On admission, chest x-ray demonstration a round and well marginated mass on right upper lobe. Electrocardiogram showed deep and symmetric T-wave inversion in precordial leads. This finding of electrocardiogram were compatible with apical hypertrophic cardiomyopathy. Under the impression of apical hypertrophic cardiomyopathy or acute coronary insufficiency, we performed 2D-echocardiogram. 2D-echocardiogram showed, about 63 cm sized, inhomogeneous right ventricular apical mass. For the differential diagnosis of myocardiac infarction, we performed coronary angioram, Which showed no significant stenosis of right and left coronary artery. We performed a biopsy from lung mass and right ventricular apical mass. Pathologic examination revealed non-small cell lung cancer with metastasis to right ventricle. 복부는 부드러웠으며 촉지되는 장기나 압통은 없었다. 검사실 소견 일반 혈액검사, 혈청 생화학 검사 및 소 변 검사상 이상소견 없었다. 흉부 방사선 검사소견 우폐 상엽에 직경 약 3 cm 크기의 둥글고 경계가 분명한 종괴가 관찰되었다. 심전도소견 전흉부 lead(v1-6), 특히 V2-5에서 대칭적인 거대 T파 역위가 관찰되었다.

3 현병력 환자는 내원 15일전부터 우측 견갑골 주변 의 통증과 종괴가 촉지 되어 타병원 방문하여 시행한 흉부 컴퓨터 단층촬영상 우측 견갑 하근의 농양과 심전 도상 Lead Ⅱ, Ⅲ, avf와 V1-6에서 T파 역위 소견이 보여 허혈성 심장질환이 의심되어 본원 심장내과로 전 원 되었다. 과거력 평균 하루 1갑씩 40년간의 흡연력과 20년 전 폐결핵으로 치료 받음. 이학적 소견 내원당시 급성병색을 보였으나 생체 활력징후는 안정적이었으며 호흡음은 청명하였다. 수축 기 심잡음이 좌측 흉골 연에서 청진되었다. 전신진찰 소 견상 좌측 경부림프절 비대가 관찰되고, 우측 쇄골 상 부와 견갑골 상부에 4 cm 크기의 종괴가 촉지되었고, Fig. 2. 2D-Echocardiogram (modified apical 4 chamber view) showed, about 6 3 cm sized, inhomogeneous right ventricular apical mass and small amount pericardial effusion. 복부는 부드러웠으며 촉지되는 장기나 압통은 없었다. 검사실 소견 일반 혈액검사, 혈청 생화학 검사 및 소 변 검사상 이상소견 없었다. 흉부 방사선 검사소견 우폐 상엽에 직경 약 3 cm 크기의 둥글고 경계가 분명한 종괴가 관찰되었다. 심전도소견 전흉부 lead(v1-6), 특히 V2-5에서 대칭적인 거대 T파 역위가 관찰되었다. 2주후 시행한 심전도상 의미있는 변화는 없었다(Fig. 1). 심초음파소견 우측 심첨부에서 약 6 3 cm 크기의 주변의 심근과 구분이 되지 않고, 고정된 불균등질의 종 괴와 경도의 삼첨판 폐쇄 부전증, 약간의 심낭액이 관 찰 되었다(Fig. 2). 관상동맥 조영술소견 심근 경색등 허혈성 심질환의 가능성을 배제하기 위하여 관상동맥 조영술을 시행하 였다. 우관동맥 원위부의 50% 이하의 협착 소견이 보 였고, 좌관 동맥에서는 의미있는 협착소견은 보이지 않 Fig. 3. Right ventriculography showed filling defect on apex. 았다. 우심실 조영술상 심첨부의 filling defect가 관찰 되어(Fig. 3), 이 부위에서 조직 검사를 시행하였다. 치료 및 임상결과 비소세포성 폐암이 간, 부신, 우 흉부 컴퓨터 단층촬영 우폐 상엽에 3 cm 크기의 종 심실, 좌측 경부 림프절, 우측 쇄골 상부, 우측 등배부등 괴가 관찰되었고, 우심실 내 장축 5 cm 크기의 불균등 다발성 전이가 있는 것으로 판단하고 환자에게 항암 화 질한 종괴가 관찰되었으며, 또한 간 좌엽의 전면을 따 학요법을 권유하였으나 거절하고 퇴원 하였으며, 이후 라 장축 4 cm 크기의 종괴 및 양측 부신에서 전이로 병원을 방문 하지않아 추적관찰이 이루어지지 않았다. 보이는 종괴가 발견 되었다. 조직학적 소견 우측 심첨부의 종괴 및 좌측 경부림 고 안 프절, 우측 쇄골상부의 종괴와 우측 등배부 종괴, 우폐 상엽의 종괴에 대해 조직검사를 시행하였으며, 비소세 심장에 발생하는 종양은 원발성 또는 이차성으로 발 포성 폐암이 우심실 및 좌측 경부 림프절, 우측 쇄골 상 생할 수 있으며, 원발성 종양은 양성 및 악성으로 구분 부, 우측 등배부로 전이된 것으로 판명되었다(Fig. 4). 되지만 이차성 또는 전이성 종양은 정의상 악성이라고 1438 Korean Circulation J 2000;30(11):
