A 19-year-old soldier was admitted with sudden cardiac arrest that presented as ventricular fibrillation requiring cardiopulmonary resuscitation. His electrocardiogram (ECG) showed normal sinus rhythm with inverted T waves from leads V1 to V4 and isolated premature ventricular beats with a left bundle branch block (LBBB) pattern. Echocardiography showed that the right ventricle was enlarged with apical trabecular formation. The coronary angiogram was normal. Cardiovascular magnetic resonance imaging (MRI) showed thinning and fatty substitution of both the right and left ventricular free walls. Ventricular tachycardia (LBBB pattern) occurred frequently despite anti-arrhythmic drug treatment. His drug-resistant ventricular tachycardia was treated with an implanted cardiac defibrillator. Left ventricle involvement in ARVC is less common and more severe. Cardiac MRI is considered the best imaging technique for evaluating the right ventricle and diagnosing ARVC. (Korean J Med 76:S64-S69, 2009) Key Words: Arrhythmogenic right ventricular cardiomyopathy; Left ventricle 서론부정맥야기성우심실이형성증 (ARVD) 은 1977년에 Fontaine 등이처음보고하였으며 1), 병리학적으로는우심실외벽특히전벽심근층의일부혹은전체가섬유조직또는지방조직으로대치되어있는드문질환으로임상적으로는우심실 기원의심실성기외수축, 심실성빈맥등의심실성부정맥을유발하여주로젊은성인에게심부전, 실신, 돌연사를일으킨다 2). 이질환의정확한원인및유병률등은알려져있지않으며젊은성인의돌연사의한원인인비후성심근병증과비교할때대략 1/10 정도의발생률을보인다고하나실제유병률을반영한다고보기는어렵다. 저자들은최근에건강 Received: Accepted: Correspondence to Jeong-Hee Kim, M.D., Department of Internal Medicinem Eulji University Hospital, 1306 Dunsan-dong, Seo-gu, Daejeon , Korea jhkim999@eulji.ac.kr - S 64 -

The chest anteroposterior X-ray showed cardiomegaly and pulmonary edema. 퓨터 게임방에서 약 10분 동안 컴퓨터 게임을 하던 중, 갑자 기 쓰러지면서 의식이 소실된 상태로 119 구대원에 의해 심 진단기준 3개에 합당한 소견을 보여 ARVC로 진단할 수 있 폐소생술이 시행되면서 응급실에 도착하였다. 응급실 도착 었다. 입원 8일째 의식은 명료해졌고, 생체 징후 역시 정상 시 심전도의 리듬은 심실세동이었으며, 제세동 및 심폐소생 적이었으나, 항부정맥 약제를 복용함에도 불구하고 좌각차 술을 시행한 후 혈압은 94/63 mm/hg, 심박동은 분당 110회, 단 형태의 심실 빈맥이 자주 발생하였다(그림 4). 이에 따라 체온은 35 가 기록되었다. 심전도 소견으로는 lead V1~V4 삽입형 제세동기를 시술하였고 증상이 호전되었다. 심장조 에서 T파의 역위와 QRS 폭의 증가(QRS prolongation in lead 직 검사와 전기생리학적 검사를 통한 심실빈맥 유발 검사 V1~V3 >120 ms)를 동반한 정상 동리듬(normal sinus rhythm) 등의 추가적인 검사가 필요하였으나, 더 이상의 검사를 원치 과 간헐적인 심실 조기 수축이 확인되었다(그림 1). 흉부단 않아 퇴원하였다. 순촬영에서는 심비대 소견과 함께 폐부종이 의심되었다(그 고 림 2). 심초음파 검사상 우심실 첨부의 섬유주 형성(apical trabecular formation)을 동반한 우심실의 확장(right ventricle 찰 부정맥야기성 우심실이형성/심근증(AVRD/AVRC)은 약 3,4) end-diastolic dimension: 50 mm)이 확인되었으며, 좌심실 구 반수에서 상염색체 우성의 유전 형태를 보이며 축률 역시 감소되어(left ventricle ejection fraction=51%) 있었 지방 및 섬유침착으로 인한 우심실 기원의 심실 빈맥이 발 다. 관상동맥 질환을 배제하기 위하여 시행한 심혈관 조영술 생하는 질병으로 알려져 있다. 초기에는 우심실 심근의 발생 상에서 관상동맥의 이상소견은 관찰되지 않았다. 심장자기 학적 결함에 의한 선천성 기형으로 여겨져 이형성증 공명촬영의 T1강조영상에서 우심실 벽이 얇아져 있는 소견 (dysplasia) 이라는 용어를 사용하였으나, 이후 계속적인 심근 우심실의 과 함께 우심실과 좌심실에서 지방 침착 확인되었으며(그림 의 손상에 대한 섬유지방(fibrofatty)의 대체로 보상하려는 기 3), 우심실 방출로(RV outflow tract)의 확장 소견이 보였다. 전 및 경과를 취하는 것이 확인되어 1996년 세계보건기구 이상의 검사에서 ARVC 진단기준의 주진단기준 한 개와 부 (World Health Organization)와 국제심장학협회(International - S 65 -

Society and Federation) 에서는심근병증의한형태로고려되어져야한다고결정내렸다 5,6). 부정맥야기성우심실심근증은우심실의미만성심근손실과더블에이부분이섬유및지방조직으로대체되는것이특징이며, 삼천판하부, 심첨부그리고누두부를연결하는삼각형형태의부위에서동맥류확장 (aneurismal dilatation) 이환자의 50% 에서관찰된다. 부정맥야기성우심실심근증은지방침윤만관찰되는 fatty form과지방침윤과더불어섬유화도관찰되는 fibrofatty variant로나뉘는데, fatty form은우심실에국한되며주로심첨부와누두부를침범하며심장벽이얇아지는소견은동반하지않는것으로알려져있다. 반면에 fibrofatty variant는지방조직의침착외에섬유화가동반되며, 우심실벽이 3 mm 이내로얇아지며동맥류확장및염증세포침윤이두드러진다. 원인은확실하게밝혀지진않았지만이질환의발생기전으로는개체발생이상, 퇴행성, 염증성, 예정세포사멸성의네가지이론이있다 6). 부정맥야기성우심실심근증은 1994년 McKenna 등 7) 이제정한진단기준에의해진단될수있는데, 가족력, 심전도소견, 부정맥, 심장의구조와기능, 조직학적소견을포함하고있으며, 두개이상의주진단기준혹은하나의주진단기준과두개의부진단기준을포함하거나, 네개이상의부진단기준에합당하면부정맥야기성우심실심근증으로진단내릴수있다 ( 표 1). 부정맥야기성우심실심근증환자의 90% 가량에서비정상적심전도소견이관찰되며 3), 이중가장흔한이상은 V1~V3에서역위된 T파혹은우측흉부유도의 ST분절의시작또는 QRS 복합체의끝에위치하는 epsilon파가관찰되는것이다. 그렇지만 12세이하의소아나여성의경우그리고우각차단이있는경우에도역위된 T파 가관찰될수있음을고려해야한다. 또한신호평준화심전도 (signal average ECG) 에서 RMS (root mean square voltage) in terminal 40 ms of QRS가 25 µv 미만으로기록되며, duration of high frequency low amplitude signal <40 µv가 40 ms를초과하거나, duration of filtered-qrs가 120 ms 이상으로연장되는 late potential의소견이나타날수있다. 부정맥야기성우심실심근증의진단에있어혈관조영술, 심초음파, 자기공명촬영등을통한우심실의기능및형태의이상을확인하는것이진단에중요하다. 우심실조영술 (right ventricular angiography) 은누두부, 심첨부그리고삼첨판하부의이상운동과무운동을비교적정확히관찰할수있으며, 90% 이상의민감도를가진다 8). 심초음파는민감도와특이도는높지않지만, 비침습적이라는장점이있어부정맥야기성우심실심근증이의심되는환자나환자가족들에대한선별검사시사용할수있다. 또한심초음파는엡스타인기형과심방중격결손과같은우심실질환을배제함으로써부정맥야기성우심실심근증을진단하는데있어유용한검사법이다. 자기공명영상은비침습적일뿐만아니라심장근육과지방조직을구별할수있는유일한검사법으로매우유용하다 9). 그렇지만이러한영상학적진단방법은해석에있어검사자간의차이가나타날수있다. 부정맥야기성우심실심근증의확진은조직검사를시행하여우심실에서지방조직, 지방섬유조직을조직학적으로확인하는것이다. 지방조직과섬유조직이각각 3%, 40% 이상이며심근의비율이 40% 미만일경우에부정맥야기성우심실심근증과정상혹은확장성심근증을감별할수있다 10). 그렇지만병변이비연속적이거나고르게분포되어있지않은경우와질환이심근외막에서부터 - S 66 -

Task Force diagnostic criteria for arrhythmogenic right ventricular dysplasia/cardiomyopathy 5) Global and or regional dysfunction and structural alterations Severe dilatation reduction of right ventricular ejection fraction with no (or only mild) LV impairment Localized right ventricular aneurysm (akinetic or dyskinetic areas with diastolic bulging) Severe segmental dilatation of the right ventricle Mild global right ventricular dilatation and or ejection fraction reduction with normal left ventricle Mild segmental dilatation of the right ventricle Regional right ventricular hypokinesia Tissue characterization of walls Fibro-fatty replacement of myocardium on endocardial boipsy Repolarisation abnormalities Inverted T waves in right precordial leads (V 2 and V 3) (people aged more than 12 years. in absence of right bundle branch block) Depolarisatin conduction abnormalities Epsilon waves or localized prolongation (>110 ms) of the QRS complex in right precordial leads (V 1-V 3) Late potential (signal averaged ECG) Arrhythmias Left bundle branch block type ventricular tachycardia (sustained and non-sustained) (ECG, Holter, exercise testing) Frequent ventricular extrasustoles (more than 1,000/24 h) (Holter) Family history Familiar disease confirmed at necroscopy or surgery Familial history of premature sudden death (>35 years) due to suspected right ventricular dysplasia Familial history (clinical diagnosis based on present criteria) LV=Right ventricle, ECG-Electrocardiography 내막으로진행하는특성이있으므로민감도는높지않다. 또한정상노인의경우에심외막하지방조직 (subepicardial adipose tissue) 이증가하며, 만성적으로알코올을섭취하거나 Duschenne/Backer 근이영양증에서도지방침윤이증가할수있다. 부정맥야기성우심실심근증은시간이지날수록점차진행하는질병으로우심실침범에국한되는것이아니라좌심실도침범하는것으로알려져있다 11). 게다가좌심실의병변은임상적으로의미있는부정맥과좀더심한심비대및심부전과연관되어있다. 부정맥야기성우심실심근증의가 장중요한치료목표는부정맥에의한급사의방지에있다. 그렇지만이전에무증상이었던젊은환자나운동선수의경우첫번째임상증상이급사로나타날가능성이높다. 임상적그리고병리학적으로좀더좋지않은결과를초래할수있는위험요소로는젊은나이, 박축률이감소한광범위한우심실침범, 좌심실침범, 실신및심실빈맥이있다 2,12,13). 급사의위험도를평가하기위해가족력및실신의과거력을포함한병력조사를비롯하여심전도, 24시간홀터검사, 운동부하검사 (exercise stress testing) 등의비침습적인검사를 - S 67 -

5 - The Korean Journal of Medicine: Vol. 76, Suppl. 1, 시행하여야하며심실조동이나심실세동이있었던환자, 실신의과거력은있지만비침습적인환자에서음성으로확인된환자, 급사의과거력이있거나우심실의기능이감소한환자는전기생리학검사 (intracardiac electrophysiologic study) 를시행하여야한다. 현재급사의위험도를정확히판단하는것이어렵고무증상인환자에대한치료여부와효과적인치료방법에대한논란이있으나 3) 베타차단제, 항부정맥약제, 전극도자절제술 (catheter ablation), 삽입형제세동기가치료방법으로알려져있다. 생명에지장이없는환자에게는베타차단제나 class I, II 계열의항부정맥약제를사용한다. 또한 Wichter 등 14) 의연구에서는급사의위험도가낮은환자에서소탈롤 (sotalol) 이나아미오다론 (amiodarone) 을베타차단제와병용하였을때효과가높다고하였다. 전극도자절제술 (radiofrequency ablation) 은부정맥을발생시키는부분을제거하는시술로약물에반응이없는환자에게시행할수있다. Fontaine 등 15) 은전극도자절제술을반복적으로시행했을때두번째와세번째시도에서각각 45% 와 66% 의성공률을보였다고보고하였다. 그렇지만부정맥유발성우심실심근증이점차진행하는질환으로새로운부정맥발생부분이생기므로전극도자절제술의경우약 60% 의재발률을보이는제한적인치료이다. 급사를예방할수있는가장효과적인치료는이식형제세동기를삽입하는것이다. 이방법은심실빈맥이나심실세동에의한심정지후소생된환자, 생명을위협하는심실빈맥, 약제에반응하지않는경우, 가족력이있는환자의경우에시행한다. 부정맥야기성우심실심근증의경우우심실혹은양심실모두에서수축기장애를야기할수있으므로심부전의치료로이뇨제, 안지오텐신변환효소억제제와강심제를사용하고혈전이발생할수있으므로항응고제를사용한다. 요약부정맥야기성우심실심근증은젊은성인이나운동선수에서발생하는급사의중요한원인으로보고가되고있다. 따라서젊은성인에서발생하는부정맥이나설명불가능한심근증이있을경우에부정맥야기성우심실심근증의가능성에대해고려해보아야한다. 급사의가족력과좌각차단의형태를보이는심실성부정맥이있을경우비침습적인자기공명영상과심초음파를통한빠른진단과이식형제세동기의삽입이반복되는심정지와합병증을예방할수있을것이다. 중심단어 : 부정맥야기성우심실이형성증 ; 좌심실 REFERENCES 1) Frank R, Fontaine G, Vedel J, Mialet G, Sol C, Guiraudon G, Grosgogeat Y.
