, 다방성낭성종양이 18 예였다.

3 이승진외 목통증 2명, 호흡곤란 1명순이었다. 수술을시행한이유는상기한 19 예처럼증상이동반된경우와, 추적관찰기간동안에크기가증가한 2예가있었으며나머지는환자자신이원하는경우로수술을시행하였다. 조직학적으로증명된종격동낭성종양은흉선낭종 24예, 기관지원성낭종 15예, 심낭낭종 10예, 성숙기형낭종 13예, 식도낭종 3예, 림프관종 2예, 신경절신경종 2예, 가슴관낭종 1예였다. 전종격동에위치하는낭성종양이 49예, 중종격동에 12예그리고후종격동에 9예였다. 크기를비교하였을때성숙낭성기형종의평균장경은 7.4 ± 3.2 cm(4~13.6 cm) 로다른종격동낭성종양에비하여길었으며기관지원성낭종은평균장경 3.6 ± 1.4 cm(1.7~5.7 cm) 로다른종격동낭성종양에비하여짧았다 (p < 0.01). 전종격동에위치하는종양 49 예중단방성낭성종양이 31 예, 다방성낭성종양이 18 예였다. 전종격동에위치하는단방성낭성종양 31 예중 21 예가흉선낭종이었으며이는통계학적으로다른낭성종양과감별하는데의의가있었다 (p = 0.027)(Table 2). 전종격동에위치하는다방성낭성종양 18예중지방을포함하는경우가 7예로이중 5예에서석회화를함께동반하였다 (Fig. 1). 이 7예모두는성숙낭성기형종이었다. 이 7예를제외한 11예의전종격동순수다방성낭성종양의경우성숙낭성기형종의벽두께가 3.1 ± 1.6 mm(1.5~5.3 mm) 로다른다방성낭종에비하여통계적으로유의하게두꺼웠다 (p = 0.008)(Table 3, Fig. 2). 기도 ( 기관, 기관지, 기관용골등 ) 에인접하여위치하는낭성종양은 13예로기관지원성낭종이 9예, 심낭낭종이 3예, 식도낭종이 1예가있었으며기도에인접한낭성종양은기관지원성낭종일가능성이통계학적으로의의가있었다 (p < 0.001). 특히기관지원성낭종의경우에는내부감쇠값이 35.5 ± 15.0 Hounsfield unit (HU) 으로측정되어기도에인접한다른종격동낭성종양에비하여유의하게높았다 (p < 0.001)(Fig. 3). 기타이번연구에서나타난심낭낭종, 식도낭종, 림프관낭종, 신경절신경종, 가슴관낭종은 CT 소견만을가지고다른종격 Fig. 1. Contrast enhanced CT scan shows a thin-walled water attenuation cyst in anterior mediastinum. 동낭성종양과감별하기어려웠다. 고찰 종격동낭종은비교적흔하지는않지만최근정기적인건강 검진이일반화되면서발견이점차증가하는추세에있다. 종격 동에서발생하는양성낭종은 1) 경계가매끄럽고타원형또는 관모양으로, 경계가명확하며조영증강되는얇은벽을갖고, 2) 물과같은감쇠값 (0~20 HU) 을갖고균질한양상을보이 며, 3) 조영증강되는고형물이없고, 4) 주변종격동으로침습 하지않는병변으로정의된다 (3, 4). 종격동낭성종양은대부 분증상이없는상태에서우연히발견되는경우가많으며간혹 주변구조물들의압박을통해서증상을나타내게된다. 본연 구에서증상이있는경우는 19 명으로대부분증상을호소하지 않았으며이는조기에진단되는이유에기인할수있으리라생 각된다. 흉선낭종은흔하지않은종양으로종격동종양의 1% 를차지한다 (5). 선천성과후천성모두나타날수있으나선천성흉선낭종은드물며흉선인두관 (thymopharyngeal duct) 에서발생한다. 50% 정도의선천성흉선낭종이 20 세이내에우연히발견된다 (6). 후천성흉선낭종은호지킨병 (Hodgekin disease) 의방사선치료 (7) 나가슴샘종양과연관되어발생하거나 (8) 가슴관삽입술 (thoracostomy) 후에발생한다 (9). 흉선낭종은단방성또는다방성이다 (6). 다방성흉선낭종은다양한염증반응의결과이다 (10, 11). 큰다방성흉선낭종은사람면역결핍성바이러스에감염된소아의대략 1% 에서관찰된다 (12). CT에서단순선천성흉선낭종은경계가잘지워지며물과같은감쇠값을갖는종괴이고낭종의벽은대게인지되지않을정도로얇다. 다방성흉선낭종은경계가잘지워지고불균질하며단방성또는다방성의종괴로관찰된다 (11). 그러나어떤흉선낭종은감염이나출혈등으로 CT에서감쇠값이올라갈수있고이로인해고형종괴로오진할수도있다. 낭종벽에곡선모양의석회화가관찰되기도한다 (6). 본연구에서흉선낭종은모두전종격동에위치했으며단방성종양 21 예다방성종양 3예로관찰되었다. 대부분물과같은감쇠값을가졌으며석회화는관찰되지않았다. 전종격동에위치하는물과같은감쇠값을갖는단방성낭성종양은다른종격동낭성종양과감별할수있는특징이었다. 성숙낭성기형종은세개의종자세포 (germ cell) 중적어도두개에서유래된잘분화된물질로구성되어있다 ( 외배엽, 내배엽그리고중배엽 ). 외배엽에서는피부, 치아, 머리카락을, 중배엽에서는뼈, 연골, 근육을, 내배엽에서는기관지, 위창자상피 (gastrointestinal epithelium) 와췌장조직으로구성된다. 전형적으로낭종을형성하며낭종은크고점액을생산하는상피세포로이루어져있다 (13). 낭종은피지 (sebaceous material) 물질로채워져있으며머리카락을함유하고있다. 털주머니 (hair follicles), 피부샘, 근육, 그리고다른조직들은벽내부에있다. 성숙기형낭종은가장흔한종자세포종양이다. 젊은성인에서더자주발생하며대부분은증상이없고우연히발견되지만큰종양의경우흉통, 호흡곤란, 기침또는압박에의한다른증상들을나타낸다 (6). 대부분전종격동에발생하고 3~8% 가후종격동에발생한다 (13). CT에서종양은불균질하며, 경계가좋은종양으로조영증강이되는다양한정도의벽두께를갖는다 (6, 14). 연조직, 액체, 지방, 석회화 4가지조직형을모두가질수도있지만액체를포함하는낭성물질이가장도드라지게나타난다 (14). Moeller 등 (15) 이보고한바에따르면성숙기형낭종의경우연조직을모든경우에갖고액체를 88% 에서, 지방은 76% 에서, 석회화는 53% 에서갖는다. 이 4가지가모두보이는경 290 대한영상의학회지 2013;69(4): jksronline.org

5 이승진외 우는 39% 였다. 15% 의증례에서는낭종으로만이루어졌으며지방이나석회화는관찰되지않았다. 대부분의성숙낭성기형종은다방성이지만단방성일때도있다. 석회화는부분적으로, 곡선형또는드문경우치아나뼈가나타나기도한다. 종괴내부의지방-액체층은이질환에매우특징적인소견이지만자주관찰되는소견은아니다 (15, 16). 본연구에서성숙기형낭종은 13예로모두전종격동에위치하였다. 단방성종양으로관찰되는경우가 1예, 나머지 12예는다방성종양으로관찰되었다. 액체, 지방, 석회화모두를보이는경우가 5예로 38.5% 에해당했으며이는이전연구와비슷한결과를보였다. 6예는지방이나석회화를포함하지않는낭종으로관찰되었으며특히이들은다른종격동낭성종양보다두꺼운벽두께를가지고있었다. 낭종으로만이루어진성숙낭성기형종이다른종격동낭성종양에비해두꺼운벽두께를가지고있다는점은이전연구에서는밝혀지지않았던점이다. 전종격동에위치하는다방성낭종중지방과석회화를포함하는경우에는비교적쉽게성숙낭성기형종을진단할수있지만본연구에서와같이전종격동에위치하는다방성낭종중석회화나지방이보이지않는경우라도두꺼운벽두께를가지고있는경우성숙낭성기형종을감별질환에포함시키는것이진단에도움이될수있을것이다. 기관지원성낭종은기관기관지분지 (tracheobronchial tree) 의비정상적인발아 (ventral budding) 또는분지 (branching) 에의해서일어난다. 기관지원성낭종은거짓중층원주상피 (pseudostratified columnar respiratory epithelium) 로되어있고이들벽에는연골, 근육그리고점액샘조직 (mucous gland tissue) 을포함한다 (17). 기관지원성낭종은장액성또는점액성물질로채워져있다. 종격동의어느부위에서든발생할수있지만대부분은기관용골근처의중종격동또는후종격동에서가장많이발생하며아주드물게폐실질조직이나흉막또는횡격막에발생한다 (18). 기관지원성낭종은때때로폐분리증 (sequestration), 엽성폐기종 (lobar emphysema) 과같은선천성폐기형 (congenital pulmonary malformation) 과연관되어있지만흔하지않다 (19). 갑자기크기가커지기도하며이로인해출혈과감염이생기기도한다. 대부분의경우증상이발생하지않지만주변장기를압박하여증상이생기기도한다. 증상에는흉통, 기침, 호흡곤란, 발열, 그리고고름가래등이있다 (20). CT에서는단방성낭종으로둥글거나타원형모양으로관찰되고인지되지않을정도의얇은벽두께를가지며균질한감쇠값을갖는다. 감쇠값은낭종의내용물에따라다르게나타나는데물과같은감쇠값에서부터연조직의감쇠값까지다양하다. 때때로 100 HU 가넘을수도있는데이것은높은단백질농도또는점액낭종의수산칼슘돌 (calcium oxalate) 에의한다 (21, 22). 낭종내부 에공기가관찰된다면감염에의한것이거나기관기관지분지와연결성이있기때문이다. 석회화는낭종벽또는낭종내부에서보일수있다. 본연구에서기관지원성낭종은총 15 예로전종격동에 5예, 중종격동에 7예, 후종격동에 3예가관찰되었다. 특히내부감쇠값이 35.5 ± 15.0 HU 로높게측정되어기도에인접해서관찰되는총 13예의종격동낭성종양중기관지원성낭종의감별점이되었다. 본연구의제한점은첫째, 후향적인연구방법으로진행되었으며흉부 CT만의영상소견을가지고분석한점이다. 향후 MRI 등다른종류의영상검사를함께시행한경우를추가적으로분석한다면본연구에서감별하기어려웠던종격동낭성종양을정확히진단하는데도움이되는소견에대해연구가필요하다. 둘째, 수술받은 70 예의환자만을대상으로했으며이는전체종격동낭성종양환자를대변할수없다는한계를가지고있다. 마지막으로일부종격동낭성종양은증례수가적어통계적으로유의성을검증하기어려웠다는점이다. 결론적으로흉부 CT는종격동낭성종양의위치, 모양, 내부성상, 기도와의관계, 그리고벽의두께를잘파악할수있도록해주며이를통해흉선낭종, 성숙낭성기형종과기관지원성낭종의감별에도움을준다. 이런결과는흉부 CT에의한종격동낭성종양의진단및치료방침을결정하는데도움을줄수있을것으로기대된다. 참고문헌 1. 7 이승진외 흉부전산화단층촬영을이용한종격동낭성종양의감별 1 이승진 1 백장미 1 송장현 1 박서연 1 선현주 1,2 김윤현 1,2 목적 : 흉부전산화단층촬영에서종격동낭성종양의감별진단에도움이되는영상소견이있는지알아보고자하였다. 대상과방법 : 수술후조직학적으로증명된 70 예의종격동낭성종양을가진 70 명의환자에대해임상소견과흉부 CT 소견을후향적으로분석하였다. 4개월에서 77세의나이분포를보였고남성이 33명, 여성이 37 명이었다. 결과 : 종격동낭성종양 70 예중전종격동에위치하는낭성종양이 49예, 중종격동에 12예그리고후종격동에 9예였다. 진단당시임상증상및징후의발현은 19예에서있었고비특이적흉부불쾌감이 10명으로가장많았다. 전종격동에위치한단방성낭종은흉선낭종일가능성이통계적으로유의하게높았으며 (p = 0.027) 다방성낭종은성숙낭성기형종일가능성이통계적으로유의하게높았다 (p < 0.001). 또한성숙낭성기형종은다른다방성낭종에비해서낭종벽의두께가두꺼웠다 (p = 0.008). 기도 (air-way) 주위에위치하는낭종이면서내부감쇠값이높은경우에기관지원성낭종일가능성이통계적으로유의하게높았다 (p < 0.001). 결론 : 흉부 CT는종격동낭성종양의위치, 모양, 기도와의관계, 감쇠값그리고벽의두께를잘파악할수있도록해주며, 이를통해흉선낭종, 성숙낭성기형종과기관지원성낭종의감별에도움을준다. 1 전남대학교병원영상의학과, 2 화순전남대학교병원영상의학과 jksronline.org 대한영상의학회지 2013;69(4):
