To evaluate the efficacy of utilizing CT scanning in deep cervical space abscess, we retrospectively revieweed CT of 17 patients confirmed by surgery. CT was found to be veπ accurate in defining the presence, location, and extent of the abscess along with the demonstration of life-threatening complications such as airway compression and mediastinal extension. In conclusion, CT can play and essential role in guiding surgical approach to deep cervical space abscess. Index Words: Neck, infection 7.0 Neck, computed tomography 7.111,. 서 론 양배액의외과적치료계획을세우는데도움을주고자한 다. 심부경부의농양은두경부의염증성질환에대한항생 제요법으로그빈도가상당히감소되었지만여전히생 명의위협을초래할수있는질환이다. 그러나, 임상소견과일반단순촬영소견만으로는농양 의정확한진단과해부학적위치결정이어려운경우가 많으며, 또한배액이필요한농양이형성된경우와단순 한세포염 ( ce Il ulitis) 과의감별도거의불가능한것으로 알려져있다 (1-3). 이에저자들은수술적배액술로심부경부농양으로확 진된환자에서경부 CT 를수술소견과비교분석하여농 양의정확한해부학적위치와범위를결정해줌으로써농 대상및방법 1990년 5월부터 1 99년 1 월까지대구가톨릭대학병원을내원하여수술로써경부농양으로확진된 17예를대상으로하여 CT 소견을후향적으로분석하였다. 남자가 1 예여자가 5예였으며연령분포는 세에서 73 세, 평균연령은 37세였다. 감염된종양, 선천성낭종및결핵성입파선염은대상에서제외하였다. 사용한 CT 기종은 HITACHI CT W- 500 이었고절편두께와간격을각각 5mm로하여전예에서조영증강 * 대구가톨릭대학의학부이비인후과학교실 *Dφartm ent 01 Otolaryngology, School 01 Mediciη e, Taegu Catholic University 이논문은 1993 년 1 월 1 3 일접수하여 1993 년 3 월 일에채택되었음

CT 상병변이관찰된부분중주된농양이위치한공간 (space) 과그에수반된인접부위의염증침윤이생긴공 간을나누어분석하여배액이필요한농양의정확한위치 를결정하는데주안점을두었다. 또한농양에동반되어 나타날수있는기도압박, 종격동침범, 내경동맥혈전 증등합병증유무도함께분석하였다. 7~ 끼 L E크 -, 농양이위치한부분은설골상부가 10 예 (5 9%) 로가장 많았고설골하부에위치한경우는 5 예 (9%), 설골상하 부공히걸쳐있었던경우는 예 ( 1% ) 이었다. 그리고농 양이위치한각공간은 CT 와수술소견이모두일치하 였다 (Table 1). CT scans through the levels of nasopharynx and orophaπnx show eccentrically located, m버 tiseptated abscess (arrows) mai띠y in the left retrophaηngcal space compressing the airway (arrowheads) and parapharyngeal fat (curved arrows) a b CT상농양은모든환자에서다양한크기의저밀도음영으로나타났고 14예에서인접한공간에광범위한연부조직의종창, 선상침윤또는지방층의소실이함께관찰되었다 (Fig.1) 동반된다른 CT 소견들로는공기음영을보인경우가 예있었으며, 인근입파선증대 1 예, 하악선증대가 8예있었다. 특히하악하공간과경동맥공간에위치한농양 7예중 5예에서는임파선의증대뿐아니라하악선의증대도함께관찰되었다 (Fig.). 경부농양으로인한합병증으로나타난소견들로는정도의차이는있으나기도압박이 1예로가장많았으며, 특히농양이인두후공간 (retropharyngeal space) 에위치한 6예의경우에는모두에서기도압박을보였다 (Fig.3) 이중 1예에서는농양이종격동까지파급되었는데이환자에서는임상적으로구인두로의자연배농도함께관찰되었다 (Fig.4). 그리고, 농양의수술적배액술시염두에두어야할다방형성 (multiloculation) 된경우도 3예있었다 (Fig.5). 심부경부농양의동반된소견및합병증을요약하면 Table 와같다. 주증상은대부분경부종창, 연하통, 발열등이었으며농양의원인이된것으로추정되는질환으로는상기도열이 4예, 치성원인 ( dental origin) 이 3예, 외상이 예, 그리고뚜렷한원인을찾을수없는경우가 8예있었다. 농 배양에서원인균이확인된경우는포도상구균 3예, 연쇄구균 예그리고아시네토박테르균 1예로모두 6예이었고배양에서균이자라지않은나머지 11예는수술적배액술을시행하기전항생제사용과혐기성균의존재와연관이있을것으로생각된다. 고 jξl E크 심부경부농양의정확한위치와범위를결정하기위해 %

LN M띠 ti - AiIway Mediastinal Space (No) Enlargement Enlargement loculation compression extension Retropharyngeal (6) 1 Submandibular (5) Carotid () Visceral (4) Total (17) Note: Submand.gl.; Submandibular gland LN: Lymph node 서는경부의해부학적구조를이해해야하므로간단히소개한다 ( 4-7). 두경부의근막구역 (fascial compartments) 은근막층 ( fascia l layer) 에의해형성된잠재성공간 ( pot e ntial space ) 이다. 경부근막 ( ce rvical fascia) 은두개의기본층이있는데피부바로아래위치하여활 경근 ( platysma mu s ci e ) 을둘러싸고있는표재성경부근 막 (sup er ficial cervical fasc ia ) 과심부경부근막 ( deep cervical fasc ia) 으로구성되어있고, 심부경부근막은 다시표재층 (s up erf ic i al layer), 중간층 ( middle layer), 그리고심층 ( deep la yer) 으로이루어져있다. 심부경부근막의표재층은등세모근 (t rap ezi u s muscie), 교근 ( n1asseter muscle), 그리고흉쇄유돌근 (s terno cieidomastoid mus ci e) 과이하선및악하선을둘러싸고 있다. 중간층또는내장층 (v i scera l l ayer ) 은식도, 기관 그리고갑상선을둘러싸고있으며위로는설골에붙어있 고아래로는종격근막 ( medi as tinal fascia ) 과연결되어 진다. 이러한이유로중간층의감엽은종격동으로빨리파급될수있다. 심층은추전근막 ( prevertebral fascia) 과익상근막 ( a l ar fasc i a) 으로나누어지는데추전근막은 척추체와바로인접하여위치하며두개기저에서미골까지 양, 특히하악선과인접한공간에위치한농양의경우임파선뿐만아니라하악선의증대도관찰할수있었는데이는농양이나통반된세포염에의한염증세포의침윤및부종에기인한것으로생각된다. 심부경부감염의치료에는크게두가지로나눌수있는데그하나는항생제요법으로이는경부감염이의심될때가장먼저시행해야할치료방법이다. 그러나, 이미경부감염이세포염의단계를넘어서농양이형성된경우에는대량의항생제요법만으로는부족하며수술적배액술이동반되어야한다. 따라서심부농양의수술적배액술시행시정확한위치와범위를아는것이무엇보다중요하며이때 CT가많은도움을주게된다. 다시말해서 CT 를통한농양의정확한위치결정이농양의적절한배액시기와외과적접큰방향을결정하는데도움을주게된다 (, 3, 10). 설골상부의인두후공간에생긴농양은후인두 점막을통한경구절개를시행하게되지만인접한인두주위간극 ( paraphar yngea l space) 까지확산된농양은구외접근 (ex traora l approach ) 을해야한다. 인두주위간극의농양은보통인두외벽을안쪽으로융기를시키지만경통맥이이공간에인접되어지나므로경구접근방 연결되어있다. 익상근막은추전근막바로앞쪽에위치하 법은금기이다. 설하공간 (Sublingual spac e) 과저작근 며두개기처에서시작하여제 - 3흉추높이에서중간층과합류하게된다. CT는이러한심부경부근막의각층을다묘사할수는없지만병변이생긴공간의위치와범위를정확하게보여줄수있다. 심부경부감염은근막구역을침범하는염증성질환으로임파관, 혈관, 또는직접파급 ( direct extension) 에의해 공간 ( masticator space) 의농양은경구절개가가능하다. 설하및하악하공간의감염을초래하는 Ludwig s angina 의경우에는경계가분명한농양은잘형성하지않지만악하접근방법을통한수술적배액술에대부분의 환자들이잘반응한다고알려져있다 ( 11 ). 내장공간의농양은종격동염을일으킬가능성이있기때문에빠른시 확산된다. 이러한심부경부감염은크게세포염과농양으 간내에수술적배액술을시행해주어야한다. 저자들의 로나눌수있는데 CT 상농양은염증성연부조직종괴 경우에서도 CT 소견에근거한위와같은외과적접근방 내부에별개의저밀도음영으로나타나지만, 세포염은저 법으로모든예에서성공적인배액을할수있었다. 심부 밀도음영없이연부조직의부종과주위지방조직층의소 실을일으킨다 (1,, 8, 9). 저자들의모든농양의예에서도 저밀도음영을 CT 상증명할수있었다. 심부경부의농 경부농양의정확한위치를결정하는것외에 CT의또다른중요한역할의하나는농양의다방형성을확인하는것이다. 이는특히조영증강후검사에서잘묘사될수

5 정혜경외 : 심부경부농앙 있는데농양이다방형성되어있을경우에는각각의농양주머니마다절개를하여배액해야하므로농양의주머니수를확인하는것또한매우중요하다 (1, 3). 저자들의경우에서도다방형성된경우가 3예있었는데 CT 의도움으로모두성공적으로배액해낼수있었다 ( Tabl e 1). 심부경부감염은신체다른부위의감염과비교해서심각한합병증을더잘유발하는경향이있는데이는경부에는여러겹의경부근막층이존재하고병원균의침입구가다양하며생명과직결되는장기가인접해있기때문이다 ( 1, 3, 8, 9, 1, 13). 심부경부농양에서나타날수있는합병증으로기도압박소견이있는데이는주로인두후농양, 양측성편도주위농양, 인두주위간극농양또는 Ludwig s angina의경우에서잘나타나게되고특히저자들의예에서보면인두후농양의경우 6예모두에서기도압박소견을보였다. 기도압박소견이외에나타날수있는합병증으로는경정맥혈전증, 종격동내침범, 농흉, 경동맥파열등이있으며저자들은인두후농양환자에서종격동침범된 1예를경험하였다. 결론적으로, 심부경부감염증을의심하는환자에서 CT를이용하면심부경부농양의존재뿐만아니라정확한위치와범위를규정할수있고또한그와동반된합병증유무를밝혀낼수있기때문에외과적치료계획을적절히세우는데많은도움을줄수있으리라생각된다. 츠 } 끼무허 1.
